Fracture of the femur, fish odour, and copper deficiency in a preterm infant.

A preterm baby boy with blood and bone changes of copper deficiency is described. Copper deficiency was suspected after fracture of the left femur during examination of the hip joint. A low serum copper concentration (2.7 mumol/l; 17.2 micrograms/100 ml) and caeruloplasmin (0.04 g/l; 0.004 g/100 ml) confirmed the diagnosis. Despite the introduction of solids at 18 weeks the copper concentration remained low, and treatment with copper sulphate (2.5 mg daily) was started at 6 months. Treatment was stopped at 9 months, when he was both physically and developmentally normal. When given a choline-containing vitamin preparation (Ketovite) he developed a fish odour because of the accumulation of trimethylamine. Withdrawal of this preparation at 6 weeks and substitution with a choline-free preparation (Abidec) was soon followed by disappearance of the odour. It is speculated that prematurity rather than copper deficiency was responsible for the poor activity of liver enzyme, trimethylamine oxidase.

Effect of feed temperature and phototherapy on gastric emptying in the neonate.

The effect of feed temperaturae and phototherapy on the rate of stomach emptying was studied in healthy infants during the first week. Emptying rate was measured by the modified serial test meal technique using 10% Dextrose feeds. The results show that emptying rate is independent of feed temperature and is not affected by phototherapy.

A comparative trial of slow-release aminophylline, salbutamol and a half dose combination in the prevention of childhood asthma.

The effect of slow-release aminophylline, salbutamol and a half dose combination of both was compared in children with chronic asthma. Drugs were given over three consecutive 5-week periods during which patients recorded daily peak flow measurements and symptom scores. At the end of each treatment period pulmonary function tests were done. Sixteen of twenty-nine children completed the trial satisfactorily and their results have been used for analysis. The data show slow-release aminophylline and salbutamol to be equally effective and the half dose combination as effective as either drug alone. The results suggest there may be a therapeutic advantage in combining a beta 2 agonist with theophylline when high doses of either drug alone are not fully effective or when toxic effects occur.

Effect of posture on the pattern of stomach emptying in the newborn.

To determine the effect of posture on the pattern of neonatal stomach emptying, 56 test meals of a 10% dextrose were given to 14 healthy infants over a period of two or three consecutive days. The infants, whose postnatal age ranged between 5 and 27 days, were studied in each of four positions. The emptying pattern was determined by the double-sampling dye-technique. The results indicate that there are no significant postural differences in the pattern of stomach emptying. They also indicate that neonatal and adult patterns are similar.

The effect of metoclopramide on neonatal gastric emptying.

1 The effect of metoclopramide on gastric emptying was studied on twenty-five occasions in fifteen low birth weight babies. 2 The serial test-meal technique was used. 3 The results indicate that metoclopramide does not promote gastric emptying in the newborn period.

Neonatal barbiturate withdrawal.

An infant with neonatal barbiturate withdrawal syndrome is reported. The clinical features, diagnosis and treatment of this condition are discussed. A retrospective review of fifty-three infants of barbiturate-treated epileptic mothers indicated a high incidence of "jitteriness" (17%) and feeding problems (36%) in the immediate neonatal period.
