[Hydrofluoric acid injuries: pathogenesis and treatment].

On the basis of two case histories, the most recent literature concerning hydrofluoric acid injuries is reviewed with particular attention to the pathogenesis and the therapeutic possibilities. It is concluded that washing with water is the primary and most important treatment. Depending upon the extent of the injury, surface treatment with 2.5% calcium gluconate solution or gel may then be employed and/or infiltration treatment with a 5-10% calcium solution. Possibly combined with local analgesia and hyaluronic acid. Intravenous or intra-arterial infusion of calcium compounds and surgical excision may be considered as specialist treatment.

[Prosthetic valve endocarditis caused by Haemophilus parainfluenzae].

A case of endocarditis preceded by dental treatment without prophylactic antibiotics and caused by Haemophilus parainfluenzae in a patient with a biological heart valve prosthesis is described. This bacteria grows slowly in the usual blood culture media and, because of this, the etiological diagnosis was not established until after incubation for 11 days. Blood cultures from patients suspected of having endocarditis should be incubated for longer than the usual five to seven days. Close cooperation between the clinical microbiologist and clinicians is necessary so that important findings are not overlooked. Dental treatment of patients with heart valve prostheses should always take place under antibiotic prophylaxis.

The prognostic value of immunohistochemical estrogen receptor analysis in paraffin-embedded and frozen sections versus that of steroid-binding assays.

Estrogen receptors (ER) were independently analyzed using dextran-coated charcoal assays (ER-DCC) and immunohistochemical assays in frozen (ER-ICA) and paraffin-embedded tissue (ER-PAR) from 130 human breast cancer specimens drawn from postmenopausal high-risk patients registered in the Danish Breast Cancer Cooperative Group. ER was best detected with the ER-DCC assay followed by the ER-ICA (relative sensitivity 87%) and the ER-PAR assays (relative sensitivity 71%). The semiquantified staining features of the immunohistochemical assays were statistically significantly correlated with each other and with ER-DCC. Analysis of disease-free interval (DFI) and overall survival (OS) showed that all assays allowed statistically significant discrimination between a high risk and a low risk group, although the sensitivity differences tended to be reflected as small differences in clinical discriminatory power. The patient groups were then stratified according to adjuvant treatment [radiotherapy (RT) versus radiotherapy and tamoxifen (RT + TAM)]. The survival advantage was tied primarily to the receptor status itself in the steroid-binding assays, but was linked to both the receptor status and the adjuvant treatment in the immunohistochemical assays. Thus, the relative risks in terms of DFI and OS were of the same relative magnitude in the RT and RT + TAM groups for ER-DCC assays using a cut-off level of 10 fmol/mg cytosol protein, while there were large differences in the relative risks between RT and RT + TAM groups for ER-ICA and ER-PAR assays. We conclude that an ER assay in fresh tissue should be given first priority, but if there is no fresh tissue, an ER assay in paraffin-embedded tissue offers a reasonably good alternative as a prognosticator and an equivalent alternative as a predictor of the response to endocrine treatment.

[Intensive dynamic back exercises for patients with chronic low back pain. A controlled study].

The effect of intensive dynamic back extensor exercises for patients with chronic low back was investigated in a controlled clinical trial where the patients underwent a three months intensive training program a total 30 sessions. The 105 patients were divided into three groups: a treatment group, an alternative group which underwent 1/5 of the treatment group's exercise program per session, and an alternative group in which the treatment consisted of thermotherapy, massage and mild exercise. Regardless of whether the outcome of treatment is assessed qualitatively or quantitatively, a statistically significant, favourable difference was found between the results for the treatment group and those for the alternative groups. The therapy was found to be without risk, but patients with clinical signs of current lumbar nerve root compression or radiological signs of spondylolysis or halisteresis of the column were excluded from the study. The authors recommend the introduction of this form of treatment as a general offer in the primary and secondary health sector. Back exercises can advantageously be conducted in a group; thus, the resources required for this treatment do not differ significantly from those required for many of the other forms of treatment generally employed, which often require individual attention.

Imported pedal chancroid: case report.

A man aged 22 who had returned from the Fiji Islands to Denmark had chancroid on the left foot, but no history or sign of primary genital infection. The pedal location only is an unusual presentation of the disease, which was diagnosed only microbiologically. Chronic tropical ulcers therefore demand special microbiological attention.

A case of Friedrich's disease of the clavicle.

Friedrich's disease is an aseptic osteonecrosis of the medial end of the clavicle. It is a rare condition that can be confused with osteomyelitis, arthritis, or tumor. Because it often resolves spontaneously, a correct diagnosis will save the patient from unnecessary treatment.

Additional studies on side effects of melperone in long-term therapy for 1-20 years in psychiatric patients.

The present study reporting on continuous melperone treatment of hitherto longest duration recorded in literature was conducted in order to reveal side effects of long-term melperone therapy. 50 patients, 24 females and 26 males, aged 37-102 (average: 81 years of age) were treated with melperone (Buronil, Bunil, Eunerpan) for 1 to 20 years. In most patients, daily dosage varied from 10 to 300 mg, total dosage ranging from 6510 mg to 1 662 225 mg, avr. 203 923 mg. Diagnoses were as follows: senile dementia (14), organic dementia (9), arterio-sclerotic dementia (8), schizophrenia (17) and nonspecific psychosis (2). The patients were examined for clinical side effects including abnormal ECGs and ophthalmological diseases. Biochemical laboratory tests comprised sedimentation rate, haemoglobin, leucocytes, creatinine, alanine-aminotransferase, gamma-glutamyl-transferase and bilirubin. The results were evaluated by a specialist in internal medicine and an ophthalmologist performed the examinations on the 20 patients who were able to cooperate. The conclusion was that no serious side effects were observed which could with any certainty be related to melperone therapy.

Chronic lymphocytic leukaemia of T cell origin. Clinical variation possibly due to involvement of different T lymphocyte subpopulations.

Based on the literature and 2 patients studied, we suggest that at least 2 different clinical entities are included in the concept of T CLL: (i) a clinical variant characterized by a relatively benign course, splenomegaly without lymphadenopathy, low lymphocyte count and granulocytopenia; the proliferating lymphocyte is morphologically mature, of medium size and a cytoplasm with azurophilic granules staining positively for acid phosphatase and corresponding to parallel tubular arrays as demonstrated by electron microscopy. The cells form E-rosettes, have no surface-membrane-bound Ig, but Fc-receptors for IgG. With monoclonal antibodies, the phenotype is OKT3+, OKT4- and OKT8+, theoretically corresponding to the suppressor/cytotoxic T lymphocyte subset, but functionally the cells demonstrate killer cell (responsible for ADCC), but not natural or suppressor cell activity. (ii) another clinical variant with an aggressive course, massive hepato-splenomegaly, lymph node enlargement and very high lymphocyte counts; the lymphocytes are small without cytoplasmic granules; their immunological and functional characteristics have not been determined, but morphologically the cells correspond to the T helper/inducer lymphocyte subset. Thus, involvement of different T lymphocyte subsets may be the reason for the clinical variation in T CLL.

Flow cytometric deoxyribonucleic acid analysis of granulosa cells aspirated from human ovarian follicles. A new method to distinguish healthy and atretic ovarian follicles.

Granulosa cell aspirates from human ovarian follicles were analyzed by flow cytometry to determine the fraction of cells in the DNA S-phase of the mitotic cell cycle. The aim of the study was to evaluate if the percentage of granulosa cells in S-phase (the S-fraction) could be used to indicate whether a follicle was healthy or atretic. A highly significant relationship was found between the S-fraction and the concentration of estradiol in the follicular fluid (r = 0.6, P less than 0.001). More than 85% of the follicles having an S-fraction of 16% or greater contained intrafollicular levels of estradiol equal to or greater than 200 ng/ml and had a low androstenedione:estradiol ratio. Conversely, 95% or more of the follicles that had an S-fraction of less than 16% contained low estradiol (less than 200 ng/ml) and had a high androstenedione to estradiol ratio. We conclude that flow cytometric DNA measurements on follicular aspirates provide a reliable and rapid method by which to distinguish healthy and atretic ovarian follicles. Since only a small fraction (less than 5%) of an entire granulosa cell population is required for S-phase analysis, the technique allows the majority of cells to be immediately available or other biochemical studies. Moreover, since excision of ovarian tissue is avoided, the technique may be acceptable for studies on women with normal ovarian function but who are undergoing laparotomy or laparoscopy for some reason.

Additional studies on side effects of melperone in long-term therapy for 1 to 15 years in psychiatric patients.

The present study which reports on the hitherto longest continuous melperone treatment recorded in the literature, was conducted in order to reveal side effects of long-term melperone therapy. 17 female and 20 male patients, aged 33-97 years, most of them with the diagnoses: schizophrenia (11 patients), dementia organica (11 patients) and dementia senilis (11 patients) were treated with melperone (Buronil) in doses of 15--800 mg/day for 1 to 15 years. The patients were examined for clinical side effects, abnormal electrocardiograms and ophthalmological diseases as well as abnormal values in sedimentation rate, hemoglobin, leucocytes, creatinine, alanine-aminotransferase, gamma-glutamyl-transferase and bilirubin. Also the thymol reaction was done. The electrocardiograms and laboratory investigations were controlled by specialists in internal medicine and the eye diseases by an ophthalmologist. We did not find any severe side effects which could be related with any certainty to melperone therapy.

DNA flow cytometry on human epidermis: I. methodological studies on normal skin.

A method for single cell DNA measurements by flow cytometry based on a technique successfully used for the hamster cheek pouch epithelium was extended for studies on human epidermis. A least squares estimation procedure was used for the statistical analysis of the initially obtained DNA frequency distribution. In 14 specimens of abdominal skin from a young healthy female the precentages of epidermal cells with an S-phase DNA content were 3.0 +/- 0.6 (SD), and the percentages of epidermal cells with a G2+M phase DNA content 1.1 +/- 0.1 (SD). No significant differences in values were found for specimens investigated in different experiments. No influence of tissue storage time for a maximum of 32 days in liquid nitrogen was discernible. Values from the corresponding dermal tissue were significantly more variable then the epidermal values, but the levels were of the same magnitude. It is concluded that this technique used for human epidermis yields sufficient stable and accurate estimates of S and G2+M fractions that it should be a valuable tool useful as an alternative or a supplement to conventional cell kinetic methods.

Cellular antitumor immune response in women with risk factors for breast cancer.

The leukocyte adherence inhibition technique was used to study cell-mediated immunoactivity to breast adenocarcinoma. In a group of 74 patients with untreated breast cancer in Stages I and II, 69% showed a positive response, while 48% of the 25 patients in Stages III and IV had reactive leukocytes. Among 43 control persons, only 2 women showed positive responses. In a group of 161 women with benign breast diseases, 24% showed a positive reaction against breast carcinoma extract. The percentage of positive responses was higher than the average of women with benign disease among those who had a mother or a sister with breast cancer, who had previously had benign breast lumps removed, who had microcalcifications in their breasts, or for whom an increased intraductal epithelial proliferation was found in their biopsies. Twenty-eight women with benign breast diseases and two or more of the above factors were assigned to the high-risk group, in which 43% had reactive leukocytes. Of the 50 women with one risk factor, 34% showed a positive response, while of the 83 women in the low-risk group, with none of the above factors, only 11% had a positive reaction. The results suggest that the leukocyte adherence inhibition test may be used to identify groups of women who have a high risk for breast cancer.
