Hemodynamic changes in the early phase of artificially created arteriovenous fistula: color Doppler ultrasonographic findings.

The objective of this study was to evaluate hemodynamic variables in arteriovenous fistulas by color Doppler ultrasonography. This study involved 28 patients with chronic renal failure who were sent to surgery clinic for creation of an arteriovenous fistula of the Brescia-Cimino type. Patients were evaluated preoperatively and on the first and seventh days postoperatively by a color Doppler ultrasound machine with a 7.5 MHz linear probe. The distal radial artery was examined preoperatively and the fistula itself postoperatively. Changes in the fistula size and the velocity, volume, and resistive index of the distal radial artery were recorded. Postoperatively the radial artery diameter, systolic flow rates, and volume flow had increased significantly, especially on the first day, in comparison to preoperative values. Resistive index values has decreased significantly at both the first and the seventh days postoperatively. Color Doppler ultrasonography is a very effective method in the evaluation of hemodynamics of arteriovenous fistulas in hemodialysis patients. It will allow an understanding of the pathology in nonfunctioning fistulas or of the cause of complications that develop secondarily.

Predictive value of colour Doppler ultrasonography in detecting failure of vascular access grafts.

The predictive role of colour Doppler ultrasonography in determining the initial success and long-term patency of polytetrafluoroethylene (PTFE) vascular access grafts for haemodialysis was investigated. Two groups of patients were studied. The upper extremities of 21 patients in the control group were assessed by clinical examination only; those of 17 in a second group (Doppler group) were also assessed by ultrasonography before and after operation. Straight PTFE vascular access grafts were used between the brachial artery and axillary vein in all patients. The median preoperative brachial artery flow rate was significantly lower in patients who later developed graft thrombosis (66 versus 87 ml/min, P < 0.01), as was the median postoperative graft flow rate (480 versus 800 ml/min, P < 0.001). Routine preoperative and postoperative colour Doppler ultrasonography is recommended for every patient in whom a vascular access graft is planned.

The diagnostic value of colour Doppler ultrasound in central venous catheter related thrombosis.

AIM: This prospective study was performed to establish the correlation between colour Doppler ultrasound (US) observations and venography in detecting catheter-related thrombosis. METHOD: Forty-four patients with subclavian venous catheters were studied. The sensitivity and specificity of colour Doppler US was determined for the diagnosis of central venous thrombosis and compared with contrast venography. Criteria considered to show the presence of catheter-related thrombosis included visualization of thrombus (T), absence of spontaneous flow (S), absence of phasicity of flow with respiration (P), incompressibility of the vein with probe pressure (C) and visualization of increased venous collaterals (V). RESULTS: Analysing each variable separately, P had the highest sensitivity (94%) and S had the highest specificity (100%). The best combinations for diagnosis were T+P, P+S, and T+P+S with a sensitivity of 94% and specificity of 88%. Overall diagnostic value of colour Doppler US had a sensitivity of 94% and specificity of 96%. CONCLUSION: Colour Doppler US is a reliable method for detecting central venous catheter-related thrombosis, especially if several parameters are evaluated in combination.

The risk factors in central venous catheter-related thrombosis.

Catheter-related venous thrombosis is one of the most important complications of central venous catheters. The aim of this prospective study was to assess the risk factors that may be important in the development of catheter-related thrombosis. Multiple lumen (n = 20) and single lumen (n = 24) polyurethane catheters were inserted to the subclavian vein by the infraclavicular approach in 44 consecutive patients. All variables that may be significant for the development of thrombosis related to the patient, the catheter, the insertion technique and the solution administered through the line were recorded. Eighteen of 44 patients (40%) were found to have venous thrombosis. Two factors were found to correlate significantly with the development of central venous catheter-related thrombosis, namely the number of vein punctures (one vs two punctures, P < 0.01) and the composition of solution (fluid replacement vs total parenteral nutrition, P = 0.01).

Evaluation of renal morphology and vascular function by color flow Doppler sonography immediately after extracorporeal shock wave lithotripsy.

Thirteen male and seven female patients with renal calculi were evaluated by color flow Doppler sonography (CFDS) for renal morphology and function before and after receiving 1000 to 2350 shocks with the Dornier MPL 9000 lithotripter. No significant morphologic change was encountered except for renal enlargement thought to be secondary to edema induced by shock wave application. Similarly, no significant differences were recorded in the systolic and diastolic flow rates or pulsatility (PI) or resistive (RI) indices of the renal vessels before and after treatment (P > 0.05). Color flow Doppler sonography appears to be a safe, noninvasive, and reliable means of evaluating kidneys subjected to extracorporeal shock wave lithotripsy.

Characterisation of renal masses with colour flow Doppler ultrasonography.

Colour flow Doppler ultrasonography (US) was used in 24 renal masses (12 benign, 12 malignant) to investigate the vascularity of the lesion and thus to characterise the mass. A colour flow Doppler US device (Toshiba 270 A) with a convex probe of 3.75 MHz was used. Colour flow and the peak-systolic Doppler shift frequency obtained from the renal mass were used to distinguish between benign and malignant lesions. Ten of 12 malignant lesions demonstrated colour flow together with Doppler signals greater than 2.5 kHz (mean 3.5 kHz). None of the 12 benign renal masses demonstrated colour flow or tumour signals. The majority of malignant lesions give rise to neovascularisation and so high frequency Doppler-shifted signals. Thus colour flow Doppler sonography is the most reliable means of differentiating benign from malignant renal masses.

Sialographic and ultrasonographic analyses of major salivary glands.

Seventy patients with major salivary gland diseases (60 parotis, 10 submandibular) were examined both with sialography and ultrasonography. The clinical and histopathological results were compared with radiological interpretations. Sialography was found to be 60% accurate for the diagnosis of salivary gland tumors and cysts and 63% for the diagnosis of inflammatory diseases. Ultrasonography diagnosed 95% of the tumors and 63% of inflammatory diseases accurately.

Acute spinal cord compression in hereditary multiple exostoses: case report.

A case of hereditary multiple exostoses with spinal cord compression by a costal exostosis is reported in a 12-year-old boy. Paraplegia is an unusual complication of hereditary multiple exostoses. In the patient with spinal cord signs, the offending exostoses should be defined with appropriate roentgenograms and myelographic and CT scanning investigations.

Sonographic findings in ectopic cervical thymus in an infant.

We present the case of a 28-day-old boy with ectopic thymic tissue in the neck. Sonography showed a well-delineated, homogeneous lesion in the left submandibular region that was isoechoic relative to the mediastinal thymus. The lesion had angular margins and lacked a mass effect despite its large size (3 x 2 cm). On power Doppler imaging, arteries with an intermediate-impedance flow pattern were seen entering the lesion from the inferior and lateral margins. On both T1- and T2-weighted MR images, the lesion was isointense relative to the mediastinal thymus and had higher and lower signal intensity than muscle and fat, respectively. A sonographically guided needle biopsy confirmed the diagnosis of ectopic thymus.

Retrorectal dermoid cyst in a male adult: case report.

We describe a 48-year-old male patient who presented with rectal fullness and pain. Magnetic resonance imaging (MRI) and computed tomographic studies revealed a noncalcified, unilocular, cystic mass lesion with well-defined borders. On MRI nondependent fat spheres were detected inside the cyst. The same pattern has been described in dermoid cyst of the ovary. We suggest that this MRI pattern is specific to dermoid cysts.

Duodenal angiodysplasia: MR angiographic evaluation.

We describe a patient in whom endoscopy failed to determine the origin of gastrointestinal bleeding, and magnetic resonance angiography showed dilated inferior pancreaticoduodenal veins that were considered indirect signs of the duodenal angiodysplasia. Incidentally, a connection between the inferior vena cava and the inferior pancreaticoduodenal veins were also noted. Repeat endoscopy and catheter angiography confirmed the presence of the angiodysplasias.

MR peritoneography in complications of continuous ambulatory peritoneal dialysis.

BACKGROUND: We evaluated the usefulness of magnetic resonance (MR) peritoneography for the examination of complications from continuous ambulatory peritoneal dialysis (CAPD). METHODS: MR peritoneography was performed in 20 patients who had suspected CAPD-related complications. Patients who had active peritonitis were not included in the study. Before MR imaging, a mixture of 2000 mL of peritoneal dialysis solution and 20 mL of gadopentetate dimeglumine was instilled into the peritoneal cavity. MR imaging was performed on a 1.0-Tesla system using a body coil. Axial, coronal, and sagittal fat-saturated fast spoiled gradient echo (repetition/echo times, 100/6.3 ms; flip angle, 90 degrees), axial fat-saturated T2-weighted fast spin echo (repetition/echo times, 6000/107 ms), and coronal fat-saturated T2-weighted fast spin echo (repetition/echo times, 3000/96.2 ms) images of the abdomen and the pelvis were obtained. After drain-age, triplanar fat-saturated fast spoiled gradient echo images were repeated with the same parameters. RESULTS: Homogeneous distribution of the dialysate in the peritoneal cavity was detected in 18 patients (90%). In 12 patients (60%), fluid leaks were discovered peripheral to the exit site, tunnel, and site of peritoneal entrance of the catheter. Fluid leaks through the abdominal wall in five patients (25%), retroperitoneum in one patient (5%), and a previous operation site in one patient (5%) were demonstrated. No abnormal finding was detected in three patients (15%) who had clinically suspected complications, whereas a dialysate leak was found in two patients (10%) who had no significant finding at physical examination. CONCLUSIONS: MR peritoneography provides detailed information about the anatomic distribution of dialysate leaks in patients treated with CAPD and poses no risks associated with ionizing radiation and nephrotoxic contrast medium.

Radiographic, CT, and MRI findings in primary pulmonary angiosarcoma.

We report a rare instance of primary pulmonary angiosarcoma presenting as a large solitary mass in the left upper lobe with mediastinal invasion. In particular, we emphasize the magnetic resonance (MR) imaging features, which included a markedly heterogeneous pattern consisting of hyperintense areas scattered throughout a background of intermediate signal intensity, rendering the lesion a cauliflower-like appearance especially on T2-weighted images. Being unreported so far in primary pulmonary angiosarcomas, these distinct MR imaging findings may be helpful in the differentiation of these neoplasms from lung cancers.

Gadolinium-enhanced three-dimensional MR angiography in jugular phlebectasia and aneurysm.

True jugular vein aneurysms and phlebectasias are very rare abnormalities, which are usually discovered in children. Though their ultrasonographic features have been well described, magnetic resonance angiographic appearance of these lesions has not been reported in the radiology literature. We describe gadolinium-enhanced MR angiographic findings in two patients with internal jugular vein phlebectasia and one patient with anterior jugular vein aneurysm. The jugular venous system and its abnormalities were successfully depicted in the coronal plane, much like the images of coronal angiography.
