Measurement of noradrenaline, dopamine and serotonin contents in follicular fluid of human graafian follicles after superovulation treatment.

We measured the noradrenaline (NA), serotonin (5-HT) and dopamine (DA) contents of 47 normally maturated and 16 cystically degenerated follicular fluid samples obtained from patients involved in the in vitro fertilization and gamete transfer program. The patients were given human menopausal gonadotropin (HMG), as a superovulation treatment, and 7,500 IU human chorionic gonadotropin (HCG) to induce ovulation 34-36 h prior to the follicular puncture done by laparoscope. The NA content of the normally developed follicles was 11.4 + 8.4 micrograms/100 ml on average. For cystically degenerated follicles, the following data were obtained: 1.1 + 0.7 micrograms/100 ml (p less than 0.001). 5-HT and DA contents in the preovulatory follicles are 14.3 +/- 8.9 and 19.3 +/- 8.2 micrograms/100 ml, respectively; at the same time, 5-HT and DA contents in the cystically degenerated follicles were 12.2 +/- 6.2 and 12.7 +/- 6.8 micrograms/100 ml, respectively. They suggest that the higher amount of NA in the follicular fluid might play an important role in the mechanism of ovulation, the regulation of postovulatory tubal motility and the release of progesterone from granulosa cells.

Comparative study of enzyme-linked immunosorbent assay and radioimmunoassay techniques in determining serum placental protein 14 levels in gynecologic patients.

A comparative analysis of radioimmunoassay (RIA) and enzyme-linked immunosorbent assay (ELISA) techniques in determining placental protein 14 (PP14) levels in gynecologic patients was performed. Serum PP14 levels were assessed in the patients in the last period of gestation, during menopause and in healthy nonpregnant controls. The possible discrepancy in PP14 levels was also examined in gynecologic patients with benign and malignant tumors. Both RIA and ELISA techniques proved to be sufficiently sensitive to measure PP14 concentration.

Placental protein 4 as a possible tumor marker in ovarian tumors.

Placental protein 4 (PP-4), a recently characterized glycoprotein from human placenta, was studied using a specific double-antibody radioimmunoassay in sera of 130 volunteers, 76 ovarian tumor patients and in ovarian tumor cyst fluid and ascites of 21 patients. Elevated levels (greater than 3 micrograms/l) were found in 45 of 52 ovarian cancer patients (86.5%). PP-4 levels correlated significantly with staging. 31 patients with malignant ovarian tumor were monitored on 2-9 occasions during 5-82 weeks. Rising or falling levels of PP-4 correlated with progression or regression of disease in 25 of 31 instances (80.6%). Elevated levels were found in 10 of 24 benign and borderline ovarian tumors. Elevated PP-4 level does not indicate malignancy in each case. PP-4 can be regarded as tumor-associated antigen and an tumor marker in oncological practice.

Red cell deformability alterations in normal late pregnancy: possible role of plasma components.

Red cell deformability of 30 nonpregnant volunteers and 20 normal pregnant women in the 3rd trimester was assessed by determination of filterability of red cells suspended either in autologous plasma or in buffer by St. George's filtrometer method. Total red cell deformability was decreased in normal pregnancy [transit time (Tc) = 7.79 +/- 0.86 vs. 6.99 +/- 0.65, p less than 0.01] as compared to nonpregnant women when erythrocytes were studied in buffer suspensions, while the number of profoundly rigid red cells was lower in pregnant patients [clogging particles (CP) = 0.864 +/- 0.225 vs. 1.103 +/- 0.246, p less than 0.01]. Total red cell deformability was no longer reduced in pregnant women when erythrocytes were suspended in autologous plasma (Tc = 7.25 +/- 0.66 vs. 7.13 +/- 0.69, p = NS) while the number of rigid erythrocytes was still lower (CP = 0.802 +/- 0.157 vs. 1.055 +/- 0.210, p less than 0.01). Our data suggest that by the end of normal pregnancy, red cell deformability decreases with a consequent accelerated turnover and a fast elimination of very poorly deformable erythrocytes; plasma alterations in late pregnancy partly counteract the intrinsic loss of deformability of red blood cells.

The influence of metoclopramide on the composition of human breast milk.

UNLABELLED: The breast milk prolactin (PRL) has been claimed to play a role in the control of electrolyte composition of the milk. Since metoclopramide has been shown to increase milk production in humans, we have made an attempt to investigate the production, the PRL and sodium concentrations in milk with (group I) and without (group II) maternal metoclopramide treatment (5 days, 30 mg/day). Both groups consisted of 11 mothers and their full-term newborn infants. The daily milk production was significantly higher in the treated group (276.4 +/- 36.6 vs 150.9 +/- 25.3 ml/day, p less than 0.01). The PRL measured by RIA was similar in the milk samples of the metoclopramide treated and control groups (80.5 +/- 17.7 vs 90.7 +/- 27.3 ng/ml). The sodium concentration in the milk of mothers taking metoclopramide was 22.1 +/- 1.6 mmol/l and 24.3 +/- 3.2 mmol/l in the control group (p = 0.59). On the 5th postnatal day the plasma PRL of the newborns of mothers treated with metoclopramide does not differ from the values of the control babies (29.8 +/- 2.6 vs 30.7 +/- 2.4 ng/ml) indicating that the amount of metoclopramide transferred into the milk has no apparent influence on the hypothalamo-hypophyseal axis of the neonate. IN CONCLUSION: the maternal metoclopramide treatment augments the milk production without having any effect on the PRL and sodium concentration of human "mature" milk.

Initial experience with placental protein 4 (PP-4) as tumor marker in cervical and endometrial cancer.

PP-4, a recently characterized glycoprotein from human placenta was studied using a specific double-antibody radioimmunoassay in sera of 130 volunteers, 74 cervical cancer patients and 43 endometrial cancer patients. Elevated levels (greater than 3 micrograms/l) were found in 35 (47.3%) cervical cancer patients and in 18 (41.9%) endometrial cancer patients. Degree of elevation were not correlated with clinical stage, histology, and histological degree of differentiation. 36 patients with cervical cancer and 20 patients with endometrial cancer were monitored on two to seven occasions during four to 50 weeks. Rising, remaining unchanged of falling levels of PP-4 correlated with progression, stabilization or regression of disease 55.5% in patients with cervical and 65.0% in patients with endometrial cancer. During and some months after external telecobalt irradiation therapy wide range of PP-4 levels were observed in some patients. The study suggest that PP-4 can be regarded as a tumor associated protein which most likely can serve as tumor marker in cervical and endometrial cancer.

Elevated sera levels of SP-1 induced by chemotherapy in patients with metastatic gestational trophoblastic diseases.

6 patients with invasive mole and 5 patients with choriocarcinoma were treated from 1983 till 1986. Serum samples were analyzed by simultaneous determining of pregnancy-specific beta-1-glycoprotein (SP-1) using enzyme-linked immunosorbent assay (ELISA) and beta subunit of human chorionic gonadotropin (beta-hCG) using Serono radioimmunoassay kit. In 2 patients with metastatic gestational trophoblastic disease (MGTD) SP-1 peaks were found during chemotherapy. In patients with MGTD with normalized beta-hCG levels a repeated, temporary elevation of isolated SP-1 levels was observed within some months following chemotherapy. After the last isolated peak of SP-1 the pulmonary metastases disappeared. This phenomenon was interpreted as a consequence of the oncolytic process in the affected tissue. In 1 patient with nonmetastatic choriocarcinoma SP-1 ELISA pseudoreaction was found. To recognize these pseudoreactions, a control plate with nonimmunized rabbit IgG was used, simultaneously with SP-1 determinations.

[Placental protein 12 (PP 12) in cyst and peritoneal fluids and serum samples of patients with ovarian cysts and cystic ovarian tumors].

Placental Protein 12 (PP 12) concentrations were measured by a radioimmunoassay in cyst and peritoneal fluids and in serum samples respectively of patients with tumor like conditions and benign and malignant cystic ovarian tumours. PP 12 can not recommended as a serum tumour marker in ovarian cancer. It must be proved the value of PP 12 in follow up of ovarian cancer with help of more extensive sample material. There are many cysts in tumour like conditions with measurable PP 12-concentrations.

Beta lactoglobulin homologue placental protein 14 (PP14) in serum of patients with trophoblastic disease and non-trophoblastic gynecologic malignancy.

Serum levels of beta lactoglobulin homologue placental protein 14 (PP14) were measured by a sensitive radioimmunoassay in various trophoblastic diseases and non-trophoblastic gynecologic malignancies. While trace amounts of protein were detected in sera of non-pregnant subjects (22.3 +/- 13.7 micrograms/l), during first half of normal pregnancy a dramatic rise of serum-PP14 levels was demonstrable with a peak-value at 7th-10th week of gestation, followed by a decline thereafter. Serial determinations of PP14 have been performed in 31 patients with trophoblastic tumour (20 hydatidiform moles, 4 invasive moles and 7 choriocarcinomas). In patients with hydatidiform moles and invasive moles (273.5 +/- 106.5 micrograms/l and 162.2 +/- 109.6 micrograms/l) respective values before therapy were much exceeding the non-pregnant controls. After therapy there was a rapid decline of the serum-PP14 levels within two weeks. In patients with choriocarcinoma the PP14 values were moderately elevated (66.4 +/- 25.7 micrograms/l), and declined following the remission of disease. In 32 gynecological tumours (21 carcinomas of the cervix, 4 endometrial carcinomas, 5 ovarian carcinomas, 2 carcinomas of the vulva) the pretreatment levels were not different to normal controls.

Serum PP12, PP14, SP1 and hCG values in the 28 days after the LH-surge in patients who do and do not conceive after artificial insemination or in vitro fertilization.

Serum levels of PP12 (somatomedin binding protein), PP14 (beta lactoglobulin homologue), Schwangerschaftsprotein 1 (SP1) and of human chorionic gonadotropin (hCG) were simultaneously measured in patients being treated for infertility in the 28 days after the LH-surge. PP14 levels were similar in the 14 days after the LH-surge in the patients who conceived when compared with those who did not and a high PP14 level was only indicative of pregnancy at 21 days after the LH-surge. hCG and SP1 levels behaved similarly in pregnant subjects. PP12 levels did not change significantly in the 28-days after the LH-surge.

Occurrence of CA 125 and CA 19-9 tumor-associated antigens in sera of patients with gynecologic, trophoblastic, and colorectal tumors.

The present study was undertaken to test the parallel detectability of ovarian cancer antigen CA 125 and gastrointestinal cancer antigen CA 19-9 in the sera of patients with malignant ovarian tumors, benign ovarian tumors, endometrial cancers, cervical cancers, colorectal cancers, and trophoblastic tumors and in early 1st-trimester pregnant as well as in healthy nonpregnant controls. In all kinds of gynecologic and colorectal tumors raised concentrations of both antigens were found with the exception of malignant nonepithelial ovarian tumors where neither of the antigens showed positive reaction. The most positive cases were found in the group with epithelial ovarian cancers. Of the two antigens CA 125 was the more responsive. No positive cases were found with either of the antigens in nonpregnant healthy controls or in patients with benign ovarian tumors. The parallel determination of the two antigens gives us a better opportunity to recognize pelvic tumors and further may enable us to distinguish ovarian and colorectal tumors.

Increased prolactin response to TRH in polycystic ovary syndrome with low basal prolactin values.

An intravenous TRH loading test with 200 micrograms TRH was carried out in 9 hypoprolactinemic (serum prolactin less than 100 mIU/ml) and 6 normoprolactinemic PCO patients and 6 normal subjects. Basal and stimulated prolactin and TSH levels were measured. The latter were within normal values. Prolactin responses to TRH were exaggerated in PCO patients, irrespective of the basal prolactin values. According to literary data, these results indicate that increased prolactin response to TRH in PCO is independent of the basal prolactin values and suggest disturbed tuberoinfundibular dopaminergic function.

Two different methods of prostaglandin administration for cervix priming before interruption of the first trimester pregnancy.

Priming of uterine cervix with PGF2 alpha has been applied to 253 primigravidae before interruption of first trimester pregnancy. Of them, 140 patients were treated with intracervical PGF2 alpha, while 113 other patients with extraamniotic PGF2 alpha. Spontaneous abortion or complete dilatation occurred in 65% of cases in the first group and in 68% in the second one. Side effects were observed less frequently with intracervical application but no serious complication occurred in either of the groups. Intracervical application of PGF2 alpha gel is recommended because of its effectiveness and low rate of side-effects.

Experiences with the treatment of clinical hyperprolactinaemia.

67 patients with high serum prolactin values were treated by bromocriptine (Parlodel). Parlodel was administered in 56 cases because of functional infertility, and 27 of them became pregnant. The previously given traditional ovulation inductors were ineffective in many cases. According to the authors' experience Parlodel can be favourably combined with clomiphene citrate. On the other hand, Parlodel treatment could be effectively introduced in cases with only moderately high prolactin values (400-800 mIU/ml) to restore fertility. Parlodel can be effective also in the treatment of polycystic ovarian syndrome.

Plasma prolactin levels in full-term newborn infants with idiopathic edema: response to furosemide.

In an attempt to explore the possible role of prolactin (PRL) in the control of neonatal electrolyte homeostasis, this study has been carried out to compare plasma electrolyte concentrations, urine volume and urinary electrolyte excretion as well as plasma PRL levels in healthy full-term neonates with idiopathic edema prior to and after furosemide treatment. No differences in plasma sodium and potassium were demonstrated, edematous neonates, however, had less urine volume and sodium excretion than neonates without edema. Plasma PRL proved to be significantly higher in the edematous group (11.0 +/- 1.9 vs. 4.2 +/- 3.1 U/l, p less than 0.01) but it remained unaltered by furosemide challenge (8.5 +/- 1.5 U/l) in spite of the marked elevation of urine flow and sodium excretion. It is concluded that PRL may be involved in the control of the volume and composition of the body fluids in the neonate but further studies are needed to define the effect of changes in body composition on the neonatal PRL secretion.

Effect of metoclopramide treatment on thyrotropin and prolactin levels in sick neonates.

The present study was carried out to define whether dopaminergic mechanisms contribute to the regulation of thyrotropin (TSH) and prolactin PRL release in the immediate neonatal period. 14 full-term neonates with a mean birth weight of 3,240 g and a mean gestational age of 39.1 weeks were administered metoclopramide (MTC), a specific dopamine antagonist, in a dose of 0.1 mg/kg/day to treat delayed gastric emptying, regurgitation and abdominal distension. Prior to and after 3-day MTC administration, blood samples were taken to determine serum TSH and PRL levels using a radioimmunoassay method. It has been demonstrated that in response to MTC administration PRL increased significantly from 4,010 +/- 383 to 5,478 +/- 441 mU/l (p less than 0.01), while TSH showed only a tendency to rise independent of the pretreatment hormone levels (from 2.85 +/- 0.44 to 3.06 +/- 0.38 mU/L. In healthy control infants and in those infants with similar functional gastrointestinal disturbances who were treated without MTC, serum PRL levels fell significantly from days 3-4 to days 6-7, serum TSH, triiodothyronine and free thyroxine, however, remained unaltered. It is concluded that dopaminergic inhibitory mechanism may be involved in the control of pituitary PRL and TSH release already in the neonatal period.
