Serotonin- and catecholamine-related substances in the brain of ornithine transcarbamylase-deficient Sparse-fur mice in the hyperammonemic state: comparison of two procedures for obtaining brain extract, decapitation and microwave irradiation.

Increased flux through the 5-HT pathway during hyperammonemia was indicated by a significant positive correlation between Gln concentration and the ratio of 5-HIAA to 5-HT in the brain of spf mice taken after microwave irradiation to prevent postmortem changes in metabolites. There were no significant changes in the catecholamine pathway in the brain during hyperammonemia despite the increases in Try and Phe.

Sodium chloride sensitivity, intracellular sodium concentration in erythrocytes and lymphocytes, and renin profile in essential hypertension.

In order to clarify the possible relationship between changes in blood pressure after salt loading, membrane sodium transport and renin profile, 19 patients with essential hypertension (8 patients with low renin hypertension and 11 patients with normal renin hypertension), admitted to our hospital, were studied. We also examined the correlation of changes in intracellular sodium concentration after salt loading between erythrocytes and lymphocytes. After a control period of one week, all subjects were placed on a low salt intake for one week followed by one week of a high salt intake. Percent increases in mean blood pressure and intracellular sodium concentration in erythrocytes and in lymphocytes after salt loading were greater in low renin hypertensive patients than in normal renin hypertensives. Percent changes in intracellular sodium concentration in erythrocytes inversely correlated with those in ouabain sensitive sodium efflux rate constant and positively correlated with those in intracellular sodium concentration in lymphocytes. These results suggest that an increase in sodium chloride sensitivity of blood pressure in patients with low renin hypertension may be due to the inhibition of Na+-K+ pump in vascular smooth muscle cell membrane.

Eimeria capricornis n.sp., E. nihonis n.sp., E. naganoensis n.sp., and E. kamoshika n.sp. (Protozoa: Eimeriidae) from the Japanese serow, Capricornis crispus.

Eimeria capricornis n.sp., E. nihonis n.sp., E. naganoensis n.sp. and E. kamoshika n.sp. were detected from the Japanese serow, Capricornis crispus. The oocysts of these species were ovoid to ellipsoid, measuring 48.02 +/- 0.53 x 33.93 +/- 0.35 microns, 29.73 +/- 0.45 x 21.01 +/- 0.29 microns, 20.04 +/- 0.05 x 15.77 +/- 0.04 microns and 30.02 +/- 4.24 x 14.58 +/- 2.03 micron, respectively. In all the species the micropyle was observed, but the micropylar cap and oocyst residuum were not present. The polar granules were observed in the oocysts of E. capricornis. The sporocysts of the above species were spherical to elongate ovoid, measuring 15-23 x 8-13 microns, 14-16 x 7-10 microns, 11-13 x 6-8 microns and 8-10 x 6-7 microns, and the sporulation time was 6, 3, 3 and 6 days, respectively. The sporocyst residuum and refractile body were seen in all the species. In the sporocysts of E. naganoensis and E. kamoshika tiny Stieda body was seen, but not in the other two species. These four Eimeria species were not infective to goats.

[Clinical evaluation of second-look operations in colorectal cancer].

Second look operations in colorectal cancer after curative resection were undertaken for liver metastasis or local recurrence. In our 247 curative cases, 35 were operated for liver metastasis and 18 for local recurrence. In 35 cases of liver metastasis, 25 had local disease amenable to resection and one had palliative resection. Among 21 pelvic recurrences, 11 had curative re-resection and 2 had palliative resection. The survivors generally experienced relief from various clinical distress, and most of those previously employed could return to work. The actual 3-year survival rate is 51.9% in the group of resected liver metastasis, and 9 of 13 cases resected for local recurrence are alive without evidence of re-recurrence. In conclusion, selected patients with recurrence may recover and resume ordinary living with the second look operation.

[Studies of diagnosis of rectal cancer using MRI, CT and intrarectal ultrasonography].

Examination using MRI, CT and Intrarectal Ultrasonography (US) were performed in 186 patients with primary rectal cancer and 127 postoperative cases, in order to diagnose the depth of invasion, lymph node metastases and local recurrence. In the diagnosis of depth of invasion, intrarectal US was superior to MRI and CT for detailed diagnosis, and MRI was the best examination modality for detecting infiltration of other organs. Also, these were available for the diagnosis of lymph node metastases, and intrarectal US was superior to MRI and CT for detecting small lymph nodes. In the detection of local recurrence, MRI, CT guided biopsy and intrarectal US were useful, especially MRI was the best examination for the decision of re-resection.

Factors determining sodium chloride sensitivity of patients with essential hypertension: evaluation by multivariate analysis.

Factors which determine sodium chloride sensitivity, defined as the difference between the mean blood pressure after 1 week of a low sodium chloride diet (3 g/day) and that after 1 week of a high sodium chloride diet (20 g/day), were studied in 60 inpatients with essential hypertension using a multivariate analysis. The sodium chloride sensitivity was independently correlated with the change in erythrocyte sodium concentration (r = 0.47) and with the change in plasma renin activity (r = 0.29); but it was not related to basal blood pressure, the change in plasma volume of the change in plasma norepinephrine concentration. These data suggest that both intracellular sodium accumulation and inadequate suppression of the renin-angiotensin system may be independently involved in the elevation of blood pressure after sodium chloride loading. We could not find the independent importance of volume retention, hyperadrenergic activity or basal blood pressure in the sodium chloride sensitivity.

[A case of pericardial cyst associated with atrial septal defect].

A 51-year-old woman with an abnormal shadow in the right upper mediastinum and systolic heart murmur was diagnosed as mediastinal cyst and atrial septal defect (ASD). Before the operation, the cyst was suspected as bronchogenic cyst or cystic thymoma based on the findings by CT and NMR. Both cystectomy and closure of ASD was successfully performed and the cyst was diagnosed as pericardial cyst by pathological examination. A brief of literature on the pericardial cyst is given.

Biochemical analysis of decreased ornithine transport activity in the liver mitochondria from patients with hyperornithinemia, hyperammonemia and homocitrullinuria.

Hyperornithinemia, hyperammonemia and homocitrullinuria (HHH disorder) is an inherited metabolic disorder which shows peculiar amino acid changes in the serum and urine. The primary defect is considered to be the transport of ornithine across the mitochondrial membrane, but there is no direct evidence for this so far. We have analyzed ornithine transport activities in the liver mitochondria from three patients with HHH disorder. In coupled liver mitochondria we demonstrated low activities of citrulline synthesis and low rates of ornithine uptake. However, there were no abnormalities in carbamoyl-phosphate synthetase activity, ornithine carbamoyltransferase activity, N-acetylglutamate levels or O2 uptake with succinate. We also performed a kinetic study of citrulline synthesis as a function of ornithine concentration. We found increased Km values for ornithine and varied Vmax values of citrulline synthesis, which suggested the presence of a mutant transport protein. From these results we conclude that the defect of hyperornithinemia, hyperammonemia and homocitrullinuria lies in the transport of ornithine across the mitochondrial membrane.

Anion conductances of the giant axon of squid Sepioteuthis.

Anion conductances of giant axons of squid, Sepioteuthis, were measured. The axons were internally perfused with a 100-mM tetraethylammonium-phosphate solution and immersed in a 100-mM Ca-salt solution (or Mg-salt solution) containing 0.3 microns tetrodotoxin. The external anion composition was changed. The membrane currents had a large amount of outward rectification due to anion influx across Cl- channels of the membrane (Inoue, 1985). The amount of outward rectification depended on the species of anion used and was strongly influenced by temperature and internal pH. In contrast to the anion conductances themselves, the conductance relative to Cl- (gA/gCl) was found to be quite stable against changes in the membrane potential, temperature, and pH. It is therefore suggested that each gA/gCl is an intrinsic quantity of the Cl- channel of the squid axon membrane. The sequence and values of gA/gCl obtained in this study were NO3- (1.80) greater than I- (1.40) greater than Br- (1.07) greater than Cl- (1.00) greater than MeSO3- (0.46) greater than H2PO2- (0.33) greater than CH3COO- (0.29) greater than SO4(2-) (0.06).

Clinical, biochemical and ultrastructural study on the pathogenesis of hyperornithinemia-hyperammonemia-homocitrullinuria syndrome.

A 10-year-old boy with the hyperornithinemia, hyperammonemia and homocitrullinuria (HHH) syndrome is described. With dietary restriction of protein intake and supplementary administration of L-ornithine and L-arginine, the high concentration of ammonia decreased and the clinical signs of truncal ataxia and lethargy improved. A deficiency of ornithine transport into liver mitochondria was demonstrated biochemically, and glycogen granules and smooth surface endoplasmic reticulum were increased, but mitochondria showed normal construction ultrastructurally. Cranial computed tomography (CT) showed diffuse white matter low density and cerebellar vermis atrophy. The impairment of ornithine transport and energy production in the central nervous system may be related to the cranial CT findings and neurological signs.

The heterogeneous distribution of argininosuccinate synthetase in the liver of type II citrullinemic patients. Its specificity and possible clinical implications.

The authors analyzed the heterogeneous distribution of hepatic argininosuccinate synthetase of type II citrullinemia in reference to its specificity and clinical implications. The low content of the enzyme in the liver of type II citrullinemic patients is associated with two kinds of the enzyme distribution that can be visualized by means of an immunohistochemical method (Saheki and colleagues. Biomed Res 1983;4:235-238). Among the 25 cases of type II citrullinemia examined, 11 exhibited homogeneous distribution of the enzyme, as in the control livers. On the other hand, 14 presented the clustered distribution, in which the hepatocytes stained positively with antisera to argininosuccinate synthetase formed a cluster among the poorly stained cells. No clustered distribution of the enzyme was present in the liver of control patients either with or without liver diseases. No clustered distribution of arginase and aldolase B was observed even in the liver of type II citrillinemic patients. These results suggest that clustered distribution is specific to argininosuccinate synthetase in the liver of type II citrullinemic patients. From considerations concerning the heterogeneous distribution of the enzyme and certain clinical parameters as well, the authors suggest that the clustered type in type II citrullinemia has a less favorable prognosis with regard to fatality.

Relationship of cellular calcium to salt sensitivity in patients with essential hypertension.

The significance of cellular calcium metabolism and systemic calcium balance in sodium chloride sensitivity was studied in 16 patients with essential hypertension and in 13 normotensive subjects. With changes in sodium chloride intake from 3 to 20 g/day, mean blood pressure, lymphocyte [Ca2+]i and the acute hypotensive response to nifedipine were increased in the hypertensive patients, but not in the normotensive subjects. Serum calcium concentration was decreased and urinary calcium excretion was increased in both groups. In the hypertensive patients, elevation of mean blood pressure was positively correlated with the increase in lymphocyte [Ca2+]i and with the enhancement of the hypotensive response to nifedipine, but it was not related to the change in serum or urinary calcium. These results suggest that enhancement of cellular-calcium-dependent vasoconstriction may lead to increased blood pressure following sodium chloride loading in patients with essential hypertension.
