PURPOSE: This study evaluates the efficacy of endoscopic laser-assisted dacryocystorhinostomy (LADCR) in the treatment of epiphora and chronic dacryocystitis. PATIENTS AND METHODS: Treatment results were prospectively reviewed for 17 patients undergoing 19 LADCR procedures of which 12 were primary and 7 were revision cases. Silastic intubation tubing was removed after surgery at a mean of 76.5 days in the primary cases and 184 days in the revision cases. Patients were followed-up for a mean of 15.6 months (range, 5 to 25 months). RESULTS: LADCR was successful in 100% of primary cases and 60% of revision procedures. The only complications were 2 conjunctival fistula, which closed without sequela. CONCLUSION: The efficacy of primary LADCR is comparable to reported external approaches. The success of revision LADCR is less favorable, yet the number of cases reported here is too small to reach a conclusion.
The pediatric pneumogram (PPG) is a technique that permits continuous, magnetic-tape recording of the respiratory pattern of infants over an extended period of time. The technique, introduced in 1975, uses an impedance method. As a diagnostic tool, PPG can objectively quantify rate of respiration, the respiratory rhythm, the relative amplitude of respiratory activity, and the incidence of apnea, as well as any resultant bradycardia. Investigations are presently under way using PPG to study the relationship between apnea and sudden infant death syndrome.
The Holter superimposition method was compared with the Argus/H computer system and the Stein/Peterson frozen display technique in the analysis of 94 long-term ambulatory ECG recordings using technical staff experienced with each analysis procedure. The use of the Holter superimposition method resulted in a failure of detection of 33-66 percent of ventricular and supraventricular arrhythmias as compared to the two other computer-based playback systems. The Argus/H system and the frozen display method of Stein and Peterson were comparable with respect to ventricular arrhythmia detection, but the frozen display method was superior to the Argus/H system with respect to the detection of supraventricular arrhythmias.
A 24-hour, 3.5-lb., fully automatic, indirect blood pressure monitoring device has been developed that permits up to 40 programmed and manual readings each hour during normal daily activities. A study comparing the automatic ambulatory device with standard conventional sphygmomanometry revealed mean absolute differences of blood pressure measurement of less than 5 mmHg and mean pairwise differences of 2 mmHg for both systolic and diastolic pressures.
