[Urinary microflora and the status of local immunity in children with cystitis].

Study of the enzymatic and adhesive properties of E. coli isolated from the urine of children with cystitis has demonstrated that P-fimbriae, DNAase- and phosphatase-positive strains with pronounced hemolyzing and metabolic properties mostly occurred in children with associated cystitis and chronic obstructive pyelonephritis. In children suffering from cystitis, the changes on the part of local immunity manifested by the decrease of the content of SIgG in the urine and by the lack of SIgA1 and SIgA2. In children with a disease standing up to 3 years, the synthesis of IgA turned out dominant in the mucous membrane of the neck of the urinary bladder and in the trigone of the bladder, with IgG and IgM output being less intensive. The authors have proved the efficacy of the treatment with tomicide instilled into the urinary bladder of children suffering from cystitis.

Laboratory criteria in the diagnostics of intestinal infections of staphylococcal etiology in children.

Faecal samples were investigated from 948 children hospitalized with the diagnose of acute intestinal infection. The authors studied the isolation and properties of a broad spectrum of microorganisms from children's faeces: Salmonella, Shigella, E. coli, S. aureus, rotaviruses, Klebsiella, Morganella, Proteus. As a result, laboratory criteria were formulated of diagnosing in children intestinal infection of staphylococcal etiology.

[Characteristics of the serological passage of Escherichia isolated from children and adults with acute intestinal infections].

The serological picture of Escherichia (5,910 strains), isolated from 1,430 inpatients (486 adults and 944 children) with acute intestinal infections by means of new diagnostic preparations (Escherichia rapid agglutinating O- and H-systems), was studied. In 15% of the adults and 26-28% of the children no Escherichia were detected. The serological picture of Escherichia proved to comprise 143 O-groups and 334 serovars; about 50% of the strains belonged to 11 prevailing O-groups: O1, O2, O4, O6, O7, O8, O9, O16, O21, O75, O85. The serological picture in the adults was more variegated than that in the children: from most of the patients (77.2%) Escherichia were isolated as a mixture of 2-9 serovars. The isolation rate of Escherichia monocultures and the incidence of Escherichia belonging to different O-groups were the same in patients of different ages, with the exception of groups O4, O6, O26, O55 and O111 which were more frequent in young children.

[Enterotoxigenic Escherichia in patients with acute intestinal infection].

Out of 4,082 Escherichia strains isolated from 989 hospitalized patients with acute enteric infections 2,979 strains belonged to 139 O-groups and the remaining strains could not by typed with O-sera (O1-O164) or were in the serological R-form. 19.3% of the strains isolated from 354 patients showed a positive reaction for thermolabile enterotoxin (TLET) determined in the radioimmunoassay. Escherichia with pronounced reaction for TLET were isolated from 5.5% of the patients. The serological picture of Escherichia with a positive reaction for TLET comprised 78 O-groups, showed practically no variations in children of different age and adult patients and was related to the intensity of reaction for TLET. The number of enterotoxigenic Escherichia greatly varied within individual O-groups: from 3.1 +/- 3.63% (O11) to 100 +/- 9.81% (O114) of the total number of known strains in these O-groups. In most O-groups the number of enterotoxigenic strains varied from 10% to 50%. The most widely spread serovars of etiologically important enterotoxigenic Escherichia belonged to 24 O-groups and were often isolated as monocultures. The capacity for producing TLET (and, probably, some other antigenically related substances) in widely spread among Escherichia; still, the determination of enterotoxicity is a necessary, but not sufficient criterion of their etiological importance in acute enteric infections.

[Use of factor-H sera for the differentiation of the flagellar antigens of Escherichia coli serological variants].

The use of factor sera permitting the differentiation of the variants, described in earlier. works, among the flagellar antigens of E. coli, formally denoted as H2, H4, H7, H10 and H34 in accordance with their official nomenclature, has made it possible to reveal that each of these variants is widely spread among E. coli and occurs in bacteria of different O-groups. Besides, this study has shown the possibility of subdividing a number of formal H: O types into 2 or more serovars on the basis of differences in the factor composition of their antigens. The results obtained in this study suggest that in the process of the evolution of E. coli H-antigen variants differing in their factor composition have been formed as independent varieties; therefore, these variants do not reflect the features characteristic of individual strains, but constitute one of the diagnostic signs of serological classification, i. e. the differentiation of the species into various serovars.

[Development of Escherichia diagnostic O-test systems].

The principles of the rational development of new diagnostic preparations, Escherichia agglutinating O-test systems consisting of polyvalent reagents (reapols), were worked out, and two O-test systems, "small" and "large", were made up on the basis of these reagents (for the identification of 22 and 151 Escherichia serovars, respectively). The agglutinogenicity of Escherichia O-antigens was studied and found to be poorly pronounced in 40% of cases. The practical scheme for the identification of Escherichia O-antigens (key to the O-test system) has been made; the scheme permitted the determination of 84.8% of Escherichia O-test strains, only 29.1% of the strains showing due positive reactions. Ways for improving these preparations are presented.

[Interrelation of the enterotoxigenic properties and the antigenic structure of Escherichia].

The enzymatic signs and serological characteristics of Escherichia enterotoxigenic strains isolated from patients with acute intestinal diseases and from healthy persons were studied. The cultures were subdivided into 24 enzymatic variants and classified with 48 serogroups and 61 serovars. The enterotoxigenic properties of the strains were compared with their serological characteristics and enzymatic signs. The strains, isolated from different persons and classified with the same serovar, belonged to the same variant with respect to the type of enterotoxin they produced (only thermostable enterotoxin, only thermolabile enterotoxin, or both), were similar in the degree of their toxigenicity and belonged, as a rule, to the same enzymatic variant. The data on the presence of manifest interrelation between the enteropathogenicity of Escherichia and their structure, as well as on the stability of the enterotoxigenic properties of these organisms, indicate that in acute intestinal diseases the determination of Escherichia enterotoxigenic strains can be carried out by common bacteriological techniques with the use of specific agglutinating sera.

[Serum resistance of Escherichia coli in chronic pyelonephritis. 1. Serum resistance in the human serum pool].

123 patients of the kidney department of the Clinic for Inner Medicine of Rostock University suffering from chronic pyelonephritis were taken into microbiological observation for between one and four years. 170 E. coli strains were bred from 59 patients with significant bacteriuria in the course of the disease and their serum resistence was determined with pooled human serum using Taylor's method. 78.24% of the strains examined were serum-sensitive, 11.18% intermediate and 10.59% serum-resistent. All strains were O-, K- and H-typed. 57.06% were successfully O-typed and were distributed over 40 O-serogroups. 24.12% were not typable and 18.82% were rough colonies. 86.50% of the resistent and intermediate strains strains were O-typable, 13.50% could not be typed. The significance of E. coli antigens (O, K, H) and serum resistence for the maintenance of a chronic infection is discussed.

[Partial composition of the O-antigens of O-group Escherichia].

The partial composition of O-antigens in Escherichia known as the causative agents of escherichioses localized in the intestine and other organs has been studied. Escherichia of groups 01, 02 and 03 are characterized by different partial composition of their O-antigens. This composition can be expressed by the formulae: 01a, 1b; 01a, 1b, 1c; 01a, 1b, 1c, 1d; 01a, 1e; 02a, 2b, 2c; 02a, 2b, 2c, 2d, 2e; 02a, 2b, 2d, 2e, 2g; 02a, 2c, 2e; 06a, 6b; 06a, 6c. No pronounced correlation between the partial composition of O-antigens within the O-group and the presence of different K- or H-antigens in the strain has been established. The scheme of adsorption has been developed and anti-Escherichia factor O-sera have been obtained. The use of these sera allows one to differentiate Escherichia strains, isolated from patients, within the O-groups according to the partial composition of their antigens, which is of both diagnostic and epidemiological importance.

[Serologic groups of O-antigens of E. coli isolated from urinary tract infections within the boundaries of the USSR and East Germany].

A total of 373 E. coli strains, isolated in urinary tract infections from children and adults in Rostock (GDR) and Moscow (USSR), were found to belong to 63 O-groups. E. coli isolated in Rostock belonged to 36 O-groups, and in Moscow to 53 O-groups. The occurrence of E. coli serogroups in the dominating O-groups (more than 2% of the total number of strains) in Rostock (O1, O2, O4, O6, O7, O8, O9, O15, O21, O25, O75, O102) differed from that in Moscow (O1, O2, O4, O6, O8, O9, O15, O16, O17, O18, O25, O28, O73, O75) in composition and frequency, in Rostock the predominant E. coli groups being O2, O4, O6, O7, O9, O21, O75, O102, while in Moscow O1, O8, O15, O16, O17, O18, O25, O28, O73. The partial O-antigen composition of E. coli strains belonging to the same O-groups, isolated in Rostock and Moscow, was different in groups O1, O2, O125.

[Evaluation of a complex method of identifying enteropathogenic Escherichia using O- and H-sera and a rational combination of biochemical tests].

A complex method for the identification of enteropathogenic Escherichia by means of adsorbed monogroup and factor O-sera, H-sera and a rational combination of biochemical tests (using triple sugar iron agar or Kligler iron agar, Simmons citrate agar, phenylalanine-, urea-, sodium malonate-containing media, indole broth, medium for mobility determination) was evaluated. The use of adsorbed monogroup O-sera in the slide agglutination test reduced the time of the study and increased its accuracy by 20.0 +/- 2.5% (6.9 +/- 2.5% to 52.4 +/- 11.1% for Escherichia of different O-groups) in comparison with the generally accepted method. The use factor O-sera and H-sera and H-sera revealed the circulation of the same O-groups belonging to different serovars and differing in partial composition a O-antigen (of the type ab - ac) and in H-antigens, which should be taken into consideration for epidemiological purposes. The combination of biochemical tests ensured correct determination of the genus of the organisms under study. The tested method is recommended for practical use.

[Highly specific agglutinating O- and OK-immunoglobulins for identification of Escherichia in the agglutination test on glass].

Escherichia agglutinating O- and OK-immunoglobulins of class G were obtained by the method of ion exchange chromatography; in studying with live and heated O- and K- cultures of the test strains from the International escherichia collection these immunoglobulins proved to be diagnostically highly specific and useful for the identification of escherichia strains in the express agglutination reaction on glass.
