[Ventricular arrhythmias and cardiac late potentials inpatients with acute coronary syndrome after reperfusion therapy].

The prognostic value of ventricular late potentials (VLP) and the character of rhythm disturbance were studied in 64 patients with acute coronary syndrome. 8 hours after system thrombolysis procedure VLP rate increased from 68.6% up to 85.7%. In all cases of reperfusion arrhythmia deterioration of signal-averaged ECG (SAECG) parameters and VLP were observed. Within the hospital treatment period VLP rate in the 1st group (patients receiving thrombolytic therapy) was lower than in the 2nd (no thrombolytic therapy). In the 1st group VLP was observed in 31.4% of the patients on the 10th day and in 11.4% by the end of hospital treatment; in the 2nd group--in 48.3% and 41.4% of the patients, respectively (p < 0.05). Administration of beta-adrenoblockers in both groups allowed improvement of SAECG parameters and cardiac rhythm variability.

[Hemodynamics, electrophysiological parameters of the heart and differential therapy of attacks of atrial fibrillation in patients with subclinical thyroid dysfunction].

Thirty patients with subclinical thyrotoxicosis and 32 with subclinical hypothyroidism with attacks of atrial fibrillation were investigated. At levels of thyroid-stimulating hormone below 0.1 or above 10.0 mIU/l average triiodothyronine and thyroxine levels in patients with subclinical thyrotoxicosis were 89.6 and 73.2%, respectively, higher than in patients with subclinical hypothyroidism. Some relationships were found between levels of thyroid hormones and hemodynamic and electrophysiological parameters of the heart. Differential therapy of attacks of atrial fibrillation was effective in 80 and 62.5% of patients with subclinical thyrotoxicosis or hypothyroidism, respectively.

[Anti-arrhythmic therapy: diagnostic possibilities of signal-averaged electrocardiography and heart rate variability].

Effects of monotherapy with class IC, II and III antiarrhythmic drugs on parameters of signal averaged (SA) ECG and heart rate variability were studied in 88 patients (mean age 45.6+/-7.8 years). Class IC drugs (ethacizine, disopyramide) caused worsening of qualitative parameters of SA ECG and appearance of ventricular late potentials. Therapy with beta-adrenoblockers, amiodarone and sotalol in patients with ventricular arrhythmias was associated with improvement of parameters of SA ECG, lowering of sympathetic and augmentation of parasympathetic activity without sings of arrhrythmogenic and negative inotropic effects. Combination of noninvasive diagnostic methods including SA ECG, temporal and spectral analysis of heart rate variability, Holter ECG monitoring can facilitate selection of appropriate antiarrhythmic therapy and control of its efficacy.

[Thyroid hormones effects on hemodynamic and electrophysiological indices of heart performance and efficiency of antiarrhythmic drugs in paroxysms of cardiac fibrillation in patients with coronary heart disease].

The article provides the results of thyrotropic hormones (TH) influence on spontaneous paroxysms of cardiac fibrillation (CFP), central hemodynamics, electrophysiology (EP) of the heart and effectiveness of antiarrhythmic drugs (AAD) in patients with coronary heart disease (CHD). It is shown that in CFP patients TH should be evaluated in sinus rhythm and in arrhythmia. Fluctuations in TH contribute to CFP. EP and hemodynamic effects of TH manifest with shorter effective refractory period of the left atrium and atrioventricular junction, disturbed diastolic function of the left ventricle. TH affect efficiency of AAD in CFP patients with CHD.

[Diagnostic value of late ventricular potentials in patients with painless myocardial ischemia].

The study of peculiarities of late ventricular potentials (LVPs) registration in patients with painless myocardial ischemia (PIMI) and painful myocardial ischemia (PMI) in unstable or stable running of ischemic heart disease (IHD) enrolled 113 patients (88 males and 25 females, mean age 52.3 +/- 6.8 years). Follow-up covered 20 +/- 3.2 months. Higher risk was associated with abnormal ECG readings, PlMI, duration of ischemic periods > 70 min for 24 hours, ST depression > 3 mm in heart rate < 100 b/m at the start of the ischemic episodes. Patients with LVPs had PlMI episodes significantly more often. 24-h duration of ischemic episodes and maximal ST depression occurred more frequently in patients with slow fragmental activity. Assessment of Holter ECG monitoring and LVPs allows to ascertain functional status of IHD patients, promotes identification of prognostically unfavourable group of patients at risk of life-threatening arrhythmia and sudden cardiac death.

[Differential diagnosis of infectious and non-infectious thrombotic endocarditis].

The aim of the study was analysis of clinical and laboratory markers of non-infectious thrombotic endocarditis (NITE) to facilitate differential diagnosis with infectious endocarditis (IE). 20 NITE patients (8 males and 12 females, mean age 32.3 years) were included in the trial. They had primary antiphospholipid syndrome, secondary antiphospholipid syndrome in systemic lupus erythematosus, nodular polyarteritis, nonspecific aortic arteritis, paraneoplastic NITE in lung and stomach cancer. NITE was also diagnosed in patients with gout and ankylosing spondylarthritis. Clinical and laboratory findings allow to differentiate IE with NITE. The former is characterized by destructive valvular lesions registered at echo-CG, positive hemoculture and arterial embolism. As a rule, NITE is associated with antiphospholipid syndrome and manifests with arterial and venous thrombosis, thrombocytopenia, livedo and presence of antibodies to cardiolipins.

[Current aspects of differentiated therapy of infectious endocarditis].

150 patients (90 males and 60 females, mean age 41.9 +/- 3.3 years) with infectious endocarditis (IE) were treated. 26.7 and 71.3% of the patients had primary and secondary IE, respectively. Clinical and laboratory manifestations of the variants indicate that each variant is associated with a certain clinicolaboratory syndrome: acute IE is associated with toxicoseptic syndrome, subacute--with immune unbalance, and thromboembolic complications, chronic--with immunocomplex visceral lesions. Different variants of the course predetermine different therapy. It is recommended to combined active antibacterial therapy with UV radiation of autoblood and hemosorption, hyperbaric oxygenation. Immunocorrective and efferent therapy hold promise in IE treatment. Glucocorticosteroids are effective in immunocomplex disorders.

[Cardio-hemodynamic effects of domestic adrenergic beta-antagonist proxodolol in hypertension in patients with pacemaker].

59 patients with mild and moderate essential hypertension have received a course treatment with beta-adrenoblocker proxodolol. The latter was studied for a hypotensive action and cardiohemodynamic effects as regards baseline type of circulation and pacing regimen. Proxodolol hypotensive action in hyperkinetic circulation is attributed primarily to a fall in the ejection (beta-adrenoblock), in hypo- and eukinetic circulation type--to reduction in cardiac postload (alpha-adrenoblock). In isolated ventricular stimulation proxodolol cardiodepressive effect was more pronounced than in atrial stimulation. An overall hypotensive effect of proxodolol in mild arterial hypertension made up 77.2%, in moderate--66.5%.

[Infectious endocarditis: immune disorders, differentiated therapy].

AIM: Immunological assessment of variants of infectious endocarditis (IE) and design of differentiated approach to the disease therapy. MATERIALS AND METHODS: Of 150 patients treated (90 males and 60 females, mean age 41.9 +/- 3.3 years) 28.7% and 71.3% had primary and secondary endocarditis, respectively. With chronization of the process, there was a trend to growing immune unbalance (IU). RESULTS: The IU in IE patients manifested with leukocytosis changing for leukopenia, reduced absolute and relative number of lymphocytes, complement, T- and B-lymphocytes, growing content of CIC, IgM, rheumatoid factor and cryoproteins. CONCLUSION: Besides antibacterial therapy, IE patients need immunocorrective and efferent therapy.

[Dynamic electrophysiological examination of heart in patients with sick sinus syndrome in continuous pacing].

AIM: Dynamic electrophysiological examination of the heart with allowances for ECG variants of this syndrome and stimulation regimen was performed to predict the results of continuous pacing in patients operated for sick sinus syndrome (SSS). MATERIALS AND METHODS: 32 patients had the diagnosis of bradycardia SSS (group 1), 15 patients--bradytachycardia (group 2). 30 patients were on ventricular VVI stimulation, 17 patients--on atrial AAI stimulation. Electrophysiological examination of the heart was performed by means of transesophageal left atrial pacing under programmed, frequent and ultrafrequent stimulation 1, 2, 3 and 4 years after the start of continuous cardiac pacing. RESULTS: In group 2 depression of the sinus function (corrected time of the sinus function recovery, frequency of spontaneous pacemaker, duration of poststimulation pause) under pacing was more distinct than in group 1. Deterioration of atrioventricular conduction was greater in group 1. Three and four years after pacing the Wenckebach point in group 1 lay significantly lower than in group 2. Within the follow-up period 6 (12.8%) patients developed AB-block of the second degree. 4 patients of them were from group 1. Arrhythmogenic atrial readiness depended on the kind of cardiac affection more than on stimulation regimen. CONCLUSION: Dynamic electrophysiological cardiac examination in the presence of pacing allows following up of natural course of sick sinus syndrome and early correction of the stimulation regimen.

[The medical rehabilitation of patients with an artificial pacemaker].

AIM: To study prognostic significance of individual rehabilitation of patients with pace-maker with account for leading cardiac syndromes. MATERIALS AND METHODS: 375 patients with artificial pace-maker were treated in a cardiological hospital for cardiac failure, blood hypertension, angina pectoris, cardiac arrhythmias. RESULTS: Congestive heart failure, blood hypertension, secondary cardiac arrhythmia, angina of effort were diagnosed in 57.4, 44.4, 33.0. 30.6% of the studied patients with artificial pace-maker. The patients died primarily of cardiac diseases, cancer and complications of cardiac pacing (71.3, 14.9 and 6.7% of cases, respectively). CONCLUSION: Aftercare of patients with pace-maker should be performed in groups of follow-up (5 groups) according to specially designed programs of diagnostic, therapeutic and prophylactic measures.

[The diagnosis and elimination of disorders in cardiac electrostimulation in patients with an artificial pacemaker].

In 378 patients with artificial pacemaker (AP) 10-year follow-up revealed disturbances of continuous pacing and cardiac arrhythmia. Diagnostically significant were: ECG, Holter ECG monitoring, functional AP control. Cardiac arrhythmias were spontaneous and pacing-induced. Programmed AP corrects complications of cardiac pacing and arrhythmia without surgical intervention. Regular technical control of the stimulation system providing early diagnosis of AP defects is an important component of check-ups of patients with AP.

[The characteristics of the effect of obzidan on the cardiac hemodynamics in patients with an artificial pacemaker when used one time and over a course of treatment].

ECG investigations of cardiodynamics were performed in 47 patients subjected to isolated ventricular electrocardiostimulation on obsidan treatment. The drug was given in a single dose or in a course. Cardiodynamic effects were evaluated in 3 groups of patients: with hyperkinetic, eukinetic and hypokinetic circulation (17, 19 and 11 patients, respectively). Cardiohemodynamic effects of obsidan are shown to depend on type of circulation and features of intracardiac hemodynamics (fixed cardiac rhythm, atrioventricular dissociation, pathological ventricular asynchronism, valvular blood regurgitation). This potentiates cardiodepressive effects of obsidan in patients with hypokinetic circulation who experience deterioration of cardiac insufficiency.
