[The motion characteristics of the interventricular septum and their effect on cardiac hemodynamics in patients with an implanted pacer].

The paper presents echocardiographic findings of 91 patients with implantable pacemaker. Ventricular septal motion was found to be abnormal in 59 out of 65 patients with endocardial stimulation. Moreover, they had early systolic motion and paradoxal septal contraction. Three types of abnormal septal motion were identified. In these patients, ECG recorded a picture of left bundle branch block. With myocardial stimulation, ventricular septal motion was normal. In abnormal septal motion, left ventricular ejection fraction was much lower than that in normal septal motion. Endocardial stimulation of the right ventricle is hemodynamically inferior to myocardial stimulation of the left ventricle. Abnormal ventricular septal motion associated with abnormal asynchronism in ventricular performance is one of the cause of the above phenomenon.

[Results of a 5-year follow-up of patients with implanted pacemaker suffering from chronic circulatory insufficiency].

The results obtained from a 5-year follow-up of 124 patients suffering from chronic circulatory insufficiency and having an implantable pacemaker were studied during various cardiotonic therapies. A combination of cardiac glycosides in subtherapeutical doses with peripheral vasodilators or cardioselective stimulants was found to enhance the efficacy of cardiotonic therapy and to prevent digitalis intoxication in low glycoside tolerance during ventricular pacing. Predictive criteria for clinical effects of cardiotonic therapy were defined.

[Comparative study of the effectiveness of Bonnecor and ethacizine in various disorders of cardiac rhythm].

The data obtained on 68 patients suffering from cardiac arrhythmia treated with bonnecor or ethacizine demonstrate that bonnecor corrects ventricular extrasystole including high-grade arrhythmia in 80.4% of the patients while ethacizine in 84.6%. Acute tests with the drugs can effectively prognosticate the outcomes of bonnecor or ethacizine long-term treatment. The drugs eliminate delta-wave and stimulation-induced arrhythmias in 90% and 44-56% of patients with WPW syndrome, respectively. Bonnecor is less effective in nodal tachycardia. Both the drugs noticeably suppress conductivity.

[Therapeutic efficacy of trimecaine in cardiac rhythm disorders and ways of improving it].

The efficacy of antiarrhythmic therapy with trimecain in 5 different regimens was studied in 87 patients, in 146 episodes of various disorders of the cardiac rhythm. On the basis of clinical and experimental (on 14 dogs with ventricular arrhythmia) studies, different schemes of trimecain treatment are compared. It was shown that combination of the intravenous and the subsequent intramuscular administration of trimecain to treat ventricular extrasystole complicating the course of acute myocardial infarction is as effective as prolonged instillation but exerts a longer action and is better tolerated by patients. Using tetrapolar chest rheography the effect of trimecain infused in the maximum single dose on the central hemodynamics and the ECG was studied in subjects without any cardiovascular pathology. It has been established that trimecain does not suppress myocardial contractility and has no hypotensive action.

[Therapeutic use of nitroglycerin in pulmonary edema and cardiac asthma in myocardial infarct patients].

The effect of sublingual medication with nitroglycerin taken in a dose of 0.5-1 mg was studied in 101 patients with myocardial infarction (77 had pulmonary edema and 34 had cardiac asthma). In patients with edema of the lungs nitroglycerin reduced dyspnoea, in some cases of cardiac asthma it arrested the attack. It was found that nitroglycerin reduced central venous pressure, the diastolic-systolic index of the pulmonary rheogram, the systolic, diastolic and mean pressure in the pulmonary artery, and arterial pressure in the greater circulation. With the intake of the drug, cardiac output decreased almost significantly, whereas the peripheral pressure did not change. It is concluded that the use of nitroglycerin in a dose of 0.5 mg in the treatment of cardiac asthma and pulmonary edema in patients with acute myocardial infraction is advisable.
