Comparison of morphologic features of intact and ruptured aneurysms of infrarenal abdominal aorta.

INTRODUCTION: Endovascular aneurysm repair (EVAR) has been suggested as a technique to improve outcome of ruptured abdominal aortic aneurysm (AAA). Whether this technique becomes an established treatment will depend, in part, on the anatomy of ruptured AAA. METHODS: The anatomy of intact and ruptured AAA seen in a university department of vascular surgery over 5 years was reviewed. Aneurysm anatomy was assessed with spiral computed tomographic angiography. Suitability for EVAR was assessed from the dimensions of the proximal neck and common iliac arteries. Neck length less than 15 mm, neck width greater than 30 mm, and common iliac artery diameter greater than 22 mm were declared unsuitable for EVAR. RESULTS: Three hundred sixty-three patients with intact AAA and 46 with ruptured AAA were identified. Larger intact aneurysms were significantly associated with longer renal artery-bifurcation distance and more complex proximal neck architecture. In this sample, patients with ruptured AAA were more likely to have larger aneurysms with shorter and narrower proximal necks. Significantly more intact aneurysms were morphologically suitable for endovascular repair compared with ruptured AAA (78% vs 43%; P <.001). CONCLUSIONS: Ruptured AAA are less likely to be suitable for endovascular repair than are intact AAA, most probably because of larger diameter at presentation. Open repair will likely remain the treatment of choice in most patients with ruptured AAA, because of current morphologic constraints of endovascular repair.

Endovascular repair of a ruptured chronic type B aortic dissection.

Aneurysm formation is a common sequel of chronic type B aortic dissection. Ruptured false lumen aneurysms have traditionally been treated with open repair. These procedures are associated with high morbidity and mortality rates. We report the first successful endovascular repair of a ruptured chronic type B aortic dissection in a patient who had been turned down for elective surgery. The endovascular management of chronic dissection with rupture is difficult and may necessitate stenting of both entry and reentry points to induce false channel thrombosis. The long-term efficacy of this technique is unknown.

Diagnostic cervical zygapophyseal joint blocks for chronic cervical pain.

OBJECTIVES: (1) To determine the prevalence of cervical zygapophyseal joint pain in a specialist clinical setting; (2) to review the number of diagnostic blocks needed to identify the segmental level of the symptomatic joints; and (3) to determine the distribution of segmental levels of cervical zygapophyseal joint pain in a clinical setting. DESIGN AND SETTING: Retrospective audit of patients of three independent rehabilitation medicine specialists who had undergone cervical zygapophyseal joint blocks in hospital outpatient clinics and private rooms. PATIENTS: 97 patients aged 18-82 years with chronic neck pain (with or without headache) of more than six months' duration refractory to conservative therapies. INTERVENTION: Diagnostic fluoroscopic cervical third occipital and medial branch blocks of zygapophyseal joints. Diagnosis required confirmation by a repeat procedure. RESULTS: 35 of 97 patients (36%) had a confirmed symptomatic cervical zygapophyseal joint (95% CI, 27%-45%). The symptomatic segmental level was found at the first attempt by reference to a standard pain diagram in 83% of cases (29 of 35). The most common symptomatic levels were C3-4 (11/35; 31%) and C5-6 (10/35; 29%). CONCLUSION: The prevalence of cervical zygapophyseal joint pain estimated in this clinical study is lower than that found in previous research setting studies, but our requirement for confirmation by a repeat block (which many patients declined) makes our estimate conservative; it is likely that the true prevalence is higher. Zygapophyseal joints are clearly a common source of pain in patients presenting with chronic neck pain, with or without headache. Cervical zygapophyseal joint pain is readily diagnosable, enabling patients to seek further, targeted treatment.

Endovascular repair of perisplanchnic abdominal aortic aneurysm with visceral vessel transposition.

PURPOSE: To report a combined endoluminal and open surgical approach for a suprarenal abdominal aortic aneurysm (AAA) with coexistent splanchnic vessel stenoses. METHODS AND RESULTS: A 64-year-old man presented with an aneurysm of the proximal abdominal aorta and severe stenoses of the celiac axis and superior mesenteric artery (SMA). An initial 2-stage plan to stent the visceral vessel stenoses and exclude the aneurysm with a fenestrated stent-graft failed when the celiac lesion could not be crossed. The approach was changed to restore visceral perfusion with a bifurcated left iliosplenic and ilio-SMA bypass graft. Exclusion of the aneurysm was achieved with a custom-made suprarenal aortic tube stent-graft (Ivancev-Malmö) system. The patient is free of symptoms at 22 months, and there was no aneurysm visible on the 14-month CT scan. CONCLUSIONS: Hybrid techniques are an alternative treatment for complex perivisceral aortic aneurysms when total endovascular reconstruction is not possible.

Static versus dynamic prosthetic weight bearing in elderly trans-tibial amputees.

The purpose of this study was to compare prosthetic weight-bearing tolerance in the standing position to the dynamic vertical ground reaction forces (VGRF) experienced during walking in elderly dysvascular trans-tibial amputees. Ten unilateral trans-tibial amputees attending an amputee clinic (mean age = 67 +/- 6.5 years) were selected as subjects. Selection criteria were the level of amputation, age, medical fitness to participate and informed consent. Each participant completed five trials of standing (static) weight bearing measurement followed by 10 walking (dynamic) trials on a 10m level walkway, five trials for each limb, Static weight bearing (SWB) was measured using standard bathroom scales. Dynamic weight bearing (DWB) was measured during gait using a Kistler multichannel force platform. T-tests for dependent means indicated that the forces borne in prosthetic single limb stance (mean = 0.97 +/- 0.03 times body weight (BW)) were significantly lower than the forces borne by the prosthetic limb during the first peak (weight acceptance) VGRF (mean = 1.08 +/- 0.08 BW; t = -4.999; p = 0.001) and significantly higher than the midstance VGRF (mean = 0.82 +/- 0.07 BW; t = 5.401; p < 0.001). However, there was no significant difference between SWB and the second peak (push-off) VGRF generated by the prosthetic limb during walking (mean = 0.96 +/- 0.03 BW). It was concluded that clinical gait training may utilise SWB as a guide to an amputees' prosthetic weight bearing tolerance and requirements during walking.

Eye splashes during invasive vascular procedures.

In recent years there has been an increased awareness among health care workers regarding possible risk of nosocomial transmission of blood borne pathogens. The aim of this study is to document the risk of contamination of radiologists' eyes during invasive vascular procedures. Radiologists performing these examinations were asked to wear glasses throughout. After each examination the glasses were inspected for droplets. 150 procedures were performed on 123 patients (M = 80, F = 43). 10 procedures (6.7%) resulted in splashes to glasses. In four of these cases the radiologist was not aware of the "eye splash" nor was there a spray event to account for it. Radiologists were aware of 13 spray events (8.7% of all procedures). There was a significantly increased risk of spray events and eye splashes during thrombolysis (chi 2 = 14.93, p < 0.001) and of spray events during angioplasty when compared with perfemoral arteriography (chi 2 = 8.816, p < 0.01). Procedures lasting longer than 30 min were associated with a significantly increased risk of spray events (chi 2 = 5.63, p < 0.02). Significantly more eye splashes were associated with more than two catheter changes (chi 2 = 8.912, p < 0.01). It is suggested that protective eye wear should be used routinely during invasive vascular procedures.

Successful pregnancy in two sisters with Wolfram syndrome.

Wolfram's Syndrome is a rare disorder comprising diabetes insipidus, diabetes mellitus, optic atrophy and high tone sensineural deafness. Successful pregnancy is extremely rare and has not previously been reported in siblings with this disorder. We report on two Irish sisters suffering from Wolfram's syndrome who have given birth to three healthy male infants.
