Diagnostic strategies for evaluation and prognosticating the outcome of jaundice among patients with cholestasis caused by neoplastic diseases of the hepatobiliary system and the pancreas.

The purpose of the present study was to assess the diagnostic approach and the outcome of neoplastic diseases of the hepatobiliary system and the pancreas that develop cholestasis. 84 patients over 40 years of age treated in our Department between January 1994 and January 1998 were included. The etiological cause of jaundice was established by ultrasonography, computed tomography, thin needle biopsy with cytological and histological examination of the obtained material or ERCP. 11 of the 84 patients died, 9 underwent surgery and 64 were referred to the Oncologic Center for further treatment.

Estimation of trends of morbidity of colorectal cancer in Plovdiv district for the period from 1985 to 1996.

The present study concerns the incidence rate of colorectal cancer (CRC) in Plovdiv region (population 1,269,464). The study was carried out over a 12-year period and aimed at establishing the increase rate of this disease. Results show that incidence rate of CRC increased progressively for the mentioned period from 22.14 to 34.98 per 100,000 people and was almost twice as high as that for the country. It is a particularly disturbing fact that the morbidity and mortality rates due to colorectal cancer is rising--2.36 times for 12 years. We propose screening for asymptomatic and high risk patients with considerable results for opportune diagnostics.

Trends in the incidence of colorectal carcinoma in three regions of south Bulgaria for the period 1985-1998.

The purpose of the present study was to evaluate the increase of colorectal carcinoma incidence in three regions of South Bulgaria (a total population of 1,269,464 persons) over a 14-year period (1985-1998). The data about the incidence rate are retrieved from the oncological hospital records of the patients and Bulgarian National Oncological Register. The census data are provided by the National Institute of Statistics, Sofia. The results show that the incidence rate of colorectal carcinoma for the studied period increased steadily from 22.14/100,000 to 37.18/100,000 (an increment of 15.04/100,000) which is almost twice the average for the country. Compared to the baseline year of 1985 the increase in these three regions is 67.93% or approximately 5% annually. The incidence rate in Plovdiv region was almost twice that of Smolyan region. The incidence rate of colorectal carcinoma in both genders was greater than that of stomach cancer. The highest incidence rate was found in the 70-79-year-age group (193.5/100,000). A major part of our study was to find the stage in which colorectal carcinoma (CRC) is detected. In 1985 only 0.57% of the patients were diagnosed as being in the I-st clinical stage, 37.76%--in the II-nd, and 61.67%--in III-IV clinical stages. This unfavorable trend was preserved in 1998, when only 3.17% were diagnosed as having the first clinical stage, 38.62%--the II-nd and 58.21%--the III-IV clinical stages. The 14-year pronounced trend of increase of the prevalence rate (a 2.91 times increment) and the mortality rate (a 2.50 times increment) of colorectal carcinoma is especially alarming. We propose screening of the asymptomatic patients and high-risk persons which gives considerable results in the timely diagnostics of colorectal carcinoma.

A rare case of fibrosarcoma of the inferior vena cava expanding into the right atrium.

The authors report in detail a case of fibrosarcoma of the inferior vena cava. A 53-year-old female presented with echographic and computed tomographic evidence of a solid formation in the porta hepatis that fused with the inferior vena cava. Because of appearance and rapid development of symptoms of right cardiac failure, the patient was transferred to the Clinic of Cardiac Surgery and underwent an emergency operation. A solid formation arising from the inferior vena cava and expanding into the right atrium was found during the operation. The operation performed was not a radical one. Fibrosarcoma of the inferior vena cava was proved on histological examination. A single therapeutic course with vincristine and farmorubicin was administered. The patient died from recurrence of the malignancy 26 months after the operation. The epidemiologic, diagnostic, and therapeutic aspects of the case are discussed in relation with the literature data.

Genetico-mathematical analysis of the inheritance pattern of ulcer disease. A posteriori methods. The maximum likelihood method.

Inheritance pattern of ulcer disease was analysed genetico-mathematically using one of the a posteriori methods--the method of maximum likelihood. We studied the families of 351 hereditary predisposed probands of whom 57 with type II gastric ulcer disease, 27 with type III gastric ulcer disease and 267 with duodenal ulcer disease. The types of ulcer disease were determined according to H. Johnson. The analysis of the patients with duodenal ulcer also included the types of familial predisposition of the probands. The results yielded by the maximum likelihood method excluded to a great degree the possibility of monogenic transmission of ulcer disease. None to the three types of peptic ulcer showed autosomal recessive pattern of inheritance; it was not found even in the families with phenotypically healthy but hereditary predisposed parents. No evidence was found of autosomal dominant pattern of inheritance of the disease in all studied groups including the families with one sick parent. This is supported by the different values of the segregation coefficients in the families with one sick father and one sick mother. The absence of evidence for monogenic mode of inheritance of the disease indicated by the detailed genetico-mathematical analysis should be further evaluated by the criteria of polygenicity.

Application of the a priori methods for genetico-mathematical analysis of the inheritance pattern of ulcer disease.

The a priori methods were used to perform a detailed genetico-mathematical analysis of the pattern ulcer disease is inherited. The study included families of 351 probands. Of these, 267 were with duodenal ulcer, 57 with gastric ulcer type II, and 27 with gastric ulcer type III. The types of gastric ulcer were defined according to the classification proposed by H. Johnson. The cases of duodenal ulcer were also analyzed depending on the type of familial predisposition of the patients. The results from Bernstein's a priori method and Li's a priori correction method exclude autosomal recessive pattern of inheritance of the three types of peptic ulcer, not even in the families with healthy but hereditarily predisposed parents. On the other hand, the difference between the numbers of actually observed and theoretically predicted sick children in all groups studied, including those in the families with one sick parent, found by applying the Hogben modification of Bernstein's a priori method, is not consistent with an autosomal dominant pattern of inheritance. Nor is this inheritance pattern consistent with the finding that there are no essential differences between the quantitative parameters of the families with one or two sick parents. It is, therefore, concluded that this disease is not possible to be passed down in generations in a monogenic mode.

[Prospective epidemiological study of arterial hypertension in middle and old age].

The arterial hypertension frequency among the advanced and old age population was studied at 5 year intervals in 4 villages in the Rhodope mountains. 1777 persons were examined--725 in advanced and old age and 1052 young persons. The high frequency of arterial hypertension among the advanced and old age population is impressing--47.94% in the second examination. The frequency is higher in women. In advanced and old age the arterial hypertension is 3-4 times more frequent which is due to the sharply increased frequency of high arterial systolic pressure in these two age groups. In prospective plan the spread of arterial hypertension continues to increase especially in old age but the disease in this age is about 10% less frequent than in advanced age. The increasing frequency of arterial hypertension in advanced and old age is attributed mostly to the changed working and living conditions of the population.

[Gastric secretion and acidity in elderly patients with duodenal peptic ulcer].

46 patients over 60 years of age suffering from duodenal ulcer, 36 young patients with duodenal ulcer and 36 healthy aged persons (control group) have been examined after maximal pentagastrin stimulation. The results revealed that the aged patients with duodenal ulcer have significantly lower gastric secretion and acidity (BVO--87.7 +/- 36.8 ml/h, MVO--220.0 +/- 52.8 ml/h, BAO--4.23 +/- 2.63 mmol/h, MAO--25.43 +/- 8.39 mmol/h, PAO--30.34 +/- 12.09 mmol/h) than the young patients with duodenal ulcer (p less than 0.01-0.001). The volume of gastric secretion and the degree of acidity of the aged patients with duodenal ulcer are significantly above the reference value (BVO--71.6 +/- 19.5 ml/h, MVO--174.8 +/- 34.9 ml/h, BAO--1.84 +/- 0.96 mmol/h, MAO--16.58 +/- 4.02 mmol/h, PAO--20.73 +/- 3.08 mmol/l) of the control group of healthy aged persons (p less than 0.02-0.001). The suggestion is put forward that the differences in gastric secretion and acidity between the patients and the healthy persons above 60 years of age are one of the basic factors for development and/or persistence of the ulcerous process in old age.

[Carcinoid tumor of the cecum--1st description in Bulgaria].

The authors describe, for the first time in Bulgarian medical literature, a carcinoid of cecum in an 77-year old woman, with a lethal end. The tumour was with the size--6 X 7 cm, with infiltrative growth in the intestinal wall and massive metastases in liver. Grimelius staining revealed the presence of secretory argentophilic granules in the tumour cells. The main tumour, light- and electrone-microscopically, is represented by conditioned non-differentiated and differentiated cellular elements, which in addition to the usual oragellae contain also granules of neurosecretory type.
