[Treatment of Wegener's granulomatosis using plasmapheresis].

A case with Wegener's granulomatosis in a young woman is described. The diagnosis was made after extirpation and histological investigation of granuloma tissue from the nasal concha and by needle renal biopsy. A successful treatment was carried out with plasmapheresis, hemodialysis and immune-suppressors, the acute paroxysm being coped as well as the developed acute renal insufficiency. After each plasmapheresis a considerable reduction of the circulating immune complexes was established. After a remission of 9 months the patient developed a stage of terminal chronic renal insufficiency and was subjected to chronic dialysis with good rehabilitation.

[Initial plasmapheresis treatment results in glomerulonephritis].

The first results from the treatment of two cases with glomerulonephritis are reported--the first was an exacerbation of nephritis after intercurrent disease and second--with systemic lupus erythematosus, after plasmapheresis application as an active therapeutic method, in combination with corticosteroid cytostatic treatment. A reduction of the three immunoglobulins in plasma was found as a result from plasmapheresis and emergence of serum complement, together with clinical data for improvement of the patients. The prospects of plasmapheresis treatment of the active forms of glomerulonephritis are discussed.

[Pathogenetic and clinical significance of pentagastrin stimulation of gastric secretion in the active phase of peptic ulcer].

The gastric secretion with pentagastrin was studied in 214 subjects (130 with duodenal ulcer, 20 with double localization--in duodenum and stomach, 32 with gastric ulcer and 32--healthy). A different gastric secretory characteristic was found and the problem raised as regards the differentiated therapeutic and surgical approach in the separate tyoes of ulcer disease. The origination and clinical course of the duodenal ulcer and the ulcer with double localization are admitted to depend most likely and to a greater extent, on the gastric secretion rate. The duration of the active duodenal ulcer probably has no substantial effect on the enhanced secretory function of the stomach in the course of many years. The indices of secretion and acid elimination in single gastric ulcers are significantly diminished.

[Age-related aspects of gastric secretion in duodenal ulcer during the exacerbation phase (a pentagastrin study)].

The age characteristics of gastric secretion were studied in 145 patients with duodenal ulcer and 32 healthy subjects via pentagastrin. With age advancing of the patients, a gradual reduction of gastric secretion and acidity are found, being statistically significant after the age of 49. Apart of that, the average values of the indices for each age group remain considerably higher than the normal. The age, at which the disease originated, has also a certain effect on gastric secretion. Significant reduction is observed after the age of 49, concerning only secretion volume and acidity elimination at basic conditions. The changes found in the gastric secretion and acidity are admitted to possibly play a definite role for the rather unusual course and relatively rare duodenal ulcer origination at the age of 50.

[Gastric secretion study by maximal pentagastrin stimulation in healthy persons and peptic ulcer patients].

Gastric secretion was investigated in 187 subjects under conditions of maximal stimulation with pentagastrin. Thirty subjects, out of them, were healthy, 30--with gastric ulcer, 110--with duodenal ulcer and 17--with double ulcer localization--in duodenum and stomach. The indices of gastric secretion in duodenal ulcer and ulcer disease with double localization are considerably higher than those on the healthy subjects and differ essentially. With single gastric ulcer they are significantly lower than those of the healthy controls.

[Several functional changes in the livers of patients with diabetes mellitus].

The authors have studied the functional liver state in 107 patients with diabetes mellitus. Various stages of functional liver disturbances were found in a considerable part of them. With a few exceptions, the latter have the highest incidence and severity among the patients with decompensated, with mixed type diabetes and with remote vascular degenerative manifestations. Radioisotope investigations with 131J-Bengal pink, reveals the earliest developed changes in some of the liver functions, as compared with rest of the tests. The functional liver state is recommended to be determined in all diabetics with a view to an early detection of the developed functional disturbances and their timely treatment.

[Several clinical-x-ray manifestations of myeloma disease in Bulgaria].

Two hundred patients with myeloma disease have been reported in Bulgarian medical literature for the period 1943-1973. The males are 62.3 per cent and the females--37.7 per cent. Out of all the cases 91.2 per cent are at the age of 40 and 48 per cent of the patients--between 51--60. The following incidence rate of the investigated syndromes and symptoms was established: osseous--96 per cent, asthenic-dynamic--93.2 per cent, consumptive--74,6 per cent, neurologic--57.4 per cent, febrility--53 per cent, hepatomegalia--50 per cent, splenomegaly--24.3 per cent, lymphoadenomegalia--23.5, per cent, hemorrhagic--37.1 per cent, skin manifestations--20.7 per cent, spontaneous fractures--42.7 per cent, osseous tumors--25 per cent, hypercalcemia--56.1 per cent, broachitis and bronchopneumonia--50.7 per cent of the patients. The distribution according to the X-ray criteria reveals: osteolytic form--81,4 per cent; diffuse-porotic--9.3 per cent and osteonegative--9.3 per cent. The diagnosis was made within 1 1/2 years after the appearance of the first manifestations. The average life span is about 2 years (2 1/2 months to 5 1/2 years). A difference in the prognosis of beta1-,beta2- and gamma-myelomas was no testablished.

[Prolonged high eosinophilia with description of a case with fibroplastic parietal endocarditis].

The authors report a case with fibroplastic parietal endocarditis with blood eosinophilia (Löffler syndrome II), observed in the course of seven years. The permanent high eosinophilia is of special interest as well as the developed aneurysm of a. pulmonalis. The past history of toxoplasmosis is thought to be the cause for the origination of the disease in the patient. A thorough literature survey on the problem is carried out.
