Protein-binding assay and radioimmunoassay were used to determine the concentrations of free androgens and C19-steroid sulphates in suprapubic abdominal skin slices obtained from healthy women and men during appendectomy. It was found that the abdominal skin of the women contained DHA in the highest concentration, followed in decreasing order by And., delta 4-dione, delta 5-diol, Test. and DHT. The sequence was the same for the men, except that delta 4-dione preceded And. There was no significant difference in the concentrations of free androgens in abdominal skin of women and men; only the concentration of delta 4-dione was somewhat higher in the skin obtained from males (P less than 0.05). Of the C19-steroid sulphates, And.-S was found in the highest concentration in abdominal skin of females, followed in decreasing order by DHA-S, delta 5-diol-S and Test.-S. The corresponding decreasing sequence for the men was DHA-S, And.-S, delta 5-diol-S and Test.-S. The concentration of DHA-S was significantly higher (P less than 0.001) in men than in women; in the case of delta 5-diol-S, only a slight degree of significance could be observed (P less than 0.05).
Beside dehydroepiandrosterone sulphate and androsterone sulphate, 5-androstene-3 beta, 17 beta-diol-3-sulphate and testosterone sulphate were isolated and identified from the axillary sweat of sexually mature women and men. The quantities of these four C19-steroid sulphates were determined individually in the sweat of eight healthy, sexually mature women and men: dehydroepiandrosterone sulphate was found in the highest amount, it was followed by 5-androstene-3 beta, 17 beta-diol-3-sulphate and androsterone sulphate. Testosterone sulphate occurred in the lowest quantity, its excretion amounting to about one-thousandth of that of dehydroepiandrosterone sulphate. Study of the C19-steroid sulphates excreted by the apocrine sweat glands permitted an insight into the steroid metabolism in human skin, which presumably is, closely connected with the steroid hormones of peripheral blood.

The pharmacologically active (methylenedioxy)- and diethoxyepialloberbane keto esters I have been synthesized with use of the readily available keto esters 2 as starting material. By choice of the appropriate reaction sequence both antipodes of keto ester 2a can be employed to provide any enantiomer of the desired raunescinone analogue 1a. Hypotensive, antihypertensive, and central depressant effects of 1a are described. The principle effect observed for 1a was a potent hypotensive and antihypertensive effect of long duration without depression of the central nervous system.
The in vitro metabolisms of [4-14C]-labelled DHA, delta 5-diol, delta 4-dione and Test were studied in skin tissue excised from the hairy hypogastric region of three patients diagnosed as suffering from "idiopathic hirsutism". The concentrations of DHA, And, delta 4-dione, delta 5-diol, Test, DHT, DHA-S, And-S, delta 5-diol-S and Test-S were determined in other portions of the same skin tissue. In the knowledge of the concentrations of the androgens and the C19-steroid sulphates in the blood and in the skin tissue, and also of the metabolisms of the main androgen precursors and Test in the hairy abdominal skin, new diagnoses can be established within the group of idiopathic hirsutisms : "pure peripheral hirsutism" and "mixed peripheral hirsutism". In the former the hyperactivity of the enzymes of the skin tissue takes part in the emergence of the disease form, while the latter involves the joint participation of the hyperactivity of the enzymes of the skin tissue and the high level of delta 4-dione in the blood. The picture of the metabolism in the hairy abdominal skin of the hirsute patients was dominated by Test formed in pathologically high amount from the precursors as a consequence of the hyperactivity of 17 beta-HSD. The formation of DHT and the activity of 5 alpha-R were of only secondary importance.
An in vitro study has been made of the steroid metabolism in the abdominal skin in two hirsute women with adrenogenital syndrome, and the concentrations of the various steroids in the skin tissue have been determined. The urinary excretion of the total 17-Ks, and particularly P-triol, was pathologically high, while of the androgens examined in the serum, primarily the levels of delta 4-dione and Test, were found to be elevated. The 21-hydroxylase deficiency meant that the plasma ACTH level was likewise extremely high in both patients. In vitro incubation studies demonstrated that in one patient (with the higher androgen overproduction) more Test. than normal was formed from the precursors (DHA, delta 5-diol, delta 4-dione), i.e. the biosynthetic pathway (17 beta-HSD, delta 5-3 beta-HSD) leading towards the androgens was enhanced in the abdominal skin. In the other patient (where the androgen production was less high as a consequence of the earlier adrenalectomy) the metabolism in the abdominal skin was not enhanced; indeed, for many metabolites the extent of the transformation did not even attain the level for normal women. The activity of Test. 5 alpha-reductase was not increased in the skin of either patient. The results on the steroid contents of the skin tissue revealed that numerous free steroids (DHA, And., delta 4-dione, delta 5-diol, Test., DHT) and C19-steroid sulphates were present in higher concentrations than in the abdominal skin of healthy women. The extents of steroid accumulation compared to the serum level in the same patient were pathologically high in the case of delta 5-diol, DHT and DHA-S in the abdominal skin of the two hirsute women with adrenogenital syndrome. This confirmed that a state of hyperandrogenism does exist in the skin of these patients.

Shock was elicited in anaesthetized dogs by maintaining a haemorrhagic hypotension of 4 kPa until 30 per cent spontaneous refusion, followed by total reinfusion. Functional residual capacity (FRC) and minute ventilation increased considerably similarly to our previous experiments. Lactate content in both the external intercostal and the biceps femoris muscles increased significantly in advanced shock. The expiratory external abdominal oblique muscle showed electromyographic signs of fatigue. At the height of the FRC changes tonic contraction of the external intercostal muscle could be demonstrated electron microscopically. This tonic contraction is the main factor in the large FRC rise in late shock forming the basis of a hitherto unknown vicious circle.
Monthly 120 serum samples were collected from healthy and clinically ill residents of North-Eastern Hungary in the years 1980 and 1981. Haemagglutination-inhibiting antibody to the strain Coronavirus OC 43 was detected in 58% of the 2881 samples. The rate of positive sera was the lowest, 23%, between 7 and 11 months of age and reached the peak of 70% between 15 and 19 years. It did not rise further with age. Paired sera was tested from 405 patients suffering from various diseases. A titre rise was shown in 23 patients; 12 of these were suffering from acute respiratory disease, another 7 showed simultaneous exacerbation of bronchial asthma or spastic bronchitis.
In vitro incubation studies demonstrated that abdominal skin slices from healthy females and males converted 1.05-2.73% (mean 1.75%) and 0.64-2.06% (mean 1.32%), respectively of [4-14C]dehydroepiandrosterone (DHA) to [4-14C]dehydroepiandrosterone sulphate (DHA-S) and that the delta 5-3 beta-hydroxysteroid sulphokinase(delta 5-3 beta-HSS) activities were almost identical in female and male abdominal skin. DHA-S formation in the skin of females with hirsutism of adrenocortical, ovarial or idiopathic origin was shown not to differ from that in healthy females; the hyperandrogenism in the skin of hirsute females is thus not a consequence of a delta 5-3 beta-HSS deficiency. In vitro synthesis of DHA-S in the abdominal skin of patients with complete testicular feminization or 46XY pure gonad dysgenesis and anorchia was similar to that in healthy subjects. Addition of synthetic human alpha 1-39-ACTH and natural (porcine) ACTH to in vitro incubates of abdominal skin slices from healthy females or females with various types of hirsutism increased the formation of DHA-S, revealing thereby a further extra-adrenal effect of ACTH. In the abdominal skin of healthy females and males the DHA concentration did not differ, but the concentration of DHA-S was much higher in males. The DHA-S concentration in the abdominal skin of idiopathic hirsutism patients was pathologically high in spite of the blood DHA-S level being normal. The considerable accumulation of DHA-S in the abdominal skin of healthy males and hirsute females cannot be explained by the normal delta 5-3 beta-HSS activity; thus, it is assumed that either the DHA-S elimination mechanism may be changed in the abdominal skin slices, or the DHA-S binding proteins might be multiplied.
Seven free androgen steroids and four C19-steroid sulphates were isolated and determined from the axillary hair of sexually mature healthy males and females. In 1 g axillary hair the free 4-androstene-3 beta, 17 beta-diol, testosterone, 5 alpha-dihydrotestosterone, dehydroepiandrosterone, androsterone, 4-androstene-3,17-dione and 5 alpha-androstane-3,17-dione occurred in nanomol amounts, whereas dehydroepiandrosterone sulphate, androsterone sulphate and 5-androstene-3 beta,17 beta-diol-3-sulphate were found in approximately 1000 times these amounts. The quantity of testosterone sulphate lies among the values for the free steroids. The free steroids present in hair are regarded as products of the sebaceous glands, and the sulphate ester steroids as products of the sweat glands.

Five females with idiopathic hirsutism, ten with adrenogenital syndrome, four with acne vulgaris, and three with Stein-Leventhal syndrome were treated for nine months with large or small doses of cyproterone acetate (100 or 2 mg daily, respectively) in combination with oestrogen. The drug had a good effect on the acne vulgaris and moderated the hirsutism. In the various patient groups the serum testosterone level was decreased to between 58.6% (+/- 28.7% S.E.) and 10.8% (+/- 8.2% S.E.) of the basal value. No essential difference in effectiveness was observed between the large and small doses. Based on study of the side-effects, it is recommended that laboratory liver tests are used for control.
