Delivery room management of meconium staining of the amniotic fluid and the development of meconium aspiration syndrome.

A 1-year prospective survey of obstetric and pediatric management of meconium staining of the amniotic fluid in 464 patients was undertaken. Pharyngeal suctioning before delivery was performed using bulb syringe (N = 130), De Lee suction catheter (N = 186), or both (N = 98); endotracheal intubation after delivery was also done in 413 instances. Using any of the three suctioning techniques, no differences were seen in Apgar scores, respiratory rates, presence or absence of meconium on or below the vocal cords, or development of meconium aspiration syndrome (MAS). If meconium was present on the vocal cords, it was present below the vocal cords in 76% of the cases. If no meconium was visualized, it was found below the vocal cords in only 7% of the cases. Of the 142 infants with meconium below the vocal cords, 10% developed MAS and all 14 survived.

Chemical and biological properties of diesel exhaust particles collected during selected segments of a simulated driving cycle.

Particle emissions, percentage of organic extractable materials, and mutagenicities of extracts from a diesel engine operating on a test stand have been determined for the full Federal Test Procedure driving cycle and several individual segments thereof. Particle samples were collected using a computer controlled high volume sampler. Extracts of the exhaust particles were screened for the potent mutagens nitropyrene/nitrofluoranthenes by mass spectrometry/mass spectrometry (MS/MS). Results indicate that a long acceleration from 0-55 mph produced approximately seven times more particles per second than the full cycle. Also, the 0- to 55-mph acceleration and a subsequent 55-mph cruise produced significantly higher amounts of mutagens than other segments or the full FTP cycle. A direct correlation of both NOx levels and temperature with mutagenicity was noted (r = 0.89 and r = 0.89). The specific activities of the extracts showed decreases or remained unchanged when assayed in TA-98 NR or TA-98 1,8 DNP6, nitroreductase deficient strains of TA-98. Three extracts were found to have high levels of nitropyrenes/nitrofluoranthenes, and two of the three had high specific activities in TA-98.

Generation and characterization of radiolabeled diesel exhaust.

To evaluate the potential health risks associated with increased use of diesel engines, information is needed on the biological fate of inhaled diesel exhaust components. Appropriately radiolabeled exhaust produced by burning radiolabeled fuel could be used to gain this information. The purpose of this study was to characterize different radiolabeled diesel exhausts with respect to their potential use in studies of the biological fate of exhaust carbon particles and particle-associated organic compounds (particle extracts). A single-cylinder diesel engine was used to burn diesel fuel containing trace amounts of 14C-labeled hexadecane, dotriacontane, benzene, phenanthrene or benzo(a)pyrene. Greater than 98% of the 14C in all additives was converted to volatile materials upon combustion. The remainder was distributed in varying amounts between the carbon particles and particle extracts. Aromatic additives labeled carbon particles more efficiently than aliphatic additives. Column chromatography of the particle extracts showed that, in most cases, the majority of the radioactivity eluted in fractions identical to the specific fuel additive employed, suggesting that a large amount of the particle-associated organic compounds consisted of uncombusted fuel constituents. Applying an electrical load to the engine-electrical generator increased carbon particle radioactivity, but had variable effects on the amount of radioactivity in the particle extracts. 67Ga-tetramethylheptanedione was also studied as a fuel additive to label carbon particles. 67Ga was incorporated into the exhaust particles and lung deposition of particles in rats was found to be approximately 10%. However, the 67Ga-radiolabel was found to separate from the particles in vivo, making it an unsuitable radiolabel for studying the long-term lung retention of diesel exhaust carbonaceous particles.

[Composition and food value of the flat pea (Lathyrus sylvestris L.)].

Flat pea (Lathyrus sylvestris L.) is suitable as 'pioneer plant' for the recultivation of slag heaps and mining areas for agricultural production. Flat pea contains between 20 and 30% crude protein in the dry matter and is richer in protein than other feed legumes. It is comparable to foxtail clover and lucerne as regards the content of amino acids (g/16 g N) and minerals. The digestibility of the crude nutrients of various dried products of flat pea was ascertained in seven experiments with five rams. The digestibility of the organic matter of the hay (before budding) was 66.2%, energy concentration 526 EFU/kg DM; 55% and 410 EFU/kg DM were ascertained for seed straw. The protein digestibility for hay and dried green fodder varied between 72.3 and 75.8%. Since there have been no reports lathyrogenous substances in the vegetative parts of flat pea, its use as green fodder or dried green fodder for feeding sheep, cattle or wild ruminants is possible.

Corticosteroidogenesis by isolated human adrenal cells: effect of serotonin and serotonin antagonists.

The direct effect of serotonin and antiserotonin agents on adrenal steroid biosynthesis was studied in isolated adrenal cells derived from patients with Cushing's syndrome. The results indicate that serotonin increases corticosterone production, while the serotonin antagonists cyproheptadine and methysergide depress adrenal steroid - particularly cortisol and aldosterone - biosynthesis.

[Case report of congenital deficiency of factor V and factor VIII].

A case of a rare congenital combined deficiency of factor V (14%) and factor VIII (30%) is described for the first time in our literature. Nine years old girl was admitted to hospital because of nightly bleedings from hyperplastic and inflamed gums. Clinical picture was mild what is in accordance with the results of coagulation studies. Under substitution therapy tooth extraction passed without complications.

[Porphyria cutanea tarda after uptake of lead--a case contribution to the etiology].

It is reported on a case of porphyria cutanea tarda. With the exception of a low risk lead exposure during work no etiological factor could be revealed. Body deposits of lead were established by means of Ca-EDTA provocation. After cessation of exposure which was suggested to be the cause the state of health markedly improved. Thus, compensation due to occupational disease was recommended to workers insurance.

[Liberation of histamine by anaesthetics (author's transl)].

Injection of Epontol (7 mg/kg i.v.) to male healthy students caused a brief increase in plasma histamine concentration from 0.7 ng/ml to 3.0 ng/ml. Combined injection (7 mg/kg) and infusion of Epontol (21 mg/kg during 15 min) or combined injection of Epontol (7 mg/kg) and suxamethonium (0.5 mg/kg) caused a more prolonged appearance of histamine in plasma without higher maximum concentrations. During anaesthesia with ketamine (Ketanest), halothane or nitrous oxide the histamine concentrations in plasma remained unaltered.
