[The spectrum of fatty acids during lactation in women].

The large spectrum of fatty acids (expressed in relative values) was studied in the breast-human milk during the first three weeks of the lactation (3rd, 5th, 10th and 20th day after delivery). All women passed the pregnancy and the delivery without any pathological syndrome and their neonates were found as a healthy and physiological too. No drug dependency was registered. Fatty acids were measured as methyl-esters (FAME) using the method of gas chromatography (1). During the 20 days after delivery and of lactation period, summarising the groups of fatty acid (saturated, monoenoic, PUFA n-3, n-6 and n-9) only few changes could be detected: the increase of the participation of saturated fatty acids of short-medium chain (8:0, 10:0 and 12:0) and of PUFA n-9. Compared with the most administered commercial formula Feminar, the visible differences in the participation of fatty acids were described. Finally: during the lactation we could obtain a large number of significant differences of such individual fatty acids as palmitic acid, arachidonic acid, eicosopentaenoic acid, linoleic acid ect. The possible reasons of such changes during the lactation are discussed and these results are compared with our previous data concerning the fatty acid- spectrum in blood plasma of healthy newborns and preterm or hypoxic infants.

Tumorous thrombosis of the vena cava inferior--experience with surgical treatment.

Patients suffering from renal carcinoma, treated at the Department of Urology in Hradec Králové, have been systematically followed up since 1962. In the present study we evaluated a group of patients treated from the year 1985 till 1991. The group consisted of 365 patients with renal carcinoma. In 55 (15%) of them invasion into the renal vein or into the vena cava inferior was found (in 43 men and 12 women). In 38 patients the tumour with thrombus in the vena cava inferior was on the right side, in 17 the tumour of the left kidney was removed. The proportion of the right side to the left was 2.2:1. The surgical treatment of tumorous thrombosis depends on the upper border line of the tumorous thrombus. The tumorous thrombus in the subhepatic segment of the vena cava inferior can be treated by its massaging into the renal vein after the obliteration of the artery or by the parietal resection of the vena cava inferior. The resection of the vena cava inferior was performed in 32 patients. In 5 patients the tumorous invasion could not be treated because of extensive metastases (into the liver and the lung) or there was a tumorous parietal infiltration of the vena cava inferior. Evaluating the whole group after 30 years inclusive of the Navrátil's set made with our cooperation, there were 1,005 patients suffering from renal tumours, operated on at the Department of Urology in Hradec Králové. In 155 (15.4%) tumorous thrombi were surgically removed from the great veins. In 38 cases there was a demarcation of tumorous thrombi to the renal vein but in 117 the thrombus penetrated into the vena cava inferior.(ABSTRACT TRUNCATED AT 250 WORDS)

[Essential fatty acids in neonates at risk and in the mother's milk].

The participation of essential fatty acids and their derivatives in the blood (serum) of physiological healthy neonates (physiological pregnancy, physiological delivery) is compared with the spectrum of essential fatty acids of risk newborns (prematurity, prematurity with the tocolytic therapy and hypotrophic newborns). In the following part the presence and participation of essential fatty acids in the human maternal milk were measured. The milk of healthy mothers, of mothers with preterm delivery (37 week of gestation), of diabetic mothers were mutually compared and finally the values of mentioned essential fatty acids in formula Feminar and in the milk from the bank were established. As a consequence of our observations, they point out the absence or serious deficit of some essential fatty acids in certain groups of risk newborns as well as in the milk of diabetic mothers, of mothers with preterm delivery and in Feminar and Milk-bank. The presented facts--after their opinion--could be well judged as a rational basis for adequate nutritional therapy in risk newborns.

[Conn's syndrome in tumors of the adrenal cortex].

During the period from Jan. 1 1989 to June 30 1992 at the Urological Clinic in Hradec Králové three women with cortical adenomas were treated, the tumour being the cause of primary hyperaldosteronism. The age of the patients was 25-40 years, in one woman the tumour was on the right side, in two on the left side. All patients complained of marked muscular weakness, fatigue, one woman developed cramps of the hands when working and another patient was repeatedly hospitalized at the neurological department on account of attacks of weakness. In one patient the interval from the onset of complaints to assessment of the diagnosis was 23 years. All patients had complete examinations at specialized departments before operation, in all the diagnosis was established by biochemical examinations and CT. In all instances minor tumours--diameter 2-3 cm--were involved which were easily removed by surgery. The postoperative course in all patients was without complications.

[Extra-adrenal pheochromocytoma].

In the course of 35 years at the Urological Clinic in Hradec Hrálové 44 pheochromocytomas were operated incl. 16 extraadrenal ones (36%). Eight men and 7 women were operated. In five men and seven women a primary extraadrenal tumour was involved. In three men and the woman a relapse of an extraadrenal pheochromocytoma was operated. The mean age of the whole group of 12 patients operated on account of a relapse of a paraganglioma was 44 years. The time interval after the primary tumour was 3-16 years. Surgically six paragangliomas on the right side and 10 on the left side were operated. The paragangliomas on the right side were at the following sites: three behind the inferior vena cava, one tumour was a multiple one and two tumours were in front of the inferior vena cava. On the left side were two tumours above the bifurcation of the aorta, five were paraaortal and one tumour was on the aorta at the insertion of two renal arteries. Three tumours were in the hilus of the left kidney.

[Tumor thrombosis of the inferior vena cava--personal experience with surgical therapy].

Patients treated at the Urological Clinic in Hradec Králové on account of carcinoma of the kidney are systematically followed up since 1962. In the submitted paper the authors evaluate patients treated during 1985-1991. During the mentioned period at the Urological Clinic in Hradec Králové 365 patients with carcinoma of the kidney were treated. In 55 (15%) invasion into the renal or inferior vena cava was proved (in 43 men and 12 women). In 38 patients the tumour with a thrombus of the inferior vena cava was on the right side, in 17 a tumour of the left kidney was removed. The right to left ratio is 2.2:1. Surgery of tumourous thrombosis depends on the level of the upper margin of the tumourous thrombus. A tumourous thrombus in the subhepatic part of the inferior vena cava can be treated either by massaging into the renal vein after obliteration of the artery or by parietal resection of the inferior vena cava. In 18 patients the authors were able to remove the thrombus by massaging into the renal vein. In 32 patients the inferior vena cava was resected. In five the tumourous invasion was not resolved, either because of extensive secondaries (in the liver and lungs) or because of direct tumourous infiltration of the wall of the inferior vena cava.(ABSTRACT TRUNCATED AT 250 WORDS)

[Fatty acids in the serum of hypotrophic neonates].

Using gas chromatography, Carlo Erba 2531 measurements were performed and the composition of serum fatty acids in 9 hypotrophic human newborns (the mean birth = 2184 g, the mean gestation age = 39 weeks) was evaluated. Fatty acids were detected as methylesters and their levels were compared with fatty acids levels of control healthy neonates born at term (n = 32). In the hypotrophic newborns the content of saturated fatty acids (short and medium-chain in particular) significantly increased, the total amount of polyunsaturated fatty acids (PUFA) series n-3 and n-6 significantly decreased and the proportion of arachidonic acid remained unchanged in comparison with the control group.

[The effect of fish oil on the secretion and effect of insulin in patients with type II diabetes].

The metabolic effect of 3-week dietary supplementation with a fish oil concentrate was examined in not markedly obese, not hypertriglyceridemic men with non-insulin-dependent diabetes mellitus (NIDDM) treated with hypoglycemic agents. Ten patients were given 15 ml/d of fish oil (Martens Oil, Norway) equivalent to 3.1 g of n-3 fatty acid (FA) per day, and compared to 10 diabetics treated with placebo (15 ml/d saline). While fish oil leads to expected increase in the ratio of n-3 to n-6 FA intake, it does not alter fasting and mixed meal stimulated blood glucose, plasma insulin and C-peptide concentrations. There were no changes in insulin action estimated by the metabolic clearance rates of glucose at plasma insulin levels of about 100 microU/ml and 1400 microU/ml during hyperinsulinemic isoglycemic clamp, and no changes were seen in insulin binding to erythrocytes. Even though our short-term study does not warrant authoritative conclusions, no adverse effects of low-dose fish oil on glucose homeostasis have been found in not markedly obese NIDDM patients treated with oral hypoglycemics.

Hypoxia and fatty acids in immature nervous tissue in the rat.

In experiments on 2-day-old rats (Wistar strain, our own breed), we studied the effect of altitude hypoxia (9000 m, 60 min) on the proportion of individual fatty acids in the brain (the cortex+the diencephalon+the cranial third of the mesencephalon). We found that hypoxia significantly altered the proportion of the various fatty acids, with a significant increase in the proportion of group n-3 polyenoic fatty acids at the expense of saturated and monoenoic acids. The results fully confirm the conception that one of the most important mechanisms responsible for the high resistance of new born mammals to oxygen deficiency is the ability of immature nervous tissue to activate, in particular, elongation (the elongation of fatty acids) and/or lipogenetic processes.

Surgical treatment of vesico-renal reflux in children aged up to 3 years.

A total of 36 children age-ranged from 2 up to 34 mos (17 boys and 19 girls) with previous surgery for the vesico-renal reflux has been evaluated. This disorder had most commonly been classified to be of higher degree (IV. and V. 45%) and often showed no response on repeated conservative treatment with even stronger antibiotics. The boys (27 ureters) and girls (31 ureters) underwent the 32 antireflux operations after Anderson-Glenn and those 26 after Politano-Leadbetter, respectively. In 6 ureters, their resection and modelling after Hendren was necessary to be done. On post-operative check-out, the 6 post-antireflux Anderson-Glenn's plastic ureters revealed on miction cystography the persistent vesico-renal reflux re-operated subsequently in accord with Politano-Leadbetter. From them, in 2 ureters the post-operative stricture has occurred in the site of ureter-to-bladder healing that was transitorily secured with percutaneous nephrostomy.

[A vesico-sigmoid fistula in a 15-year-old girl].

The authors diagnosed in a fifteen-year-old girl. other wise healthy, a double vesicosigmoidal fistula. It developed probably at the age of two after an acute abdominal attack of inflammatory character which was treated conservatively. The fistulae caused repeated inflammations of the urinary pathways. In the late diagnosis participated among others evaluation of miction cystography which was imperfect from the technical aspect. When repeated, this examination contributed to the diagnosis. After surgery the girl recovered.

Effect of acute hyperinsulinemia on fatty acid composition of serum lipids in non-insulin-dependent diabetics and healthy men.

The fatty acid pattern of serum phospholipids, cholesteryl esters, triglycerides and free fatty acids was measured before and after a 5-h two-step euglycemic hyperinsulinemic clamp (75 and 1400 microU/ml) in 21 non-insulin-dependent diabetics and 14 age-, weight-, and sex-matched healthy controls. Acute hyperinsulinemia was associated with a statistically significant increase in essential fatty acid and a decrease in non-essential fatty acid contents in triglycerides while the levels of serum triglycerides and free fatty acids dropped in both groups. The fatty acid composition of phospholipids and cholesteryl esters remained unchanged as did the levels of serum phospholipids, total cholesterol and HDL cholesterol.

Fatty acid composition of serum lipids and erythrocyte membranes in type 2 (non-insulin-dependent) diabetic men.

The fatty acid (FA) composition of serum lipids and erythrocytes was studied in 21 men with non-insulin-dependent diabetes mellitus (NIDDM) and in 14 normal subjects matched for age, sex, body weight, and dietary intake. Lower levels of linoleic acid and higher levels of highly unsaturated FA (daughter) of n-3 and n-6 family FA, reflected in a higher unsaturation index, were found in serum phospholipids (S-PL), in phospholipids of erythrocyte membranes (ery-PL), and in serum cholesterolesters (S-CHE). The unsaturation index of serum phospholipids significantly correlated with glycosylated hemoglobin A1c (P less than .05) and blood glucose levels after glucose load (P less than .001). The results suggest that elongation and desaturation of essential FA (linoleic acid in particular) are increased. The above changes may be associated with accelerated atherosclerosis in type 2 diabetics.

[Treatment of patients with clinical stage I and IIA nonseminomas--15 years' personal experience].

The author evaluates experience with the treatment of patients with non seminomas in the Ist and IInd clinical stage treated at the Urological Clinic in Hradec Králové in 1975-1989. In a total of 90 patients chemotherapy was administered as primary treatment in 29, in 86 retroperitoneal lymphadenectomy was performed and adjuvant chemotherapy was administered to 49 patients. Six patients were irradiated. The author advocates retroperitoneal lymphadenectomy and gives the reasons for this view. He evaluates retroperitoneal lymphadenectomy which is important for the reliable assessment of the clinical stage.

[Treatment of patients with clinical stage IIB and IIC nonseminomas-- 15 years' personal experience].

The author evaluates the experience with treatment of patients with nonseminomas stage II B and II C treated at the Urological Clinic in Hradec Králové in 1975 to 1989. In a group of 55 patients initial chemotherapy was administered in 42, in 53 retroperitoneal lymphadenecomy was performed (95.3%), to 47 adjuvant chemotherapy was administered and 11 patients were irradiated. In 47 specimens collected during retroperitoneal lymphadenectomy for histological examination in 26 a vital tumour was detected, in one instance a mature teratoma and in 20 preparations necrosis of the tumour and fibrosis was revealed. The author who in the initial stage advocated surgical reduction of the tumourous mass later abandoned this concept after introduction of cisplatinum into combined chemotherapy and now he uses on principle initial chemotherapy followed by revision of retroperitoneal nodes.

[Treatment of patients with clinical stage III and IV nonseminomas-- 15 years' experience].

At the Urological Clinic in Hradec Králové in 1975-1989 a total of 182 patients with germ cell tumours of the testis--nonseminomas were treated. Of these patients 37 were in stage III-IV (20%). In 27 (72.9%) initial chemotherapy was administered, since 1980 combined with cisplatinum, as outlined. In 20 patients also revision of retroperitoneal nodes (54%) was made, to 19 patients adjuvant chemotherapy was administered (5th and 6th cycle). In 12 patients irradiation was indicated (32.4%). Histological examination of the preparation removed from the retroperitoneum was implemented in 15 patients: in four a vital tumour was revealed, in two a mature teratoma and in nine fibrosis or necrosis of a metastatic tumour was found. In seven patient a relapse of the disease occurred, 14 patients survive for 1-15 years. Twenty-three patients died.
