[Sex life and fertility after comprehensive treatment of malignant testicular tumors].

The author evaluates a group of 96 patients treated on account of malignant tumours of the testicle. From the psychosocial aspect it may be assumed that no change in the relationship with the partner occurred, the marital bond was rather reinforced, the percentage of failing marriages is substantially lower than in the normal population. Of 96 men where inquiries were made during the investigation period of 1-15 years only two men were divorced. As regards sexual life no problems arose. In the foreground is inadequate ejaculation of seminal fluid mentioned practically by 50% of the patients where inquiries were made. This is associated with impossibility of conception. Inferiority complexes are not frequent, 81.2% of the men do not suffer from inferiority complexes and the remaining 18% suffer from an inferiority complex associated with the loss of the testicle or inadequate ejaculation of semen.

[The fatty acid spectrum in the serum of mothers and infants after premature delivery].

Using the method of gas chromatography on a Carlo Erba 2531 apparatus in Base's modification, the authors identified serum fatty acids of mothers (n = 23) and premature infants (17 girls and 6 boys). The mean birth weight was 2224.3 (800-2650 g), the mean gestation age 34 weeks (28-37 weeks). The fatty acid levels were compared with fatty acid levels of control mothers (n = 7) and control healthy neonates born at term (n = 15). Individual fatty acids were expressed as the ratio in the total volume of fatty acids (100%). The neonates and their mothers were divided into three groups. The most numerous group (n = 12) are mothers who lost the amniotic fluid prematurely (6 hours-12 days), mothers under the influence of Partusisten (n = 10); the third group is formed by mothers treated with ATB (n = 8). In these groups the authors assessed, as compared with controls, the rise of the saturated fatty acid ratio, in particular short-chain fatty acids. The greatest increase was recorded in mothers and children under the influence of Partusisten. The ratio of mono- and polyunsaturated n-6 fatty acids was not altered in the infants, there was, however, a marked reduction of the n-3 fatty acid ratio which are known to play an important role in the regulation of immune responses and to have an anti-inflammatory effect; they are also important for the development of the CNS and retina.(ABSTRACT TRUNCATED AT 250 WORDS)

[Aneurysms of the renal artery in childhood].

The authors describe the case-history of a nine-year-old boy engaged in sports, suffering from serious hypertension due to an aneurysm of the renal artery of the right kidney. Vascular reconstruction was performed. They boy died, however, suddenly 20 hours after operation and on necropsy serious hypoplasia of the adrenals was revealed.

[Insulin secretion and insulin resistance in type II diabetes. Relation to fatty acid composition of serum phospholipids].

Non-obese middle-aged men (n = 21) in an early stage after manifestation of Type 2 diabetes mellitus, and free of signs of atherosclerotic complications, when compared with an age-, weight- and sex-matched control group (n = 14), were found to show: 1. a decrease in linoleic acid content and increase in polyunsaturated elongated forms of n-6 and n-3 families fatty acids in serum phospholipids; 2. fasting hyperinsulinemia and impaired dynamics of insulin secretion after glucose load; 3. insulin resistance due to both receptor and postreceptor defect. Under physiologic conditions (i.e., in the control group), the drop in linoleic acid content and the rise in saturated fatty acids were associated with increased insulin secretion, and decrease in maximal insulin action. The nature of the changes in fatty acid pattern, the decrease in linoleic acid in particular, resembles findings made in persons who died middle-aged from serious complications of atherosclerosis. Combined with the relationship between the fatty acid composition of lipids and insulin secretion and action, our findings suggest a common metabolic defect of atherosclerosis and Type 2 diabetes which is probably insulin resistance in glucose metabolism.

[Diagnosis and treatment of retrocaval ureter].

The authors present a brief characteristic of the genesis and development of a retrocaval ureter. They describe the case-history and treatment in a 3.5-year-old girl with right-sided hydronephrosis of unknown aetiology. Genetic examination revealed Turner's syndrome. The anomaly was diagnosed on operation. Resection and antiposition of the ureter were performed and suture of pyeloureteroanastomosis using the method Anderson-Hynes.

[Introduction to the problem of markers in urologic oncology].

In the introduction the author explains the term marker and its synthesis. Demands on markers are as follows: 1. strict specificity, 2. correlation with the clinical stage, 3. prognostic information. The authors discusses in more detail the problem of human choriogonadotropin, alpha-fetoprotein, basic fetoprotein and specific beta-1-glycoprotein in germinal testicular tumours. The author mentions placental alkaline phosphatase, ferritin, lactic dehydrogenase in the above tumours, in carcinoma of the prostate prostatic specific antigen, in carcinoma of the bladder oncofoetal antigens.

Proportion of individual fatty acids in the non-esterified (free) fatty acid (FFA) fraction in the serum of laboratory rats of different ages.

In experiments on Wistar strain rats of both sexes, aged 5, 10 and 14 days and adult (90-120 days), of their own breed, the authors determined the quantitative proportion of individual fatty acids in the serum free non-esterified fatty acid (FFA) fraction, using mixed blood (obtained by decapitation) and the titration method of Trout et al. (1960). The proportion of the individual fatty acids was then determined in this fraction by gas chromatography (Base 1978) and their concentration (in mumol.1-1) was determined by simple calculation from the relative chromatogram data. Animals in the first three age groups were killed in the morning, directly from the nest; in adult rats the FFA fraction was measured after a 20 h fast. It was demonstrated that the increase in the proportion of monoenoic acids was highly statistically significant (about fivefold) during ontogenesis and that there was also a marked increase in the quantitative expression of polyenic acids, especially in group (n-6). The n-6/n-3 acid index in the FFA fraction altered during maturation (despite some fluctuation it basically rose from 4.3 in 5-day-old young to 10.0 in adult rats). It was further demonstrated that the concentration of fatty acids with a very short chain fell significantly during development, so that C 8:0, for example, could be detected only in the first two age groups, but not in 14-day-old and adult rats. The concentration of the saturated fatty acids C 15:0 to C 18:0 in the serum FFA fraction showed a statistically significant increase, while the index expressing the ratio of saturated to unsaturated fatty acids displayed a downward trend during development.

[The effect of repeated fasting on the ratio of individual fatty acids in the brain and blood of rats of various ages].

In locally bred, 5-day old and full grown rats, the authors conducted experiments where they investigated the influence of repeated fasting (3 X 18 hours food deprivation on three consecutive days) on a wide spectrum of fatty acids in the cerebral cortex, medulla oblongata and serum. The fatty acids were assessed as methyl esters by gas chromatography, as described in previous work (1, 13, 14, 17). In 5-day old rats after repeated fasting an evident decline in the ratio of saturated fatty acids in serum was recorded; at the same time the ratio of arachidonic acid increased markedly and in the cerebral cortex there was a significant drop of the palmitic acid participation. In adult rats 16 statistically significant changes took place (treble as compared with the 5-day old rats). The large number of significant changes pertained to 18 carbon fatty acid (a rise in all instances in the brain; the ratio of 18:3n-6 in plasma declined significantly). In the serum of adult rats the ratio of arachidonic acid increased also. The authors discuss possible reasons of the described changes.

Insulin secretion and insulin action related to the serum phospholipid fatty acid pattern in healthy men.

In order to decide whether the phospholipid fatty acid pattern is related to variables determining glucose tolerance, 11 healthy volunteers with normal glucose tolerance were studied. The relationship was evaluated between the proportions of individual fatty acids (FA) in serum phospholipids and (1) insulin secretion, determined by fasting and postglucose plasma insulin levels, and (2) in vivo insulin action, assessed as metabolic clearance rates of glucose during euglycemic clamp studies at two insulin concentrations of approximately 70 microU/mL (MCRglu70) and 500 microU/mL (MCRglu500). It was found that both insulin secretion and insulin action are significantly related to the ratio of omega-6 class essential FA to saturated FA in serum phospholipids. An increase of this ratio is associated with a decrease in total insulin response (r = -0.84, P less than .01), and an increase in MCRglu70 (r = .66, P less than .05) and MCRglu500 (r = .82, P less than .01). The data presented support the hypothesis that phospholipid FA composition might play a role in blood glucose regulation.

[Monitoring clinical stages of testicular tumors using markers].

The authors evaluate a group of patients with a germinal testicular tumour treated at the urological clinic in Hradec Králové in 1975-1987. The group of 212 patients comprised 60 with seminomas, 152 with non-seminomas. Of these 152 138 men were examined for HCG and 84 for AFP. According to different diagnoses and the clinical stage the authors analyze in detail the assessed values of HCG and AFP. Both markers were on average elevated by 30 to 40%, only in patients with teratocarcinoma stage B 3 AFP is elevated in 100% of the men. The findings are consistent with data in the literature.

Erythrocyte insulin receptor characteristics and erythrocyte membrane lipid composition in healthy men.

The cell membrane plays an important role in the mechanism of insulin action. To test whether erythrocyte insulin receptor characteristics are related to the erythrocyte membrane lipid composition, 11 healthy volunteers were studied. The relationship between insulin binding to erythrocytes, the number of receptors per cell and the affinity of receptors to insulin on the one hand and total phospholipid fatty acid (FA) composition and cholesterol/phospholipid molar ratio in the erythrocyte membrane on the other hand were evaluated. 1. We found a significant negative correlation between specific insulin binding and the proportion of n-6 essential FA in erythrocyte membrane phospholipids, especially linoleic acid (r = -0.82, p less than 0.01) and arachidonic acid (r = -0.73, p less than 0.05). On the other hand, a significant positive correlation between insulin binding and the proportion of nonessential FA (r = +0.65, p less than 0.05) was seen. Number of receptors per cell and the affinity of receptors were not significantly related to phospholipid FA composition. 2. There was no significant correlation between insulin receptor characteristics and the cholesterol/phospholipid molar ratio in the erythrocyte membrane. The data presented support the hypothesis that the FA pattern of membrane total phospholipids may modify the properties of insulin receptors.

[Extratesticular germinal cell tumors].

In a group of 212 patients with germinal tumours of the testicle treated at the urological clinic in Hradec Králové in 1975-1987 there were 7 with an extragonadal tumour in the retroperitoneum. In all the dominating symptom was a palpable resistance in the epigastrium, backache and secondary signs of malignant disease. In the first four a radical extirpation of the tumour was performed, in one with replacement of the aorta always with left nephrectomy. After introduction of cisplatinum into combined chemotherapy this became the initial treatment. Of the seven patients two died from generalization of the basic disease, the third one from exsanguination through an erosion of an aortal graft.

Determination of fatty acids as phenacyl esters in rat adipose tissue and blood vessel walls by high-performance liquid chromatography.

Twenty-two biologically relevant (6:0-22:6) saturated, monounsaturated and polyunsaturated fatty acids were separated by reversed-phase high-performance liquid chromatography after derivatization with phenacyl bromide. An optimal resolution of the critical combinations linolenic-myristic, docosahexaenoic-palmitoleic-arachidonic and palmitic-oleic acids and cis and trans isomers of octadecenoic (n9) and octadecadienoic (n9, 12) acids was achieved by continuous gradient elution with methanol-acetonitrile-water. Elution of mixtures of 6:0-22:1 fatty acids was completed within 80 min at a flow-rate of 1 ml/min. By the use of UV detection at 242 nm the detection limits for short- and long-chain fatty acids were found to be about 0.8 and 12 ng per injection, respectively. Linearity was tested up to 100 ng. The method was applied to the determination of fatty acids in rat adipose tissue and blood vessel walls of animals fed hydrogenated fat diets. The results are comparable to those obtained by gas chromatography and surpass the latter for the resolution of oleic and elaidic acids.
