BACKGROUND: Cause-of-death statistics are widely used for comparing health characteristics of European Community (EC) countries. Before attempting to interpret between-country differences, it is essential to assess the biases affecting the comparability of the data. EUROSTAT decided to address globally this problem with the objective to improve the quality and comparability of cause-of-death data within the EC. METHODS: The material is based on a review of results of international comparative cause-of-death studies and on specific inquiries among EC. Both cause-of-death certification and codification practices are analysed. Certification is studied comparing the models of death certificates, the type of information captured, certifiers training and querying practices. The different coding systems are analysed (International classification of diseases (ICD) in use, interpretation of the ICD rules, implementation of automated coding systems). RESULTS: International studies on comparability of certification and coding practices between countries are rare. These studies are based on certification of cases histories and recoding of samples of death certificates. Recent studies on respiratory diseases, cancers and diabetes outline differences that influenced on the reported level of mortality. The specific EUROSTAT investigation (1997) outline general discrepancies: models of death certificates, nature and amount of information entered, way to establish the diagnosis, degree of consistency of the certification process, autopsy practices, certifiers practices, implementation of ICD-10 and implementation of automated coding systems. CONCLUSION: EUROSTAT studies are now focused on causes of death requiring special attention for comparability (e.g. suicide, accidental deaths, drug and alcohol related deaths, unknown and ill-defined causes), on procedures to improve the homogeneity of certifiers training and querying practices, on the effect of the transition to ICD-10. The international model of death certificate recommended by the World Health Organization should be adopted as widely as possible. Uniform complementary information (e.g. surgery, pregnancy, autopsy, place of occurrence of accidental deaths, work accident) should also be adopted. The EUROSTAT investigations must result in definitions of common recommendations and guidelines to EC.
The recently published "Survival and Ventricular Enlargement" (SAVE), prospective controlled trial over 4 years including over 2,000 patients, has shown that long-term treatment with captopril reduced cardiovascular mortality (- 19%) and morbidity in patients with post-infarction left ventricular dysfunction without signs of cardiac failure. Based on the trial data, the extra cost of treating the patients with captopril compared with the placebo branch is estimated at 9.3 million french francs. This expense is compensated by a reduction in the cost of hospitalisation for cardiac failure of 1.6 million francs and a reduction in the cost of further coronary events of 3.4 million francs. Above all, the extra cost is compensated by a gain of 47 deaths avoided, corresponding to about 493.5 years of life saved. The cost-effectiveness of captopril administration is therefore about 8.750 francs per year per life saved. The ratio is significantly less than that of other primary or secondary prevention therapeutic strategies. On this basis, we conclude that captopril therapy of high risk post-infarction patients is a solution that is not only medically effective but also economically sound.

Twenty-one patients were enrolled in a pilot study in order to evaluate the clinical efficacy of the GnRH analogues in the treatment of hormone dependent gynaecological malignancies. Seven patients with recurrent or advanced endometrial adenocarcinoma and fourteen patients with ovarian carcinoma had a 4-weekly subcutaneous injection in the abdominal wall of 3.6 mg goserelin (Zoladex--ICI Pharmaceuticals). Overall, four of the 21 patients showed regression, nine had their disease stabilised and eight showed progression. No important side effects were noticed.
In order to improve the results of hormonal therapy of gynecologic malignancies by means of medroxyprogesterone acetate two approaches are explored. In the first approach medroxyprogesterone dosage increased up to 400 mg a day, achieved better results in curative as well as adjuvant treatment of advanced or early gynecologic cancers. In the other approach, the combined, simultaneous or successive treatment by means of tamoxifen and medroxyprogesterone was even more successful for these therapeutic purposes compared to monohormonotherapy.
Serum squamous cell carcinoma antigen (SCCa) concentrations were determined by a radioimmunoassay kit before and during the treatment of 50 patients with cervical carcinoma: 44 with squamous cell carcinoma (SCC) and 6 with adenocarcinoma. The positivity rate of SCCa was 50% (52% for SCC and 33% for adenocarcinoma). The sensitivity of SCCa for SCC was twice as high as that of CEA and CA-125. Low serum concentrations were observed in early-stage carcinoma, indicating that SCCa is not useful for diagnosis. In advanced cases, serum levels were directly and significantly correlated with the stage of the disease.
Computed tomography can assist the surgeon in planning the exact three-dimensional positioning of endosseous implants in the jaw bone. This study examined the reliability of three CT techniques: direct imaging (normal coronal and sagittal slices), standard reconstruction (based on axial slices), and multiplanar reconstruction and display (also based on axial slices). The three imaging techniques were used to estimate the bone height, bone width, and maximal implant length in six dissected human jaw bones. These jaws were then sawed perpendicular to their axis to enable measurement of real values for comparison. The sagittal and coronal slices frequently were overestimated, especially in the canine and premolar regions. The mean absolute deviation was 1.4 mm. Standard reconstruction offered the most reliable cross-sectional images, with a mean absolute deviation of 0.5 mm. The multiplanar reconstruction and display technique frequently demonstrated underestimations (mean absolute deviation was 2.3 mm). The standard reconstruction technique seems to be the method of choice in preoperative radiographic examination of patients before implant placement.

The effects of sodium ascorbate (vitamin C) and 2-methyl-1,4-naphthoquinone (vitamin K3) administered separately or in combination on the in vitro cultured human neoplastic cell lines MCF-7 (breast carcinoma), KB (oral epidermoid carcinoma), and AN3-CA (endometrial adenocarcinoma) have been examined. When given separately, vitamin C or K3 had a growth inhibiting action only at high concentrations (5.10(3) mumol/1 and 10(5) nmol/l, respectively). Combined administration of both vitamins demonstrated a synergistic inhibition of cell growth at 10 to 50 times lower concentrations. At this level separately given vitamins are not toxic. The sensitivity to this treatment was somewhat different in the three cell lines, being slightly higher for KB line. This tumor cell growth inhibitory effect was completely suppressed by the addition of catalase to the culture medium containing vitamins C and K3, suggesting an excessive production of hydrogen peroxide as being implied in mechanisms responsible for the above-mentioned effects.
156 abdominal preparations were explored by arteriography, corrosion and dissection. The stomach is vascularized by four well-anastomosed main arteries: the arteria gastrica dextra and sinistra and the arteria gastroepiploica dextra and sinistra. Other important vessels include: the arteria gastroduodenalis, the arteriae gastricae breves for the upper half of the greater curvature, a posterior gastric artery (36%), an accessory left gastric artery (12%) and an arteria supraduodenalis. The main vessels give rise to some very specific collaterals, e.g. the omental arteries that may form an omental arcade (44%), supra- and infra-pyloric branches, retroduodenal branches, rami cardiaci, esophagei and tuberales and an accessory left hepatic artery. The gastroduodenal artery always arises at a fixed point.

The value of bipedal lymphography and computed tomography (CT) in the presurgical staging of early carcinoma of the uterine cervix was analysed in 62 patients by histologic examination of pelvic lymph nodes and parametrial extension at the time of surgery. Macroscopic invasion of the lymph nodes was detected in 33% of the patients by CT and in 17% by lymphography. Microscopic invasion was suspected in 9% by CT scan and in 38% by lymphography. Physical examination with measurement of the tumor diameter was more predictive for regional extension of the disease as compared to lymphography and CT. CT may detect subclinical parametrial invasion but overestimation of the extent of the disease is common. Bipedal lymphography and CT add only limited information to the routine presurgical staging of cancer of the uterine cervix.

156 abdominal preparations were explored by arteriography, corrosion and dissection. Within the meanders of the splenic artery a basic pattern can be traced. The division of the artery can easily be summarized by introducing the term of a truncus lienogastroepiploicus. The short gastric arteries are always present, and usually they are longer and more important than may appear from their name. In nearly half of the preparations (52%) an artery for the extremitas lienalis posterior is found; an artery for the extremitas lienalis anterior is very exceptional. A posterior gastric artery can be identified (36%) but it must be distinguished from several other vessels. The arteria colli pancreatis (for the neck of the pancreas) is a constant vessel that may have a superior (usually arteria lienalis) or an inferior origin (arteria mesenterica superior). It commonly supplies the transverse pancreatic artery and often the prepancreatic arcade. The arteria corporis pancreatis, too, is a constant artery. The pancreatic tail is thoroughly irrigated by usually more than one arteria caudae pancreatis. An important part of the greater curvature is not accompanied by the left gastroepiploic artery, but is supplied by the arteriae gastricae breves.
