Modalities and results of a combined anti-estrogenic therapy by means of tamoxifen and medroxyprogesterone in gynecologic cancerology.

In the therapeutic schemes of the hormone-dependent gynecologic cancers, the combined administration of tamoxifen and medroxyprogesterone, generally, scores better results. The highly hormone-dependent tumors (ER+, PgR+) respond very well to a combined, simultaneous treatment. The strictly hormone-dependent (ER+, PgR-) and the potentially hormone-dependent (ER-, PgR-) tumors seem to be most efficiently treated by means of a combined, successive scheme.

The branches of the celiac trunk.

156 abdominal preparations were explored by arteriography, corrosion and dissection. Classification of the celiac trunk becomes easy if one considers the trunk to be composed of three main stems: the splenic, the hepatic and the left gastric artery, other vessels being less important collaterals. Usually, the celiac trunk bifurcates into the splenic and the hepatic artery; the left gastric artery is a mobile vessel whose origin may slide between the aorta, all over the celiac trunk up to a trifurcation. The direction of the celiac trunk is influenced by the topography of the pancreatic neck and by the origin of the hepatic artery: if the celiac trunk is not the origin of the hepatic artery it is not directed to the right but to the left. It is the hepatic artery that pulls the celiac trunk to the right. Variations in the origin of the splenic artery are exceptional. In contrast to adults, the diameter of the hepatic artery in young children is larger than that of the splenic artery.

The endometrial adenocarcinoma as a model for hormone-dependency and hormone-responsiveness of gynaecological cancers.

The endometrial adenocarcinoma is studied as a model for the hormone-dependency of gynaecological cancers to estrogens and for their hormone-responsiveness to progestogens and anti-estrogens. The evident direct and the possible indirect mechanisms of action of medroxyprogesterone and tamoxifen on the endometrial adenocarcinoma are elucidated in vitro as well as in vivo with special regard to histological, histochemical, cytological, biochemical, endocrinological and clinical data. The modalities, side-effects and monitoring of the treatment by means of medroxyprogesterone and tamoxifen in early and recurrent or metastatic endometrial adenocarcinoma are discussed. Finally, new perspectives are opened on combined anti-estrogen + progestogen treatment of all gynaecological malignancies, with special regard to the cytosol steroid receptor concentrations and to the bioavailability of progestogens and anti-estrogens.

Arcus paracolicus.

The paracolic arcade is the most important vessel for the blood supply of the colon. It can be compared with the felly of a wheel, whose spokes are the colic arteries. It is the immediate source of the colic supply and the most important way for collateral circulation between the colic arteries and even between the superior and inferior mesenteric arteries.

Influence of dose rate on fast neutron OER and biological effectiveness determined for growth inhibition in Vicia faba.

The influence of dose rate on the effectiveness of a neutron irradiation was investigated using growth inhibition in Vicia faba bean roots as biological system. d(50) + Be neutron beams produced at the cyclotron CYCLONE of the University of Louvain-la-Neuve were used, at high and low dose rate, by modifying the deuteron beam current. When decreasing the dose rate from 0.14 Gy.min-1 to 0.2 Gy.h-1, the effectiveness of the neutrons decreased down to 0.84 +/- 0.05 (dose ratio, at high and low dose rate. Dhigh/Dlow, producing equal biological effect). Control irradiations, with 60Co gamma-rays, indicated a similar reduction in effectiveness (0.84 +/- 0.03) when decreasing dose rate from 0.6 Gy.min-1 to 0.7 Gy.h-1. In previous experiments, on the same Vicia faba system, higher RBE values were observed for 252Cf neutrons, at low dose rate (RBE = 8.3), compared to different neutron beams actually used in external beam therapy (RBE = 3.2 - 3.6 for d(50) + Be, p(75) + Be and 15 MeV (d, T) neutrons). According to present results, this higher RBE has to be related to the lower energy of the 252Cf neutron spectrum (2 MeV), since the influence of dose rate was shown to be small. As far as OER is concerned, for d(50) + Be neutrons, it decreases from 1.65 +/- 0.12 to 1.59 +/- 0.09 when decreasing dose rate from 0.14 Gy.min-1 to 0.2 Gy.h-1. Control irradiations with 60Co gamma-rays have shown an OER decrease from 2.69 +/- 0.08 to 2.55 +/- 0.11 when decreasing dose rate from 0.6 Gy.min-1 to 0.7 Gy.h-1. These rather small OER reductions are within the statistical fluctuations.

Re-evaluation of the colic irrigation from the inferior mesenteric artery.

156 abdominal preparations were explored by arteriography, corrosion and dissection. The arteria mesenterica inferior (AMI) ends by bifurcating into the two arteriae rectales superiores. The key to the interpretation of the AMI is the recognition of an arteria colosigmoidea that gives off one or more rami sigmoidei. In the presence of an arteria or ramus colic. sin. access, usually from the superior mesenteric artery, the left colic artery is absent, atrophic or displaced. The sigmoid branches (usually three) arise from the colosigmoid, the left colic or the distal portion of the AMI. Usually, the last sigmoid artery gives branch to the rectosigmoid colon. The rectosigmoid artery arises from the AMI between arteria sigmoidea ima and the terminal bifurcation of the former. It may be replaced by the descending branch of the a. sigmoidea ima. They irrigate an extensive part of the anterior wall of the bowel.

A new look at the blood supply of the ileocolic angle.

87 ileocolic preparations are studied by means of postmortem arteriography combined with dissection and corrosion. The ileocolic stem is to be continued beyond the colic and ileal branches up to its division into the caecal branches. This allows to distinguish a long ileocolic trunk which is the direct origin of the different tributaries and which ends into one or both caecal arteries. An arteria colica dextra from the ileocolic artery cannot be supported. Many rules are established that govern the relations between the various collaterals, and several conclusions of theoretical and practical importance are made.

[Correlation between cell fluorescence for estrogen receptors and the hormone-dependence of breast cancer].

In 27 postmenopaused patients cellular ER has been assessed after fine needle aspiration by estimation of the cytoplasmic or nuclear fluorescence intensity after incubation with 27 beta-estradiol-6-carboxymethyl oxime-bovine serum albumin-fluorescin isothiocyanate. The cellular changes in the fluorescence intensity and tumor size after a 7 days treatment with Tamoxifen (TMX) have been studied. These factors have been correlated with the anatomoclinical factors. Three different groups of patients can be distinguished; in the first group of 10 patients with predominantly cytoplasmic ER fluorescence before treatment with TMX, tumor reduction and disappearance of the cytoplasmic ER fluorescence are observed in a group with the best anatomoclinical factors. In a second group of 7 patients moderate cytoplasmic and some nuclear ER fluorescence is observed. After the 7 days treatment with TMX tumor reduction is found in cases with diminished nuclear ER fluorescence, on the contrary an increase in nuclear ER fluorescence is found in fast growing tumors, with stationary tumor size after 7 days TMX treatment. A third group of 10 patients displays only nuclear ER fluorescence. After the 7 days treatment with TMX, no change occurred in tumor size, with increase of the nuclear ER fluorescence in 8 patients with fast growing tumors, in 2 patients the diminished nuclear fluorescence was accompanied by a decrease in tumor size.

A field study of the validity of static paper sampling in fluoride pollution surveys.

A field study of fluoride pollution and of its consequences was made over a period of five years in the vicinity of an alumina reduction plant. This study, based upon the use of a static soda impregnated paper sampler, shows that the results obtained are in good agreement with the atmospheric fluoride concentrations obtained with dynamic samplers, especially when fluoride is present in gaseous form (HF). The results suggest that vertically mounted paper samplers are less sensitive to the collection of particulate fluorides. Collection rate is strongly influenced and increases with speed. If this parameter is known, an accurate estimation of the absolute atmospheric concentration is possible; the values shown by this technique are in good correlation with the value (annual mean) of pasture fluoride content. As a correlation exists between pasture fluoride content and the symptoms shown by cattle, the static filter samplers can predict disease in the case of slow chronic fluorosis. The field study agrees with earlier laboratory experiments and shows the validity of these simple and inexpensive types of exposure methods.
