The authors examined sera of patients suffering from idiopathic infertility, unsuccessful IVF, the diagnosis of habitual abortion and endometriosis, using the test of the passive haemagglutination reaction with sheep erythrocytes sensitized by 11 antigenic fractions of pig zona pellucida, molecular weights from 13.3-114 kilodaltons. The authors analyze the assembled results with regard to individual antigenic fractions and with regard to the diagnoses. The frequency of positive reactions in patients is investigated in particular in the area of higher molecular weights, as compared with the control group of healthy fertile women.
The effect of induction of parturitions in the 1-2 weeks of pregnancy was tested as exerted on the health of calves and on some biochemical and hematological indicators. The observations concerned 26 cows, out of which 14 cows (experimental group) were applied intramuscularly 500 micrograms of synthetic analog of prostaglandin F2 of alpha- cloprostenol. The other 12 cows were a control group. These indicators were determined in the blood of calves before colostrum intake, in 24 h, 5 and 15 days from the intake of the first colostrum ration: acid-base balance, hematocrit and leucocyte counts, in the blood plasma it was concentrations of glucose, urea, Na, K, Ca, inorganic P, vitamin A and E levels and AST and GMT activities. The same range of examinations was performed in the blood and/or in the blood plasma of cows, in addition to urea screening. Blood and urea samples were taken a month before and a month after parturition. No deviations from the normal state were found by the examination of highly-pregnant cows. Afterbirth retention was found in 87% cows of the experimental group after parturition, while it was only 9.8% in the control group. All the calves in the experiment had good vitality, very good health in the course of 15-day observation. There were not any significant differences in the birth weight and weight gains of the calves of experimental and control groups.(ABSTRACT TRUNCATED AT 250 WORDS)
The annual incidence of malignant neoplasms of the ovary has increased in the Czech Republic between 1961 and 1988 from 14.3 to 17.2 newly notified cases per 100,000 women. The highest incidence, 47.03, is in the age group from 70 to 74 years. It participates in the overall mortality from malignant tumours of the genitalia by 41.6%, although in the total incidence it accounts for 25%. The lethality calculated from the incidence and mortality in the Czech Republic in similar as in other countries--79%. This is consistent with the generally reported survival period after treatment which is 20-30%. Hospital statistics report more favourable results. Introduction of new therapeutic methods so far did not lead to marked improvement of therapeutic results. The latter are influenced more by detection of the disease in early stages.
The authors present an analysis of 33 oesophageal injuries treated at the surgical clinic of Faculty Hospital Plzen in 1976-1991. Iatrogenic injuries predominated (a total of 24 cases). Of these seven developed during oesophagoscopy performed on account of dysphagic complaints caused by benign or malignant disease. Ten injuries developed in conjunction with endoscopic extraction of a foreign body, three occurred during operation of hiatus hernia, two during sclerotization of oesophageal varices and one each during insertion of a nasogastric tube and oesophageal endoprosthesis. In the remaining cases perforation occurred during traumatic injury of the neck four times, twice as a result of a swallowed bone, and three perforations were spontaneous. The total mortality was 27.3%. The authors analyze factors which influence the morbidity and mortality--the site of injury, the time which elapsed between injury and treatment, the patient's age, mode of injury, associated diseases and method of treatment. They found that the most frequent complication and most frequent cause of death was septicaemia from mediastinitis and empyema of the chest.
The authors describe the frequency, incidence, number of deaths and mortality from malignant tumours of the uterine corpus in the Czech Republic in 1961-1988. The mean incidence of 13.06 during the five-year period 1961-1965 rose to a mean incidence of 25.12 during the five-year period 1984-1988, while during this period a paradoxical decline of the mortality from 9.00 to 6.7 occurred. This implies a decline of the lethality during 1961-1965 from 76.56% to 28.98%, i.e. to 37.85% of the original value. The reasons of the decline in lethality may be earlier detection of the disease and possibly a rise in the incidence of less aggressive forms of hormone-dependent carcinomas only, because therapy did not change essentially during the mentioned period.

Subclinical ketosis of dry cows was studied as to its effect on the composition of colostrum and on the health condition and selected clinico-biochemical parameters in the new-born calves of these cows. The experimental group consisted of eight cows with ketonuria and their calves. The control group consisted of six cows without ketonuria and their new-born calves. Calves of both groups were reared in similar conditions. Colostrum samples were taken from the first milking and then from the milking 24 hours after parturition. The blood of the calves was sampled before the intake of colostrum and 24 hours after birth. The concentration of ketone bodies, particularly beta-hydroxybutyrate, was much higher in the colostrum of both groups of cows than in the blood (62.6 mg total ketone bodies per 1 litre of the first colostrum of the cows of the experimental group). The concentration of ketone bodies slightly increased after parturition. The cows with ketonuria secreted into colostrum a larger amount of oxidated ketone bodies. Colostrum quality was good in both groups. Decreased viability was not recorded during the clinical examination of the calves after birth. The level of total ketone bodies in the blood of pre-colostral calves in the experimental group was 10.4 mg per litre (a much lower concentration than in blood of their dams), and exhibited no appreciable change after taking in the colostrum. The blood of calves born to cows with ketonuria contained significantly higher amounts of oxidated ketone bodies. No correlation was demonstrated between the contents of ketone bodies in the blood of cows, in the blood of calves, and in the colostrum. The calves of the experimental group were found to have a significantly higher AST activity in plasma, as compared with the control group. Twenty-four hours after birth, the quantity of immunoglobulins in the blood serum was significantly lower in the calves of the experimental group. Thanks to good attendance and hygiene, the morbidity did not increase and no losses occurred.
The more frequent incidence of cancer of the uterine cervix and thus of precanceroses in younger age groups, the introduction of new therapeutic methods (cryotherapy, CO2 laser, modernized diathermocoagulation) call for an estimate of the histological basis of the cervical lesion and in precanceroses assessment of its grade. These data must be provided by prebioptic methods (cytology and colposcopy). This diagnosis is essential to provide optimum therapy under the given circumstances. During the two-year period of 1985-1986 at the Gynaecological Clinic in Plzen 308 women detected by cytological screening were examined who were subsequently operated on account of a cervical lesion. In the investigated group a prebioptic predicition of high reliability was made. A concordant finding was recorded in 286 women, i.e. 92.9%. False negative, i.e. so-called risk errors were made in 9 women, i.e. 2.9%. False positive errors without risk, where the only consequence was unnecessarily radical treatment, were made in 13, i.e. in 4.2% women.

The activity of the colostral trypsin inhibitor (CTI) was studied in seven milkings of colostrum in the course of the first five days after calving in 15 cows. The activity of the total trypsin inhibitor in the colostrum was ascertained in the course of the first two days after calving in ten cows in their second to fourth lactations and also in the colostrum of the first milking after calving in the case of 11 cows. The greatest CTI activity was found in the colostrum of the first milking after calving (301 micrograms X ml-1). Between the CTI activity and the concentration of total protein (CB) in the whey of the colostrum from the first and second day after calving there was found to be a statistically significant correlation (r = +0.929). The activity of total trypsin inhibitor was also highest in the colostrum from the first milking (499 micrograms X ml-1). After calving the CTI and the total trypsin inhibitor activities in the colostrum gradually dropped. Between the activity of the total trypsin inhibitor and the CB concentration in whey from the first, third, fifth and seventh milkings after birth a statistically significant correlation was found (r = +0.972). Statistically significant correlations were also found between the activity of the total trypsin inhibitor, the specific gravity of colostrum, the concentration of total protein and gamma-globulins in the whey of the colostrum from the first milking after calving. In metabolic alkalosis the concentration of total protein in the whey and the activity of trypsin inhibitor were found to be considerably lower than the average values.
Two milk feeding systems were investigated as influencing the health and development of calves. After the termination of colostrum feeding, the ten animals of the experimental group were given whole milk whereas the control group (also ten calves) was given the Laktosan milk replacer. By the age of three months, blood was collected from the calves for biochemical examination in weekly intervals, later once a month. The content of urea, determined in the blood plasma of the calves of the experimental group was significantly lower in the fourth to seventh week. The plasma levels of nitrogen, potassium, calcium and magnesium were about the same in the experimental and control groups, being within the limits of the reference values. At the age of six to nine weeks, the content of inorganic phosphorus in the blood plasma of the tested animals was statistically significantly higher. Vitamin A concentration in the blood plasma was about the same in both groups. The content of vitamin E in the blood plasma of the calves of the experimental group was statistically significantly higher in the fourth to eight week of age. No significant differences between the two groups were observed in the plasmatic activities of aspartate aminotransferase (AST) and gamma-glutamyl transferase (GMT). The activities of alkaline phosphatase (ALP) were significantly higher in the third to fifth week of life. From the fifth to eighteenth week of age, the average daily weight gains were significantly higher in the calves given whole milk.
