Malignant fibrous histiocytoma of the oral soft tissues.

Between 1944 and 1984, six patients with a malignant fibrous histiocytoma of the oral soft tissues were treated at the M.D. Anderson Hospital and Tumor Institute. From the time of the first description of this tumor until 1984, another 10 documented cases of malignant fibrous histiocytoma involving the oral soft tissues were reported in the English-language literature. With respect to these 16 patients (including our 6 cases), the clinicopathologic aspects of malignant fibrous histiocytoma, treatment, and survival figures are discussed. The treatment of choice is early and radical surgery, including neck dissection, since in four of the six patients treated at the M.D. Anderson Hospital and Tumor Institute regional lymph node metastases were found. The response to chemotherapy, except for the response of patients with the inflammatory type of malignant fibrous histiocytoma, and radiotherapy is usually poor. The 2-, 3-, and 5-year survival rates respectively, are, 36%, 30%, and 20%.

Osteomyelitis of the jaws in pycnodysostosis.

Pycnodysostosis is a rare sclerosing bone disorder in which osteomyelitis of the jaws is a frequent complication. Treatment of osteomyelitis of the jaws in pycnodysostosis is difficult and may lead to large resections. A review of the literature and 3 new cases in which osteomyelitis was successfully treated by a combined surgical and antibiotic approach, are reported.

Localized aspergillosis involving the nose and paranasal sinuses.

Two patients with non-invasive aspergillosis of the paranasal sinuses are reported. One patient had this disease in the maxillary sinus while the other had it in the ethmoid and sphenoid sinus. Caldwell-Luc and a transnasal ethmoidectomy operation were performed. The treatment and prognosis of this rare nasal infection are discussed.

[Epileptogenesis caused by the kindling effect in diving at 3 ATA of air].

The kindling effect is an experimental model of epilepsy which results from an intermittent application of focal electrical stimulations to certain regions of the brain. Generalized seizures are usually obtained after about fifteen stimulations of amygdala in the rat. We studied the kindling effect in hyperbaric-hyperoxic conditions. Eleven Wistar rats were stimulated daily in a caisson and all stimulations were delivered after 30 min of diving at 3 ATA of air. Diving inhibited kindling in half animals. In the others, which all presented generalised convulsions, diving produced a reduction in seizure duration, especially during the generalisation phase.

Radiographic interpretation of the mandibular angular cortex: a diagnostic tool in metabolic bone loss. Part II. Renal osteodystrophy.

Panoramic radiographs of patients with chronic renal failure showed a loss of cortical bone at the mandibular angle; the loss of cortical bone correlated very well with the degree of renal osteodystrophy, established by biopsy of the undecalcified iliac crest. The value of using the cortical thickness at the mandibular angle as a parameter in diagnosing metabolic bone loss is discussed.

Epidermal growth factor receptor targeting enhances adenoviral vector based suicide gene therapy of osteosarcoma.

BACKGROUND: Despite improvements in the treatment of osteosarcoma (OS) there are still too many patients who cannot benefit from current treatment modalities. Therefore, new therapeutic approaches are warranted. Here we explore the efficacy of targeted adenoviral based suicide gene therapy. METHODS AND RESULTS: Immunohistochemistry and FACS analysis detected low or absent expression levels of the primary adenovirus receptor CAR on human primary OS and human OS cell lines. These results predict a low infection efficiency and thus a reduced therapeutic effect. Targeting the adenoviruses to another receptor highly expressed on OS could overcome this limitation. We found epidermal growth factor receptor (EGFR) to be widely expressed on primary OS. Immunohistochemistry on primary tumor samples and FACS analysis on primary short-term cultures and four OS cell lines showed that EGFR was consistently expressed. The recombinant bispecific single-chain antibody 425-s11 redirects adenoviral vectors towards the EGFR. Adenovirus transduction experiments in the presence or absence of 425-s11 showed significantly enhanced gene transfer with the targeted adenoviral vector compared with the native vector (OS cell lines 2.5 to 7.2 times enhanced gene transfer and OS primary short term cultures 1.7 to 10 times enhanced gene transfer). On this basis, targeted suicide gene therapy experiments with AdCMVHSV-TK in combination with ganciclovir were performed. These experiments demonstrated up to 3.5-fold enhanced kill of OS cell lines and primary short-term cultures by the EGFR targeted vector. CONCLUSIONS: Suicide gene therapy with adenovirus targeted towards EGFR may have favorable therapeutic characteristics for future gene therapy applications in OS.

Dermolipoma: characteristic CT appearance.

Dermolipomas are choristomas usually located at the external canthus. Clinically they can be confused with dermoid cysts. Preoperative diagnosis of a dermolipoma is important because the surgical approach is less aggressive than for dermoid cysts. We describe the clinical aspects of ten patients with a dermolipoma and demonstrate the posterior extension of these lesions by CT.
