Mixed signet-ring and transitional cell carcinoma of the bladder. Report of a case.

A case of transitional cell carcinoma of the bladder displaying prominent signet-ring cell features is reported. Despite combined therapy, the patient died within a few months of the diagnosis, thus confirming the poor prognosis of this tumor. We understand this entity could be considered as an example of neoplastic-associated metaplasia and therefore propose that it should be distinguished from genuine adenocarcinoma.

Intrathoracic kidney and vertebral fusion: a model of combined misdevelopment.

We report a case of right intrathoracic kidney associated with a complex somite malformation that comprised vertebral fusion and right intrathoracic supernumerary ribs. The interpretation of this association as a disease model of nature allows us to propose a unifying hypothesis on the pathogenesis of this form of renal dystopia.

Squamous cell carcinoma of the male urethra mimicking a paravertebral abscess.

In the context of a misleading diagnosis of paraurethral abscess, we report a case of unrecognized metastatic squamous cell carcinoma of the male urethra that mimicked a picture of paravertebral abscess with accompanying massive caval vein thrombosis. Fine needle aspiration cytology of both paravertebral abscess and urethral nodule allowed the diagnosis. This report calls the attention of clinicians to think of a malignant process in case of a paravertebral abscess. Fine needle aspiration cytology once more proves a useful tool in the diagnosis of unsuspected malignancies.

Adnexal tumour of probable Wolffian origin. Report of a case.

An adnexal tumour of probable Wolffian origin occurring in a 36 year old woman is reported. The lesion was asymptomatic and incidentally discovered during laparotomy. No recurrences have been demonstrated to date. Its origin from Wolffian remnants is briefly.

[Renal carcinoma and non-Hodgkin's lymphoma. A clinical report].

Presentation of one case of a 61-year old male with a non-Hodgkin lymphoma, where a renal cell adenocarcinoma was unexpectedly discovered during and extension study. Review of the pathogenic significance of such association in the light of the various explicative hypothesis of this concurrence.

[Focal urinary Salmonella infection].

A case of focal Salmonella sp. urinary infection in an elderly diabetic patient is described herein. The therapeutic approach in this uncommon type of infection is discussed.

[Bladder carcinoma and brain metastases before systemic chemotherapy].

Clinico-pathological study of six patients with cerebral metastasis from vesical carcinoma with no prior administration of systemic chemotherapy. In two cases the symptoms of intracranial mass were the initial reason for infiltrant vesical carcinoma examination. Despite the rarity of such occurrence, the possibility of vesical tumours showing in such a way must be taken into account. The singularity of cerebral metastatic seeding throughout the natural history of a vesical neoplasia is analyzed. Also, a review is made of the factors hypothetically responsible for the increase of cerebral metastasis establishment following current chemotherapy.

[Paratesticular adenomatoid tumor: clinico-pathologic study].

Display of the clinico-pathological correlation of three cases of adenomatoid tumour (AT) of the male reproductive organs. From the anatomo-pathological point of view we observe the architectural patterns traditionally described, and discuss their controversial mesothelial origin. From the clinical point of view we highlight the benignancy of the process, often underdiagnosed, as well as its association to post-inflammatory hydrocele in one case. Such coincidence could favour the assumption that inflammation plays a decisive role on the production mechanism.

[Double renal tumor: histogenetic implications].

Presentation of a new case of synchronic concurrence of renal adenocarcinoma and papillar carcinoma of renal pelvis on the same kidney. The origin of this double malignant renal tumour remains unknown, although several hypothesis have been raised. We believe that coexistence of both tumours is more than casual and we would underscore the morphogenetic interpretation of such coincidence.

Long term survival in metastatic nephroblastoma. A case report.

We describe a case of metastatic nephroblastoma presenting as renal rupture. Histology was "favorable". Lung metastases were discovered early during follow-up. Radiotherapy and surgical excision were therefore combined with intensive chemotherapy. Ten years later, the patient is symptom-free.

Congenital mesoblastic nephroma, nephrocalcinosis, and hypertrophic pyloric stenosis.

A cellular variant of congenital mesoblastic nephroma (CMN) occurring in a newborn is presented. Coincidental findings were congenital hypertrophic pyloric stenosis (CHPS) and nephrocalcinotic hypercalcemia that reverted after nephrectomy. As of the day of writing, this multiple association has not been reported. The authors believe that both CMN and CHPS could be interpreted as fibroblastic proliferative-related entities. Transient hypercalcemia seems to be a paraneoplastic phenomenon.

Kaposi's sarcoma of the penis as an initial urological manifestation of AIDS. A report of two cases.

Kaposi's sarcoma of the penis is an initial manifestation of AIDS and the practising urologist should be aware of this form of presentation. We report 2 young intravenous drug abusers with this tumor in the male genitourinary system. The clinical course is unfavorable; therefore prompt diagnosis and management, based on the early recognition of the lesion, must be achieved.

Acquired Immunodeficiency Syndrome

Metastasising carcinoma of the urinary bladder presenting as a retro-orbital mass. Case report.

A 61-year-old man presented with macroscopic haematuria. Infiltrating transitional cell carcinoma of the urinary bladder was diagnosed after transurethral resection, and he was treated first with three doses of cisplatinum (100 mg/m2 at three week intervals) and then by radical cystectomy. Eleven months later he complained of progressive diplopia, which was found on computed tomography to be caused by a retro-orbital metastatic mass. There was no evidence of a space occupying lesion in the brain or of other metastases. The patient died four weeks later and permission for necropsy was refused. Despite the fact that carcinoma of the urinary bladder rarely metastasis in this way, urologists should be aware that it can happen.

Hydrops fetalis and congenital mesoblastic nephroma.

The autopsy finding of congenital mesoblastic nephroma in a stillborn hydropic fetus is reported. We discuss the pathogenetic relation of both entities and summarize the optimal therapeutic attitude to follow in order to prevent this misfortunate outcome.

[The use of computerized tomography in the localization of impalpable testes].

Computed tomography is the most reliable method in the localization of the impalpable testis. This imaging technique permits precise preoperative localization where ultrasound cannot. Moreover, it can provide very useful information in the complicated cases of cryptorchidism (torsion, trauma, tumor) and can disclose other previously undetected coexisting anomalies.
