[Specificity and sensitivity of the solid phase hemadsorption test: a study of treated and untreated syphilitics].

SPHA-test was performed on 317 treated and 78 untreated patients with syphilis and partly compared to 19S-IgM-FTA-ABS- as well as VDRL-test. Equivalent results were obtained in a reference laboratory, the correspondence being 94.8 percent regarding SPHA and 95.6 percent concerning 19S-IgM-FTA-ABS. In comparison to 19S-IgM-FTA-ABS, increased numbers of weakly reactive values occurred in SPHA correlating with the Latex-test. Because of its specificity and sensitiveness, SPHA turned out to be superior to VDRL with regard to the control of treatment. The biological significance of weakly reactive SPHA results is still unclear and does not indicate a necessity for therapy.

Rapid laboratory diagnostic of X-linked ichthyosis.

Steroid sulfatase and aryl sulfatase C activities were assayed simultaneously in peripheral blood leucocytes of 4 patients with X-linked ichthyosis (XLI) and 4 patients with autosomal-dominant ichthyosis; 11 healthy subjects served as controls. The deficiency of steroid sulfatase as well as of aryl sulfatase C found in leucocytes of patients with XLI was confirmed by the histochemical demonstration of aryl sulfatase C deficiency in skin sections. Although results are obtained earlier with the histochemical method than with the biochemical assay in leucocytes, several disadvantages brought us to the conclusion that the biochemical assay of microsomal sulfatase activity in leucocytes offers a fast and safe method for the identification of patients with XLI.

[Microsomal sulfatase deficiency in X chromosome-linked ichthyosis].

The microsomal sulfatases are known to be deficient in the X-linked recessive inherited type of ichthyosis (XLI), which is closely related to the placental steroid-sulfatase deficiency. Our group demonstrated biochemically the deficiency of steroid-sulfatase activity as well as arylsulfatase C activity in cultured skin fibroblasts and leukocytes of patients with XLI, whereas all cases of ADI investigated hitherto expressed high activities of microsomal sulfatase. On the other hand, the analysis of microsomal sulfatase in membranous preparations of uncultivated skin and hair follicles failed to distinguish between XLI, ADI, and controls. Possible relations between this enzyme defect and the hyperkeratotic condition of XLI are discussed.

Onychomycosis (trichophyton mentagrophytes). A scanning electron microscopic observation.

Nails from four patients, infected with dermatophytes, were investigated with the scanning electron microscope (SEM) to gain insight into the spatial arrangements of the dermatophytes within the nail. Fungal hyphae could be detected in nails of all four patients. A toenail from one patient infected with Trichophyton mentagrophytes was more extensively studied and the results are presented in this paper. Light microscopic observations with bright field illumination and Nomarski interference contrast confirmed the dermatophytic infection. Fungal hyphae found with the SEM distally on the ventral part of the toenail showed typical T. mentagrophytes structures and the comparison with cultured material clearly demonstrated the correspondence in morphology and size. Besides the fine structural morphology of the invasion of fungal hyphae into the nail plate between the horny cells and/or directly into corneocytes, "tunnel"-like holes were observed in paraffin embedded sections after removal of the paraffin. Scanning electron microscopy, with its enormous focal depth, yielded far more information than light microscopic techniques about the three dimensional behavior of dermatophytes in the nail.

Inhibitory effect of 8-methoxysporalen plus UVA (PUVA) on the systemic induction of contact sensitivity to dinitrochlorobenzene (DNCB) in guinea pigs.

To investigate the influence of 8-MOP and UVA on the induction of cell-mediated immunity, guinea pigs, sensitized with a single injection of dinitrofluorobenzene (DNFB) in Freund's complete adjuvant (FCA) in all footpads and the nuchal skin, were treated with 8-MOP, UVA, or 8-MOP + UVA on days 0, 2, and 5 and tested with dinitrochlorobenzene (DNCB) on day 14 after sensitization. Two control groups, exposed in a covered condition to the PUVA 4000 lamp, to observe the heat effect, showed a slightly enhanced contact sensitivity in comparison to the only sensitized control group. No altered reactivity was observed after irradiation with longwave UV light alone, whereas a statistically significant enhanced resp. reduced contact sensitivity was obtained after the treatment with 8-MOP alone and the combined action of 8-MOP + UVA, respectively.

[Delayed hypersensitivity against staphylococcal antigens in guinea pigs (author's transl)].

Delayed type hypersensitivity against staphylococcal antigens could be induced in guinea pigs by injecting the animals with a staphylococcal homogenate in Freund's adjuvant in all four foot pads and the nuchal skin. Maximal skin reactivity, tested intracutaneously, was observed 21 days after sensitization. Highest stimulation of lymph node cells and peripheral blood lymphocytes was also obtained 21 days after sensitization. When comparing cell walls, cell materials (nonsoluble material obtained after the separation of the cell wall fraction) and soluble fraction (obtained after sedimentation of all nonsoluble material, the strongest skin test reactions occurred after testing intracutaneously with the cell wall fraction.

Determination of water soluble imidazo-1,4-benzodiazepines in blood by electron- capture gas--liquid chromatography and in urine by differential pulse polaragraphy.

A sensitive and specific electron-capture gas--liquid chromatographic (GLC--ECD) assay was developed for the determination of 8-chloro-6-(2'-fluorophenyl)-1-methyl-4H-imidazo(1,5a)(1,4)benzodiazepine (I) or 8-chloro-1,4-dimethyl-6-(2'-fluorophenyl)-4H-imidazo (1,5a)(1,4)benzodiazepine (II) in blood. The assay for both compounds involves extraction into benzene--methylene chloride (9:1) from blood buffered to pH 12.6 The overall recovery of I and II from blood is 86% +- 5.0 (S.D.) and the sensitivity limit of detection is of the order of 2 to 3 ng of I or II per milliltre of blood. The major urinary metabolite of I is 8-chloro-6-(2'-fluorophenyl)-1-hydroxymethyl-4H-imidazo(1,5a)(1,4)benzodiazepine, (IA) present as a glucuronide conjugate while 8-chloro-6-(2'-fluorophenyl)-4-hydroxyl-1-methyl-4H-imidazo(1,5a)(1,4)benzodiazepine, (IB) and 8-chloro-6-(2'-fluorophenyl)-4-hydroxy-1-hydroxymethyl-4H-imidazo(1,5a)(1,4) benzodiazepine, (IC) are minor metabolites. The major metabolite IA is extracted into benzene--methylene chloride (9:1) from urine buffered to pH 11.0 (after incubation with glucuronidase--sulfatase as pH 5.0), and analyzed by differential pulse polarography (DPP) in 0.1 M phosphate buffer PH 3). The overall recovery of IA is 84 +- 3.0% (S.D.) with a sensitivity limit of 50 ng per millilitre of urine. The metabolites of compound II have not as yet been elucidated. The GLC--ECD and DPP assays were applied to the determination of blood levels and urinary excretion in dogs following single 10 mg/kg intravenous and oral doses of I and following single 6 mg/kg intravenous and 10 mg/kg oral doses of II. Blood levels of compound I were also evaluated in man following intravenous infusion of single 10 mg doses.

Induction of delayed-type hypersensitivity to staphylococcal antigens in guinea pigs.

Delayed-type hypersensitivity against staphylococcal antigens was induced in guinea pigs using Freund's incomplete as well as Freund's complete adjuvant. In animals sensitized with Freund's complete adjuvant the induction time and the duration of delayed-type hypersensitivity were longer and the skin reactions were stronger compared to animals sensitized with Freund's incomplete adjuvant. The difference between animals sensitized with Freund's incomplete or complete adjuvant may reflect the difference between tuberculin- and Jones-Mote type hypersensitivity.

Determination of the anti-inflammatory agent carprofen, (D,L)-6-chloro-alpha-methylcarbazole-2-acetic acid, in blood by high-pressure liquid chromatography.

A rapid, sensitive, and specific high-pressure liquid chromatographic (HPLC) assay was developed for the determination of (D,L)-6-chloro-alpha-methylcarbazole-2-acetic acid (carprofen) in blood. The assay involves extraction into diethyl ether from blood buffered to pH 6. The overall recovery of carprofen from blood is 97.3 +/- 5.3% (S.D.), and the sensitivity limit of detection is 100-200 ng/ml of blood using a fluorescence detector with excitation at 240 nm and emission at wavelengths greater than 350 nm. The HPLC assay is amenable to rapid routine analysis of clinical specimens, and the data obtained using this assay showed an excellent correlation coefficient (0.99) compared with a previously published spectrofluorometric assay. The method was used to monitor the blood level-time fall-off profiles in four subjects following single and multiple dose administration of carprofen.

Spectrofluorometric determination of substituted tetrahydrocarbazoles by a methylene blue sensitized photolytic reaction.

A spectrofluorometric method was developed for the determination of 6-chloro-9-[2-(2-methyl-5-pyridyl)ethyl]-1,2,3,4-tetrahydrocarbazole-2-methanol hydrochloride and its carboxylic acid analog in blood and urine. It involves extraction of both compounds at neutral pH, either from blood into ethyl acetate (the residue of which is dissolved in either) or from urine directly into ether. Both the alcohol and the acid are separated from each other by selective extraction into acid or base, respectively, and then reextracted into either from the respective aqueous medium by appropriate pH adjustment. The residues of the ether extracts containing the compounds are dissolved separately in 0.25 N NH4OH. Methylene blue is added to all samples, which are then exposed to UV energy for 15 min to produce the fluorophores. The fluorescence of the solutions is read at 370 nm, with excitation at 340 nm. The linear range of quantitation of both compounds is 0.02-10 mug/each/ml of final solution. The method was applied to the determination of blood levels and urinary excretion of the alcohol and its acid metabolite in a dog.

Insulin stimulation tests in pituitary dependent Cushing's syndrome after complete adrenalectomy.

The present study shows that in a group of 6 euadrenal patients, previously treated by complete adrenalectomy for pituitary dependent Cushing's syndrome, the stress stimulus of insulin induced hypoglycaemia is followed by a plasma ACTH response which is of similar magnitude as the response obtained with lysin-vasopressin. Both observations indicate that the central nervous system-pituitary axis is basically normal in pituitary dependent Cushing's syndrome as assessed by insulin induced hypoglycaemia. It is concluded that non-responsiveness of the pituitary-adrenocortical system to insulin induced hypoglycaemia in untreated patients with pituitary dependent Cushing's syndrome does not represent a fundamental defect of the stress mechanism, but is due to hypercorticism per se.
