Mucinous sweat gland adenocarcinoma of eyelid: a clinicopathologic study of 21 cases with histochemical and electron microscopic observations.

This is a clinicopathologic study of 21 cases of mucinous sweat gland adenocarcinoma involving the eyelid. The tumor occurred in middle-aged adults (median age 60 years) and has a predilection for males. In this series eight patients (40%) had one or more local recurrences, one of whom died with extensive local invasion of the face after multiple recurrences in a 15-year interval. Only one patient had metastasis to the submandibular lymph nodes, which was treated by radical neck dissection. Involvement of the head, especially the face and scalp, was observed in 78% of the cases from the literature. Combining our cases with those previously reported, we found that the eyelid was involved in almost half of the cases (21 out of 45 lesions). We believe that the identification by light microscopy of a mixed population of light and dark secretory cells within the tumor lobules, the histochemical findings (presence of sialomucin and absence of iron as well as periodic acid-Schiff positive, diastase-resistant granules), and the results of ultrastructural studies support the histogenetic postulate that this mucinous adenocarcinoma is probably derived from the secretory cells of the eccrine coil. Our interpretation concurs with Headington's view of origin from the eccrine glands.

Effects of work scheduling on time-income tradeoffs.

Existing theory and data on work--leisure preferences suggest that how work and free time are scheduled may influence workers' preferences regarding the exchange of income for free time. In general, hours and income are not traded in a strict one-to-one fashion; rather, the value of leisure time is in part a function of the "lumps" in which free time is made available. Exploratory research on an accidental sample of 248 employees in ten work organizations supports this hypothesis. Some societal implications of these findings are discussed.

Location of light chain tyrosine peptides from the combining site of a rabbit anti-p-azobenzoate antibody.

Peptic peptides containing a tyrosyl residue from the binding site of a rabbit anti-p-azobenzoate antibody were isolated by means of the paired-iodination procedure. The peptides were from the light chain, and the tyrosyl residue is 29, 30, 31, 32, 32A, 32B, 33 at position 30 in the sequence -Val-Tyr-Asn-Asx-Lys-Gly-Leu- and thus is in the first hypervariable region. The sequence of the N-terminal 40 residues was determined. The major antibody-site peptide isolated was a diiodotyrosyl (DIT) tetrapeptide representing residues 30-32A; the monoiodotyrosyl (MIT) tetrapeptide was also isolated, but in a smaller yield. By isoelectric focusing, the light chain appeared to be homogeneous. No heterogeneity was apparent in the light chain sequencing until position 32B when, in addition to the phenylthiohydantoin derivative of tyrosine present as the major residue, a significant amount of the phenylthiohydantoin derivative of glycine was obtained. The glycine presumably represents a light chain variant population and explains the source of the other antibody-site peptides isolated, i.e. two pentapeptides, apparently of the sequence Tyr-Asn-Asx-Lys-Gly, isolated as the DIT and MIT derivatives. The tetrapeptides must have been derived from the peptic cleavage between Lys 32A and Tyr 32B in the major light chain variant and the pentapeptides from the peptic cleavage between Gly 32B and Leu 33 in the other variant. It is interesting that position 30 is occupied by a tyrosyl residue in five out of twelve other rabbit antibody light chains of known sequence (Margolies, M.N. et al., Proc. Nat. Acad. Sci. US 1975.72: 2180). One light chain is from another rabbit anti-p-azobenzoate antibody in which Tyr 30 is apparently not important in hapten binding although a tyrosyl at position 96 is clearly involved in hapten binding (Roholt, O.A. et al., J. Immunol. 1973.111:1367). The other four of the five light chains are from anti-pneumococcal polysaccharide antibodies in which the role of this tyrosyl residue is not known.

Anterior urethral valves.

Two cases of anterior urethral valves are reported, one occuring in a child with renal osteodystrophy and the second being the first recorded case of anterior urethral valves in an adult. These 2 cases represent the thirteenth and fourteenth cases of anterior urethral valves reported in the literature.

Evaluation and optimum use of directed horizontal filtered air flow for surgeries.

Airborne micro-organisms, a suspected cause of surgical wound infection, are significantly reduced by the use of filtered unidirectional air flow (UAF) in operating rooms. A horizontal UAF system was installed at UCLA for evaluation and to determine procedures which would optimize its use. Major findings are listed: though the air flow was found to be somewhat turbulent and non-uniform in velocity downstream from the filter-bank entrance, the gross flow direction was maintained throughout the protected region; airborne particles were swept downstream with little vertical drop and no large scale recirculation; proper orientation and placement of objects in the clean air stream maximized its purging effect; correlation of non-viable with viable particle counts was not possible; better control of sources of contamination, particularly the operating team, would further reduce the potential for airborne infection; as yet there is no definitive proof that the addition of filtered UAF in a modern operating room reduces occurrences of deep wound sepsis.

Adolescent pregnancy.

The results of a retrospective study of adolescent pregnancy indicated that obstetric and perinatal complications were fewer than expected. The case histories of 178 pregnancies, all occurring in patients aged 15 years or less at the time of delivery, were reviewed. During the ten-year period 1963 to 1972 there were no maternal deaths, no stillbirths and no neonatal deaths. As expected, the majority of patients were primigravidae, but 35 patients (approximately 20 per cent) subsequently became pregnant for a second time before their sixteenth birthday. This emphasizes the need for improvement in contraceptive advice and follow-up for this age-group so that further illegitimate pregnancy or abortion can be avoided.

The role of the plasma membrane in fatty acid uptake by rat liver parenchymal cells.

1. Suspensions of isolated rat liver parenchymal cells incorporate [(14)C]palmitic acid into glycerides at about 40% of the rate obtained with liver slices. 2. At short time-intervals most of the incorporation is into phosphatidylcholine and this is recovered mainly in the plasma-membrane fraction. 3. At later times (5min to 2h) the [(14)C]palmitic acid is mainly found in triglyceride, but this is not recovered in the plasma-membrane fraction. 4. Addition of lysophosphatidylcholine increases incorporation of palmitic acid into both phosphatidylcholine and triglyceride, with maximum effect at about 0.1mm. 5. In vivo, 1min after injection of [(14)C]palmitic acid, radioactive phosphatidylcholine is concentrated in the plasma-membrane fraction, but the proportion present in this fraction declines rapidly. 6. The phosphatidylcholine of the plasma-membrane fraction has, at 1min after injection, a specific radioactivity 30-fold greater than that of the whole tissue. 7. This phosphatidylcholine reaches its maximum specific radioactivity before the tissue phosphatidic acid or diglyceride. 8. The phosphatidylcholine of the plasma-membrane fraction has a very rapid turnover. 9. It is proposed that the rapid formation of phospholipids in the plasma membrane is by acylation of their lyso-derivatives and the role of this process in fatty acid uptake is discussed.

Assessment of paracervical nerve block anaesthesia during labour.

An assessment of paracervical nerve block anaesthesia was made in 118 women during labour using 0.5% bupivacaine with 1:200,000 parts of adrenaline added. The 82 multiparae were each given a single block and the 36 primigravidae a continuous block to try to provide the sole means of analgesia throughout the first stage of labour.Only 48% of the primigravidae required no other form of analgesia, as against 72% of the multiparae.Complications included a 17% incidence of maternal haemorrhage during insertion of the continuous needle, an overall fetal bradycardia rate of 11%, and two perinatal deaths following single paracervical block. It is concluded that the use of continuous paracervical block should be discontinued, and that amide-type local anaesthetic agents should not be used for single paracervical block.
