Correlation between local oxygen tension in muscle tissue and survival time in tourniquet shock.

Tourniquet shock was induced in rabbits by constricting one hind leg for 4 hr. Local pO2 on muscle surface was measured by a multiwire platinum electrode according to the method of Kessler. Untreated animals showed anoxic tissue areas after a period of 30 min of reperfusion. All these animals died in shock within 3-4 hr. The glucocorticoid, prednisolone (20 mg/kg), and the proteinase inhibitor, Trasylol (20,000 KIU/kg), were administered 5 min bfore release of the tourniquet. These animals showed normal pO2 values during the 2-3 hr observation period. Tourniquet shock was prevented and survival time prolonged by these agents. This protective effect could be correlated to the oxygen tension in the ischemic region.

[Is curative treatment of breast cancer necessarily connected with radical mastectomy after Rotter-Halsted? (author's transl)].

Lokal recidivation rate in carcinoma of the female breast represents besides the 5- to 10-years survival rate an important parameter for primary surgical treatment. At the Chirurgische Universitätsklinik Tübingen radical mastectomy in the Rotter-Halsted technique as well as extended radical mastectomy were abandoned because they did not improve the survival rate. Surgery of the female breast is now performed according to different stages of the carcinoma. A comparative study in non-selected patients of our hospital showed that local relapse of the breast cancer did not depend on surgical technique, if the tumour was removed with a sufficient safety distance and postoperative megavolt radiation was performed.

[Morphological findings in tourniquet-shock (author's transl)].

Studies on 15 adult rabbits revealed that after removing a 3 hours lasting tourniquet of the left hind limb, the average survival time is 3,6 hours, whereas after a tourniquet lasting 4 hours animals die on the average within 22,8 hours in tourniquet shock. The maximum of interstitial edema and myolysis - estimated by light microscopy - was reached in the ischemic muscles of the hind leg after a 3-6 hours period of recirculation. Extraordinary strong muscle lesions regulary were found in the adductor. In all animals shock lungs could be demonstrated in which hyaline membranes were absent however. 54% of all animals partly showed extensive necroses of liver epithelial cells. Necrosis of cardiac muscle fibres were found in one animal only. In the kidneys merely slight degenerative lesions were identified in the thick descending and ascending limb of Henle's loops, kidneys of longer surviving animals contained many casts.

[Occult blood in stool. A field trial in cancer screening (author's transl)].

4000 male probands aged 45 to 69 were investigated in a field trial according to random distribution for undetected intestinal bleeding with the Haemoccult - method. With a participation of 28% only 39 out of a total of 65 positive cases could be evaluated. Four cases of colon or rectal carcinoma and four cases of colonic polyps were ascertained. In view of complete lack of experience with false negative test results the Haemoccult test is at present a useful addition to the diagnostic possibilities. However, in suspected cases even a repeatedly negative test cannot replace complete colo-rectal investigation.

[Frequency of local recidivism in breast carcinoma].

Frequency of local recidivation in carcinoma of the female breast represents a good parameter for the success of primary surgical treatment. A comparative study on nonselected patients of the Chir. Univ.-Klinik Tübingen showed, that local relapse of breast cancer did not increase after a conservative surgery due to different stages of carcinoma if: 1. the safety distance to the tumor was sufficient, 2. postoperative irradiation had been applied in megavolt technique. According to our results and regarding the 10-year-survival rate as well as the rate of local recidivation it does not seem imperative any longer to see a rigid indication for radical mastectomy.

[Toxic megaileum. Relapse of ulcerative colitis in the ileum following colectomy].

A 19 year old male patient suffered from ulcerative colitis and was colectomized 5 1/2 years ago. He presented now with an acute illness, characterized by massive discharges from the ileostoma, fever, leucocytosis, meteroism, melaena and a tender abdomen. A severe ulcerative inflammation of the total ileum was found. A "toxic megaileum" developed. At an operation the ileum was opened and drained. The patient recoverd under treatment with prednisolone, salazosulfypyridine, parenteral nutrition and Vivasorb. It is suggested that in this case the severe ileitis was due to the same disease which led to the total colectomy 5 1/2 years ago: ulcerative colitis.

The role of temperature factors in the epidemiology of listeriosis.

A total of 5000 samples were examined for Listeria monocytogenes, and 117 strains had been isolated. Out of these, 36 strains were selected according to their low virulence. An increasing virulence could be observed, when they were cultured on optimal media at 4 degrees C, with inoculations at every third week. This increased virulence caused by temperature factors, can have an important role in the epidemiology of listeriosis.
