[Clinical analysis of patients operated for adenocarcinoma and adenosquamous carcinoma of uterine cervix].

OBJECTIVES: The aim of our study was clinical analysis of patients with operated adenocarcinoma and adenosquamous carcinoma of uterine cervix. STUDY DESIGN: The mean age, number of pregnancies and labours, the incidence of adenocarcinoma and carcinoma adenosquamous in the stage Ib and IIa, and also in 2 following decades (1972-81 and 1982-91), the influence of positive lymph nodes on the survival have been estimated. MATERIALS AND METHODS: Between 1972 and 1991 at the 2nd Department of Obstetrics and Gynecology, Medical University of Gdańsk, 499 radical hysterectomies were performed in the patients with cervical cancer stage Ia, Ib and IIa. In this group there were 41 (8.2%) patients selected with adenocarcinoma and 16 (3.2%) with carcinoma adenosquamous. In the group of 18 pregnant women (3.6% of the whole group) there was diagnosed 1 case of adenocarcinoma and 1 case of carcinoma adenosquamous. The incidence of both-adenocarcinoma and carcinoma adenosquamous amounted to 11.4%. RESULTS: The number of pregnancies and labours was similar to other histological types. Nulliparas made up respectively 10.5% adenocarcinoma and 6.25% carcinoma adenosquamous. The incidence of adenocarcinoma stage Ib has been found to be more than twice higher than the incidence of adenocarcinoma stage IIa (9.0% and 3.8% respectively). The percentage of carcinoma adenosquamous in those two clinical stages was similar. The three times decrease in the incidence of adenocarcinoma, and the four times increase in the incidence of carcinoma adenosquamous has been found in the years 1982-91 in comparison to 1972-81. It was also stated that an adenouse type, comparing to other histological types, most often occurs without any clinical symptoms--in 23.1% a course of the disease was asymptomatic. As for an adenosqamous type the figures were lower--12.5%. In the patients with adenocarcinoma the incidence of positive pelvic nodes in the stage Ib-IIa was 34%. The incidence of positive lymph nodes in the patients with carcinoma adenosqamous and adenocarcinoma was lower and similar--25% and 23.7% respectively. The analysis of the 5-year survival showed 69.8% in patients with adenocarcinoma stage Ib-IIa and 66.3% in patients with carcinoma adenosquamous the (same stages). The difference is not statistically significant (p = 0.8).

[A comparison of clinical and surgical-pathologic staging of advanced vulvar cancer].

A surgico-pathological staging system introduced in 1988 by FIGO replaced previously used clinical classification of vulvar cancer extent. The aim of our study was to compare retrospectively the prognostic value of both of these staging systems (with the modification of surgical staging introduced in 1994) in the group of 123 vulvar cancer patients undergoing radical surgery including lymph node dissection in our institution. The comparison of both staging systems is presented in the table. Ninety patients (73%) have been assigned to the same stage in both systems, in 27 patients (22%) the use of surgical system resulted in upstaging, as compared to the clinical classification and 6 patients (5%) have been down-staged by the surgical system. In the analysis of prognostic value of both classifications the surgical staging system proved to have better discriminating power for prediction of survival in vulvar cancer patients.

[Borderline ovarian tumors: clinical analysis of 114 cases].

OBJECTIVES: The aim of this study was to analyse the group of patients with borderline ovarian tumours. DESIGN: The analysis included 114 patients, operated for ovarian tumours of borderline malignancy in the Gynaecological Department of Medical University of Gdańsk between 1978-1997. The study takes into account comparison of: age of patients, obstetrical past, clinical signs and symptoms, clinical stage (according to FIGO), type of surgery, tumour pathology, post surgical treatment. Furthermore, long-term follow up was assessed. RESULTS: Middle age in the group was 48 years, main symptoms: pain of lower part of abdomen (47%) and ascites (26%). 92% of tumours were recognised in stage I (FIGO). 44.7% of the tumours were histological serous, 36% were mucinous. All patients were treated by surgery and 12% received additional treatment. Mean follow up was 104 months (1-247). 9.6% of the patients died because of main disease, next ten persons for reasons not connected with the main disease. 5 year survival rate was 91.2%, 10-year 84.2%. CONCLUSIONS: 1. Borderline ovarian tumours are most frequently recognised in stage I. 2. Serous and mucinous borderline ovarian are dominant. 3. Type of surgery is dependent on age of patients, obstetrical past, clinical stage and tumour pathology. 4. Prognosis in borderline ovarian tumours is excellent.

[A case of cystic adenomatoid lung malformation treated by thoracocentesis].

A case of Cystic Adenomatoid Lung Malformation (CALM) treated by serial thoracocenteses followed by implantation of the feto-amniotic shunt is presented. The idea was to prevent prolonged lung compression leading to lung growth retardation and maturation restriction. Postnatally the baby presented good respiratory condition. She underwent lung lobectomy in the 2nd day of life. The histological assessment confirmed the ultrasound diagnosis of CALM. The postoperative period was not complicated. The method of feto-amniotic shunting is presented as promising in the treatment of CALM prenatally.

[Heterotopic osteogenesis in postoperative care in ovarian cancer treatment].

The case of calcification in postoperative scare in patient with ovarian cancer was described. The patient was operated because of ovarian cancer and had subsequently chemiotherapy. Afterwards the second look operation was performed which showed calcification (size 9.5 x 2 x 0.4 cm) in postoperative scare. The calcification wasn't a metastasis from the ovary. The etiology of calcification in unclear. The heterotopic osteogenesis in postoperative scars is quite rare. They usually appear as a result of calcification of connective tissue. Sometimes the relation between osteogenesis and neoplasmatic disease is supposed. Till now the reason of these kind of calcification is still unknown.

[The correlation between clinical and histological features and complexity of chromosomal changes in primary ovarian cancer].

The aim of the study was to determine the correlation between the complexity of chromosome aberrations in ovarian cancer and its clinical stage and histological grade. 38 primary ovarian cancer tumors were analysed cytogenetically. The chromosomes were obtained from primary cell cultures in vitro and were stained using GTW technique. Advanced clinical stage correlated with complex chromosomal rearrangements. Similarly, histologically poorly differentiated tumors more often displayed complex chromosomal rearrangements (p = 0.004). Simple chromosome aberrations or normal karyotype were more frequently detected in well-differentiated tumors. The analysis of the results revealed that the increase in histological grade and clinical stage correlates with the increase in the number and complexity of chromosomal rearrangements.

[The role of cytogenetic studies in prognosis of treatment results in patients with ovarian cancer].

UNLABELLED: The aim of this study was to determine the correlation between chromosome aberrations complexity in carcinomatous cells and survival in 38 patients with ovarian cancer. The material of cytogenic studies were 38 specimens taken from primary neoplastic ovarian tumors, which were than crumbled enzymatically. The suspension of neoplastic cells was used to set up primary cell culture in vitro. Only numerical aberrations or one of structural aberrations were assumed to be simple aberrations. More than one of numerical aberrations or structural aberrations were thought to form complex aberrations. The curve of survival in patients were determined using nonparametrical method of Kaplan and Meier and the differences between the curves were compared using logrank test. CONCLUSIONS: 1. The existence of complex chromosome aberrations in ovarian cancer cells is unfavourable prognostic factor. 2. The cytogenic analysis of neoplastic ovarian tumors together with numerous clinical and histological factors is an important prognostic factor.

[Primary malignant schwannoma in peritoneum of pelvis minoris: a case report].

There are reports of rare cases of malignant schwannoma of the small bowel, ovary or urinary bladder. We report a case of malignant schwannoma found in peritoneum without any other localisation in 66-year-old woman. The first diagnosis was tumor of right ovary. Laparotomy was performed and a big, solid tumor of urinary bladder involving the small bowel was found. There were some small tumors in peritoneum and parametrium. All the tumors were removed. The histological changes of the small lesions were described as malignant schwannoma. The mass in urinary bladder and small bowel was found to be of an inflammatory origin. The cases described by other authors suggest, that the diagnosis of malignant schwannoma in pelvis minoris is very difficult. Surgery is an adequate way of final diagnosis and management.

[Pregnancy and delivery after liver transplantation: case report].

We report a case of successful pregnancy in a 22-year-old primigravida who became pregnant 19 months after orthotopic liver transplantation for end-stage liver dysfunction caused by Wilson disease. The mother continued immunosuppressive therapy (tacrolimus) during the pregnancy. No complications were observed. Labour started spontaneously at 40th week's gestation. As a result the healthy male infant weighing 3900 g was born. Patient and the baby were discharged from hospital, both in good condition, on the fifth day after delivery.

[Problems of contraception in women with Wilson disease].

Because of the constant progress in medical therapy, the fertility status of patients with Wilson's disease may be preserved. Contraceptive counselling is, therefore, essential to these women. Some contraceptive methods can have a negative influence on hepatic function or hepatic insufficiency may reduce the efficacy of other contraceptives. Different contraceptive options for patients with Wilson's disease are described. Second generation intrauterine devices with copper and estrogen-containing oral contraceptives are contraindicated. Spermatocide and barrier contraceptives and progesterone-only preparations can be safely used.

Cytogenetics of uterine sarcomas: presentation of eight new cases and review of the literature.

Tissue from 14 uterine tumor samples from eight patients-four with endometrial stromal sarcoma (ESS), two with leiomyosarcoma (ULMS), and two with malignant mixed mesodermal tumor (MMMT)-were investigated cytogenetically after short-term culturing. Clonal chromosome aberrations were found in 12 tumors. One ESS showed a recombination between 7p14 and 17q12, a rearrangement characterizing a subset of ESSs. In our series, chromosomes 1, 6, 7, and 16 were involved in structural aberrations most frequently (four cases each). Net loss of 6q material was found in four cases and bands 11q13, 16q13, and 22q13 were each rearranged in four cases. Among 43 uterine sarcomas, including 12 MMTs, now available for evaluation, some differences in breakpoint distribution among different tumor types were found. Rearrangements of bands 1p32, 3p24, and 10q22 were found exclusively in ULMS, whereas aberrations of bands 6p21, 7p21, and 17q12 were found predominantly in ESS.

[Hypertension in pregnancy, which resulted from intrauterine insemination].

The authors present an analysis of 117 pregnancies after intrauterine insemination (AIH) to indicate presence of disease during this period. We found out only 2.6% of EPH-gestosis among treated women, which means that frequency of PIH after AIH is lower then in natural population. Propriate decisions concerning pregnancies and positive relation of women to keep pregnancy have an essential value. We have not obtained any statistically significant differences between natural and after AIH evaluation of pregnancy.

[A clinical analysis of fetoplacental blood flow in prognosis of fetal emergency in pregnancies complicated by hypertension].

DESIGN: The aim of our study was a clinical analysis of fetoplacental blood flow in prevision of fetal hypoxia in pregnancy complicated by a hypertension. MATERIALS AND METHODS: Between 1995-1997 105 pregnant women delivered in 2nd Dept. Of Gynecology and Obstetrics Medical University of Gdansk. They were divided into two groups: 53 patients with hypertension and control group of 52 pregnant women. In all cases a doppler umbilical blood flow was estimated using 5 MHz Convex sounder. RESULTS: In a group of pregnant women with hypertension abnormal results of doppler fetoplacental blood flow were found. Twice more often a caesarean section was made because of fetal emergency symptoms. CONCLUSIONS: The results of fetoplacental blood flow are one of more important prognostic factors in pregnancies complicated by a hypertension.
