[A clinical analysis of risk factors in women operated because of endometrial cancer].

OBJECTIVES: The aim of the study was a clinical analysis of risk factors of endometrial cancer such as diabetes mellitus, hypertension, obesitas, using oral contraceptives, smoking cigarettes and neoplasm diseases in a family. MATERIAL AND METHODS: Between 1983-1997, 336 patients was operated in 2nd Department of Obstetrics and Gynaecology Medical University of Gdańsk because of endometrial cancer. 117 women, who answered inquiry form questions was selected. RESULTS: The median age of patients was 58.9 years (41 to 83 years). In 71.79% the neoplasm disease was found in postmenopausal age. Nulliparas was 16.2%. In 96 cases (82.5%) an adenocarcinoma was found, in 14 cases (12.0%)--a adenosquamous cell carcinoma and in 1 case (0.8%)--a squamous cell carcinoma of uterine corpus was found. In 51 patients (43.6%) a obesitas was found, in 44 patients (43.6%)--hypertension and in 14 patients (11.9%)--diabetes mellitus. In 11 cases (9.4%) diabetes mellitus and hypertension coexist and in 9 cases (7.7%)--all diseases. Only 2 women (1.7%) used oral contraceptives, so it is impossible to estimate its influence on risk of endometrial cancer. 18 patients (15.4%) was smokers and 49 women (41.9%) had a neoplasm diseases in their families (the most often--43.7%--a neoplasm disease was in families of women with adenocarcinoma). CONCLUSIONS: The percentage of nulliparas was lower then it is described in literature (40-50%) and was 16.2%. The highest percentage of nulliparas was found in a group of women with adenosquamous carcinoma. The most often histological type of endometrial carcinoma was high-differentiated adenocarcinoma.

[A complementary treatment of patients with endometrial carcinoma after surgical treatment].

OBJECTIVES: The different methods of a complementary treatment used in a group of 117 patients, aged 41 to 83, after surgical treatment because of endometrial cancer in 2nd Department of Obstetrics and Gynaecology Medical University of Gdańsk between 1983-1997, were analysed. RESULTS: The hysterectomy with removal of uterine adnexa was performed in all cases, in 18 patients supplemented by pelvical lymphadenectomy. The median age of women was 58.9 years. In 96 cases (82.5%) an adenocarcinoma was found, in 14 cases (12.0%)--a adenosquamous cell carcinoma and in 1 case (0.8%)--a squamous cell carcinoma of uterine corpus was found. In 52 patients (44.4%) a complementary treatment was used: in 43 patients (82.7%)--radiotherapy, in 4 patients (7.7%)--chemiotherapy, in 5 patients (9.6%)--radio- and chemiotherapy and in 1 case (1.9%)--radio- and hormonotherapy. In 8 women (6.8%) a second laparotomy was made but only in 1 case it was caused by ileus because of neoplasm metastases. The median survival of all patients was 6.1 years; without complementary treatment--6.48 years, after radiotherapy--6.25 years, after chemiotherapy--2.75 years and in a group of women after associated treatment--4.4 years. CONCLUSIONS: Between all patients with endometrial cancer after complementary treatment, the longest survival was found in a group of women after complemetary radiotherapy then in a group of patients after chemiotherapy or associated treatment (radio- and chemiotherapy). The highest percentage of women without any symptoms of disease was in a group of patients after radio- and chemiotherapy or only chemiotherapy against to patients after chemiotherapy.

[Retrospective assessment of the feasibility of conservative surgery in the vulvar squamous cell carcinoma].

OBJECTIVES: Radical vulvar surgery is associated with significant morbidity. DESIGN: The aim of the study was to assess retrospectively the feasibility of less extensive surgery in vulvar cancer patients operated in our institution with radical intent. The feasibility of less extensive surgical procedures was investigated, based on the retrospective analysis of preoperative assessment of tumour location and extent in a group of 162 patients. RESULTS: In the next step, the pathology reports of actually performed extensive operations were carefully reviewed in a search of microscopic evidence of tumour outside the limits of proposed conservative procedures. Conservative surgery was regarded technically possible in 127 patients (85.8%). Macro- or microscopic multifocality was detected in 22 patients (14.9%), of whom in 15 cases conservative surgery was regarded possible on the basis of clinical assessment. However, only in 11 cases tumour was found outside the limits of planned excision, in 9 of those cases--due to multifocal lesions. CONCLUSIONS: Less extensive surgery is technically feasible in a vast majority of vulvar cancer patients, however the selection for conservative procedures based only on clinical assessment of tumour extent is not sensitive enough to assure radical treatment in all patients and better methods of preoperative evaluation, such as vulvoscopy or random vulvar biopsies need to be investigated before the decision on routine use of conservative surgery is made.

[Risk factors for groin lymph node involvement in patients with vulvar cancer].

OBJECTIVES: Morbidity of vulvar cancer surgery is predominantly related to groin lymph node dissection. DESIGN: The aim of the study was to determine factors influencing the probability of groin lymph node metastases and to select a subgroup of patients in whom the risk of nodal involvement is low and nodal dissection can be omitted. MATERIAL AND METHODS: A retrospective analysis was performed on the group of 123 patients who had undergone full inguino-femoral lymph node dissection as a part of their primary vulvar cancer treatment. RESULTS: The risk of involvement of groin nodes was related to the clinical assessment of the stage of disease according to 1970 FIGO classification (p < 0.001), depth of stromal invasion (p = 0.004), primary tumour diameter (p = 0.009) and to the presence of pain in the vulva (p = 0.028). In multivariate analysis the probability of groin node infiltration was related to the diameter of primary tumour (p = 0.008) and depth of stromal invasion (p = 0.013). CONCLUSION: The analysis allowed for division of vulvar cancer patients into subgroups of similar risk of lymph node involvement.

[The use of PCR technique in the evaluation of the frequency of HPV infection in vulvar and cervical carcinoma].

OBJECTIVES: In this study we estimated the frequency of human papillomavirus (HPV) infection in patients with vulvar and cervical carcinoma by polymerase chain reaction (PCR). We took into consideration the age of patients, the HPV infection status and frequency of HPV types. MATERIALS AND METHODS: Vulvar and cervical smears obtained from 88 patients with vulvar and cervical carcinoma, treated in 1997 in II Department of Obstetrics and Gynecology, Medical University in Gdańsk were investigated for HPV DNA. We used the multiplex polymerase chain reaction (M-PCR) with four primer pairs: two primer pairs located in the E6, E7 open reading frame (ORF) for HPV6, 11, 16, 18, 31, 33, 35, 39, 45, 52b i 58 and primer par located in L1 ORF and type-specific primer for HPV16 (E6 ORF). We used restriction fragments length polimorphism PCR (RFLP-PCR) method for HPV typing. RESULTS: HPV DNA was detected in 3 (19%) of the 18 patients with vulvar carcinoma, of whom 3 (100%) had HPV16. In cervical carcinoma, DNA HPV identified in 50 (71%) of the 70, of whom 43 (86%) had HPV16, 3 had HPV31, 3 HPV33 and one was positive for HPV52-b. CONCLUSION: We did not find significant differences between the frequency of the HPV infection in different age groups of patients with cervical carcinoma. In vulvar cancer, we found the HPV infections significantly more often in group of patients in the age below 55.

[A long-time presence of a foreign body in an episiotomy scar].

OBJECTIVES: The object of our study was a long-time presence of a foreign body in an episiotomy scar. STUDY DESIGN: A case of 46-year old woman with a palpable tumour in a perineal region is described. RESULTS: This false tumour was caused by a surgical needle which was left in the body of the patient 19 years ago during the episiorrhaphy after the last labour. CONCLUSION: Because of a character and localisation of the change the method of choice was surgical treatment which completely eliminated all symptoms.

Pharmacological characterization of the contractile effects of galanin (1-29)-NH2, galantide and galanin (1-14)-(alpha-aminobutyric acid8)scyliorhinin-I in the rat gastric fundus.

Porcine galanin (1-29)-NH2, galantide (M15) and galanin (1-14)-(alpha-aminobutyric acid8)-scyliorhinin-I used in concentrations of 300, 1,000 and 3,000 nM respectively caused contractions of rat fundus strips. The contractile responses to galanin(1-29)-NH2 were not modified by atropine (10 microM), guanethidine (10 microM), naloxone (1 microM), a mixture of propranolol (10 microM) and phentolamine (10 microM), indomethacin (10 microM), a mixture of mepyramine (10 microM) and cimetidine (10 microM), saralasin (10 microM), and spantide (100 microM). The effects of M15 and galanin(1-14)-(alpha-aminobutyric acid8)-scyliorhinin-I were significantly decreased by atropine for 36 and 18% and by spantide for 37 and 26% respectively. Indomethacin inhibited the muscle response to M15 without influence on the galanin (1-14)-(alpha-aminobutyric acid8)-scyliorhinin-I-induced action. These results support findings that galanin (1-29)-NH2 contracts rat gastric fundus strips by stimulating specific receptors localized on the surface of smooth muscle cells. M15 and galanin(1-14)-(alpha-aminobutyric acid8)-scyliorhinin-I seem to contract smooth muscles not only by acting at galanin receptors, but by interacting with muscarinic or tachykinin receptors or modulating the release of acetylcholine and substance P. Diltiazem (EC50 825 nM), dantrolene (EC50 30.2 microM) and the phospholipase C inhibitors U-73122 (EC50 549 microM) and U-73343 (EC50 751 microM) lowered the contraction to galanin(1-29)-NH2 in a concentration-dependent manner. These observations imply that though the extracellular Ca2+ influx plays a major role in the action of galanin(1-29)-NH2, the release of Ca2+ ions from the intracellular stores contributes to the response of smooth muscles of galanin(1-29) NH2. Norepinephrine (30, 60, 100 and 300 nM) concentration-dependently reduced the Emax to galanin (1-29)-NH2 and reduced the slopes of the concentration-contraction curves, without a notable change in EC50. Pertussis toxin pre-treatment (10 and 30 mg/kg intravenous [i.v.]), 120 h before the experiment, notably increased the maximal response of the rat gastric fundus to galanin(1-29)-NH2, without a significant change in the properties of the concentration-contraction curves (EC50, slopes). The observations may suggest that pertussis toxin-sensitive GTP-binding proteins are involved in the modulation of the excitatory effects of galanin(1-29)-NH2 in the rat gastric fundus.

[The value of peritoneal cytology in the staging of ovarian cancer].

The results of peritoneal cytology obtained during primary surgery for stage I ovarian cancer and "second look" operations was assessed. Positive peritoneal smears were found in 10.7% of patients, while neoplastic cells were present in peritoneal fluid in 32.5% of cases. Of 144 patients undergoing "second look" surgery, positive cytology occurred in 55.5% of cases: in 26.4%--in peritoneal smears and in 29.1%--in peritoneal washings. Intraoperative peritoneal cytology was found to be a valuable diagnostic tool in early stage and minimal disease ovarian cancer.

[The use of biodegradable valtrac-bar rings for intestinal anastomosis in advanced ovarian cancer patients].

Intestinal anastomoses using Valtrac-Bar biofragmentable rings were performed in 9 advanced ovarian cancer patients presenting with symptoms of acute or subacute intestinal obstruction. Two ileo-transversal anastomoses, six sigmo-rectal and one ileo-ileal ones were performed. A part of sigmoid colon with tumour causing obstruction was resected in seven patients and in two patients a resection of an ileal loop was performed. In two patients a colostomy was necessary. The result of anastomosis with Valtrac-Bar was satisfactory in all patients: no cases of peritonitis, anastomosis leak or fistulae were observed. The treatment of intestinal obstruction allowed for a prolongation and improvement of quality of life of these patients and enabled further treatment with chemotherapy. We conclude that biofragmentable Valtrac-Bar ring is a safe and effective device enabling easy and fast intestinal anastomosis even in patients with inappropriate healing.

[Malignant non-epithelial neoplasms of the uterine body--clinical analysis].

Twenty six cases of primary uterine sarcomas treated between 1980-1990 are analyzed. There were 16 cases (62%) of endometrial stromal sarcoma, 8 cases (31%) of leiomyosarcoma and 2 cases (7%) of botryoid rhabdomyosarcoma. Symptoms of uterine sarcoma are presented. In 19 patients (73%) pathological diagnosis was obtained by endometrial curettage before surgery. Stage of disease was classified according to intraoperative scale of Lewis. Median survival for the whole group treated surgically, by radiotherapy and chemotherapy was 22.2 months. Five patients (19%) in whom the disease was limited to the endometrium survived more than 5 years. Recurrences of the neoplasm and their treatment by secondary cytoreductive surgery are described.

Alveolar rhabdomyosarcoma of the uterine cervix.

Alveolar rhabdomyosarcoma is an uncommon tumor occurring mostly in extremities of children and adolescents. This type of tumor located in the uterine cervix has not been reported previously. A case of pure alveolar rhabdomyosarcoma arising from the uterine cervix in a 45-year-old patient and having a very aggressive clinical course is described. The patient presented with rapidly progressive cervical tumor, which was originally diagnosed as mesonephroid adenocarcinoma. She was treated with a radical two-stage surgical procedure and postoperative external beam and intracavitary irradiation. Despite treatment, she developed metastatic disease and died 3 months after the surgery. The aggressive course of the disease at this location was consistent with that of the majority of alveolar rhabdomyosarcoma cases of other sites reported in literature. No standard effective treatment is known for this tumor in adult patients.

[Alveolar rhabdomyosarcoma of the uterine cervix].

A rare case of cervical alveolar rhabdomyosarcoma in a 45-year old female is presented. The removal of the tumour was performed in two stage-procedure, due to its large dimensions: first the bulk of the tumour was removed per vaginam, proceeding afterwards to a radical hysterectomy. Metastases were detected in pelvic lymph nodes. The patient was given postoperative radiotherapy, but a dissemination of the tumour was found at the termination of the treatment and the patent died 3.5 months after surgery.

[Use of intraperitoneal chemotherapy in patients with clinical stage I ovarian neoplasms].

Results of treatment with intraperitoneal cisplatin chemotherapy via Tenckhoff catheter of 14 high-risk stage I ovarian cancer patients are presented. Seven patients had malignant ascites and in 6 a spillage of the tumor occurred before or during surgery. All patients achieved pathological complete remission after IP chemotherapy. During a median follow up of 38.5 months (range: 15-58) one recurrence was noted 16 months after treatment.
