[Differences by sex in the presentation and follow-up in the first year of acute myocardial infarct].

Three-hundred and nine patients, 242 males and 67 females diagnosed of myocardial infarction were studied during the acute phase and followed for a year to evaluate any differences. The corrected incidence was greater amongst males, 11.1 vs 3.6/10,000 inhabitants. The presenting age was significantly lower in males below 50 and greater in women above 70. Hypertension, hypercholesterolemia and sedentariness were more prevalent amongst women; smoking was more prevalent amongst males. Infarction presentation was typical in both sexes being significant a 5 hour delay in arriving to the hospital in female patients. On hospital admission Killip III-IV was significantly more frequent amongst women (17% vs 4.6%). Hospital mortality although it was higher in women this was not statistically significant (19.4% vs 14.4%). During follow up there was a better control of risk factors in the male patients. Heart failure and mortality were greater amongst women during the first three months (p less than 0.001). Mortality differences continued being statistically significant during the following three months. From the sixth month to the first year, mortality rates were similar. The study suggests that infarction prognosis is worse in females.

[The single coronary artery. Presentation of a case].

A patient with a single coronary artery originating in the right coronary sinus is presented. The condition was diagnosed during aortic valve replacement surgery and later confirmed by coronary angiogram. The clinic significance in relation to the anomalous course of the coronary artery and the importance of correct diagnosis prior to cardiac surgery is discussed.

[Bone marrow autotransplant: preliminary results in 20 patients with acute leukemia].

The results of bone marrow autograft (BMAG) in 20 patients with acute leukemia (AL) consecutively treated from October 1985 and May 1988 are reported. The follow up was continued until November 1988. The mean age of the patients was 20 years (range 10-48) and their diagnoses were acute myeloblastic leukemia (AML) in 12 and acute lymphoblastic leukemia (ALL) in 8. The preparation for BMAG included cyclophosphamide and whole body radiation in all cases. The procedure was carried out in the first complete remission (CR-1) in 5 patients, in CR-2 in 11, in CR-3 in 2, and in CR-4 in another 2. Two patients died as a direct consequence of BMAG, 11 relapsed and 7 are alive and free from relapse. The likelihood of survival free from prolonged illness was calculated as 24% for the whole series, 60% for the cases of BMAG in CR-1 and 50% for the group of patients with AML. The results were poor in the cases of BMAG carried out for AL in an advanced stage. On the basis of these results the experience of other authors with this therapeutic modality is reviewed, and its current indications are discussed.

Relationship between myoplasmic calcium transients and calcium currents in frog skeletal muscle.

Ca2+ currents (ICa) and myoplasmic Ca2+ transients were simultaneously recorded in single muscle fibers from the semitendinosus muscle of Rana pipiens. The vaseline-gap voltage-clamp technique was used. Ca2+ transients were recorded with the metallochromic indicator dye antipyrylazo III. Ca2+ transients consisted of an early fast rising phase followed by a late slower one. The second phase was increased by experimental maneuvers that enlarged ICa, such as augmenting [Ca2+]o (from 2 to 10 mM) or adding (-)-Bay K 8644 (2 microM). When [Ca2+]o was increased, the second phase of the Ca2+ transients and ICa showed an average increase at 0 mV of 2 +/- 0.9 microM (4) and 1.4 +/- 0.3 mA/ml (4), respectively. (-)-Bay K 8644 increased the late phase of the Ca2+ transients and ICa at 0 mV by 0.8 +/- 0.3 microM (3) and 6.7 +/- 2.0 mA/ml (4), respectively. The initial fast rising phase of the Ca2+ transients was not modified. (-)-Bay K 8644 slowed the time constant of decay of the transients by 57 +/- 6 ms. In other experimental conditions, Ca2+ release from the sarcoplasmic reticulum (SR) was impaired with repetitive stimulation in 1 mM [EGTA]i-containing fibers. Under those circumstances, Ca2+ transients directly followed the time integral of ICa. Pulses to 0 mV caused a large Ca2+ transient that became suppressed when large pulses to 100 mV were applied. In fibers with functioning SR, pulses to 100 mV elicited somewhat smaller or similar amplitude Ca2+ transients when compared with those elicited by pulses to 0 mV. The increase in ICa after raising [Ca2+]o or adding (-)-Bay K 8644 cannot directly explain the change in Ca2+ transients in fibers with functioning SR. On the other hand, when Ca2+ release from the SR is impaired Ca2+ transients depend on ICa.

[Uncomplicated acute male and female gonorrhea: treatment with a single oral dose of epicillin-probenecid].

We treated 142 patients with acute uncomplicated gonorrhea with a single oral dose of epiciline-probenecid. All cases were confirmed by culture on Thayer Martin medium at the first day and they were all laboratory tested seven days after treatment. At the control time the patients were asked about drug tolerance. All isolated strains were tested for beta-lactamase producing capacity and susceptibility to epiciline and penicillin. All patients who were culture positive seven days after treatment were considered to be treatment failures. The treatment was effective in 94.4% and we detected 8 Neisseria gonorrhoeae strains resistant to penicillin (5.6%); seven of these were PPNG strains. Tolerance to the drug was excellent in 90% of all cases; 9% presented minor adverse reactions for a short period of time. Therefore, the combination of epiciline-probenecid is a valid alternative for routine treatment of acute uncomplicated gonorrhea.

[Pericardiocentesis: usefulness in the routine determination of intrapericardial pressure].

The usefulness of routine intrapericardial pressure determination and hemodynamic monitoring during therapeutic (n = 22) or diagnostic (n = 2) pericardiocentesis was analyzed. Hemodynamic data confirmed the presence of cardiac tamponade in all cases, including the 2 patients without clinical suspicion. Prior to fluid evacuation, abnormally elevated levels of intrapericardial (13 +/- 5 mmHg), right atrial (14 +/- 4 mmHg) and pulmonary capillary (16 +/- 3 mmHg) pressures were observed. A significant correlation was found between intrapericardial and right atrial pressures (r = 0.97, p less than 0.05) and also between intrapericardial and pulmonary capillary pressures (r = 0.69, p less than 0.05). The basal cardiac output was low (3 +/- 1 l/min) and increased after the pericardiocentesis in all but one patient. According to the results of pericardiocentesis, patients were classified into three groups. Group I: intrapericardial and right atrial pressures decreased to normal levels. The increment of cardiac output was high (72%) and the procedure was considered successful. Group II: right atrial and intrapericardial pressures remained elevated (7.7 +/- 4 and 7 +/- 2 mmHg, respectively) and the cardiac output increment was less marked (52%), thus indicating incomplete drainage. Group III: intrapericardial pressure decreased to normal levels but right atrial pressure remained elevated. All these patients were found to have associated cardiac disease, most frequently constriction. We conclude that routine determination of intrapericardial pressure: 1) reveals the presence of tamponade in cases without clinical suspicion; 2) indicates the efficacy of drainage; and 3) together with right atrial pressure recording, provides early diagnosis of associated cardiac disease, especially effusive-constrictive pericarditis.

Squamous and glandular differentiation in urothelial bladder carcinomas. Histopathology, histochemistry and immunohistochemical expression of carcinoembryonic antigen.

This paper reports the immunochemical expression of carcinoembryonic antigen in the squamous, glandular and mixed differentiation areas observed in 190 urothelial urinary bladder carcinomas. The antigen was found to occur in 75% and 81% of all papillary and solid infiltrating carcinomas, respectively. The use of the immunohistochemical determination of CEA in improving the morphological examination of these differentiation areas and their scant presence in the prognosis of urothelial carcinomas involving squamous and/or glandular focal differentiation are discussed.

[Molluscum contagiosum. Clinicopathologic and ultrastructural study].

26 usually multiple Molluscum contagiosum (MC) lesions corresponding to 14 patients (x age, 12 years old) mainly located in face and body, and unfrequently in genital area, were studied. Under light microscopy, we emphasize the presence of molluscum bodies corresponding ultrastructurally to large bodies of viral particles within a protein matrix. Intranuclear inclusions of suspected viral origin were noted too. Lesions previously embedded in paraffin showed a perfect preservation of their viral particles.

Appropriate conditions to record activation of fast Ca2+ channels in frog skeletal muscle (Rana pipiens).

Frog skeletal muscle fibres possess slow Ca++ channels which are currently studied. Only recently a fast Ca++ current in frog skeletal muscle was reported. This finding is related to the fact that this current is very labile. Here we report the appropriate conditions to record the fast calcium current (ICa-f) in skeletal muscle of the frog Rana pipiens, in isotonic solutions and resting fibre length.

Chromosomal anomalies in children treated for cancer.

Cytogenetic studies were carried out in peripheral blood lymphocytes of 31 children with cancer (12 retinoblastomas, 9 non-Hodgkin's lymphomas, 7 neuroblastomas and 3 Wilms' tumors) with the purpose of investigating the prevalence and persistence of chromosomal aberrations before onset of antineoplastic treatment and at different intervals thereafter. The number of treated patients with chromosomal anomalies was significantly higher (p less than 0.01) and so was the percent of cells with aberrations, as compared to untreated patients or healthy controls. The most frequent aberrations were of chromatid type. One patient had a number five trisomy in the third posttreatment study and another presented abnormal aneuploid cells in the third and fourth posttreatment studies. Congenital chromosomal anomalies were not observed, nor anomalies described for some of these tumors. There was no regularity in the effected chromosomes. We conclude that the observed chromosomal aberrations were due to treatment. We consider a prolonged and stringent cytogenetic follow up of such patients necessary to detect the induced aberrations.
