[Antithrombin III in various conditions in internal medicine].

In a group of 993 patients with serious medical diseases an important deficiency of antithrombin III was found in patients with hepatic insufficiency, pulmonary embolism and with disseminated intravascular coagulation. Acquired antithrombin III deficiency in these conditions develops when the antithrombin production in the liver is low and also in patients with shock syndrome and disseminated intravascular coagulation. Assessment of antithrombin III is of diagnostic and prognostic value in thrombotic and prethrombotic conditions and its results is decisive for adequate substitution. Adequate AT III substitution without concurrent heparin administration in patients with septicaemia and manifestations of DIC improves the prognosis of patients with an increased endothelial resistance.

The effect of reperfusion on plasma tumor necrosis factor alpha and C reactive protein levels in the course of acute myocardial infarction.

Severe acute response, the synthesis of human acute-phase proteins and the increase of plasma cytokines and adhesion molecules occur in patients in the course of acute myocardial infarction. We examined the plasma tumor necrosis factor alpha (TNF alpha), plasma creatinkinase (CK) and C-reactive protein (C-RP) levels in patients with acute myocardial infarction (AMI) in the course of 96 hours. Venous blood samples were taken at 3-hour intervals during the first 48 hours, and at 6-hour intervals during the next 48 hours. All patients were treated using thrombolytic therapy (streptokinase). Detection of the reperfusion was based on the method of measuring the time to achieve peak serum creatinkinase activity. The study was done on a group of 24 patients. Plasma levels of the parameters were compared between the group of patients with expected reperfusion versus the group of patients in which reperfusion is not suggested. The plasma TNF alpha level was elevated constantly without any significant peak. The mean plasma TNF alpha concentration was 46.8 pg/ml, SD 2.13, vs. normal level 4.35 pg/ml, p < 0.001. The plasma TNF alpha level in the group of patients with reperfused coronary artery showed a significant decrease especially during the 3rd and 4th day (the mean peak plasma TNF alpha concentration was 35.2 pg/ml, SD 15.8, vs. 66.9 pg/ml, SD 38.3 pg/ml, p < 0.005). The plasma C-RP levels were elevated throughout the time of observation in the both groups. The elevation of the plasma C-RP levels was more significant in the group of patients without successful reperfusion (80.6 mg/ml, SD 31.2, the mean plasma C-RP level of the group of the patients with successful reperfusion was 45.7 mg/ml, SD 18.1, p < 0.005). We conclude, that TNF alpha can play a role in the mechanisms of tissue injury. The successful reperfusion of coronary artery leads to significant decrease of plasma TNF alpha and C-RP levels.

Combined vaccination against yellow fever and typhoid fever: a comparative trial.

In order to evaluate a combination of yellow fever and typhoid fever vaccine, we conducted a controlled trial comparing reactogenicity and immunogenicity of Vi polysaccharide (ViPS) vaccine and yellow fever 17D (YF) vaccine after single, simultaneous and combined administration. The combined YF/ViPS vaccine was prepared by using the liquid ViPS vaccine as a diluent for the YF vaccine. The stability of such a reconstitution had been assessed in vitro. Safety was evaluated using a self-surveillance form and by repeated clinical visits. Immunogenicity was evaluated by a plaque reduction test for YF and by radioimmunoassay for ViPS. Tolerability was satisfactory in all groups. There was no increase in local or general reactions in groups receiving both vaccines, whether given simultaneously or combined. The serological response to ViPS was similar after single and simultaneous or combined administration. Interestingly, the immune response to YF was significantly enhanced in groups receiving the vaccines simultaneously or combined, suggesting a potential adjuvant effect of ViPS.

Constrained Markov networks for automated analysis of G-banded chromosomes.

Automated analysis of chromosome band patterns using probabilistic Markov networks has been reported in previous work. Band patterns are represented as strings of symbols. Inferred from a set of learning strings, a Markov network is a model of intraband and interband relations in these strings. The inference is entirely data-driven and is accomplished using dynamic programming. This paper presents a new model of chromosome band patterns, the constrained Markov network, which is a special case of its predecessor. Substantial experimental evidence of the superiority of the new model over the old is given in terms of equal results in centromere finding and improved results in classification for the 22 autosomes. Furthermore, a method for simplification of constrained Markov networks is shown to be of considerable importance with respect to computational complexity.

Skin side-effects of amiodarone therapy.

Amiodarone is a antiarrhythmic drug with many side-effects which include also the induction of photosensitivity and the development of greyish-blue pigmentations on the skin exposed to light. The authors examined 64 patients treated with amiodarone and 32 controls by means of a phototest using a xenon lamp. In 9.4% photosensitivity was recorded, in 9.4% there were greyish-blue hyperpigmentations in the face and on the back of the hands. Histological identification of these hyperpigmentations including electron microscopy was also performed.

Finding chromosome centromeres using band pattern information.

A method for finding centromeres of chromosomes using band pattern information only is described. Rather than using shape-related characteristics to identify the position of the centromere constriction, its position in relation to the band pattern is encoded into a structural band pattern model for each chromosome type individually. These models are automatically inferred from strings of symbols representing band pattern profiles. When used for analysis, a sample string is compared with a model from which the position in the string corresponding to the centromere can be derived. The approach is experimentally investigated with centromere finding both for class of the chromosomes known and unknown. A scheme for simultaneous centromere finding and classification is also proposed and tested on different band pattern representations with encouraging results.

[The significance of changes in fibrinogen levels during thrombolytic therapy in myocardial infarct].

The submitted work was concerned with two problems: a) investigation of the dynamics of changes of the fibrinogen and fibrin breakdown products after a single dose of streptokinase, b) evaluation of the effectiveness of thrombolysis from changes in the fibrinogen concentration. Two groups of patients were examined with the Q form of acute myocardial infarction who were treated at an intensive care unit. The first group comprised 62 patients (mean age 56.3 years) who were given 1 million units of streptokinase by the i.v. route. The second group comprised 52 patients who were given 1.5 million units of streptokinase. The authors investigated the fibrinogen plasma concentration and the fibrin breakdown product concentration in serum after 3-hour intervals. Concurrently they examined the creatine kinase curve. They proved a significant increase of the fibrinogen and fibrin degradation products before thrombolysis (p less than 0.001). The fibrinogen concentration declined in both groups highly significantly with a maximum after 12 hours. The fibrinogen level reached normal levels in both groups after cca 24 hours. Patients with a high initial fibrinogen level (above 5 g/l) did not attain hypofibrinogenic values during thrombolysis. This phenomenon did not depend on the size of the streptokinase dose. The decline of fibrin breakdown products in both groups declined to baseline values within 24 hours. The authors confirmed that the effectiveness of thrombolysis is influenced among others also by the residual fibrinogen value.

[Long-term study of disorders of heart rhythm in patients with chronic diseases of the respiratory system].

The authors investigated the incidence of arrhythmias by the method of continuous recording of ECG tracings according to Holter in 16 patients with chronic respiratory diseases three times in the course of two years. The incidence of arrhythmias was very frequent, more serious disorders (class 3 or a higher class according to Lown's classification) were very rare. The authors provided evidence of a great variability in the incidence of arrhythmias in the same patients during different periods of investigation. They did not reveal a relationship between the incidence of arrhythmias and the degree of functional affection of the lungs, the severity of hypoxaemia and the drop of the maximum aerobic capacity of the lungs. They did not find a relationship between the incidence of arrhythmias and the pressure in the pulmonary artery, or the size of the right or left ventricle or the thickness of their wall. The authors do not consider the finding of arrhythmias in patients with chronic lung disease an unequivocally serious finding, as reported sometimes in the literature.

The decomposition of a distribution into Gaussian components (Algol program.

The paper attempts to give a practical solution to the problem of decomposition of a mixture of Gaussian components. The results of an earlier paper [1] have been used and the described algorithm has been revised and improved. A complete Algol program is given with all the necessary comments and descriptions and with results of testing examples.
