Influence of frusemide on cis-dichlorodiammineplatinum (II) pharmaco-kinetics.

Cis-dichlorodiammineplatinum (CDDP) pharmacokinetics were evaluated in eighteen patients with cancer who received 80 mg/m2 CDDP as a 20 min infusion. One hour before, 10 of them had 20 mg/m2 frusemide. Fifteen blood samples and fifteen urine samples were collected over the 5 hours following the infusion of CDDP. Platinum was assayed by flameless atomic absorption. The data did not detect any difference between patients with or without frusemide for the following platinum parameters: plasma concentration, urinary concentration, cumulative urinary excretion, renal clearance. These results suggest that frusemide has no influence upon cisplatin (CDDP) pharmacokinetics and if it protects the kidneys, it is not via kinetic modifications.

An uncommon apudoma: a functional chemodectoma of the larynx. Report of a case and review of the literature.

The authors report a case of laryngeal chemodectoma in a 53-year-old man who died from general dissemination and lymphatic and subcutaneous metastases. This functional neoplasm secreted calcitonin revealed by fluorescence and biochemical tests and perhaps adrenaline-like substances. Its cells looked like the light chief cells of the human carotid body; they contained secretory granules and lysosomal formations. Thirty similar cases had been previously reported. The superior laryngeal glomi and more occasionally the inferior ones were the seat of these tumours which occurred with an equal frequency in both sexes. A hoarseness was often the unique symptom noticed during the several months or years preceding the first examination of this slow growing neoplasm. Nevertheless among the chemodectomas of the head and the neck region those arising in the larynx had the highest incidence of malignant evolution. Five of the patients presented a general dissemination, two had lymphatic metastases and one a local recurrence. The ultra-structural features and sometimes the rarely detected functional activity of these chemodectomas are those of apudomas whose cells might originate from the cephalic portion of the neural crests.

[Bilateral breast localization of Kahler's disease: cytological and histological aspects].

Breast localisation of multiple myeloma is quite unfrequent. In one case of a 69 years old man, the diagnosis was suspected by a cytological aspiration biopsy of the lesion, and later confirmed by histological and biological investigations. The analysis of the literature has enabled the authors to gather 7 other cases and to describe the characters of this peculiar tumoral localisation. The breast involvement is mostly observed in women. The average is 38 years old. Only 2 cases deal with men, both treated with estrogens. The lesion is usually bilateral and nearly always associated with other visceral and bone localisations. It is not modified by chemiotherapy and appears shortly before death.

[Course of blood creatinine during repeated injections of cisplatin. Influence of drug combinations].

Cisplatin (CDDP) is known to induce nephrotoxicity. In this retrospective study, we have investigated the evolution of plasma creatinine in long term therapy with CDDP. We have studied case history, mode of administration of CDDP and associations with other chemotherapeutic agents or drugs which could potientiate CDDP renal damage (non steroidal anti-inflammatory agents, analgesics, antibiotics, antihypertensive agents, diuretics and contrast media). Mean creatinine concentration versus time rises significantly. This elevation is significantly higher in patients with hypertension, diabete, one functional kidney, or abdominoperineal irradiation. The association with other drugs has not proved a real influence on creatinine evolution.

[Adjuvant chemotherapy in treatment of non seminomatous germinal testicular tumor. Results and reflexions about 77 cases (author's transl)].

To define prognostic factors of testicular cancer and to develop a strategic therapy based on clinical, pathological and biological findings, 77 cases of non seminomatous testis tumor, observed in G.F. Leclerc Center from 1967 to 1977, have been analysed. For all these patients early chemotherapy program was associated with surgery; radiation therapy was reserved for stages with retroperitoneal metastatic disease. Results of this review confirmed the interest of such medical treatment which improved survival in all stages of disease. Some prognostic factors could be detailed from these cases: nature of tumoral components, invading of lymph node, presence or not of biological markers; identification of these would unable us to choose the best adjuvant chemotherapy program and to define its place in the management of testis tumor's treatment.

[Carcinoembryonic antigen in carcinoma of the uterine cervix. Correlations with clinical stage, histopathology, and survival about 115 cases (author's transl)].

Carcinoembryonic antigen is elevated in the serum of approximately 35% (35/98) of patients with carcinoma of the uterine cervix. The incidence of elevated serum CEA is not related to stage of disease, histopathology, lymphangiogram and survival rate. Carcinoembryonic antigen levels return to normal after curative radiation therapy (25/32). Persistently high values after irradiation are indicative of residual disease (3/32). Reappearance of CEA, with a progressive rise evoke a recurrence, and may precede the clinical diagnosis.

[Anticancerous chemotherapy trial with duborimycin. Analysis of 151 cases].

Duborimycin is a new antimitotic agent approaching danorubicin and adriamycin in activity which has been tried on 151 patients suffering from cancer of different types, is an advanced local/regional stage and/or metastatic disease. It was administered intravenously every fortnight in a mean unit dose of 400 mg, and the duration of the treatment ranged from 2 to 52 weeks. Objective improvement was registered in 56 patients of the 135 cases in whcih the results were assessed (around 41.4% of cases). In 4 cases the regression of tumour volume was greater than 50% (one of these cases was in melanoma, the other a sarcoma) and in 2 cases regression was complete (a squamous cell carcinoma and an embryonal testicular tumour). The subjective effects were appreciable in 53 of the 115 cases which could be studied (46%) and above all in the refractory pain of bony secondaries from breast cancer (a favourable response in 78% of cases). Manifestations of intolerance/toxicity were of a minor nature on the haematologic side, that cardiologic ones relatively frequent (18% of treated cases) and occasionally serious (2 cases of asystole). Great care is therefore necessary in supervision of the treatment. However, the first results obtained by this line of approach, notably in chemo-resistant forms of tumour such as melanoma and sarcomas, utilizing the very strict criteria in one analysis encourage further study of duborimycin in cases of this sort (preferably in association and in accordance with protocols of comparative trials) so that its place in cancer chemotherapy may be more precisely defined.
