Percutaneous needle muscle biopsy in the diagnosis of neuromuscular disorders in children. Histological, histochemical and electron microscopic studies.

Percutaneous needle biopsy was carried out in 75 patients with various neurological and muscle diseases (myopathies in 7, lower motor neuron diseases in 22, upper motor neuron diseases in 6, and unlocalized or miscellaneous diseases in 13) and its usefulness as a diagnostic procedure was evaluated. The ages of the subjects ranged from 2 months to 35 years, 77% were younger than 6 years. The materials were examined histologically, histochemically and electron microscopically with the following results. 1. The muscle volume obtained by needle biopsy was sufficient for studies in 64 cases (85%), insufficient in 8 (11%), while in 3 cases no muscle tissue was obtained. 2. Histological staining properties were excellent in all samples examined. In histochemical preparations, staining was judged as good in 61 cases, but poor in 11 due to an inappropriate freezing procedure. 3. In 33 cases, both needle and open biopsies were carried out simultaneously, and the morphological findings of the specimens were compared. There were no significant differences between them histologically as well as histochemically. Electron microscopic examinations also provided identical information in the seven cases studied so far. 4. The method of needle biopsy has many advantages over open biopsy as a routine diagnostic procedure for neuromuscular disorders in infants and young children, but there are a few disadvantages such as the requirement of technical skill and the inappropriateness for histogram construction in some cases.

Monocistronic and polycistronic bacteriophage T4 gene 23 messages.

We studied transcription of T4 late genes by in vitro translation of size-fractionated late RNA and by hybridization of T4 late RNA to plasmids containing identified T4 late genes. We identified mRNA species that coded for the late proteins gp10, gp18, gp21, gp22, gp23, and gp24. Functional mRNA's that coded for the early proteins gp32 and IPIII were also detected after fractionation of late RNA. As in preparations of early RNA, gene 32 message activity was present in two species of RNA, which had molecular weights of approximately 0.5 x 10(6) and 0.8 x 10(6), and IPIII message activity was present in multiple species of RNA. Gene 24 and gene 10 message activities migrated as single species that had approximate molecular weights of 0.5 x 10(6) and 1.2 x 10(6), respectively. mRNA activity for gp18 migrated heterogeneously. We detected multiple transcripts from gene 23 by in vitro translation and by hybridization of late RNA to plasmids containing genes 21 through 23. Both types of analysis indicated that the major gene 23 transcript had a molecular weight of 0.75 x 10(6). In addition, two gene 23 transcripts having molecular weights of about 1.0 x 10(6) and 1.3 x 10(6) were present; these RNA species also coded for gp21 and gp22. Physical linkage of transcripts from genes 21, 22, and 23 was demonstrated by hybridization.

CT of abdominal malignancies: dynamic approach.

By sequential CT scanning, the dynamics of intravenously injected contrast medium were observed in 21 patients with abdominal malignancies. Within 30 sec, and one breath, four sequential cuts of the same section were obtained. The corticomedullary junction of the kidney was clearly delineated in the early phase, and tumor vascularity was well appreciated within vascularized malignancies. Angiographic CT was found to be superior to conventional CT and is promising in the assessment of the nature of abdominal tumors. In some cases, use of CT angiography may enable deferring catheter angiography.

Various manifestations of stratification phenomenon during intravenous cholangiography.

The authors classify various types of stratification phenomenon during intravenous cholangiography. The stage of gallbladder opacification in the recumbent position has been classified as (I) mottled, (II) dendritic, (III) ring-like, and (IV) homogeneous. 'Dendritic' type of stratification phenomenon has never been reported in the literature to our knowledge. At 20 min following infusion of contrast material homogeneous opacification of the gallbladder was noticed in only 14% of patients. The others fell into types I, II or III of stratification phenomenon. In contrast, 87% of the opacified gallbladders were homogeneous on the after fatty meal film. It is therefore mandatory for diagnosis that either a 24 h film or a fatty meal film be taken to avoid the stratification phenomenon.

Labelled gelfoam in transcatheter arterial embolization of renal cell carcinoma.

Six patients with renal cell carcinoma underwent transcatheter arterial embolization with Gelfoam labelled by metallic clips. The embolization procedure was more easily monitored by fluoroscopy, and the complication of peripheral embolization was prevented. Tumor regression can be evaluated by changes in the distribution of the metallic clips on plain films.
