QT dispersion and T-loop morphology in late pregnancy and after delivery.

The aim of the study was to detect changes of both the QT dispersion and T-loop morphology resulting from the changed spatial position of the heart during pregnancy. Electrocardiographic and vectorcardiographic recordings were obtained from 37 healthy women 19-36 years old in the 36th to 40th week of physiological pregnancy and 2 to 6 days after delivery. The same recordings were obtained from 18 healthy women of the same age. The average QT dispersion (+/- S.D.) in normal subjects was significantly lower (34 +/- 12 ms) than in those in late pregnancy (73 +/- 18 ms) (P < 0.001). The average amplitude of T-loop (Ta) in women in late pregnancy was significantly (P < 0.001) smaller (532 +/- 98 microV) and the width of T-loop (Tw) was wider (21.24 +/- 11.48 deg) than in the control group (793 +/- 114 microV and 7.17 +/- 3.02 deg, respectively). The partial post-partum restoration of all parameters was not significant. In all groups, the QT dispersion was significantly correlated with Tw but not with Ta. According to these results we can conclude that the QT dispersion is an indirect reflection of the complete process of ventricular repolarization, reflected in the morphology of the T-loop.

Application of Corynebacterium parvum lysate into the brain of rats--morphological study.

The authors investigated the influence of application of the Corynebacterium parvum lysate (CPL) into the brain of rats with aim to define the resulting morphological changes. Variable amount of the CPL (0.1, 0.35, 0.7 mg) was injected under stereotactic control into frontal lobe of the left cerebral hemisphere. Morphological changes were studied after intervals of 3, 7, 14, and 30 days respectively. CPL lead to inflammatory and reparative changes in direct relations to the administered dosage. The most pronounced reaction was observed on days 3 and 7. Morphological changes were still visible on day 14. There were no valid difference between the application of efficient CPL and the control physiological saline solution (PSS) after thirty days. The authors suggested that CPL is likely to be safe also in human malignant brain tumor therapy.

Preliminary experimental experiences with xenotransplantations.

In our xenotransplantation experiments has been a major aim to induce specific tolerance. The results from the meeting of the donor's and the recipient's immunocompetent systems already in the period of embryonic development and/or the influence of methylprednisolone are described.

Histological and biochemical characteristics of kryoprotected embryonic nervous tissue for transplantation purpose.

In clinical and experimental conditions, a biochemical equipment and a histological image of a fresh embryonic nervous tissue and a tissue conserved by deep freezing for transplantation purposes were examined. By repeated examination of neurotransmitters, the contents of dopamine and serotonin were found unchanged by freezing. In a human embryo, serotonin predominates over dopamine, while in chicken embryos, used for xeno-transplantations, it is vice versa. As for the microscopic structure of nervous tissue, no influence of kryopreservation for 4 to 14 days was found.

[Directed photonucleolysis of the intervertebral disk and its herniation. Preparation for percutaneous laser diskectomy].

Under model conditions it proved possible, using a whole-body stereotactic apparatus, to focus on a human intervertebral disc and its prolapses and eliminate them by photocoagulation, using a laser. The suitable cannulae have a 3 mm diameter and an output of 6 to 12 W. The necessary lesion is reached in a fractionated manner within 1 minute. Thus the conditions were created for the new technique of percutaneous disectomy which has been done hitherto by means of chemical substances or mechanically by incision and suction.

[Technics of preparation and collection of embryonal nerve tissue for transplantation in clinical practice].

The proper author's experience is used in discussing possibilities on how to obtain the embryonal nerve tissue's samples for transplantation in various CNS diseases. The most reliable, though a unique one is the obtention of the whole foetus. This may be exclusively done by the miniabortion using suction curettage, or eventually at greater dilatation of cervix with the use of fine abortive forceps providing the ultrasound control. The most secure source of tissue in order to make transplantation is the removal of pregnant uterus, e. g. for its myomatosis. The proper preparation of tissue for transplantation is provided with maximally aseptic conditions.

Contribution of pregnan steroid Althesin to the pathophysiology of epilepsy.

The original study with Althesin in the development of epilepsy was carried out in nine patients. EEG and SEEG records show that sensitivity to Althesin, which is a mixture of pregnandiol and pregnan acetate, precursors of sexual hormones, raises in the direction to the oldest brain formations. A physiological substance in provoking epilepsy was discovered.

Functional complications of therapeutic stimulation.

A new category of complications during therapeutic stimulation of the brain was described and classified as functional complications. They occurred by combined stimulation in one level of the brain, namely in thalamus, by unprecisely localisation of the electrode or by its spontaneous dislocation from the target, what is dependent on the quality of the electrode. Mostly they can endanger the patient.

On the treatment of vertebrogenic pain using laser-induced lesion of the spinal ganglia.

Recently, there has been an increase in the use of spinal-ganglia lesions as a means of surgical treatment of postdiscotomic algic syndromes. After relapses following thermocoagulation, the authors began to use lesions caused by a laser beam of an output of 30 W and temperature of 2,000 degrees C. As the resulting changes are irreversible, lasting effects of the procedure can be expected.

Our own clinical experiences with deep electrodes in psychophysiological studies.

The experiences obtained during 30 years work with the deep electrodes gave the possibility to formulate some psychophysiological concepts of 1) memory, 2) vertical homunculus and 3) cooperation between limbic system and hypothalamus. Simultaneously they hinted at the exploitation of Au-Fe dipole for the stereotactic treatment of epilepsy and at the kindling phenomenon in man.

[The role of the whole-body stereotaxic apparatus in directed interventions in humans].

All-body aiming apparatuses in conjunction with computed tomography are a new promise in the diagnostic and therapeutic complex. They make it possible to reach any point inside the body from any point on its surface. The space where the target is found is operated by the arms of the apparatus in three planes normal to each other and by rotating two angles round the longitudinal and transverse axis. Guiding of the apparatus on the target is controlled by the computer programme. Immobilization of the patient is achieved by a vacuum cushion. The apparatus was tested under model conditions and is ready for clinical use.

[Transplantation of embryonal nerve tissue into the brain of donors under experimental and clinical conditions].

The authors, stimulated by the results of transplantations of embryonic nervous tissue under experimental conditions and by findings on the biochemical background of some manifestations of mental diseases, dared to use for the first time transplantation of portions of human embryonic brain in the treatment of schizophrenia. Before the experiment they evaluated the bilateral xeno as well as autotransplantations in ten cats. Under clinical conditions they used transplantation of embryonic nervous tissue in two patients with the acute and chronic form of schizophrenia. The operation was performed, using Mundinger's stereotactic apparatus controlled by a computer. The tissue from the central portion of the embryonic brain from the 5th and 7th week of gestation with a high serotonin content was deposited bilaterally in the septal area in the recipient's brain. They recorded improvement in particular in a case of acute schizophrenia. By monitoring the foci of transplanted tissue by computed tomography and magnetic resonance, they provided evidence that embryonic nervous tissue is incorporated in the recipient's brain without reverting into malignant growth and that it is not rejected by the immune system of the recipient.

[A multipurpose vacuum mattress in health care].

The authors draw attention to vacuum mattresses which are a modern appliance for the immobilization of casualties and patients. They present their initial experience with the use of these mattresses during examinations on a computed tomograph, during all-body stereotaxis and minor surgical operations. The material from which the mattress is made allows X-rays to penetrate, therefore it can be used also for immobilization of restless patients and children during X-ray examinations. It is, however, possible to manufacture also various types of vacuum splints for fixation of the extremities after injuries.

Position of whole body stereotactic device among targeted interventions into human organism.

An universal targeting device coupled with computerized tomography is a new promise in the complex of diagnostics and therapy. It allows any point inside the human body to be reached from any spot on its surface. The space containing the target is controlled by moving the arms of the device on three mutually perpendicular planes and by turning at two angles around the longitudinal and transversal axes. The targeting is controlled by computer. The patient is immobilized on a vacuum mattress. This device was tested in model conditions and is now ready for clinical application.

[Perspectives in the treatment of some types of pain using lasers. Histological correlation with experimental results].

Treatment of some painful syndromes in the innervation area of spinal ganglia, particular in persisting pain after operations of prolapsed intervertebral discs or operations of the discs, call for new therapeutic methods. Promising possibilities are elimination of the spinal ganglion or nucleolysis of the disc by laser rays, as revealed by experimental studies in vitro and histological check-up of the affected tissue. Three concentric layers of the focus, the carbonization zone, the zones of evaporization and coagulation display irreversible changes which are prerequisites of a permanent effect.

[Development of a moral consciousness in medical schools].

In the conditions of developed socialism, changes in social consciousness are also reflected in one of its forms - the morals. The problems of moral education as an inseparable part of communist education is coming to the foreground. It plays its role in all the stages of the education system, having its unsubstitutable place also in the conditions of universities and technical colleges, the faculties of medicine not excluding. It is its task to form the personality of the student and graduate, to make them, among others, acquire the moral codex of a builder of communism (habits of moral behaviour in professional activities - medical ethics). Such crucial situations must be found that are of cardinal importance for the formation of the medical student's moral profile (transition from the secondary school to university, entering the clinic, meeting the patient, running health institutions, positive and negative influences in the health service). The student must live the moral situation, obtain the experience from moral acting on his own self and be an active participant in the events creating his moral qualities, primarily in collective conditions. We stress the necessity of engagement - mobilisation of students to all-round social activities, connection of the theoretical findings of marxist ethics with their particular problems and actual activities. A special accent is laid on the method of personal example, the moral example of the university teacher himself, in the first place his authority based on all the progressive components of his personality.(ABSTRACT TRUNCATED AT 250 WORDS)

First model of whole body computerized stereotactic device.

The whole body computerized stereotactic device was created by a group of specialist in various professions. Its connection with computer tomography has overcome the limitation of the stereotactic method to the brain surgery and the changeability of anatomic relations inside the human body. Computing technique and special patients' fixation are new supplements in the contemporary stereotaxy.
