Myocardial revascularization by CO2 laser.

An original method of myocardial revascularization using CO2 laser has been studied in a pig model with acute myocardial infarction. Transparietal channels were created by myocardial vaporization, using a CO2 laser beam. A comparative evaluation was made against a control group using as parameters survival rate, ECG changes, anatomopathologic findings, and angiographic studies. With the exception of favorable changes in the ECG there was no other evidence of improved regional perfusion. No evidence of long-term patency of the transmyocardial channels could be demonstrated by angiographic and histologic studies.

Arterial healing in rabbits following carbon dioxide laser endarterectomy.

CO2 laser open endarterectomy of small atherosclerotic vessels appears feasible. Under the conditions described, the endarterectomized arterial wall is not smooth but the thrombogenicity of the laser-treated surface is low, and the endothelial healing rapid. These advantages are to be weighed against the consequence of thermal injury, in particular, the late development of aneurysms. Before human clinical trials are made, further experimental studies are required to determine the best type of laser and optimal wave length to limit thermal injury.

[Experimental realization of a continent ileocolic valve in rats].

The ileocecal valve plays a primordial role in pediatric digestive disorders. Various technical procedures have been proposed based on formation of an anastomosis with invagination of the distal extremity of the ileal segment into the colic portion. A resection of 20% of the small intestine and an ileocolic anastomosis were performed in the rat using three different methods: simple end-to-end ileocolic anastomosis, ileocolic anastomosis with invagination of ileal segment, and ileocolic anastomosis with ileal hemi-valve intubated into colon. Three animals died in the first group, the surviving three animals presenting massive reflux on barium meal examination. Of the 7 animals in the second group, 6 developed signs of occlusion from anastomotic stenosis. Two of the 10 animals the third group died, one before the 15th and one before the 40th day, as a result of anastomotic stenosis. Autopsy of the 8 survivors sacrificed after 3 months showed permeable anastomoses.

[Reconstructive microsurgery of the vas deferens. Experimental study of suture materials].

Microsurgery is the procedure of choice for vasectomy reversal. The aim of this study was two compare two different suture materials for vasovasectomy - a nonresorbable material (nylon 10/0 with a BV 6 needle), which is widely used, and a resorbable material (polyglycolic acid, also with a BV 6 needle), which has not yet been evaluated for this use. 28 Sprague-Dawley rats were operated on under microscope. Two groups were then compared, group A with nylon (n = 14) and group B with polyglycolic acid (n = 14). In each group, 8 animals had a vas deferens section and 6 had a previous vasectomy by ligature. Ten days postoperatively, the patency rate of the anastomosis was evaluated by the presence of sperm on both sides of the suture line. The contractility was assessed by mechanical stimulation. The existence of a sperm granuloma was considered as indicative of a non functional anastomosis. Three days later a fertility test was performed, lasting three months, and the number of litters was checked. The rats were sacrificed after three months, and each vas deferens was examined histologically or by electron microscope. The macroscopic results were: 57% patent anastomoses in group A and 77% in group B. 16% patent anastomoses after ligature in group A (n = 6) and 75% in group B (n = 6). The pregnancy rate was 54% in group A and 77% in group B. After previous ligation, the corresponding figures were 20% and 83% respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

New fiber for ligament and tendon prosthesis.

A new material has been used as tendon and ligament prosthesis on animals. The long-term clinical results obtained after ACL and Achilles tendon replacement suggest an adequate tolerance and a function of the prosthesis.

[Experimental study of a biological micro-arterial prosthesis].

An arterial prosthesis prepared from human placental arteries, diameter 1.0 mm, length 20 mm, was evaluated after implantation into the infrarenal region of the aorta of the rat. The permeability rate after one year (70%) compares favorably with results after fresh arterial autografts. Histology showed that the prosthesis acted as a matrix colonizable by the tissues of the recipient. The internal surface of the tube was covered by a tissular layer in which fibrillary elements suggestive of elastic fibers were developing.

[Longitudinal salpingotomy and fertility. Experimental study in the female rabbit].

A microsurgical technique of salpingotomy has been put forward for the treatment of ampullary distal lesions responsible for infertility. --An experimental research has been carried out before proposing any application on women. --The consequences on the fertility of a female rabbit of a longitudinal salpingotomy followed by a microsuture have been thoroughly studied. --21 female rabbits have been operated under microscope. --In every case the tubal permeability has been maintained and the rabbits became pregnant. --In some cases histological lesions in front of the suture area have been noticed, what aroused several possibilities as to their origin.

[Experimental study of a new technic of sutureless tubal anastomosis in the rabbit].

The authors have tested an original method of tubal anastomosis in the rabbit, which does not require suture material. The principle of the operation is the intubation of the ovarian tube by a silicone elastomere tube which acts as a guide for healing. From the 11 rabbits operated, the authors obtained 7 pregnancies, i.e. 63,6%. The results of this series do not appear to be any different from the pregnancy rates obtained by other teams with microsurgical techniques in the rabbit. The authors conclude that their method is as reliable, but more simple.

[Experimental test of two tissue adhesives (GRF and IBC 2) in vascular microsurgery in the rat (author's transl)].

Tissues adhesives have been evaluated on rats, during microvascular surgery: the IBC 2 and the GRF glues. The short and long term evolution is studied. The patency rate of the anastomosis is analyzed and the histological aspect is studied. The GRF glue exhibits a rapid resorption rate, but the long term results show a high toxicity against the muscular layers of the artery. The IBC 2 glue has a slower resorption rate and its long term effect are restricted to a foreign body reaction.

[Small-caliber vascular prosthesis].

Further research on biomaterials should permit the development of an adequate microarterial prosthesis. This program should also allow improvements of the presently available arterial substitutes. Various types of conduits have been tested on an experimental model: prosthetic tube, biological conduits either preserved or chemically modified. The most encouraging results have been observed with an arterial conduit, prepared from fresh aorta, treated by trypsin and glycosaminoglycans.
