Resection of the rectum with construction of a colonic reservoir and colo-anal anastomosis for carcinoma of the rectum.

Rectal resection with colo-anal anastomosis was performed in 65 patients with carcinoma of the lower rectum. In 20 a pelvic colonic reservoir was constructed while in 45 a direct anastomosis was carried out. There were no postoperative deaths and morbidity was comparable in the two groups. Functional results were determined by clinical examination and manometry. The frequency of bowel movements was inversely related to the maximum tolerated volume (P less than 0.001). During the first year 60 per cent of the patients with a reservoir and 33 per cent of the patients without had one or two stools per day (P less than 0.05). After one year, 86 per cent of the patients with a reservoir and 33 per cent of the patients without had one or two bowel movements per day (P less than 0.01). The maximum tolerated volume was increased by the reservoir (P less than 0.05). The loss of reservoir capacity of the rectum increases frequency of bowel movements in colo-anal anastomosis. The creation of a colonic reservoir improves function by increasing the maximum tolerated volume without any increase in mortality or morbidity.

[Lymphatic and lymph node diffusion of ceftriaxone. Incidence in the treatment of typhoid fever].

Following intravenous administration of 2 g ceftriaxone, concentrations of the drug were assayed in serum, in thoracic duct lymph from dogs, and in mesenteric lymph nodes in patients. Antibacterial activity of lymph against S. typhi was also studied. Results show that ceftriaxone concentrations in serum and lymph are comparable; with a satisfactory antibacterial activity of both fluids against S. typhi. In mesenteric lymph nodes, mean ceftriaxone concentration was approximately 1000 times the MIC for S. typhi. Our data contribute to explain the successful clinical results achieved with ceftriaxone in patients with typhoid fever.

[Serologic diagnosis of syphilis by the passive hemagglutination test].

The passive haemagglutination test in the diagnosis of syphilis was used with: 139 sera posing serological problems, 965 sera from subjects with syphilis at all stages of the disease and non-syphilitics. The percentages of agreement seen both in comparison of THPA with the TPI (94,5 p.cent) and the FTA abs test (97 p.cent) demonstrate its high degree of specificity and sensitivity. The reaction may be performed rapidly on a routine basis and requires little in the way of specialised material. These qualities should lead, in the near future, to the adoption of the T.P.H.A. as a routine examination in all laboratories, reserving the use of the F.T.A. abs test and the T.P.I. for difficult cases.
