Classification of commercial apple beverages using a minimum set of mid-IR wavenumbers selected by Procrustes rotation.

When infrared spectral data are used in classification and/or multivariate regression methods there can be problems related to both chemical understanding and computation speed due to the large number of wavenumbers in each spectrum. Here, it is shown that the Procrustes rotation technique can be used to select a minimum set of spectral variables (wavenumbers) to perform classification and regression. Procrustes rotation was coupled to several multivariate methods as PLS, SIMCA and potential curves (a maximum likelihood classification method). The practical problem of implementing a screening methodology for classifying apple juice-based beverages according to their contents of "pure" apple juice was addressed using attenuated total reflectance, mid-IR spectroscopy. It is found that two of the original wavenumbers are almost as good predictors as all the 176 initial ones.

Hemorrhagic ascites associated with endometriosis. A case report.

BACKGROUND: Endometriosis associated with massive, bloody ascites is an unusual occurrence. This report draws attention to this condition as a complication of endometriosis, with the description of a case and a review of 31 others. CASE: A 41-year-old, black nulligravida with massive, bloody ascites and a pelvic mass underwent laparotomy, and an intraoperative microscopic examination ruled out malignancy. The histologic report was compatible with endometriosis. The patient was treated with a GnRH analog, with progressive reduction of ascitic fluid and full remission after six months. CONCLUSION: Bloody ascites should be considered a complication of endometriosis, especially in nulliparous women of childbearing age with abdominal distention, a pelvic mass, dysmenorrhea, abdominal pain, weight loss and eventual pleural effusion, suggesting a diagnosis of ovarian malignancy.

Colposcopic scoring system for biopsy decisions in different patient groups.

Colposcopic scoring system have been used for distinguishing low-grade from high-grade cervical lesions. However, none of the previous studies have reported colposcopic scoring systems for biopsy decisions in different patient groups. The purpose of our study was to evaluate the safety of biopsy decisions using the colposcopic score elaborated by Stellato and Paavonen (IL) in 21 nonpregnant HIV infected patients (NP+) and 36 uninfected patients (NP-), 12 HIV infected pregnant patients (P+) and 20 uninfected pregnant patients (P-) in the diagnosis of cervical intraepithelial neoplasias (CIN) and HPV infection. The receiving operator curve was used for the establishment of a cut-off point in the scoring system graduation. The chi-square test was used for the statistical analysis. We obtained a safety cut-off value in the colposcopic scoring system for each patient group: 4.5 for NP+; 4.0 for NP- and 3.5 for P+ and P- patients. The sensitivity and specificity of the colposcopic score for the detection of high-grade lesions for each group were respectively: 87.5 and 92.3% for NP+ patients; 90.9 and 92% for NP- patients; 100 and 87.5% for P+ patients and 100 and 91.7% for P- patients. Our results suggest that the colposcopic scoring system is a practical tool for a colposcopy-guided punch biopsy decision and detection of high-grade cervical lesions in different patient groups. Further studies are needed to prove its clinical utility.

Vaginal hysterectomy for correcting genital prolapse. Long-term evaluation.

OBJECTIVE: To determine whether a transvaginal hysterectomy with anterior and posterior repair is effective in the long term in treating uterovaginal prolapse and stress urinary incontinence (SUI). STUDY DESIGN: Seventy-four patients subjected to vaginal hysterectomy for the treatment of severe genital prolapse, on average five years before the study, were contacted by letter for evaluation. Four of these patients had died, and 47 (67.1%) responded to the letter. The mean age of the patients at the time of reevaluation was 66.1 +/- 10.6 years, and mean parity was 6.6 deliveries. RESULTS: All patients but two presented some degree of genital prolapse at the time of reevaluation, with three cases of total vaginal vault prolapse. White patients (87.2%) predominated over African (black) patients (12.8%). SUI associated with prolapse persisted in 14 of the 20 patients, and 6 others had this complaint after surgical correction (22.2% of previously continent patients). CONCLUSION: The rate of unsuccessful surgical correction of severe genital prolapse was very high (95.7%), and cure of SUI was low (30%), with SUI actually arising after surgical correction in 25% of continent patients. In addition to parity, there seems to be a racial factor linked to the onset and maintenance of this pathology, with a higher prevalence among white patients.

A morphometric study of maternal smoking on apoptosis in the syncytiotrophoblast.

OBJECTIVES: To study syncytiotrophoblast apoptosis in the placenta of smoking and non-smoking pregnant women. METHODS: Twelve neonates, pregnancies and placentas were available for study. Eight mothers smoked during pregnancy and the remaining four were non-smokers used as control subjects. The main outcome measure was the apoptotic syncytiotrophoblast index for each group. Apoptosis was detected by immunohistochemistry using the TUNEL method and quantitatively measured using a Merz grid. The apoptotic syncytiotrophoblast index was calculated as the ratio of mean apoptotic labeling to percent terminal villus area using high-power field microscopy. RESULTS: Significant differences in apoptotic syncytiotrophoblast index were observed between the control group (15.06+/-3.72) and the smoker group (1.66+/-1.74) (P < 0.0001, Mann-Whitney test), but no differences were detected in clinical or morphometric data between groups. CONCLUSIONS: The human placental syncytiotrophoblast undergoes apoptosis and this process is associated with inhibition of apoptosis by the smoking habit. The same way as the presence of trophoblast apoptosis is associated with modifications of the maternal-fetal exchange, the inhibitory effect of the smoking habit on syncytiotrophoblast could be responsible for the poor prognosis of pregnancy in the presence of maternal smoking.

Diarrhoea mortality in Aracaju, Brazil.

We report diarrhoea mortality rates in the city of Aracaju, Brazil from 1992 to 1994 when there were 318 deaths due to diarrhoea. Eighty-nine per cent of deaths occurred in infants: 53.1% were boys and 78.9% died in hospital. Diarrhoea mortality rates for children under 5 years of age were 2.63, 1.78 and 1.28 for 1992, 1993 and 1994, respectively. The district with the most slums and least sewerage services had the highest mortality rates (4.29, 2.53 and 1.87 for 1992, 1993 and 1994, respectively) and the district with most services and fewest slums had the lowest rates each year (0.79, 0.55 and 0.38). Malnutrition was associated with diarrhoea on 26.5% of death certificates. Mortality rates fell during the study period but still remained high in infants from poor areas of the city. Study of the geographical distribution of mortality allowed identification of priority areas for targeted interventions.

Cadmium analysis in soil by microwave acid digestion and graphite furnace atomic absorption spectrometry.

The need to determine micronutrients and toxic elements in soils has grown in recent years and cadmium is of special interest. A method has been developed for the determination of cadmium in soils based on a prior acid digestion of the samples with nitric acid in closed Teflon vessels, into a microwave over. The cadmium determination was carried out by graphite furnace atomic absorption spectrometry (GFAAS) with L'vov platform. Optimum operating conditions, analyte modifiers and matrix interferences have been investigated. The best matrix modifier was found to be ammonium dihydrogen phosphate. The interferences are greatly reduced under these operating conditions and calibration can be performed with simple aqueous solutions of the metal standard. The method is rapid and provides accurate and precise results that agree with certified values for two reference materials: BCR 141 (calcareous loam soil) and BCR 277 (estuarine sediment).

Prediction of FCC gasoline octane numbers using FT-MIR and PLS.

A method for predicting "octane numbers" (RON and MON) in fluid catalytic cracking (FCC) gasolines is proposed. Using FT-MIR and PLS, improvements have been obtained in sample throughput, reduced delay times, accuracy (repeatability and reproducibility), amounts of samples and reagents and environmental working conditions when compared with current standard methods. A total number of 140 daily production samples were taken; and from there, a learning group was prepared (44 samples); a validation set (96 samples) was prepared, as well. Sample spectra were recorded from 4000 to 600 cm(-1) at 4 cm(-1) intervals (traditional sealed NaCl cells). The PLS technique was used in its two variants (1 and 2-block). Both provided similar results. Their predictive characteristics are very good: SEP(RON)=0.38; SEP(MON)=0.40; repeatability <0.1 O.N.; reproducibility <0.3 O.N. (SEP=Standard Error of Prediction).

Natural killer cells and interleukin-12 in patients with advanced cervical cancer under neoadjuvant chemotherapy.

Patients with advanced cervical cancer have deficient natural killer (NK) cell activity, usually as a consequence of tumor invasion, which results in tumor NK cell sequestration. The reason for the occurrence of such alterations in patients under chemotherapy is unknown. We evaluated the activity and number of NK cells and T cell subpopulations in ten patients before and three weeks after neoadjuvant chemotherapy (CT). The schedule used was cis-platinum (100 mg/m2 per cycle) and bleomycin (15 mg/cycle), repeated every 28 days. Although there were similar levels of NK cells before and after CT in both groups, we observed greater cytotoxicity of peripheral blood lymphocytes and increased levels of CD4+ and CD8+ T cells (P < 0.01) in five patients who presented a good clinical response when compared to the group with a poor response. IL-12, known to increase NK cell activity when added to peripheral blood lymphocyte cultures, markedly increased lytic activity before and after CT only in the group with a good clinical response. These results suggest that NK cells from the poorly responding patient group express less lytic activity per NK cell and are insensitive to IL-12 stimulation, probably as a result of reduced IL-12 receptor expression or a defective intracellular transduction mechanism. The present findings may be useful as a prognostic factor in clinical practice and also provide support for human clinical trials of IL-12 and neoadjuvant CT for the treatment of malignant cervical tumors.

Evaluation of cardiovascular control by neurons in the dorsal medulla of rats.

The contribution of sympathoexcitatory neurons in the dorsal medulla to the regulation of arterial pressure and the involvement of such neurons in integration of physiological responses or in the genesis of basal vasomotor tone are not well defined. In the present study discharge of neurons in the dorsal medulla of anesthetized rats was increased or decreased by microinjections of amino acids to examine effects on systemic arterial pressure, heart rate and blood flow and conductance of the renal and femoral vascular beds. Microinjections of excitatory D, L-homocysteic acid caused increases in arterial pressure of 18 +/- 2 mmHg, increases in heart rate ranging from 5-40 beats/min and renal vasoconstriction; the femoral bed constricted after some injections and dilated in response to others. Injections of the inhibitory amino acid glycine caused no consistent decreases in arterial pressure and heart rate and injections of the gamma-aminobutyric acid analog, muscimol were ineffective. These data demonstrate that neurons in the dorsal region of the rat medulla can contribute to regulation of arterial pressure and can integrate generalized differential changes in regional vascular resistance, but do not appear to be essential for the genesis of basal vasomotor tone.

A pilot study for the statistical treatment of nutritional parameters in the quality control of canned fishing goods.

This pilot study was derived as a consequence of European Directives 496/90 and 493/91 in which a regulation on the labeling of canned fishing goods was established. The study was intended primarily to assess whether different Spanish canned fishing goods might be differentiated by their basic nutritional composition (i.e., ash, chlorine as NaCl, fat, humidity, total proteins, and dry residue) and, second, to study each particular type of good. Accordingly, a univariate nonparametric statistical test and 2 multivariate chemometric techniques (factor and cluster analyses) were used. The pilot study revealed that (1) the basic nutritional variables did not allow a clear distinction among canned goods when different commodities were considered, but they seemed useful for obtaining information for only one type of good; and (2) the variables that gave the most useful information to visualize the appearance of groups in the data sets were humidity, dry residue, fat, and proteins, although their particular usefulness was found to be different when different species were considered.

[Executive dysfunction in attention deficit hyperactivity disorder in childhood].

INTRODUCTION: The principal problem of ADHD is the difficulty to execute inhibitory control. The inhibition is an executive function that is develop during childhood. AIM: To know if other executive functions shower a lower performance in ADHD versus control group and these were different between ADHD-I and ADHD-C. PATIENTS AND METHODS: Fifty three children, between 7 to 10 years old, participated and were assess with EMIC and Simon task. RESULTS: The results showed similar profile in working memory and verbal span. In contrast, ADHD-C showed lower performance in Simon task and more impulsively. On an other hand, ADHD-I showed lower performance in memory working tasks and planning. CONCLUSIONS: These results suggest differences in the executive profile between ADHD-I and ADHD-C and these support the hypothesis of Barkley about the necessity to differ both clinical subtypes.
