[Coralliform lithiasis in children. Apropos of 20 case reports].

The authors recall the characteristics of infantile staghorn calculus, in relation to twenty personal cases. The complaint is more frequently encountered in boys than in girls. A frequent etiology is proteus urinary infection. The morphology of the staghorn and the state of the parenchyma are determining factors in the treatment. A sinusoid morphology and thickness of the renal parenchyma are operative indications. The bivalving operation is exceptional. Results are generally satisfactory (80% of cases), and recurrences are rare.

[Therapeutic approach to bilateral hepatoblastoma].

The treatment of a case of bilateral hepatoblastoma by continuous infusion of adriamycin in the hepatic artery accompanied by ligature of the right hepatic artery enabled the authors to carry out six weeks later an extensive right lobe hepatectomy. Unfortunately the patient died seven weeks after the partial hepatectomy from complication of surgery. Thus it was possible to demonstrate the effectiveness of the intra arterial chemotherapy. No residual tumor could be found at autopsy. As in the case with the adult, arterial ligature combined with carefully targeted chemotherapy would seem to be potentially important therapeutic option in the treatment of malignant liver tumors in children.

[Epidermoid cyst of the spleen in children. Apropos of 2 cases].

Two cases of epidermoid cyst of the spleen are reported. A splenectomy was necessary because the topography of the lesions was not consistent with a conservative procedure for this children fourteen and ten years old. The histological criteria and pathogenic hypotheses of this infrequent lesion are analysed. Ultrasonography and eventually computerized tomography bring the best findings concerning the topography and are useful for the surgical treatment. When it is possible, the partial conservation of the spleen is to be wished.

[Fractures and slipped epiphyses of the proximal humerus in children. Place and methods of surgical treatment].

Fractures and Epiphyseal separations of the proximal humerus are usually non-operatively treated with a generally good result. However, sometimes, a surgical treatment may be required. Twenty fractures of the proximal humerus occurring in children were treated operatively. From these 20 cases, the authors precise the indications and the ways of surgical treatment in this type of lesion. They insist upon the value of the angulation on the lateral radiograph of the shoulder which, if important, may often signify an interposition of the tendon of the long head of the biceps between the fracture fragments causing irreductibility; they propose a classification of the fractures referring to this angle. Considering surgical procedure, delto-pectoral approach is the most commonly used, although axillary way is more esthaetic. Kirschner wires and screws are the best means of synthesis. Anyway, this surgical treatment has to remain exceptional.

[Infrahepatic bile duct proliferation in the gestating rat treated with 4,4'-diaminodiphenylmethane (4,4DDPM)].

We have attempted to produce experimentally the histological hepatic lesions observed in the case of biliary infant atresia. Though the etiopathogeny of this disease remains uncertain, the possibility of a toxic origin for the liver was evoked. Female rats were treated per os from the 10th to the 20th day of gestation with 44' DDPM, a molecule able to provoke an intra-hepatic biliary ductular proliferation. If we always observed hepatic lesions in pregnant rats sacrificed 24 hours before delivery, in some cases we also noticed the existence of macro- and microscopic hepatic lesions in the foetus. With a dose superior to LD 50, we have sometimes observed neural tube defects without hepatic lesions.

[Value of neuroleptoanalgesia in preventing recurrence of fetal shunts during treatment of diaphragmatic hernias].

A leptanalgesia (Chlorpromazine-morphine) was used in the treatment of postero lateral diaphragmatic hernia in six neonates. The treatment which was begun immediately after the arrival of the child and continued during and after surgery seemed to contribute to the favorable outcome observed in five of the babies. In spite of the empirical nature of their argument, the authors suggest the possible mechanism of action of the treatment. However they recognize that its effectiveness can only be assessed after careful hemodynamic studies.

[Serious pelvic traumatisms in children].

The rareness of pelvic fractures in children is due to the resilience of bony tissue and to the thickness of the periosteum at this age. Therefore, such fractures are most often seen after violent traumatisms, which explains their frequent association with visceral lesions that determine the gravity and dominate the prognosis. The authors have presented 9 observations of fractures of the pelvis, with or without pelvis dislocation, which were the most often associated with urological lesions either acute (urethral rupture, bladder rupture) neither chronic (hydronephrosis) but sometimes with vasculo-nervous, gynecological or others lesions. The different therapeutic techniques and their indications in the treatment of such fractures are similar to those used in the adult. Excepting those cases of polytraumatisms, where mortality remains elevated, the treatment of such fractures is most often satisfactory in the short term, but requires a thorough and prolonged monitoring in children presenting associated urological and gynecological lesions in order to evaluate with precision the long term prognosis.

[Traumatic displacement of the upper femoral epiphysis (author's transl)].

Three cases of traumatic displacement of the upper femoral epiphysis in young children are presented. The authors emphasized that these lesions are very rare (about fifty cases in the literature) and that the incidence of major complications including avascular necrosis of the proximal fragment, coxa magna and premature epiphyseal fusion is very high. However, an early orthopedic management including soft reduction with X-ray control and fixation by percutaneous kirschner pinning give the best chance for these complications not to occur. Two of the three described in this paper can be considered as a good result after a course of four years. The third case including traumatic separation through the epiphyseal plate with dislocation of the hip and fracture of the femoral head needed a surgical ablation with poor result.

[Skin lesions and joint involvement in purpura fulminans (author's transl)].

Until recently, purpura fulminans had a very prognosis. It can now be treated and the patient may survive but extensive skin necrosis may lead to septic arthritis. The authors have observed 4 cases of this disease. 3 had to suffer amputation of a lower limb. They describe the type of care recommended in order to limit the extent of the necrosis.
