[Essential deformans osteolysis. Report of a case with progressive and generalized evolution (author's transl)].

The authors report a clinical and radiographic course of a 16 years old caucasian girl with a progressive deformation of her skeleton. The primary lesions occur in knees and wrists. This disease was at first considered as a rheumatoid arthritis, then as a dysplasia epiphysialis multiplex, and now as essential deformans osteolysis (with carpal lysis, shortening of the forearm bones, dislocation of the elbows, disparition of the humeral and femoral heads, contracture of hips and knees, posterior tarsal lysis, and kypho-scoliosis). The clinical (particularly ophtalmologic), biological (including inflammatory, phospho-calcic and nephrologic evaluation with mucopolysaccharidosis urinary excretion) and histological (skin, muscle and bone) check-up were normal. They review the different classification established on lesion topography, on association or not with a nephropathy, finally on an dominant or recessive autosomal inheritance. Then the authors think that their case is similar to the ones of Winchester and Hollister. They discuss the etiopathogenic factors, and do not consider their case as a new mucopolysaccharidosis, but rather as a generalized disease of the collagen of bones.

[Hypospadias with chordee one-stage operation. An original procedure (author's transl)].

The common management of hypospadias involves the chordee correction before reconstructing the urethra; then staging of operative cure is so often necessary. This inconvenience is avoided by neglecting some moderate chordees as it was recorded by the authors in their own retrospective series. For the last two years they are performing a one stage procedure with simultaneous correction of chordee and urethroplasty; they briefly review main other one-stage procedures before describing their own using a tubulated and inverted flap. Twenty cases were managed with this procedure and the results were found satisfactory as well as functionally and aesthetically in spite of three transient fistulas and two meatal stenosis. However, definitive conclusion cannot be drawn because of a too short follow-up.

[The place of lavage-irrigation of the peritonium in peritonitis from the appendix in children. A report of 10 cases (author's transl)].

Confronted with the difficult problems presented in acute appendicular peritonitis, whether generalized or multiple focused, the authors used the "lavage irrigation" method in ten cases. They analysed the local and general conditions that led them to employ this method, in light of the evolution of this technic which they had observed in their experiences whit it. They have attempted to elucidate, using information previously available in the literature, the indications, the steps, and the eventual complications of the "lavage irrigation" of the peritonemum in the child.

[Thoracopagus. Possibilities of surgical separation].

A new case of thoracopagus twins is reported. Radiological, hemodynamical and surgical explorations showed a communication between the two hearts, situated at the atrial level and associated with digestive and cardiovascular malformations. These abnormalities were definned by the anatomical study realized after vascular injection:hypoplastic pulmonary artery (associated with a persistent atrio-ventricular canal in one case, with a tricuspid atresia in the other case), a large fused liver, common pancreas and ileum. A bibliographic study allows the authors to define surgical separation possibilities. The prognosis depends essentially on cardiovascular abnormalities associating abnormal connections between the twins' hearts with malformations often too severe for surgical repair.

[Double venoma (varicosity) of the neck in an 8 year old child (author's transl)].

Venous ectasia in the neck, can for esthetic reasons, justify radical surgical treatment. But as it is a benign condition, the decision to operate also depends on the topography of the ectasia. The authors present the case of a double jugular localization, internal and external, explored by the techniques of direct and retrograde phlebography.
