Unusual presentation of glomerulocystic kidney disease in an adult patient.

A 38-year-old Latin-American man developed uremic syndrome without any evidence of previous kidney diseases or any other health problems. Ultrasound and CT scan confirmed normal size of the kidneys without evidence of urinary tract obstruction or renal parenchymal cysts. Kidney biopsy showed cystic dilatation of Bowman's space (glomerulocystic kidney disease). The patient was started on hemodialysis. Severe renal dysfunction and uremic symptoms are a rare initial manifestation of glomerulocystic kidney disease. This pathology should be considered in the differential diagnosis of patients with normal size kidney and chronic renal failure.

Histopathological study of Clara cells in lungs from linoleylanilide intoxicated mice.

A histopathological study was made of Clara cells in lungs from mice intoxicated by sc injections of linoleylanilide dissolved in olive oil (20% w/v) at doses of 1,000, 2,500 or 4,000 mg/kg administered at 500 mg/kg on 2, 5 or 8 occasions, respectively. Cells were evaluated for biotransformation capacity. Morphological alterations included degeneration processes in the Clara cells that correlated with the doses administered. The observed lesions were very similar to those caused by xenobiotics with pneumotoxic capacity due to their biotransformation to electrophilic intermediates. The changes observed in Clara cells under our experimental conditions may reflect the activity of reactive catabolites from oleoanilides.

Medical nephrectomy: the last resort for intractable complications of the nephrotic syndrome.

The large protein excretion of nephrotic syndrome leads to severe hypoalbuminemia and massive edema. The ensuing hyperlipidemia may not respond to any treatment unless protein losses are corrected. Urinary excretion of clotting factors due to nonselective proteinuria explains, at least in part, the development of a procoagulable state. We report herein four cases of intractable nephrotic syndrome that required medical nephrectomy by bilateral embolization of renal arteries. This procedure, although unusually performed, is an important therapeutic option in selected patients with nephrotic syndrome.

Primary pulmonary lymphoma. Diagnosis by immunoglobulin gene rearrangement study using a novel polymerase chain reaction technique.

A 63-yr-old-woman presented with pulmonary infiltrates and macroglobinemia. Open lung biopsy revealed an interstitial plasma cell infiltrate. Immunocytochemical staining to determine clonality of the infiltrate was negative for both kappa and lambda light chains and therefore was not helpful. Polymerase chain reaction (PCR) amplification of the CDR-III region of the immunoglobulin heavy chain gene revealed that the plasma cells were either monoclonal or biclonal, thus suggesting the diagnosis of a plasmacytoid pulmonary lymphoma. The advantages and limitations of the PCR technique over the traditional method of detecting gene rearrangements, i.e., restriction-digestion of DNA and Southern blotting, are discussed. To our knowledge this patient represents the first reported application of the PCR technique for detecting gene rearrangements to determine the clonality of a lymphoid infiltrate in the lung. This technique can also be applied to determine the clonality of lymphocytes obtained by bronchoalveolar lavage or needle biopsy, or from a pleural effusion.

Circumscribed myositis ossificans. Report of nine cases without history of injury.

The clinical features of 9 patients with circumscribed myositis ossificans (CMO) are described and the effects of treatment with surgical removal of ectopic bone are assessed. The average age of these patients was 24.4 years, and the average follow-up period was 7.4 years. Early correct diagnosis remains unusual, mainly because myositis may be mistaken for bruising, sarcoma or mumps. Once histological diagnosis was established in biopsy, surgical resection of the mass was found in these series. Although spontaneous recurrence was found in this series. Although spontaneous regression of the clinical findings has been reported, we consider surgery to be necessary in CMO in order to establish the diagnosis; furthermore, when the clinical and radiological diagnosis is uncertain or when the lesion causes pain or mechanical blocking of a joint, the removal of the mass is mandatory.

Secondary chondrosarcoma. Four cases and review of the literature.

The authors present four new cases of chondrosarcoma secondary to multiple osteochondromatosis (MOC). As MOC is a familiar and hereditary condition, the patients should be evaluated throughout their lives, from childhood to adulthood. Because of the risk of malignant transformation in this condition, any adult patient complaining of pain and excessive increase in tumor size should be investigated for sarcomatous degeneration. Wide resection, when possible, must be the elected treatment in this type of patient, in order to avoid the risk of local recurrence.

Mode of replication, size and distribution of naturally occurring plasmids in Bacillus thuringiensis.

Cloned replication origin regions, derived from both small (4.9-7.5 MDa) and large (43-60 MDa) plasmids of Bacillus thuringiensis subspecies kurstaki strains HD73 and HD263 were used as hybridization probes in a Southern-blot analysis to assess both the size and horizontal distribution of native plasmid replicon groups among different subspecies of B. thuringiensis. In general, resident plasmids hybridizing to the replication origin regions derived from strains HD263 and HD73 were more commonly found in kurstaki strains than in non-kurstaki strains, suggesting a non-random distribution of plasmid incompatibility groups. Replication origin regions derived from the large HD263 plasmids (43-60 MDa) hybridized almost exclusively with large plasmids (greater than 30 MDa) of widely varying sizes. In contrast, replication origin regions derived from small plasmids hybridized exclusively with small plasmids (less than 10 MDa) showing little size variation. These results are consistent with previous observations concerning the relationship between plasmid size, mode of replication, and structural stability.

Evidence for a fourth rat isoform of the plasma membrane calcium pump in the kidney.

This study was conducted to identify plasma membrane Ca(2+)-transporting ATPases present in rat kidney. Characterization of the cDNAs of the plasma membrane Ca(2+)-ATPases revealed a family of proteins with regions of highly conserved amino acid sequence. To examine the extent of the diversity of rat renal plasma membrane Ca(2+)-ATPases, we used the polymerase chain reaction to detect additional gene products in rat kidney mRNA that shared these conserved regions. Sequences corresponding to three previously known rat plasma membrane Ca(2+)-ATPases were obtained. In addition, we found sequence corresponding to a new putative plasma membrane Ca(2+)-ATPase. Our results demonstrate that the rat kidney contains at least four different plasma membrane Ca(2+)-ATPases and the complexity of this multigene family is greater than previously thought.

Infection by hepatitis B and C virus in non-intravenous drug using female prostitutes in Spain.

We have studied the prevalence of hepatitis B (HBV) and hepatitis C virus (HCV) serologic markers in female blood donors and in female prostitutes and the relationship of antibodies to hepatitis B core antigen (anti-HBc) and of antibodies to HCV (anti-HCV) with the presence of treponemal antibodies (FTA-ABS) in non-intravenous drug using female prostitutes. Hepatitis B surface antigen (HBsAg) was found in 1.0% of the female blood donors, anti-HBc in 15.6% and anti-HCV in 0.7%. In the prostitutes, the prevalence of HBsAg was 6.1%, anti-HBc was positive in 29.0% and anti-HCV in 8.8%. No significant statistical association between the prevalence of anti-HBc or anti-HCV and the age of prostitutes (p = 0.9111 and p = 0.8254 respectively) or the length of time as prostitutes (p = 0.3583 and p = 0.5770) was found. FTA-ABS positive prostitutes had a significantly higher prevalence of anti-HCV than FTA-ABS negative prostitutes (p < 0.001). No statistical association was found between anti-HBc antibodies and positive FTA-ABS prostitutes (p = 0.336).

[Muscular lesions and enzymatic activities in fighting bulls].

The aim of this study was to determine whether some muscular pathology existed in fighting bulls, in relation or not to the weakness shown in these animals during the bullfight (corrida for males and tienta for females). Creatinine kinase (CK), lactate dehydrogenase (LDH) and aspartate transaminase (AST) serum enzyme activities were increased after the corrida or the tienta. Most of the fighting bulls (78%) had some histological lesions in the skeletal or cardiac muscle, with predominance of chronic lesions. Clinical signs of these chronic lesions could only be seen after some trigger-effect, such as physical, exercise or stress, as shown after the corrida or tienta.

Size-selective grazing on bacteria by natural assemblages of estuarine flagellates and ciliates.

The small average cell size of in situ bacterioplankton, relative to cultured cells, has been suggested to be at least partly a result of selection of larger-sized cells by bacterivorous protozoa. In this study, we determined the relative rates of uptake of fluorescence-labeled bacteria (FLB), of various cell sizes and cell types, by natural assemblages of flagellates and ciliates in estuarine water. Calculated clearance rates of bacterivorous flagellates had a highly significant, positive relationship with size of FLB, over a range of average biovolume of FLB of 0.03 to 0.08 microns3. Bacterial cell type or cell shape per se did not appear to affect flagellate clearance rates. The dominant size classes of flagellates which ingested all types of FLB were 3- to 4-microns cells. Ciliates also showed a general preference for larger-sized bacteria. However, ciliates ingested a gram-positive enteric bacterium and a marine bacterial isolate at higher rates than they did a similarly sized, gram-negative enteric bacterium or natural bacterioplankton, respectively. From the results of an experiment designed to test whether the addition of a preferentially grazed bacterial strain stimulated clearance rates of natural bacterioplankton FLB by the ciliates, we hypothesized that measured differences in rates of FLB uptake were due instead to differences in effective retention of bacteria by the ciliates. In general, clearance rates for different FLB varied by a factor of 2 to 4. Selective grazing by protozoa of larger bacterioplankton cells, which are generally the cells actively growing or dividing, may in part explain the small average cell size, low frequency of dividing cells, and low growth rates generally observed for assemblages of suspended bacteria.

Transbronchial fine-needle aspiration. Reliability and limitations.

Transbronchial needle aspiration is a new method of sampling pulmonary, paratracheal, and mediastinal masses. A total of 116 aspirations of the lung performed during a two-and-a-half-year period were reviewed. The results were compared with those of bronchial wash, bronchial brush, sputum, transbronchial forceps biopsy, and histologic material subsequently obtained during surgery or at autopsy. Clinical information was also reviewed. Transbronchial needle aspiration was performed on 104 patients, yielding 116 specimens. With the use of a combination of aspiration, wash, brush, sputum, and forceps biopsy, malignancy was detected in 48 patients. Open biopsy, mediastinoscopy, and transcutaneous needle aspiration increased the number of malignant neoplasms detected to 72. The tumors were detected in 67% of these 72 cases by a combination of all bronchoscopic methods. Transbronchial aspiration correctly identified 36% of malignant cases compared with 35% identified by wash, 39% by brush, 25% by sputum, and 59% by forceps biopsy. The overall accuracy of the techniques was 56% for aspiration, 48% for wash, 56% for brush, 35% for sputum, and 71% for forceps biopsy. Malignant cases were associated with cellular transbronchial aspirates, good preservation of cell detail, and a high degree of cytologic/histologic correlation regarding cell type. Transbronchial fine-needle aspiration has the potential for improving the diagnostic ability of bronchoscopy. However, the high incidence of false negative diagnoses is the main limitation for the technique, and the responsible factors are discussed.

Isolation and characterization of EG2158, a new strain of Bacillus thuringiensis toxic to coleopteran larvae, and nucleotide sequence of the toxin gene.

A novel strain of Bacillus thuringiensis was isolated from soybean grain dust from Kansas and found to be toxic to larvae of Leptinotarsa decemlineata (Colorado potato beetle). The strain (EG2158) synthesized two parasporal crystals: a rhomboid crystal composed of a 73116 dalton protein of approximately 30 kDa. Plasmid transfer and gene cloning experiments demonstrated that the 73 kDa protein was encoded on an 88 MDa plasmid and that the protein was toxic to the larvae of Colorado potato beetle (CPB). The sequence of the 73 kDa protein, as deduced from the sequence of its gene (cryC), was found to have regions of similarity with several B. thuringiensis crystal proteins: the lepidopteran-toxic P1 proteins of var. kurstaki and berliner, the lepidopteran- and dipteran-toxic P2 (or CRYB1) protein of var. kurstaki, and the dipteran-toxic 130 kDa protein of var. israelensis. While B. megaterium cells harboring the cryC gene from EG2158 synthesized significant amounts of the 73 kDa CRYC protein, Escherichia coli cells did not. The cryC-containing B. megaterium cells produced rhomboid crystals that were toxic to CPB larvae.

Pulmonary and hepatic lesions in experimental 3-hydroxymethylindole intoxication.

Histopathological examination of liver and lung in guinea pigs experimentally intoxicated by 3HMI, a degradation product of an allelochemic of Brassica spp, showed the harmful effects of this compound on these organs. The 3HMI produces a hepatic steatosis and interstitial pneumonia with septal hyperemia. These results suggest that the 3HMI could develop a synergic toxic action together with skatole (3MI) as inducing agents of different forms of some diseases detected in cattle grazing Brassica pasture.

Androgen receptor content in cytosol from non-tumoral human kidney and its relation to steroid hormone environment.

The content of cytoplasmic androgen receptors (Bmax) was analyzed in non-tumoral renal tissue of 12 men and 15 postmenopausal women using a synthetic androgen (methyltrienolone) as ligand and a method of dextran-coated charcoal. The Bmax in both sexes was compared, establishing correlations between it and circulating levels of testosterone, dehydroepiandrosterone sulfate, androstenedione, and estradiol to find out the possible influence of the hormonal environment on androgen receptors in the human kidney. No differences in Bmax were observed between males (46.3 +/- 24 fmol/g of tissue) and females (45.4 +/- 26 fmol/g), in spite of the significantly greater (p less than 0.01) levels of circulating testosterone in the former group. No significant linear correlations existed between any of the steroids analyzed and the Bmax. These results demonstrate the existence of androgenic receptors in non-tumoral human kidney and indicate that its content is not regulated by circulating levels of testosterone. The concentrations of the principal extratesticular androgens and estradiol do not seem to have a quantitative influence on these androphilic proteins either.

Inhibition of airway reactivity by nifedipine in patients with coronary artery disease.

Nine patients with coronary artery disease were challenged with aerosolized methacholine before and 3 days after the daily oral administration of 30 mg of nifedipine. Resting airway tone appeared to be generally unaffected by nifedipine, as there were no significant changes in the baseline ventilatory functions. Serial measurements of spirometry and airway resistance were made after cumulative methacholine doses of 1.25, 3.75, and 8.75 mg. Identical doses of methacholine induced smaller changes in specific airway conductance (p less than 0.02), airway resistance (p less than 0.001), FEV1 (p less than 0.02), and FEF25-75% (p less than 0.01) after nifedipine therapy than before. These differences were significant at all methacholine dosage levels by paired t test except for FEV1 at the 3.75 mg dose. This study demonstrated that the bronchospastic response to methacholine in patients with coronary artery disease is reduced after the administration of nifedipine. This salutary effect on ventilatory function may be of importance to cardiac patients with concomitant chronic airway obstruction who are intolerant of beta-adrenergic blocking agents.
