[Malignant melanoma of the meninges--MR and CT diagnosis].

Malignant melanoma of the meninges is a rare neoplasm derived from melanocytes of the cranial or spinal meninges. Histologically classified as grade IV tumours, malignant melanoma may present either as a diffuse meningeal neoplasm, first described by Virchow in 1859, or as a circumscribed tumour attached to the meninges. Although diagnosis is rarely established prior to surgery or autopsy, MR and CT may provide indispensable information probably leading to earlier diagnosis. In 4 patients, diagnosis of a primary meningeal melanoma was based on MR and CT findings and histology. Histology was obtained in 3 cases by surgery, in one patient by autopsy and showed a melanotic and an amelanotic malignant melanoma in 2 patients each. Autopsy was carried out in 3 cases after survival of 4, 5, and 18 months; in a single case, the follow-up period is almost 3 years.

Maffucci's syndrome with bilateral cartilaginous tumors of the cerebellopontine angle.

Maffucci's syndrome is characterized by the combination of multiple enchondromas (Ollier's disease) and hemangiomatosis. These hemangiomas develop in the subcutaneous tissue and form red-blue tumors dispersed over the whole body. Intracranial involvement is rare, making a preoperative radiological diagnosis and differentiation from other tumors rather difficult. The radiological characteristics and successful removal of the intracranial part of a chondrosarcoma of the cerebellopontine angle in a case of Maffucci's syndrome are reported in this paper.

Stereological estimates of nuclear volume in normal germ cells and carcinoma in situ of the human testis.

Carcinoma in situ of the testis may appear many years prior to the development of an invasive tumour. Using point-sampled intercepts, base-line data concerning unbiased stereological estimates of the volume-weighted mean nuclear volume (nuclear vV) were obtained in 50 retrospective serial testicular biopsies from 10 patients with carcinoma in situ. All but two patients eventually developed an invasive growth. Testicular biopsies from 10 normal adult individuals and five prepubertal boys were included as controls. Nuclear vV in testicular carcinoma in situ was significantly larger than that of morphologically normal spermatogonia (2P = 1.0 x 10(-19)), with only minor overlap. Normal spermatogonia from controls had, on average, smaller nuclear vV than morphologically normal spermatogonia in biopsies with ipsi- or contra-lateral carcinoma in situ (2P = 5.2 x 10(-3)). No difference in nuclear vV was found in carcinoma in situ with or without co-existing invasion, and no characteristic pattern of nuclear vV was disclosed by following the lesion in serial biopsies over time from individual patients. Estimation of nuclear vV may represent an adjuvant tool in morphologically puzzling cases of testicular carcinoma in situ, but the prognostic value requires further evaluation in larger series of patients.

In vitro susceptibilities of Bordetella pertussis and Bordetella parapertussis to six new oral cephalosporins.

Of six new oral cephalosporins, cefixime and cefpodoxime were the most active (MIC for 90% of isolates tested [MIC90], 16 micrograms/ml) against Bordetella pertussis, followed by cefetamet, cefprozil, and loracarbef (LY163892) (MIC90, 64 micrograms/ml) and ceftibuten (MIC90, 128 micrograms/ml). Against Bordetella parapertussis, loracarbef was more active (MIC90, 32 micrograms/ml) than the other compounds tested (MIC90s, 64 to greater than 128 micrograms/ml). The new oral cephalosporins are unlikely to play a role in pertussis treatment.

Effect of a herbal yeast food supplement and long-distance running on immunological parameters.

The effect of a food supplement on immunological parameters of 16 long-distance runners was tested in a randomized, double-blind and placebo-controlled trial. The supplement comprised plasmolysed herbal yeast, malt, honey, and orange juice. No statistically significant differences between the two groups regarding the following variables were detected at three sessions at rest and immediately after a 21 km run: total and differential white blood cell counts, numbers of B- and T-cells and T-subpopulations, concanavalin-A-induced lymphocyte proliferation, serum levels of immunoglobulins, neopterin, IL-2 receptors, beta 2-microglobulin, complement factor b, c4 and c3c, and c1-inactivator. These findings suggest that the effects of the tested food supplement on these parameters are negligible with respect to improvements in the immunological status of long-distance runners. The changes observed immediately after the run had a transient character. In both groups, however, low lymphocyte counts, IgG subclass 2 levels and c1-inactivator levels were noted at rest, which indicate that the immune status of endurance athletes may be affected by training.

Panencephalopathic type of Creutzfeldt-Jakob disease with primary extensive involvement of white matter.

The case of a 66-year-old woman with atypical Creutzfeldt-Jacob disease (CJD) presents several peculiarities. Pathologic examination confirmed a rare CJD case with primary generalized spongiform changes of the white matter and only moderate, but typical changes of the gray matter. Besides an essentially typical clinical course, the patient developed temporary diplopia and vertical eye movement paralysis. Intoxication had been excluded. Isoelectric focusing revealed oligoclonal CSF-IgG. Magnetic resonance imaging studies showed periventricular accentuated flat and striped hyperintense structures. EEG had CJD-typical periodic 1/s synchronous discharges. This case illustrates for the first time that in panencephalopathic type of CJD the cerebral white matter can be involved primarily and more extensively than the gray matter.

Cis- and trans-isomeric fatty acids in plasma lipids of newborn infants and their mothers.

The proportionate fatty acid composition of plasma lipids was determined by high-resolution gas-liquid chromatography in 30 pairs of mothers and their term infants at the time of birth. Trans-fatty acids were found at similar percentage levels in maternal and infantile plasma, evidence for the first time of their placental permeability. In umbilical plasma, linoleic and alpha-linolenic acids contributed markedly smaller portions to total fatty acids, in contrast to clearly higher proportions of their long-chain polyunsaturated metabolites (LCP). Significantly larger percentage values in cord than in maternal plasma were found for those LCP that are structural components of brain lipids, which may reflect a discriminating placental transport mechanism for certain physiologically important LCP.

Clinical and biological significance of carcinoma in situ of the testis.

Carcinoma in situ (CIS) of the testis is a preinvasive lesion that with time progresses into invasive germ cell tumour. CIS precedes all types of germ cell tumours except spermatocytic seminoma. An increased risk of harbouring CIS has been reported in men with a history of cryptorchidism, in contralateral testes of men previously treated for unilateral germ cell tumour, in intersex patients, in men with assumed extragonadal germ cell tumour and possibly in infertile men. CIS develops into invasive germ cell cancer in 50% of untreated patients within five years of diagnosis, and it is believed that with time CIS will progress into germ cell cancer in almost all patients. A testicular biopsy is necessary to diagnose CIS. Cells with the characteristics of CIS cells have been detected in seminal fluid, and it may be possible to develop methods for diagnosing the condition by analysis of the semen. We recommend orchidectomy as the treatment of choice in cases of unilateral CIS, if the other testis has a potential for fertility. In other cases, such as patients with testicular tumour and CIS of the contralateral testis, we recommend localized irradiation of the testis with CIS. Leydig cell function seems to be slightly impaired, but sufficient for sexual function, after irradiation. CIS cells show morphological and immunohistochemical similarities with primordial germ cells (gonocytes), whereas they share only few features with spermatogonia. Therefore, we suggest that CIS cells are malignant cells derived from gonocytes during embryonic development.

Ontogeny of ATP and ADP hydrolysis by cerebral cortex synaptosomes from rats.

In the present study, we examined the ontogeny of ATP and ADP hydrolysis by cerebral cortex synaptosomes from rats of various ages (0-, 7-, 14-, 21- and 60 to 90-day-old rats) in order to learn whether hydrolytic activity increases during the period of intense brain growth, as has been reported for other enzymes involved in neurotransmitter metabolism. The results demonstrate that ATP and ADP hydrolyzing activities increase in parallel from birth until the second postnatal week (about 4-fold), followed by a slight and statistically insignificant increase until the animal reaches adulthood. The maximum increase in nucleotide hydrolysis coincided with maximum brain growth, which may indicate a role for the enzyme in neurotransmission. Furthermore, the parallel development of both activities (ATPase and ADPase) strongly suggest that a single enzyme, an ATP diphosphohydrolase, is involved in ATP and ADP hydrolysis by the synaptosomal fraction.

[Neuroleptic malignant syndrome--3 case reports].

An account is given of three patients who exhibited a malignant neuroleptic syndrome after the administration of neuroplegics. Like other authors, we identified typical elicitation factors and clinical characteristics. In one case a catatonic dilemma with lethal issue was observed. In the final case dealt with in this paper, the renewed application of neuroplegics following the termination of the malignant neuroleptic syndrome was tolerated without complications, whereas in the case of an epileptic patient with damage to the brain, the malignant neuroleptic syndrome returned upon dosage increase.

[Loss of marginal alveolar bone and tooth mobility].

In the article the relation between initial mobility of incisors of both jaws and the level of marginal alveolar bone is described. The regression lines representing the dependence of mobility on marginal bone level at various loading forces confirm stepwise the nature of the force transmission from tooth to periodontal support tissues. The dependence of regression line slope on increasing inflammatory periodontal pathology indicates increasing loading of alveolar bone over tension of connective tissue fibers as well as increasing of pressure. This can further accelerate alveolar bone resorption.

[Treatment of labial herpes with acyclovir].

Recurrent infections with herpes simplex virus (HSV), occurring in as many as 20% of the population, often interfere with dental treatment. HSV (type 1 and type 2) belongs to the family of DNA viruses. The new generation of antiviral drugs promises to provide more successful management of viral diseases. Acyclovir, an analogue of viral deoxyguanosine, is an inhibitor of viral DNA and viral DNA polymerase. The aim of the present study was to evaluate to efficacy of the Virolex ointment, an acyclovir preparation manufactured by KRKA Novo mesto, in the treatment of labial herpes. The results show that Virolex is a highly effective agent. If started in the initial phase of infection it can prevent the development of herpetic lesions. Compared to other antiviral drugs used in the treatment of labial herpes, the duration of acyclovir therapy is shorter.
