Population-based incidence and survival of gastrointestinal stromal tumours (GIST) in Girona, Spain.

BACKGROUND: Gastrointestinal stromal tumours (GIST) are rare malignancies characterised by their association with KIT oncogene mutations. Until now, population-based reports of the incidence or survival of kit-confirmed GIST have been rare, and none have originated in Southern Europe. MATERIALS AND METHODS: We used the Girona Cancer Registry to identify malignant mesenchymal tumours of the digestive tract between 1994 and 2001, and performed c-kit testing in the tumour samples. Age-adjusted incidence rates and survival rates were calculated, and they were also analysed by sex and NIH risk categories. RESULTS: Forty-six cases were categorised as GIST. Fifty percent were localised in the stomach, 43.5% in small intestine, 4.3% in the omentum, and 2.2% in colon. Thirty-seven percent were classified as high risk of an aggressive behaviour, 30.4% as intermediate risk and 32.6% as low or very low risk. Only one patient received treatment with imatinib mesilate. The annual incidence by 100,000 inhabitants in crude rate, European age-standardised rate and world age-standardised rate was, respectively, 1.09, 0.90 and 0.65 cases. The relative 5-year survival rate was 74.7% for the entire cohort, and it was markedly lower in the high-risk cases (20.3%). CONCLUSIONS: We report the first population-based study of GIST incidence and survival in Southern Europe. The incidence rate is low and comparable with that of cancer registries from Northern Europe. Survival was favourable in our pre-imatinib population although it was low in high risk cases. Prognostic discrimination of the cases with intermediate, low, or very low risk is inadequate, and these categories should be considered jointly in the future. Our results will help researchers in establishing baseline values against which they can compare, in the future, the impact of imatinib and other Kit tyrosine inhibitors on survival.

Sperm motility patterns and metabolism in Catalonian donkey semen.

The Sperm-Class Analyzer detected four subpopulations of spermatozoa with different motility characteristics in the ejaculate of the Catalonian donkey. Significant differences (P < 0.001) in the distribution of these subpopulations, as well as in total sperm number and percentage total motility, were seen in the diluted semen of four sampled donkeys. All the ejaculates evaluated showed excellent semen quality characteristics; the sperm they contained was more rapid than horse sperm. Principal components analysis showed sperm l-lactate production to be a good predictor of semen condition. This, plus the characteristics of the motility patterns of the different sperm subpopulations, provides an excellent overall indicator of semen quality.

Markov chains: computing limit existence and approximations with DNA.

We present two algorithms to perform computations over Markov chains. The first one determines whether the sequence of powers of the transition matrix of a Markov chain converges or not to a limit matrix. If it does converge, the second algorithm enables us to estimate this limit. The combination of these algorithms allows the computation of a limit using DNA computing. In this sense, we have encoded the states and the transition probabilities using strands of DNA for generating paths of the Markov chain.

[Gastrointestinal inflammatory fibroid polyps. Clinical characteristics and follow-up in a series of 26 patients].

INTRODUCTION: Inflammatory fibroid polyp (IFP) is a protuberant lesion, located near the muscularis mucosae and composed of a proliferation of fusiform cells and conjunctive fibers surrounding capillaries and a variable inflammatory infiltrate. It is believed to be a poorly controlled inflammatory repair response. Our aim was to study the clinical, pathological and follow-up characteristics of a series of patients with IFP. PATIENTS AND METHOD: We studied 26 IFPs from 25 patients (16 women and 9 men) registered between 1985 and 2001 in a specific register of 3 centers in the city of Gerona (Spain). The variables analyzed were age, sex and clinical presentation, IFP localization and size, mucosal characteristics and associated disease, as well as follow-up information. Routine statistical analyses were performed. RESULTS: IFPs were antral in 16 patients, ileal in 7, jejunal in 2 and colonic in the remaining patient. Size determined whether they were symptomatic (35 +/- 13.6 mm) or asymptomatic (8.4 +/- 6.3 mm). Gastric polyps were significantly smaller than intestinal polyps. Symptomatic polyps (5 out of 16 gastric polyps and 9 out of 10 intestinal polyps) predominated in women and occurred at a significantly lower age than asymptomatic polyps (59.2 versus 74.1 years). Most gastric IFPs were associated with chronic atrophic gastritis while only one ileal polyp was associated with Meckels diverticulum. The mean length of follow-up was 60.6 months and, except in one patient who underwent incomplete resection, no recurrences of IFP were observed. CONCLUSION: IFP is a heterogeneous entity, depending on age at presentation, sex, size and location in the digestive tract. IFP does not recur after resection. The association of gastric IFP and chronic atrophic gastritis could suggest a modulatory effect of the mucosa on IFP growth.

Identification of sperm subpopulations with specific motility characteristics in stallion ejaculates.

The aim of this study was to test the presence of separate sperm subpopulations, with specific motility characteristics, in stallion ejaculates by using a computer-assisted semen motility analysis (CASA) system. Motility data were analyzed with a hierarchical clustering of variables based on a correlation or covariance matrix to select like parameters of sperm motility descriptors that better explain the kinetics of spermatozoa. The statistical analyses clustered the whole motile sperm population in both fresh and 24 h stored ejaculates into four separate groups. There were significant differences in the distribution of the four subpopulations (P < 0.001) as well as in the total sperm number and the percentage of total motility (P < 0.01) in fresh semen among the five stallions tested. Our results show that separate subpopulations of spermatozoa with different motility characteristics coexist in stallion ejaculates. These subpopulations were maintained, although with a less-progressive motion pattern, after 24 h of storage. The study of subpopulations in ejaculates that have confirmed fertilizing capacity showed that the majority of the motile spermatozoa in these ejaculates are included in a subpopulation with high progressive motility and low linearity, and the ejaculates with proven fertility that have a total sperm count > or = 20 x 10(9) spermatozoa/ejaculate show all of their motile sperm included in this subpopulation. Our results show that the use of the CASA system is a relatively simple approach to the study of sperm subpopulation patterns in equine ejaculates.

A clinical longitudinal study of multiple sclerosis in Cantabria, Spain.

INTRODUCTION: There are many studies that analyze the clinical course of multiple sclerosis (MS), but only a few of them observed patients from their first episode. We report a MS series of patients longitudinally evaluated from the onset of the disease and compare outcome between immunosuppressive treatment and non-treatment groups. PATIENTS AND METHODS: Patients were included when their follow-up was at least three years and serial examination was available. Poser's criteria were used for diagnosis. We measured neurological impairment according to Kurtzke's expanded disability status scale (EDSS). RESULTS: Fifty-five MS patients were followed-up. Forty-six had clinically defined and nine clinically probable MS. Thirty-three patients had relapsing-remitting, 11 had secondary-progressive and 11 had primary-progressive MS. Mean age of onset of the disease was 31.1 years and mean duration of follow-up was seven years. Pyramidal weakness and sensory symptoms were the most common initial manifestations. Median time to reach EDSS-3 and EDSS-6 from onset was 4.5 and 7.5 years, respectively. From a total of 23 patients with follow-up of 10 or more years, 11 (48 %) had benign MS. The average time from onset of MS to secondary-progressive was 6.1 years. There were no significant differences between treatment and non-treatment patients. CONCLUSION: Longitudinal studies starting from early clinical course of MS are most useful in determining its semeiology, clinical pattern and outcome. Our results call for caution in initiating early immunosuppressive or immunomodulatory therapy.

Laparoscopic biliopancreatic diversion with duodenal switch: technique and initial experience.

BACKGROUND: The duodenal Switch (DS) is a variant of the biliopancreatic diversion (BPD) for the surgical treatment of morbid obesity. MATERIALS AND METHODS: The laparoscopic DS (LapDS) operation is described, and the early surgical outcomes of 16 patients are reported. RESULTS: Postoperative stay was 5 to 8 days. Local wound infection at a trocar site was the most common local complication. CONCLUSION: LapDS is an advanced, complex and feasible technique in bariatric surgery.

Duodenal switch: an effective therapy for morbid obesity--intermediate results.

BACKGROUND: The duodenal switch (DS) is a variant of the biliopancreatic diversion (BPD), with a vertical subtotal gastrectomy and pylorus preservation. METHODS: DS was used to treat morbid obesity in 125 patients, with mean BMI 50, with 65% of the patients super obese (SO). Patients have been followed for an intermediate period. RESULTS: The percentage of excess weight loss (%EWL) was > 70% at 1 year, and reached 81.4% at 5 years when 97% of the patients had a %EWL > 50%. Comorbidities were cured or improved in all patients. CONCLUSION: DS was very effective for the treatment of the morbid obesity in the SO patients.

Doppler and M-mode ultrasonography to time fetal atrial and ventricular contractions.

OBJECTIVE: To compare ease of recording and reliability of ultrasonographic approaches used to time fetal heart atrial and ventricular contractions. METHODS: Seventeen consecutive fetuses seen at our fetal cardiology unit for possible fetal cardiac arrhythmia were included in this study. The same ultrasonographer obtained M-mode tracings of atrial and ventricular free walls, atrial wall and opening of the aortic valves, a peak of the mitral valve, and the opening of the aortic valves; and Doppler signals of flow-velocity waveforms in the outflow tract of the left ventricle and simultaneous flow-velocity waveforms in the aorta and superior vena cava. The outcome measures were rate of successful attempts and intra- and interobserver reliability coefficients. RESULTS: Valid recordings were made for all patients with one M-mode (atrial and ventricular free walls) and two Doppler (intraventricular, superior vena cava, and ascending aorta) approaches. Atrioventricular intervals were significantly longer with M-mode compared with Doppler ultrasonography. Reliability coefficients were excellent (at least 0.89) for all intraobserver measurements. Comparisons of atrioventricular and ventriculoatrial interval measurements made by two observers gave the following intraclass correlation coefficients (95% confidence interval): atrioventricular = M-mode: 0.87 (0.79, 0. 91), left ventricular outflow: 0.93 (0.89, 0.96), superior vena cava-aorta: 0.98 (0.97, 0.99); ventriculoatrial = M-mode: 0.79 (0.67, 0.87), left ventricular outflow: 0.97 (0.95, 0.98); superior vena cava-aorta: 0.99 (0.98, 0.99). CONCLUSION: Fetal atrioventricular intervals measured indirectly from M-mode or Doppler tracings were equally reliable when measured by the same observer; the Doppler approaches had better correlation between measurements made by two different observers.

Correlation between electrocardiographic and ultrasonographic time-interval measurements in fetal lamb heart.

The objective of this study was to establish the echocardiographic modality that best correlates with electrical events in the fetal heart. No documentation on the relationship between electrical events recorded with a surface ECG and fetal M-mode or Doppler echocardiographic measurements is available. The following ultrasound tracings were recorded simultaneously with a surface ECG on six exteriorized near-term fetal lambs: 1) M-mode echocardiography of atrial and ventricular contractions; and 2) Doppler flow velocity waveforms in the right superior vena cava (SVC) either alone or 3) in association with those of the ascending aorta. In the SVC, the onset of the retrograde A wave and the beginning of the forward wave during ventricular systole were used as markers for the start of the P wave and QRS complex, respectively. For the simultaneous SVC and ascending aorta tracings, the beginnings of the A and of the aortic ejection waves were used as markers. On average, the atrioventricular interval was 84 ms longer than the PR interval with the M-mode, corresponding to an increase of 107%. A similar observation was made for the simultaneous Doppler signals from SVC and ascending aorta, but the difference between the atrioventricular and PR intervals was smaller, averaging 35 ms. When the SVC Doppler was taken alone, no significant difference was found between atrioventricular and ventriculoatrial compared with PR and RP intervals, respectively, and a strong correlation was found between the two methods of measurement, both for the atrioventricular (r = 0.91) and ventriculoatrial (r = 0.89) intervals. Doppler interrogation of the SVC alone and, to a lesser degree, of the SVC and ascending aorta are reliable indirect markers for the timing of electrical events of the fetal lamb heart in sinus rhythm.

Internal hernias and gastric perforation after a laparoscopic gastric bypass.

A 27-year-old woman underwent laparoscopic Rouxen-Y gastric bypass. A retrocolic-retrogastric herniation of most of the small bowel and later a gastric perforation due to internal hernia at the mesenteric defect of the jejuno-jejunostomy occurred. These unusual, but not rare, complications are directly related to the neoanatomy that follows gastric bypass and can lead to rapidly progressing and life-threatening situations. Proper evaluation of clinical signs and symptoms, early abdominal CT scan, and urgent operative intervention are mandatory to achieve a successful outcome.

[Electroencephalographic and hemodynamic monitoring at various desflurane concentrations].

OBJECTIVES: To assess the electroencephalographic patterns and hemodynamic changes produced by desflurane at 0.5, 1 and 1.5 MAC. MATERIAL AND METHODS: Twenty-four patients undergoing gynecological surgery under general anesthesia were enrolled prospectively. We monitored electroencephalographic (EEG) patterns and hemodynamic parameters as well as inspired and expired desflurane fractions. Anesthetic induction was with 2 mg/kg of propofol, 2 to 3 mg/kg of fentanyl and 0.1 mg/kg vecuronium. Spectral edge frequency (SEF90) and the EEG delta ratio were recorded along with mean systolic and diastolic pressures, heart rate, oxygen saturation and expired CO2 fraction during induction, intubation, 5 min after intubation and at 0.5, 1 and 1.5 MAC during weaning from anesthesia and extubation. RESULTS: Blood pressure fell significantly at 1.5 MAC and increased when anesthesia was withdrawn; heart rate did not change significantly. SEF90 and the delta ratio changed, however, at each phase. SEF90 fell notably at the moment of induction but gradually increased, rising upon intubation, 5 min later, at 0.5 MAC and upon withdrawal of anesthesia. A significant decrease in SEF90 appeared when 1.5 MAC was reached, related to achievement of a deeper plane of anesthesia. CONCLUSIONS: Automated EEG processing can provide a good measure of depth of anesthesia, as it reflects significant changes related to level of anesthesia at each phase. These differences are not always observable with routine monitoring of hemodynamic parameters.

[Relationship of increased body weight, androgens, insulin and family history of hypertension with blood pressure in premenopausal women].

BACKGROUND: The aim of this study was to evaluate whether the body weight increased, the androgens, the insulin and the family history of hypertension were independently associated with hypertension. PATIENTS AND METHODS: Sixty-two premenopausal women aged 42 y, whose did not have any situation that change the variables to study: blood pressure, body mass index (BMI) and her increase since age of 20, waist to hip ratio, family history of hypertension, smoking, alcohol, exercise, fasting and after oral glucose tolerance test (GTT) glucose and insulin, sex hormone binding globulin (SHBG), serum and salivary testosterone and the free testosterone index. Hypertension was defined as blood pressure > or = 140/90 mmHg. Multivariate analysis and prevalence odds ratio was calculated. RESULTS: Diastolic blood pressure correlated positively with BMI, waist to hip ratio, increase of BMI, GTT glucose, fasting and GTT insulin, free testosterone index and negatively with SHBG. Systolic blood pressure was only positively correlated with GTT glucose. Into a logistic regression model, the increase of BMI (OR: 1.23; IC: 95%: 1.04-1.5), the family history of hypertension (OR: 7.6; IC 95%: 1.9-29.9) and the free testosterone index (OR: 1.7; IC 95%: 1.1-2.6) were the only variables independently associated with hypertension. The prevalence odds ratio of hypertension according to the presence or not of family history of hypertension decreased when highest increase of BMI was considered. CONCLUSIONS: Higher free testosterone levels with body weight increased in adulthood and family history of hypertension are independent predictors of hypertension in premenopausal women, whereas fasting insulin levels may be related to family history of hypertension.

Gender effect on apolipoprotein E epsilon4 allele-associated risk for sporadic Alzheimer's disease.

INTRODUCTION: The role of gender in Alzheimer's disease (AD), and its possible interaction with apolipoprotein E (apoE), has been controversial. MATERIAL AND METHODS: ApoE allelic frequencies and the effect of apoE epsilon4 allele dosage on risk and age at onset of AD were evaluated, separately for men and women, in 100 patients with sporadic AD and 100 age-matched controls. RESULTS: The distribution of apoE alleles and the odds ratio for AD, when associated with 1 or 2 epsilon4 alleles, were not statistically different between men and women. No effect of the dosage of the epsilon4 allele was found on the age at onset of dementia in the 2 sex groups. CONCLUSION: Our data suggest that the relation of the apoE genotype to AD is not dependent on sex.

Vertical banded gastroplasty at more than 5 years.

BACKGROUND: Optimal evaluation of the results of surgery for morbid obesity requires a long-term follow-up for at least 5 years. METHODS: One hundred patients were operated by vertical banded gastroplasty (VBG) and revised with a follow-up of no less than 5 years. Sixty patients were morbidly obese with a body mass index (BMI) of between 40 and 50 kg/m2, and 40 were super-obese with a BMI of >50 kg/m2. Follow-up included 93 patients (93%). RESULTS: Initial surgical mortality was nil. Twenty-five patients required surgery for complications related to the technique (25%) and one patient died due to pulmonary embolism after a re-stapling operation. The percentage excess weight loss was 54.3%, and the BMI was 33 kg/m2 for the 84 patients followed to 5 years post VBG. Only 40 out of 92 patients (43.5%), obtained the weight loss benefit due to the operation. None of them is able to eat a regular diet, and the quality of food intake has been severely affected in some of them. CONCLUSIONS: VBG is, in our experience, a safe and technically simple operation, but the long-term results are questionable. The reoperation rate was high, and weight loss and quality of life are superior with other operations.

Resistance to osmotic stress of horse spermatozoa: the role of ionic pumps and their relationship to cryopreservation success.

To study the resistance of horse spermatozoa against hyperosmotic stress, cells were incubated in solutions of 600 to 4000 mOsm(undisturbed media). Then, semen was immediately placed into an iso-osmotic solution (disrupted media). Incubation in undisturbed media decreased sperm viability in an osmolarity- and temperature-dependent manner. Viability was further decreased in disrupted media, with the effect dependent upon the initial osmolarity of the media and on the temperature. Treatment with ouabain or amiloride impaired the resistance of horse spermatozoa to hyperosmotic stress. Very few correlations were strong between viability after hyperosomotic stress and quality parameters of fresh and frozen-thawed horse semen. The results indicate that the usefulness of resistance to hyperosmotic stress in assessing frozen-thawed semen quality is compromised, since other factors are involved in the resistance to freezing-thawing. Both Na (+)K (+) ATP-ase and the Na (+)H (+) antiporter act in the resistance to hyperosmotic stress in horse spermatozoa.

Subjecting horse spermatozoa to hypoosmotic incubation: effects of ouabain.

Although hypoosmotic tests are widely used to assess spermatozoal quality in different species, they have not been used extensively in the stallion. Moreover, the role of the Na (+)K (+), ouabain sensitive-ATP-ase in the response of equine sperm to hypoosmotic shock is not well understood. This study tests two hypotheses: 1) that equine spermatozoa will respond to a hypoosmotic medium by swelling of the tail, and 2) that addition of ouabain will increase the percentage of swollen sperm tails. Ejaculates from 3 stallions were collected with an artificial vagina and diluted in Kenney's medium (Time = 0). Aliquots were randomly selected to be incubated in an isoosmotic (297 mOsm) or different hypoosmotic media that were composed of citrate or of citrate wïth fructose. The osmolarity of the hypoosmotic media with citrate ranged from 18 to 96 mOsm, and the medium composed of citrate plus fructose (HOS medium) was of 153 mOsm. Moreover, aliquots of spermatozoa pretreated with ouabain were added to the isoosmotic medium and also to the HOS and the 96 mOsm citrate medium (ORT medium). Incubation of equine sperm in the hypoosmotic media resulted in a time- and osmolarity-dependent swelling of the sperm tail, reaching maximum values after incubation for 20-30 min in both the HOS and ORT media. Ouabain induced a dose-dependent effect on swollen tails and viability in fresh semen and also affected some parameters related to motility. Ouabain also increased the swelling response in a hypoosmotic medium although viability decreased. The percentage of swollen tails after incubation in ORT and HOS media snowed significant correlations to viability, altered acrosomes and total motility, but not to other parameters of horse semen analysis. Our results suggest that hypoosmotic tests could be used to improve standard horse semen analysis. Additionally, Na (+)K (+)-ATP-ase activity could be related to the response against hypoosmotic shock of horse spermatozoa.

Ageusia associated with thalamic plaque in multiple sclerosis.

Ageusia and the cheirooral syndrome developed in a patient with a relapse of multiple sclerosis. Magnetic resonance imaging revealed an area of demyelination in the thalamus. This lesion presumably affected the most medial part of the ventralis posterior nucleus, where taste information is located. Given the proximity of the taste area and somatosensory representation of the hand and oral cavity in the ventralis posterior nucleus, we propose that a diagnosis of thalamic lesion should be considered when ageusia occurs with the cheirooral syndrome.
