Rheological properties of the anterior vaginal fluid from superovulated dairy heifers at estrus.

Anterior vaginal fluid samples were collected from 25 dairy heifers induced to superovulation at 12 to 20 hours after estrus detection. Thixotropy and flow behavior were evaluated. In samples from 19 heifers, structural variation was registered and the fluids were considered to be thixotropic; in the remaining samples (6 heifers), structural variation was not registered and the fluids were considered to be nonthixotropic. The mean estradiol concentration of the thixotropic fluids (20.8 pg/ml) was lower (P=0.0002) than of the nonthixotropic samples (56.7 pg/ml). All the nonthixotropic samples of anterior vaginal fluid exhibited Newtonian behavior. Of the 25 thixotropic anterior vaginal fluid samples, 10 exhibited Newtonian behavior while 9 exhibited non-Newtonian behavior. Heifers in which samples exhibited Newtonian behavior had a higher mean estradiol concentration (38.8 pg/ml; P=0.003) than heifers in which samples exhibited non-Newtonian behavior (12.8 pg/ml). Estradiol concentrations were correlated negatively (P=0.02) with the index of consistency (r = -0.46) and were not related to structural variation (P=0.08), although a certain tendency was shown. These results have shown that thixotropic and flow behavior measurements in the anterior vaginal fluid of superovulated estrous heifers vary over a wide range and even qualitatively. This variation is related to estrogen levels: Fluidity increases with increased plasma estrogen values, and the integrity of the gel structure tends toward elimination.

Decreased plasma atriopeptin response to volume-overloading maneuvers and exercise after atriopulmonary anastomosis of Fontan.

Atriopulmonary anastomosis results in a chronic right atrial pressure-volume overload. Water and salt retention is a frequent clinical observation in patients after atriopulmonary anastomosis. The purpose of this study was to examine if this could be related to an inability to increase already elevated circulating atriopeptin (ANP) in response to central volume-overloading conditions. Eighteen patients (mean age 16 +/- 6 years) with an atriopulmonary anastomosis underwent routine cardiac catheterization during which a 5-minute head-down 10 degrees tilt was performed. Peripheral venous and right atrial blood samples were obtained under basal conditions, and after tilting and angiography for determination of ANP concentrations. At a different time, circulating ANP levels were measured during a maximal graded exercise protocol. Increased circulating ANP concentrations were found under basal conditions (114 +/- 10 pg/ml). Tilting and cardioangiography resulted in significant increases in mean atrial pressure (basal: 12 +/- 0.7 mm Hg; tilt: 13.4 +/- 0.63 mm Hg; after angiography: 15.8 +/- 0.8 mm Hg), but not in atrial or peripheral ANP. Compared with the expected threefold increase in plasma ANP induced by maximal exercise in healthy control subjects, only a slight (0.25-fold) increase was found in patients. These observations suggest a reduced stimulus-release response after atriopulmonary anastomosis, which could be related to a loss of atrial stretch receptor sensitivity, achievement of the limit for maximal right atrial secretion, or an alteration in right atrial compliance, or a combination.

Intrathecal immunoglobulin synthesis in multiple sclerosis: effect of corticosteroids and azathioprine.

The objective of this study was to investigate quantitative and qualitative intrathecal IgG synthesis in 51 patients with clinically definite multiple sclerosis, taking previous immunosuppressive treatment into account. Four formulae were used to assess quantitative synthesis. Oligoclonal bands (OB) were investigated using isoelectric focusing (IEF) and silver staining. Abnormal quantitative values were detected in 42 (82%) patients, whereas OB occurred in 38 (74.5%) patients. Steroid therapy lowered quantitative synthesis in 6 out of 9 patients when given within 3 months before lumbar puncture (LP). Untreated patients with OB systematically had abnormal formulae. Patients treated with corticosteroids 10 or more months prior to LP had abnormal formulae. This fact suggests a transient depressor effect of steroids on quantitative synthesis. OB were present in patients recently treated with corticosteroids. Quantitative synthesis was higher in patients with OB than in those without OB. Azathioprine treatment did not significantly lower quantitative synthesis. We conclude that for routine purposes evaluation of intrathecal immunoglobulin synthesis could begin by performing quantitative tests. IEF seems to be mandatory for patients with normal formulae who have recently been treated with steroids.

Within-subject and between-subject biological variation of prothrombin time and activated partial thromboplastin time.

The within-subject and between-subject biological variation of prothrombin time (PT) and activated partial thromboplastin time (APTT) have been studied in a group of 19 women and 20 men over a period of 9 months. All measurements were performed with a mechanical coagulometer. The between-subject coefficients of variation were 6.8% for PT and 8.9% for APTT. The medians and the 90th percentiles of within-subject coefficients of variation were 1.7% and 5.9%, respectively, for PT, and 0.0% and 6.1% for APTT. These data were used to calculate the critical difference for significant change detection and the desirable between-day imprecision.

Biological variation of the leukocyte differential count quantities.

The biological variation of the blood concentration and number fraction of the different kinds of leukocytes have been estimated, over a one-year period in a group of 19 women and 20 men, apparently healthy volunteers. All measurements have been performed with a Toa E-5000 haematological analyser. The medians of the within-subject biological coefficients of variation, separated by sex when significant differences exist, are the following for concentrations: 9.4% in men and 15.4% in women for the leukocytes, 16.9% in men and 26.9% in women for the neutrophils, 12.3% for the lymphocytes, 14.9 for the basophils+eosinophils+monocytes (as measured by Toa E-5000); and for number fraction are the following: 8.6% for neutrophils, 0% in men and 7.2% in women for lymphocytes, and 16.7 for basophils+eosinophils+monocytes (as measured by Toa E-5000). With these data the index of individuality, the critical difference for significant change detection and the desirable imprecision have been calculated.

Within-subject biological variation of hematological quantities and analytical goals.

For several common hematological quantities, the within-subject biological variation during a 1-year period was estimated in a group of 39 apparently healthy volunteers: 19 women and 20 men. All measurements were performed with a hematological analyzer (Toa E-5000, Toa Medical Electronics, Kobe, Japan). The means of the within-subject biological coefficients of variation, separated by sex when significant difference was shown, were as follows: 2.8% for hemoglobin concentration; 3.0% for erythrocyte concentration; 2.9% for erythrocyte volume fraction; 1.1% for mean erythrocyte volume; 1.9% for arbitrary SD of erythrocyte volume; 1.6% for mean erythrocyte hemoglobin concentration; 1.3% for mean erythrocyte hemoglobin; 10.8% in men and 15.7% in women for leukocyte concentration; and 6.7% in men and 8.6% in women for platelet concentration. The desirable between-day imprecisions for the measurement of each quantity were established as one half of the average of within-subject biological variation.

Pain sensitivity range: a useful parameter to measure experimentally induced pain?

Although the Pain Sensitivity Range (PSR) has been presented as a useful parameter to measure pain in the laboratory, it has not received much attention from pain researchers. The goals of the present investigation were to analyze the reliability and stability of the PSR with the cold pressor test method. Results of our study indicate that useful data can be obtained from this parameter. The PSR is a reliable measure which has not shown any variability due to the cyclic changes in pain perception nor due to the supposedly greater pain sensitivity on the left side of the body. The authors postulate that PSR might be a good measure in laboratory research to evaluate the effectiveness of analgesic therapies, for its resemblance to the acute pain experience.

Prevalence of primary Sjögren's syndrome in patients with multiple sclerosis.

Sixty-four consecutive patients with clinically or laboratory-supported definite multiple sclerosis (MS) were evaluated prospectively for evidence of primary Sjögren's syndrome (SS). This diagnosis was established when a patient had objective keratoconjunctivitis sicca, xerostomia, or both together with positive labial salivary gland biopsy. We found 2 patients (3.1%) with clinical evidence of primary SS. Whether this association is fortuitous or whether there is pathogenetic linkage between MS and primary SS remains to be established.

Assessment of the hypothalamic-pituitary-adrenal axis function after corticosteroid therapy for MS relapses.

Doses of corticosteroids usually given for relapses of MS are able to suppress the hypothalamic-pituitary-adrenal (HPA) axis. We evaluated HPA axis function using rapid ACTH stimulation and rapid overnight metyrapone tests, just after cessation of regular oral prednisone therapy for relapses in 14 MS patients. Nine additional patients treated with i.v. boluses of methylprednisolone before beginning conventional oral therapy were also evaluated. Sixteen patients had normal response to both tests and 6 patients had only a discordant response to one test. These data indicate that most patients had normal HPA axis function, which make corticosteroid replacement unnecessary after cessation of therapy for relapses.

Marrow aplasia during high dose mebendazole treatment.

A patient with chronic liver disease was treated with large doses of mebendazole for a hepatic hydatid cyst. Eighteen days after beginning treatment he developed marrow aplasia which reverted to normal after the drug was stopped. This is the marrow aplasia which reverted to normal after the drug was stopped. This is the sixth patient described as developing marrow aplasia when treated with large doses of mebendazole. We suggest that the aplasia is related to the dose of the drug, and that the patient's chronic liver disease was an important factor in its genesis. Patients treated with large doses of mebendazole should have their blood counts monitored during treatment.

A prospective and multicenter safety-monitoring study of a short up-dosing schedule of immunotherapy with a mass-units-standardized extract of mites.

A prospective, multicenter pharmacovigilance study was carried out to evaluate the safety of a new 7-dose treatment schedule of subcutaneous immunotherapy as opposed to the conventional 13 doses normally recommended. The study was carried out in 14 centers and included 261 patients (children and adults) with respiratory allergic disease due to sensitization to mites (Dermatophagoides pteronyssinus and/or farinae). A total of 2290 doses were administered under the direct supervision of the participating specialists. One hundred and ten reactions in 63 patients (24.1%) were recorded, representing 4.8% of the total doses administered. Most of the reactions (98) were local. Only 12 were systemic (0.5% of the administered doses) and occurred in 10 patients (3.8% of the sample). Ten reactions reverted quickly with rescue medication. The maintenance dose had to be lowered in one patient and another patient was withdrawn from the study after suffering two asthmatic crises after two consecutive doses. In view of the results obtained, we can conclude that the new schedule shows an acceptable tolerance profile and does not present a greater risk of reactions than the conventional scheme of 13 doses using an identical extract. Moreover, the new schedule represents substantial savings in the number of doses and visits required to reach the maintenance dose.

Simple colorimetric method for seminal plasma zinc assay.

Zinc concentration in seminal plasma is one of the best biochemical parameters with which to assess prostatic function in the study of male infertility. Nevertheless, this parameter is not determined in many laboratories because of technical difficulties. In this paper a simple colorimetric method for seminal plasma zinc assay is presented. This assay is performed with only one reagent solution making deproteinization and extraction with organic solvents unnecessary. The method is based on the reaction between 1-(2-pyridylazo)-2-naphthol and zinc which yields a coloured complex, that is soluble in the reagent solution when dodecyl hydrogen sulfate sodium salt is present. The solubilized complex has an absorbance peak at 560 nm. The accuracy of this method is proved by statistical comparison with atomic absorption spectrophotometry. Furthermore, the method has a good precision and is free of interference.

[Depressive states in multiple sclerosis. Critical bibliographic review].

The aim of this paper is to review the question of depression in patients with multiple sclerosis (MS). Morbidity and methodological difficulties in the investigation of depressive states in MS are analyzed. Prevalence of depression in MS in previous series is critically compared. The role of chronic invalidity and topography of demyelinating lesions in the development of depression are considered. We also review the treatment of depressive states and future guidelines for investigation of this topic.

[The "Sickness Impact Profile" questionnaire (SIP): Spanish version of a measure of health status].

The objective of this paper is to present the results of the translation and adaptation into Spanish of a questionnaire (The Sickness Impact Profile --SIP--) for measuring the physical and psychosocial dysfunctions generated by a disease. For this the SIP, together with some clinical measures of physical and psychosocial status were administered to a series of 50 patients afflicted by multiple sclerosis. We found not only that the SIP could be considered a good instrument for measuring the physical and psychosocial impact of illness, but also that it is adequate to differentiate in it a physical and a psychosocial dimensions.

[Nasosinusal inverted papilloma].

Inverted papilloma is a papillar lesion of the nasal cavity and paranasal sinuses that arise from the Schneiderian membrane. This tumor has a high rate of local recurrence. We present a review of 10 cases of inverted papilloma of the nose and paranasal sinuses. A review of the literature is done.

[Verrucous carcinoma of the larynx].

From 1981-1984, we have diagnosed and treated 5 cases verrucous carcinoma of the larynx. The clinical findings, histopathology, and management of verrucous carcinoma of the larynx are discussed.
