Rapid and simple PCR assay for quantitation of human immunodeficiency virus type 1 RNA in plasma: application to acute retroviral infection.

A method for quantitating human immunodeficiency virus type 1 plasma viremia may be useful in monitoring disease progression and the responsiveness of patients to a therapeutic regimen or vaccine. A quantitative assay for viral RNA in plasma or sera that differs in several aspects from those reported previously was developed. First, whereas conventional reverse transcriptase-PCR assays involve a two-step process and use two enzymes, the method described uses a single enzyme, rTth DNA polymerase, for both reverse transcription and PCR. The reactions are carried out in a single tube and with a single buffer solution with uninterrupted thermal cycling. Second, uracil-N-glycosylase and dUTP are incorporated into the reaction mixtures to ensure that any carryover of DNA from previous amplifications will not compromise quantitation. Third, a quantitation standard is incorporated into each reaction mixture so that differences in amplification efficiency caused by sample interferents, variability in reaction conditions, or thermal cycling can be normalized. To ensure comparable amplification efficiency, the quantitation standard has the same primer-binding regions as the human immunodeficiency virus type 1 target and generates an amplified product of the same size and base composition. The probe-binding region was replaced with a sequence that can be detected separately. Fourth, a colorimetric detection format was modified to provide at least a four-log-unit dynamic range. The quantitative assay requires only a single amplification of the sample and can be completed in less than 8 h. The procedure was used on archival samples to demonstrate the viremic spike in acute infection and the suppressed levels of circulating virus following seroconversion.

Caries prevalence amongst schoolchildren in The Hague between 1969 and 1993.

Since a first survey in 1969, juvenile caries prevalence in the second largest city of the Netherlands has attained a European minimum of DMFT = 0.8 in 11.9-year-old children. However, the curves of improvement have been flattening out since the mid-eighties. Incidental variations between 1984 and 1993, especially of caries prevalence in the deciduous teeth of 5- and 7-year-old children, do not indicate a turn to the secular downward trend. The stability of juvenile oral health does not seem to be due to changes in dietary habits, nor to public health measures, but is mainly due to good oral health and use of fluoride dentifrices by the children.

A multi-practice clinical study on posterior resin-bonded bridges: a 2.5-year interim report.

A clinical trial of 201 posterior resin-bonded bridges (PRBBs) was conducted partially in a University clinic and partially in general dental practices, for study of the influence of some experimental variables on the survival of these restorations. This report contains the results of an interim analysis which was done after 2.5 years' follow-up. No significant differences were found among the bonding systems used to adhere the PRBBs (Clearfil F2/etching, Panavia EX/sandblasting, and Microfill Pontic C/silicate-coating), or among the different preparation forms. PRBBs made in general dental practices were as retentive as those made in the University clinic. The "location" factor was found to be highly significant (p = 0.0001). The survival rates were 81% for maxillary PRBBs and 56% for mandibular PRBBs.

Antiperinuclear factor and disease activity in rheumatoid arthritis. Longitudinal evaluation during methotrexate and azathioprine therapy.

OBJECTIVE: To study the correlation between antiperinuclear factor (APF) titer and disease activity variables in patients with rheumatoid arthritis (RA) treated with methotrexate (MTX) or azathioprine (AZA) and to investigate whether changes are dependent on the drug used. METHODS: Serial measurements of APF titers (2-fold dilutions) and disease activity variables in a 48-week double blind trial comparing MTX and AZA in 64 patients with RA. APF titers at baseline and during followup, and correlations between APF titers and disease activity variables and their changes from baseline were studied in the patient group as a whole and in the 2 treatment groups. RESULTS: The prevalence of the APF at baseline in the MTX group and in the AZA group with undiluted serum was 15/31 (48%) and 19/33 (58%), respectively. With serum diluted 1:10 this was about 25% higher. The APF titer ranged from 1/10 to 1/640. No sustained changes in APF titers were observed during followup. Statistically significant correlations were found between APF titers and 2 of the 4 disease activity variables, as well as for their changes from baseline at some time points and were most pronounced in the AZA group. However, no consistent correlation between APF titers and disease activity variables could be established. APF changed from negative to positive during followup in 4 patients (6.3%) and from positive to negative in 4 (6.3%). Changes in APF titer between 2 consecutive measuring points did not exceed 2 dilution steps. CONCLUSION: The APF titer showed no sustained change during the followup period. There was no consistent correlation between APF titer and disease activity variables. We conclude that serial measurements of the APF in longitudinal studies do not give additional information.

Cosmetic dental treatment in an adult Dutch population.

In 1992 a follow-up to the Dutch national dental survey of 1986 (DNDS) was performed. The main objective was to analyse changes in oral self care, dental attendance, and oral health status. A postal questionnaire was used: 936 persons (44 per cent response) participated in the study. This paper will focus on the results concerning cosmetic dental treatment. Stepwise regression analyses were used to study the effect of dental and social variables. Relative risks (RR) were computed to clarify the effect of these variables. Of the group of respondents, 90 persons reported having been treated with veneers on their anterior teeth. The variables indicating cosmetic dental treatment needs established during the DNDS clinical dental examination, showed a statistically significant influence. 'Objective need': RR = 2.6; 'Subjective need': RR = 2.1. Out of 877 respondents with restorations present in 1986, 19 per cent reported having one or more of them replaced by composite or porcelain. Four variables were found to have a statistically significant influence: 'Subjective need': RR = 1.7; 'Number of filled tooth surfaces': RR = 1.6; 'Number of decayed tooth surfaces': RR = 1.4; and 'Frequency of toothbrushing': RR = 1.6. Whereas the dentists' opinions had the most influence on having veneer treatments, the patients' opinions had more influence on having amalgam restorations replaced with composite or porcelain.

Circulating soluble tumor necrosis factor receptors, interleukin-2 receptors, tumor necrosis factor alpha, and interleukin-6 levels in rheumatoid arthritis. Longitudinal evaluation during methotrexate and azathioprine therapy.

OBJECTIVE: To assess whether circulating concentrations of soluble tumor necrosis factor receptors (sTNFR; p55 and p75), soluble interleukin-2 receptors (sIL-2R), tumor necrosis factor alpha (TNF alpha), and interleukin-6 (IL-6) reflect clinical response and whether changes are dependent on the drug used in rheumatoid arthritis (RA) patients taking methotrexate (MTX) or azathioprine (AZA). METHODS: These cytokines and soluble receptors were assessed in 20 control subjects and serially for up to 48 weeks in 61 RA patients, by bioassay (IL-6) and immunoassays (sTNFR, sIL-2R, TNF alpha, and IL-6). RESULTS: Concentrations of p55 and p75, sIL-2R, and TNF alpha (but not IL-6) were significantly higher in RA patients than in controls. Significant decreases in sIL-2R and p55 concentrations were associated with clinical improvement and were observed in patients treated with MTX, but not AZA. Both treatments induced decreases in IL-6 concentrations, but circulating AZA (or its metabolites) appears to interfere with the measurement of IL-6 bioactivity. TNF alpha and p75 levels did not show significant changes. CONCLUSION: Measurement of circulating sIL-2R, p55, and IL-6 may be useful in the evaluation of RA disease activity and response to therapy. Interference by circulating levels of drugs must be ruled out when bioassays are used to evaluate cytokine levels.

Survival rate and failure characteristics of the all metal post and core restoration.

In this retrospective study 516 teeth restored with a cast post and core build-up were followed from 1970 till 1990. The data was derived from the dental records of 283 dental clinic patients treated by senior students. The survival rate was found to be 82% after 10 years for post and cores in the anterior region. The most frequent failure characteristic was recementation (46%), followed by rerestoration (32%). The solitary provisions in posterior teeth showed a relative high survival rate, compared with other tooth-types and locations.

[Fit of anchors in frame prostheses].

The fit of clasps was assessed in 101 removable partial dentures (RPD), including 54 free-end prostheses without any tooth bound replacements and 47 tooth bound prostheses replacing only premolars and molars. The age and the design of the RPD had a significant influence on the fit: in older and in stress broken free-end RPD's the clasps had a worse fit in terms of standing away from the abutment tooth.

[Dental caries in school children in The Hague. A comparison of the research results of 1993 with those from 1989].

Starting in 1969, periodic cross-sectional examinations of schoolchildren have been carried out in the city of The Hague. The results of 1993 show that since 1989 no further change in the caries prevalence of the deciduous and permanent dentition has taken place in 5- and 7-year-old children of Dutch origin. Between 1989 and 1993 a marked DMFS reduction amongst Dutch 11-year-olds was found. Compared to 1981 and 1989 a further reduction of dfs and DMFS values for 7-year-old children of ethnic minorities (Turkey and Morocco) could be observed in 1993. Between 1989 and 1993 a significant caries decrease was found among 11-year-old children from Turkey and Morocco.

Exposure to low levels of fluoride and dental caries in deciduous molars of Tanzanian children.

In general, the prevalence of caries in African children may be classified as low to very low. In order to reduce this level even further, methods including the practice of good oral hygiene and administration of fluoride, e.g. water fluoridation, have been suggested. In 1984, 1986 and 1988, a mixed-longitudinal study amongst schoolchildren was carried out in a rural and urban area of Tanzania. In the rural area, shallow wells had been constructed at different periods in time in eight villages since the late 1970s. The drinking water in three of the villages contained fluoride in the range of 0.5-0.8 ppm (fluoridated) and contained less than 0.4 ppm fluoride in the remaining five villages (non-fluoridated). These fluoride levels were disclosed to the authors only during the course of the study. Thus the data were reanalysed to investigate the effects of fluoride and length of fluoride exposure on caries experience in the deciduous dentition. The study was carried out amongst 522 7- and 8-year-olds. Fluoride tablets were not used and toothpaste was virtually unavailable. Three periods of fluoride exposure were identified, i.e. 3, 5 and 7 years. The outcome variable was the mean dmft score in deciduous molars whereas the explanatory variables were age, length of fluoride exposure and year of investigation. Nutrition was considered a co-variate. Analysis of co-variance revealed a fluoride effect (p = 0.0004). Regression analysis did not show significant relationships between the mean dmft score and the three periods of fluoride exposure.(ABSTRACT TRUNCATED AT 250 WORDS)

Induction of autoantibodies to human enzymes following viral infection: a biologically relevant hypothesis.

Macro enzymes, i.e. complexes of normal (iso-)enzymes with an immunoglobulin, may be due to immunological cross-reactions evoked by specific viral antigenic determinants that are homologous to regions in the target enzymes. A search of the National Biomedical Research Foundation protein databank with the amino-acid sequence of human pancreatic amylase revealed a marked homology with a fragment of the yellow fever virus major envelope protein E: i.e. an overall identity of 19.7 per cent and a high degree (40.9 per cent) of conservative amino-acid substitutions over 119 amino acids. At each identical position, the corresponding residue of Taka amylase A was examined by three-dimensional structure analysis, to determine whether the position is likely to be buried or exposed. The existence of a site (epitope) on amylase recognized by an anti-amylase antibody is discussed.

Methotrexate versus azathioprine in the treatment of rheumatoid arthritis. A forty-eight-week randomized, double-blind trial.

We conducted a double-blind, randomized trial comparing azathioprine (AZA) and methotrexate (MTX) in the treatment of patients with rheumatoid arthritis in whom parenteral gold and/or D-penicillamine treatment had been unsuccessful. Patients were randomly assigned to receive either AZA (100 mg daily) or oral MTX (7.5 mg weekly). After 8 weeks, the dosage was increased depending on the clinical improvement. Sixty-four patients were followed up for 48 weeks (33 AZA, 31 MTX). Comparison of values at week 24 with baseline values revealed significant improvement in 12 of 13 disease variables in the MTX group and in 6 of 13 in the AZA group. Comparison between the 2 treatment groups at 24 weeks, by area-under-the-curve analysis, showed significantly more improvement in the MTX group in terms of the swollen joint count, pain score, erythrocyte sedimentation rate, C-reactive protein level, hemoglobin level, thrombocyte level, and disease activity score. A significant overall clinical improvement (disease activity score) was found in 7 of 20 patients treated with AZA and 18 of 30 patients treated with MTX after 24 weeks of therapy, and in 6 of 12 AZA-treated patients and 19 of 25 MTX-treated patients after 48 weeks. The number of withdrawals due to side effects was significantly higher in the AZA group. After 48 weeks, only 12 patients from the AZA group (36%), but 25 from the MTX group (81%), were still using the initial drug. These results demonstrate MTX to be superior to AZA in the treatment of rheumatoid arthritis, with a more rapid clinical improvement which is sustained after 1 year, accompanied by a lower rate of serious adverse reactions.

A tool for measuring the asthma self-management competency of families.

A tool for measuring a family's asthma self-management behaviour has been developed for a study examining the relationships of asthma self-management behaviour, knowledge, and psychosocial factors with various indices of morbidity in children with asthma. The tool involves a structured interview which includes three typical situations of asthma self-management (scenarios). Each of the scenarios is divided into graded challenges. A scoring schedule was developed according to the critical incidents of self-management inherent in each situation. This schedule was then applied to the verbatim transcripts of the subjects' responses. These were presented to 380 asthmatic children aged 5-11 yr and their primary caregivers. Inter-rater reliability, inter-rater agreement and test-retest reliability coefficients indicate that the scores obtained are stable across raters and time. Correlations between scores on different scenarios suggest there is a common factor of self-management competency across all scenarios, but families are better at some aspects of self-management than others. The distribution of scores for each scenario is presented. The place of this tool as a means of assessing self-management behaviour is discussed.
