Enhancement of alcohol-induced hypoglycaemia by H2-receptor antagonists.

The oral ethanol loading test (0.5 g/kg body mass) was carried out in 3 groups with 10 healthy male volunteers each before and after 7 days of administration of either cimetidine (CAS 51481-61-9), ranitidine (CAS 66357-59-3), or famotidine (CAS 76824-35-6). The parameters determined during 6 h comprised the blood levels of ethanol, acetaldehyde, glucose, lactate, pyruvate and bicarbonates, as well as blood pH, PCO2 and PO2. Only ranitidine significantly increased the mean blood ethanol concentration and none of the drugs modified the blood acetaldehyde concentration. Hypoglycaemia following alcohol ingestion was significantly enhanced by all H2-receptor antagonists, but was most pronounced after famotidine. The alcohol-induced rise in blood pyruvate and lactate rather had a tendency to decrease during the second test. The presented results suggest that the evident enhancement of alcohol-induced hypoglycaemia by H2-receptor antagonists is not dependent on the increase of ethanol absorption from the gastrointestinal tract, but represents rather a specific effect of these drugs on glucose metabolism.

[Ischemic hepatitis].

The patients with chronic congestive heart failure and acute deterioration of heart failure (pulmonary oedema, significant reduction of blood pressure) have decrease liver's perfusion with signs of acute damage of liver's cells--ischemic hepatitis. Aspat, AIAT and LDH in blood rich very high level. The level of bilirubin, alkaline phosphatase and glucose increase slightly. Hepatotoxic viruses are never observed. The authors described a case of 34 years old man, who two years earlier had large myocardial infarction with aneurysm of heart and congestive heart failure. He was admitted to hospital in shock. The shock was caused probably by overdose of nitroglycerin. In ECG and Echo examinations he had no signs of acute myocardial infarction, but we observed serious damage of liver's cells with very high levels of AspAT, AIAT and LDH. Based on clinical and biochemical examinations we diagnosed ischemic hepatitis. The patient's clinical and biochemical tests were normalized after improvement of heart failure. Biopsy of liver was normal at that time. Prognosis in ischemic hepatitis depends on course of heart failure.

[Effect of age and sex on the occurrence of gastritis changes in gastric mucosa].

Natural course of gastritis is far from being clarified. Having this fact in mind we undertook this study to establish how age and sex did influence the frequency and intensity of inflammation in particular stomach portions. Possible relation between gastritis on the one hand and cholelithiasis and idiopathic non-ulcer dyspepsia on the other was also checked. The study comprised 392 persons, including 148 subjects with cholelithiasis, 130 with non-ulcer dyspepsia and 114 healthy volunteers. Endoscopy of upper digestive tract was done in all of them, the specimens of mucosa of gastric antrum and body being taken for histologic examination. Frequency of gastritis was found to rise with age, particularly up to the 40-th year of life, being analogical in both sexes. In men, gastritis showed higher intensity and activity as well as more rapid progression toward glandular atrophy. In particular age intervals no significant difference in the frequency of gastritis was found between the population of healthy persons, that suffering from cholelithiasis as well as that with non-ulcer dyspepsia.

[Changes in gastric mucosa and Helicobacter pylori infection in young health volunteers].

They examined 114 young health volunteers to establish a frequency of inflammation of gastric mucous membrane and(or) duodenal bulbous, a frequency of Helicobacter pylori infection and interdependence between infection and inflammation, smoking and nourishment. They evaluated a frequency and intensity of inflammation in antrum, fundus and duodenal bulbus and present of Helicobacter pylori using an urease test and microscopic examination. They noted in young inhabitants of Warsaw, appearance of asymptomatic gastritis in more than 53% cases, an inflammation of duodenal bulbus in 34% and H.pylori infection in 50% cases.

[Changes in secretion of cholecystokinin after cholecystectomy and the effect of these changes on biliary reflux and the state of the gastric mucosa].

Cholecystokinin's role in regulations of gallbladder and gastric function was well documented. After cholecystectomy a secretion of cholecystokinin may be changed and observed symptoms are able to create a new clinical picture including biliary gastric reflux. In the study was noticed that in 12 patients after cholecystectomy the cholecystokinin secretion was increased in comparison to the period before operation. Observed increased levels of the enzyme were not in connection with biliary gastric reflux.

Is Helicobacter pylori infection a real risk factor for gastric carcinoma?

BACKGROUND AND METHODS: That Helicobacter pylori has a role in the pathogenesis of gastric carcinoma is widely accepted, although not all doubts are definitively clarified. The purpose of this work was to detect the differences in presence and mean titer of anti-H. pylori antibodies between groups with gastric (n = 65), colonic (n = 70), and lung (n = 43) carcinoma. RESULTS: The highest prevalence of anti-H. pylori antibodies was found in patients with pulmonary carcinoma (88.4%), which significantly surpassed (p = 0.02) that in the group with gastric carcinoma (69.2%). The groups with colonic and gastric carcinomas failed to show any difference in this respect. Mean antibody titer was significantly higher in subjects with lung carcinoma than in those with gastric carcinoma (p = 0.005). This difference was unrelated to age. CONCLUSIONS: These results contradict the hypothesis assuming a relationship between H. pylori infection and the sequence of phenomena leading to gastric carcinoma.

Reduced inhibitory effect of somatostatin on the exocrine function of the pancreas and on serum insulin (IRI) levels in chronic relapsing pancreatitis.

In 3 healthy men and 4 healthy women, and in 5 men and 2 women with confirmed chronic relapsing pancreatitis (CRP) the inhibitory effect of somatostatin cn pancreatic exocrine function and insulin secretion stimulated with pancreozymin and secretin was determined. In all 7 patients with CRP the volume of pancreatic juice and the bicarbonate and amylase and protein output after stimulation were lower than in healthy subjects, and somatostatin had a reduced inhibitory effect on both the basal secretion and particularly on the pancreozymin-secretin-stimulated secretion. A similarly lower inhibitory effect of somatostatin on the secretin-pancreozymin induced increase in blood insulin (IRI) level was observed in these patients. The presented data suggest that the damaged pancreas reacts less effectively not only to the stimuli enhancing its exocrine and endocrine secretory activity, but also to the stimuli inhibiting this activity. This observation may be of practical value for the evaluation of the functional efficiency of the pancreas.
