TSH stimulation and other ways to increase the uptake in normal and tumorous thyroidal tissue.

The use of bovine TSH is still the most important means for increasing the radioiodine uptake in functionally active tissue. The increase is more marked in normal thyroid tissue (where 2.5 fold increase of the initial value can be expected); but the tumorous tissue reacts to the stimulation, though less substantially (mean increase to 1.5). In intact thyroid gland the increase of uptake is more pronounced with lower uptake in the remaining thyroid tissue; in functionally active tumor this relation cannot be proved. With very low uptake in the tumorous tissue (below 2.5%/24 h) the chance to achieve practically significant increase is negligible. The complications with TSH administration appear especially with repeated application, the proportion of more serious side effects being low. The use of diuretic regime with the iodine depletion simultaneously with TSH stimulation is rather pretentious for the patient; it substantially increases the uptake in the tumorous tissue (again to 2.6 times), although more markedly decreases the iodine excretion in the urine what leads to higher danger of complications caused by radioiodine therapy. The use of TSH stimulation to increase the uptake in the tumorous tissue had not yet been shown to be useful.

Cardiovascular effects of diazepam and nitrazepam in combination with ethanol.

As little attention has been given to cardiovascular effects of a combination of benzodiazepines with ethanol up till now, this paper describes some studies of arterial blood pressure and heart rate in anaesthetized rats treated with diazepam or nitrazepam 5 mg X kg-1 and ethanol 2 g X kg-1. It was observed, that ethanol enhanced the mild hypotensive effect of both benzodiazepines and alone reduced arterial blood pressure in proportion to the rate of its absorption and actual plasma level. This effect may be supposed a synergistic effect caused by a changed biotransformation of benzodiazepines or by intensified central sedative action. The heart rate was not substantially influenced by this drug combination.

Some effects of triamcinolone pretreatment on intravenous glucose tolerance test in patients with different thyroid function.

In 42 patients (17 hypothyroid -Hypo, 14 euthyroid -Eu and 11 thyrotoxic -TX) the intravenous glucose tolerance test was performed before and after triamcinolone sensitization. The blood sugar (BS), immunoreactive insulin (IRI), growth hormone (GH), inorganic phosphorus (P), calcium (Ca), and total proteins (TP) were assessed and glucose disappearance rate was calculated from absolute values (KGt) and from increments (KGi) of glucose. Triamcinolone caused a rise of BS in Eu an Tx subgroups and a decrease of KGt and KGi in Hypo and decrease of KGt in Tx. No differences were found in patterns of IRI, GH, TP and Ca, respectively, but a significant rise of P after triamcinolone was found in Hypo and Eu subgroup. When comparing the subgroups according to thyroid function, only mild differences could be demonstrated in BS pattern both during IVGTT and TIVGTT, but significant differences of KGt and KGi were found between Eu and Hypo subgroups. No differences were observed in patterns of IRI, GH and Ca (except fasting values in Eu and Hypo subgroups during IVGTT, but significant differences were found in the pattern of P and particularly of TP. Except of differences of TP and P, changes during TIVGTT were not more pronounced than those during IVGTT. The comparison of effects of triamcinolone pretreatment on IVGTT and on OGTT (as were described previously) demonstrated similar effects of triamcinolone in Eu subgroups (significant changes of BS, IRI and P), but different action in Hypo- and Tx subgroups (where the significant changes of BS, IRI and P during OGTT and IVGTT were not parallel). These findings may suggest a possible role of some gastrointestinal and/or hepatic factors in action of triamcinolone in impaired thyroid function, but not in normal persons.

[Medullary carcinoma of the thyroid gland with Cushing's syndrome].

The authors describe generalized medullary carcinoma of the thyroid gland with a typical slow 23-year progression which due to prolonged hypercortisolism led to the development of classical "paraneoplastic" Cushing's syndrome. The authors describe the diagnostic difficulties as regards laboratory tests. The case confirms not only the need of early radical treatment of these carcinomas which is the optimal solution, which however can be implemented only in exceptional instances, but also of the detection of this syndrome in all patients with medullary carcinoma of the thyroid in the preclinical stage. Clinically developed Cushing's syndrome or hypokalaemic alkalosis are in the majority of cases a late symptom associated as a rule with advanced generalization of this carcinoma.
