[Recent data in the diagnosis of scaphoid non-union (author's transl)].

An analysis of the problems of the scaphoid non-union diagnostics is given here, based on their experiences with more than 400 operations. Bennet's x-rays are always mandatory, and if problems arise, microfocal x-rays and tomography should be done. They draw the attention to certain x-rays signs described by them. Intraosseous venography has been introduced to help the diagnosis. The distinctive signs between bipartite scaphoid and scaphoid non-union are given too.

Carpal Bones↗

[Carpal bone substitution with silicone prosthesis (author's transl)].

The authors reviewed the development of the carpal bone prosthesis implantations. The advantage of the silicone prostheses type Swanson was described opposed to the metal and acrylic prostheses. Silicone rubber prostheses replacing carpal bones were implanted in 56 cases (30 lunatum, 22 scaphoid, 4 trapezium) with success. To keep the well defined indication conditions is underlined by the authors. They created the home conditions of the carpal bone prostheses manufacturing.

[The place of palliative operations in treatment of complaints caused by scaphoid pseudoarthrosis (author's transl)].

The application of palliative operations intending the termination or the reduction of complaints caused by scaphoid pseudarthrosis have been reviewed in details on the basis of literature and according to own experiences. The indications of styloidectomy and the indications of the removal of the proximal end, as well as the extent of styloidectomy have been exactly defined. Styloidectomy as supplemental surgical intervention is not more applied. The results obtained by styloidectomy have been reviewed (57) cases. They did not do denervation, the Bentzon's-operation is regarded to be out of date.

Arthrodesis↗

[Operative treatment of non-recent and old perilunate dislocations (author's transl)].

Experiences of the authors with the open reduction of 65 nonrecent and old perilunate dislocations are reported here. They analyse the diagnostic failures at these injuries and focus the attention to the correct assessment of the clinical and radiological signs. A survey of the literature on the possibilities of the operative treatment is given. A detailed description of their own technic and the results are given too.

[Operative treatment of lunate malatia (author's transl)].

Authors give a review on the most up-to-date explanation for the aetiology of the lunate malatia and the characteristic features of the different stages of this disease. A report is given on the result of operative treatment in 60 cases of lunate malatia. In the non-arthritic stage the Graner-procedure or silivon prosthetic implant, in grave painful carpal arthritis a complete arthrodesis with AO-plate are recommended. The importance of the correct operative indication and method has been emphasized.

[Experiences with carpal arthrodesis (author's transl)].

Report is given on good results of 41 carpal arthrodesis with AO-plate. Literature is briefly surveyed, indications for arthrodesis and detailed technical description are given too. The importance of compression and stability has been stressed here. They find that fixation with AO-plate is the answer for the problems of carpal arthrodesis.

Study of the renin-angiotensin system in essential hypertension.

320 patients with essential hypertension divided into three groups (minor, medium and major severty) were studied against 50 normal controls. Plasma renin activity (PRA) was estimated in case of normal and low Na intake, furthermore, of low Na intake in association with administration of chlorothiazide, in the recumbent position as well as at the end of 4-hr walk. The hypertensive group of medium severity (Group II) revealed a significant decrease in PRA, compared with the controls and with the hypertensive group of minor severity (Group I), in each period of observation. A significant increase of PRA in response to insulin hypoglycaemia was demonstrable in all three groups and in the controls, but the increase was of significantly lesser degree in Group II than either in the controls or in Group I. The plasma catecholamine concentrations were found lower in Group II than in the controls and in Group I. An elevation of the plasma catecholamine levels was demonstrable in response to insulin hypeglycaemia, but it was of lesser degree in Group II than a Group I and in the controls. The poor responsiveness of the RAS in Group II is attributed to an inadequate adrenergic activity and to an increased mean arterial pressure in that group, but the higher age of patients and longer duration of hypertensive disease, both characteristic of the group of medium severity, may also effect the renin-angiotensin system.

The effect of pindolol on plasma renin activity in patients with essential hypertension.

Twenty-two patients with essential hypertension were treated for 3 months with pindolol, and blood pressure and plasma renin activity were measured at rest and after stimulation (upright posture stimulation and insulin induced hypoglycaemia stimulation). Beta-receptor blockade produced a significant decrease in systolic and diastolic blood pressure. After treatment with pindolol the plasma renin activity was significantly lower. Under conditions of renin stimulation such as orthostasis and insulin produced hypoglycaemia, plasma renin activity was significantly lower in treated patients. There was no correlation between the fall of plasma renin activity and the decrease of blood pressure. Renin suppression is probably only one of the factors involved in the reduction in the blood pressure in these patients.

Caries prevalence of preschoolchildren in Baja, Hungary in 1955 and 1975.

In 1975 caries epidemiologic investigations were performed in 1,017 preschoolchildren of Baja, aged 3--6 years. The data were analyzed and compared with those of 620 preschoolchildren of the same city in 1955. A mean increase of 10.9% of the caries frequency (percentage of examinees with caries), and a mean increase of 43.5% of the caries intensity(dmft count per examinee) could be observed in spite of a better vitamin D prophylaxis. The possible cariogenic role of the increased surgar consumption (37.5 kg from 24.4 kg per individual during 20 years) is discussed.

[Possibilities of treatment of open fractures of the tibia].

Results of healing in 477 cases of open leg fractures treated in the Central Institut of Traumatology in Budapest are analysed in the article. 47,2% of cases were treated by closed methods. The authors are convinced that careful conservative management leads to good results, if the type of the fracture is suitable. Conditions of operative treatment are outlined. In favourable circumstances a stable internal fixation should be performed, otherwise inramedullary wiring and plaster may hold the fragments until healing occurs. Soft tissues should be handled carefully and placing sutures under tension must be avoided, by all means.
