The role of synovial fluid filtration by cartilage in lubrication of synovial joints--III. Squeeze-film lubrication: axial symmetry under low loading conditions.

A mixture model of synovial fluid filtration and synovial gel formation at normal approach of cartilage surfaces in the human synovial joints loaded by a compressive force has been recently presented in Parts I and II of this paper (Hlavácek, 1993, J. Biomechanics 26, 1145-1150; 1151-1160). In the model synovial fluid is taken as a mixture of two incompressible fluids (ideal and Newtonian viscous), while the biphasic model of Mow et al. (1980, J. Biomech. Engng 102, 73-84) is used for cartilage. A system of partial differential equations for the normal approach of axially symmetric cartilage surfaces in the human hip joint obtained in Part II is solved numerically for low loads. A shallow pocket-type configuration of the synovial film is formed shortly after the load application at time t = 0. For constant loads the fluid film pressure profile follows very closely that in a dry frictionless contact. To this approximation and with the exception of a close vicinity of the squeeze-film edge the flux of the ideal fluid across the synovial fluid-cartilage interface varies quadratically with the radial distance r and decreases as t-1/2 with time. The ideal fluid is forced into cartilage at the central region and out of cartilage at the low-pressure periphery of the squeezed synovial film. The maximum gel-forming concentration (the 20-fold of the original value) of the hyaluronic acid-protein macromolecular complex of the synovial fluid is reached at the film centre first, then the gel film starts spreading quickly sideways. Later, the process slows down approaching the value r/2 1/2 where r is the radius of a dry frictionless contact. The final gel-film thickness decreases very slowly with the increasing r for 0 < or = r < r/2 1/2.

Mutacin II, a bactericidal antibiotic from Streptococcus mutans.

Mutacin II is an antibiotic that is produced by group II Streptococcus mutans. It inhibits the growth of other streptococci as well as many other gram-positive microorganisms by a hitherto unknown mechanism. Mutacin II possess bactericidal activity against susceptible cells. It transiently depolarizes the transmembrane electrical potential (delta psi) and the transmembrane pH gradient (delta pH) and partially inhibits amino acid transport. However, it rapidly depletes the intracellular ATP pool in glucose-energized cells and prevents the generation of ATP. It is concluded that mutacin II does not belong to the group of pore-forming antibiotics (type A) or to the type B antibiotics, which inhibit phospholipases or interfere with peptidoglycan biosynthesis. Mutacin II acts by inhibiting essential enzyme functions at the level of metabolic energy generation, an activity that has not yet been classified for antibiotics.

[Neovascular glaucoma with ocular ischemic syndrome].

Three cases of the neovascular glaucoma with signs of the chronic ocular ischemia in the occlusive disease of the arteria carotis (communis or interna) document the unfavorable prognosis of the disease. None of the patients was submitted to the carotis surgery. The visual acuity of the first patient decreased to 2/60. The eye of the second patient became blind and the third patient died after the second stroke.

[Rare late complication of transcystic drainage].

The authors performed 3539 successful endoscopic retrograde cholangio-panreatographies between 1982 and 1992. In 1160 (32.7%) of the cases was revealed some kind of pathological condition. In connection with one case, following conventional cholecystectomy, a very rare complication was diagnosed by endoscopic means. There was a stent-like object, hanging into the duodenal lumen, through a parapapillar fistula. This was surgically removed. It turned retrospectively out that the removal of K-32 feeding tube used as a transcystic drain 18 years earlier had managed only partially. After this period of time did it cause a biliary flow obstruction and a choledocho-duodenal fistula. The endoscopic diagnostic possibilities, course of disease and treatment is presented hereby.

Procedure for refolding and purification of recombinant proteins from Escherichia coli inclusion bodies using a strong anion exchanger.

Using Escherichia coli system expressing papilloma virus HPV16 E7MS2 fusion protein as a model system, a novel procedure was applied to solubilize, purify and refold recombinant proteins from E. coli inclusion bodies. The necessity to reactivate proteins at low protein concentrations (owing to their tendency to aggregate at high concentrations) was overcome by solubilization of inclusion bodies in alkaline solution and immobilization of proteins on a strong and resistant anion exchanger. This procedure has an inherent advantage of combining refolding and purification procedures in one step. The solubilization of the fusion protein in an alkaline reagent with the use of an anion exchanger resulted in considerable purification of the recombinant protein at a fairly high concentration. The protein was soluble under mild conditions and reacted with antibodies against the "native" papilloma virus.

Adjuvant effect of liposomes and adamantylamide dipeptide on antigenicity of entrapped synthetic peptide derived from HIV-1 transmembrane region glycoprotein gp41.

The synthetic peptide antigen (Ag) (the primary structure Tyr-Leu-Lys-Asp-Gln-Gln-Leu-Leu-Gly-Ile-Trp-Gly-Cys-Ser-Gly-Lys-Leu-Ile- Cys-Thr derived from the envelope glycoprotein gp41 of the human immunodeficiency virus type 1 (HIV-1) and exerting specificity with all HIV-1-positive sera available in the Czech Republic (and also in a panel of 10,000 sera from WHO)) was conjugated with bovine serum albumin (BSA) and encapsulated into liposomes. Adjuvant activities of liposomes with various lipid compositions were compared with Freund's complete adjuvant (FCA) and with aluminium hydroxide (AL). The immune response to BSA-Ag liposomes with coentrapped adamantylamide dipeptide (AdDP) was comparable with that of FCA in terms of longevity and levels of specific antibodies in mouse sera.

Presence of antibodies to seven human papillomavirus type 16-derived peptides in cervical cancer patients and healthy controls.

Sera from 133 cervical cancer patients and 154 healthy women (controls) from Spain and Colombia were tested in IgG-specific ELISAs for the presence of antibodies against seven peptides derived from five open-reading frames of human papillomavirus (HPV) type 16. Three of the peptides corresponded to overlapping regions of the N-terminal half of E7 protein; the other peptides corresponded to selected regions of E2, E4, L1, and L2 proteins. The prevalence of antibodies against E2 and E7 peptides was significantly different between patients and controls. The most marked differences were for E7 peptides. HPV DNA polymerase chain reaction diagnoses of cervical scrapes were available; these were correlated with serologic findings. In HPV-16 DNA-positive patients, E7 antibodies were more broadly and more strongly reactive than in other patient groups.

Isolation and biochemical characterization of a novel lantibiotic mutacin from Streptococcus mutans.

Certain members of the indigenous biota of humans produce antimicrobial substances called bacteriocins, which inhibit other bacteria, including members of their own species. One of these substances, mutacin, is made by Streptococcus mutans, a member of the oral biota. Mutacin inhibits other mutans streptococci as well as many gram-positive exogenous pathogens. Here, we report for the first time the purification and partial biochemical characterization of a lanthionine-containing mutacin peptide from S. mutants T8. The biologically active peptide was isolated from the broth cultures by ultrafiltration and differential precipitation. The final mutacin preparation was homogeneous as shown by sodium dodecyl sulfate-polyacrylamide gel electrophoresis and N-terminal amino acid sequencing. A molecular mass of the peptide was estimated by electrospray ionization mass spectroscopy to be 3,244.64 +/- 1.15 Da. Its amino acid composition indicates the presence of lanthionine and likely beta-methyllanthionine in a total of about 25 amino acids. Because alpha,beta-unsaturated amino acids, the precursors of lanthionine residues, are often found in lantibiotics, we carried out the addition reaction of the mutacin with N-(methyl)mercaptoacetamide. The subsequent electrospray ionization mass spectroscopy analysis indicated the presence of two reaction products with M(r)s of 3,350.45 and 3,456.0. These are interpreted as the mutacin molecule with the addition of one and two molecules of reagent to the unsaturated amino acids, respectively. Sequencing of the peptide revealed an N-terminal amino acid sequence of Asn-Arg-Trp-Trp-Gln-Gly-Val-Val.

Histochemical and functional parameters in Nordic combination athletes.

Bioptic samples from the vastus lateralis muscle were analyzed in a group of Czechoslovak representatives in the Nordic combination (ski-jumping and 15 km cross-country skiing). The distribution of individual muscle fibre types (FG, FOG and SO) was detected and correlated with values obtained by motor and functional performance tests. Histochemical analysis of the bioptic samples revealed a considerably heterogeneous distribution of muscle fibre types in the group studied. No typical profilation for this sport discipline was found. Weak correlation between the proportion of fast muscle fibres and explosive strength parameters was ascertained. The correlation between the proportion of slow muscle fibres and the capacity of O2 utilization (VO2max) was statistically significant. Strong correlation between the proportion of fast twitch fibres and relative maximal strength of knee extensors (N/kg) was disclosed. A non-linear relation between the area of fast twitch fibres and vigour of take-off was found.

[Massive colorectal hemorrhage in adult Schönlein-Henoch purpura].

Schönlein-Henoch purpura is a well known syndrome, rare in adults, and is seldom accompanied with extensive colorectal haemorrhage. In abdominal purpura, the small bowel is more frequently involved, but cases of oesophageal, gastroduodenal and colorectal localisation also have been reported. The cases with colorectal bleeding, appearing in adulthood and elderly patients, could cause differential diagnostic problems. Described endoscopic features of one patient, who was referred to our department, because of colorectal bleeding. The treatment, course of disease and endoscopic follow-up is presented. No previous endoscopic documentation of this type of localisation has been reported in Hungarian literature, so far.

Family therapy in a transitional society.

This paper is concerned with the application and adaptation of traditional models of family therapy as developed in Western societies, to the understanding and therapy of families in a society where the sociopolitical structures are changing rapidly. References were found in the literature only in connection with certain ethnic groups (Gray and Cosgrove 1985; McGoldrick-Pearce 1982), religious fellowships (Aradi 1988), and sociopolitical contexts (Auerswald 1983; Elkaim 1982). The authors began to deal with these phenomena several years ago. Our first publication examined differences in religious, social, and class relations within the family and the way in which these factors affect family roles and their implementation (Füredi et al. 1989). Subsequent events directed our interest to the effects of political changes on family relationships and what the strategy of the family therapist could be in this situation.

Instability of the production of avermectins, sporulation, and pigmentation in Streptomyces avermitilis.

Production of avermectins, sporulation ability, and colony pigmentation were followed in Streptomyces avermitilis C-18/6 cultures during serial transfer (for eight subcultures) in four different liquid media. These phenotypes were found to be unstable and independent of each other. A procedure was established whereby depressed avermectin production could be reversed but the degree of reversion was dependent on the history of the isolate, i.e., the type of medium used. The presence of isoleucine as the sole N source caused an immediate loss of avermectin production and this could not be reversed.

Application of digital image processing to evaluation of eye fundus pictures. Preliminary report.

An application of computerized digital analysis to photographically recorded pictures of the fundus of the eye is described. This approach has been found as a reproducible and objective method for detection and description of different changes in ocular tissues (exudative diabetic maculopathy especially) for the diagnostic or prognostic purposes (progress of the illness, results of the treatment). Possibilities offered by this method in other relevant retrospective studies are also discussed.

[Quadrant cyclocryocoagulation. Initial experience].

The authors describe a modification of cycloccryocoagulation /CCC/ they used in the treatment of different types of glaucoma in a group of 320 eyes. Cryodestruction of the ciliary body was implemented with a probe at a temperature of -60 degrees C for a period of 2 x 20sec. in two points of opposite qaudrants, using a transconjunctival approach. The probe was applied at a distance of 3 mm from the limbus. Following a period of 12 months to 3 years after operation they investigated the intraocular pressure and visual acuity of the treated eyes. They recorded a hypotonic effect comparable to cryocoagulation performed by the classical procedure; the incidence of complications was lower.

[Ischemic colitis].

Based on experience with 12 patients with ischaemic colitis observed in 1968-1992 the following conclusions can be drawn: ischaemic colitis is not as rare as assumed and its rising incidence must be foreseen. The clinical picture is not typical, but when suitable diagnostic methods are used, it can be diagnosed in a large number of cases. In non-complicated cases the prospects of conservative treatment are good. Surgery is associated, with regard to the age of the majority of patients, with a considerable risk. Therefore decisions on surgery should be made by the surgeon in cooperation with the gastroenterologist.

[Perianal pyoderma].

Perianal pyoderma (hidradenitis suppurativa-acne conglobata) is a chronic disease which in its progressive form cannot be treated by conservative methods. Extensive suppurative foci must be radically excised incl. all systems of fistulas right down to the fascia and the defects which develop are allowed to heal per secundam or after several weeks they can at a different period of time be covered by a skin transplant. The authors present their 30-year experience with surgical treatment of 39 patients. Treatment was in all instances successful after radical surgical excision. All these patients have fully recovered as regards work capacity as well as social aspects.
