Modulation of chronic lymphocytic leukemia cells by phorbol ester: increase in Ia expression, IgM secretion and MLR stimulatory capacity.

When cultured with 12-O-tetradecanoylphorbol 13-acetate (TPA) at a concentration of 1.6 X 10(-7) M, chronic lymphocytic leukemia (CLL) cells differentiated into mature cells of B lineage and increased their expression of surface Ia antigens when compared with cells cultured in the absence of TPA. Concurrently, TPA enhanced the ability of CLL cells to stimulate in a mixed lymphocyte reaction (MLR). The events induced in vitro by TPA that are characteristic of B cell maturation included morphologic changes, reduction in surface immunoglobulin (Ig), appearance of cytoplasmic Ig, and secretion of IgM. The increase in Ia expression and the enhanced capacity to stimulate in an MLR after incubation with TPA might also be associated with maturation of the CLL cells. The changes induced in vitro by TPA in neoplastic B cells provide new information concerning the terminal events in normal B cell differentiation.

Human erythroid cell lines derived from a patient with acute erythremia.

Three continuous human cell lines, designated KMOE, derived from a patient with acute erythremia (Di Guglielmo's disease) are reported. The cell lines are the cultures of (1) bone marrow cells, (2) peripheral blood cells, and (3) cells from a tumor developed into an athymic nude mouse after transplantation of the cultured bone marrow cells. Cells of all three lines show morphology of immature erythroblast and have i(17q) marker chromosome. They are negative for both Philadelphia chromosome and Epstein-Barr virus nuclear antigen. Although all KMOE cells in suspension culture are benzidine-negative, benzidine-positive cells are found within colonies formed in semi-solid culture media. The relative number of colonies with benzidine-positive cells is increased when sodium butyrate is added to the culture.

Vincristine and prednisolone therapy in blast crisis of chronic myelogenous leukaemia in childhood.

6 cases of chronic myelogenous leukaemia (CML) blast crisis in childhood were treated with vincristine and prednisolone (V-P). In 3 terminal transferase ((TdT)-positive cases, blast cells were lymphoid in appearance. All 3 TdT-positive patients entered a complete remission but developed meningeal leukaemia. The mean duration of survival from the onset of blast crisis was 32 months. In 3 TdT-negative cases, V-P therapy was ineffective. The results of this study suggest that V-P therapy is more effective for CML blast crisis in childhood with TdT positive blast cells and that prophylactic central nervous system treatment is necessary to prevent meningeal leukaemia.

Role of adenosine deaminase in activation of macrophages.

The intracellular adenosine deaminase activity of induced macrophages was twice that of resident cells. Coformycin, a specific inhibitor of adenosine deaminase, inhibited the activation of macrophages, as measured by superoxide generation.

Hemobilia after percutaneous transhepatic biliary drainage.

Hemobilia after the inception of percutaneous transhepatic biliary drainage (PTBD) has been reported as an uncommon, but sometimes fatal, complication. We examined the incidence, pathogenesis, and management of hemobilia in 94 patients who received PTBD. There were seven cases (7.4%) with transient and six (6.4%) with severe hemobilia; one patient (1.1%) of the latter group died. There was no correlation between hemobilia and hemostatic or hepatic insufficiency. Angiography during PTBD was performed in 47 patients, and abnormalities restricted to arterial changes were noted in nine (19.1%). All but one patient with hemobilia showed angiographic abnormalities. Our findings indicate that hemobilia occurs more often than has been suspected and that it is usually due to intrahepatic vessel injury rather than to hemorrhagic diathesis or hepatic insufficiency. The primary management of hemobilia consists of maintaining continuous patency of the drainage catheter.

Hepatocellular carcinoma in a case of Dubin-Johnson syndrome treated successfully with right extended lobectomy.

A 57-year-old man with hepatocellular carcinoma and liver cirrhosis combined with Dubin-Johnson syndrome was successfully treated by extensive right hepatic lobectomy. While the preoperative serum bilirubin level is one of the criteria in determining the indication for radical hepatic resection, it does not play a decisive role in the case of combined Dubin-Johnson syndrome.

[Transhepatic biliary drainage as a method in the diagnosis and treatment of obstructive jaundice].

Percutaneous transhepatic biliary drainage was performed on eleven jaundiced patients, who had obstruction of the bile duct, in order to determine the exact site and nature of the obstruction and to alleviate the jaundice. In the present series, six patients were treated with external biliary drainage. Four of them received this treatment prior to radical surgery. In the remaining two cases the external drainage was a palliative procedure. In five patients, the drainage catheter was internalized through the obstructed bile duct (internal biliary drainage). One of these patients was subsequently treated with radical surgery. In two cases, the internal biliary drainage was palliative. In the two remaining cases, intraluminal selective irradiation was introduced via Iridium 192 flexible wires. Severe hemobilia occurred in one patient, who was successfully treated by lithotomy and "T" tube drainage. We conclude that the simplicity and low cost of these drainage procedures recommend them as extremely useful for the diagnosis and treatment of obstructive jaundiced patients.

A rapid and quantitative assay of phagocytosis-connected oxygen consumption by leukocytes in whole blood.

We have devised a simple method for quantitative assay of the phagocytosis-connected oxygen consumption by human peripheral leukocytes. Time-consuming and leukocyte-debilitating cell fractionation procedure is avoided by the use of whole blood as assay material. Heparinized venous blood was treated with CO and put into a cuvette. A Clark-type oxygen electrode was equipped to the cuvette. A decrease in oxygen concentration in the whole blood was induced by the addition of bacteria. The decrease was exclusively dependent on phagocytizing leukocytes. All procedure can be performed within half an hour. Blood from boys with CGD showed little or negligible increase in oxygen consumption, and that from their mothers showed about 30% activity of control blood.
