The surgical management of adrenal cortical carcinoma.

Experience with the surgical treatment of 12 patients with carcinoma of the adrenal cortex is presented. They were 6 men and 6 women. Their ages ranged from 23 to 66 years old (mean 38 years). Four carcinomas had detected hormonal activity. The location of the carcinoma involved the right adrenal in 8 cases and the left in 4 cases. The mean diameter of the mass was 15 cm. Excision of the adrenal cancer with the ipsilateral kidney was the usual procedure. Lymph nodes were involved in 5 cases and interestingly the kidney was only involved in 3 cases. One patient died during hospitalization from pulmonary embolus, whereas another one required re-exploration for postoperative hemorrhage. Nine patients developed pulmonary, hepatic and bone metastasis within 3 to 10 months, postoperatively. Radiation treatment and chemotherapy had poor results. Nevertheless, one patient in whom left adrenalectomy, splenectomy and partial pancreatectomy was performed, is doing well, despite the presence of hepatic metastases, with the addition of o,p' DDD, 24 months postoperatively. Another patient underwent reoperation and excision of recurrent local disease 12 months after adrenalectomy/nephrectomy. She is now alive 16 months following her second surgery. Based on the above, an aggressive surgical approach is advocated in the management of adrenocortical carcinoma.

Influenza A virus and its influence on the outcome of pregnancy in the mouse.

The effect of sub-lethal doses of influenza A strains WSN (H0N1), MEL (H0N1) and MRC-7 (H3N2) administered intranasally during pregnancy was studied in C3H inbred and Prince Henry outbred mice. Maternal and neonatal mortality rates were significantly increased by infections in the last third of the gestational period. Infection with influenza strain WSN in the last part of the first third of the gestational period significantly depressed the growth rate of neonates. No evidence of viraemia, transplacental transmission or congenital malformations were observed. Invasion of alveolar spaces by mononuclear phagocytes and erythrocytes was more pronounced in pregnant mice infected in the last third of the gestational period than in non-pregnant mice, and paralleled an ablation of cell-mediated immune responsiveness.

[Surgical management of varicocele with vein-to-vein anastomosis].

The authors mention the technique of Koichi Ishigami, relative to 15 cases. This involves the anastomosis of the spermatic vein with the long saphenous vein. The authors use phlebography to show the permeability of this venovenostomy, and emphasize the quality of their results.

Serum transferrin and ceruloplasmin in patients with cancer of the gastrointestinal and other systems.

Serum transferrin and ceruloplasmin were determined in 20 healthy subjects, 23 patients with cancer of the gastro-intestinal system (G.I.S) and 22 patients with other types of cancer. Serum transferrin in patients with cancer of the G.I.S. was significantly decreased in comparison to that of healthy subjects (p < 0.05) and to that of other cancer patients (p < 0.005), but no significant difference was found between the values of healthy subjects and those with other types of cancer. Serum ceruloplasmin was significantly increased in patients with cancer of the G.I.S. (p < 0.01) and other types (p < 0.05) in comparison to that of healthy subjects. No significant difference was found between patients with cancer of the G.I.S. and those with other cancer types. In conclusion, transferrin was significantly decreased in the serum of patients with cancer of the G.I.S. while ceruloplasmin was significantly increased in both groups of patients with G.I.S. and other types of cancer.

Surgery of varicose veins on ambulatory basis. Early and late results.

Varicose veins resection on a day-case basis has the advantage of the early return of the patient to his usual activities, thus also reducing the cost and occupation of hospital beds. Between 1980-1992, 1710 patients (2325 extremities) were operated on as outpatients. The preoperative management included: clinical examination, Doppler ultrasounds, Duplex scanning, ambulatory venous pressure and phlebography. The surgical method was based on two axioms: resection of affected segments only, and functional operation according to the hemodynamic parameters. At the end of the first trimester, 1.572 (91.9%) patients showed satisfactory results. One to twelve years later 72.1% with surgical therapy alone and 82% in combination with sclerotherapy. The serious complications were injury in 2 cases, one of the common peroneal nerve and the other of the posterior tibial artery. In conclusion, patients with varicose veins may be operated as outpatients with good results.
