[Tentative results after implantation of isoelastic cementless hip-joint socket].

During the 3 years from 1979-1981 in 3 Austrian Accident Departments, 196 isoelastic acetabular components for total hip replacements were implanted. 3 different models were used as components of the femoral head. 194 acetabula were implanted without cement. In two cases cement had to be used because of extreme porosity or protrusion. Follow-up examination of 178 patients after 14-48 months yielded a good clinical result in 161 cases. 16 patients reported occasional pain, 38 patients had a slightly positive and 16 a definitely positive Trendelenburg's sign. On X-ray film 48 patients showed no change in bone structure at the borderline between bone and implant. Sclerosis and increased density of the surrounding bone were observed more frequently than formation of radiolucent seams or cysts. No definite signs of loosening were seen.

[Treatment results of late surgery in subcutaneous ruptures of the Achilles tendon].

15 patients with neglected subcutaneous ruptures of the tendo Achilles where operated at an average interval of 7 weeks after the accident. Up to 7 weeks after the rupture direct suture was possible, when the scar tissue was not removed. After a longer interval additional reinforcement of the suture or plastic repair was necessary. As immobilisation a low leg cast for 6 weeks is sufficient. Weight bearing is permitted after wound healing. Late results on 11 patients showed in all of them a loss of muscle strength of at least 10% as compared to the other leg. Also limitations of motion of the ankle joint is to be expected. 3 patients had infections. The use of non absorbable suture material is not to be recommended. The "pen-box operation" (Griffelschachtelplastik) leads to less devascularisation of the material then other methods with twisting a flap from the proximal tendon-stump for 180 degrees (Umkipp-Plastik).

[Initial results with the dynamic hip screw in comparison to other osteosynthesis procedures].

30 fractures of the proximal end of the femur were stabilised with so-called "dynamic hip screws" (AO/ASIF). This method seems to be more suitable for some inter-trochanteric fractures which pose problems questioning our usual treatment with Ender's elastic round nails. The dynamic hip screw system is also more stable for femoral neck fractures of type Pauwels III than 4 cancellous bone screws. One phenomenon which can not yet be fully explained is the slipping out of the screw to an extent unobserved in other procedures.

[Results of Bankart procedure in recurrent anterior dislocation of the shoulder].

Of 41 patients with recurrent anterior dislocation of the shoulder operated on during a nine-year period (1970-1979), twenty-eight were re-examined and six answered a questionnaire. There were two recurrences (6%) after repair by the Bankart-Procedure. The results at follow-up were rated excellent in 59%, good in 29%, and poor in 12%. In 35% limitation in the external rotation was noted. We concluded that with the technique of the Bankart repair as described, only short postoperative immobilisation is necessary, early return of motion and function can be expected.

[The influence of primary closed reduction on the incidence of posttraumatic femoral head necrosis (author's transl)].

24 patients with fractures of the femoral neck were younger than 65. Eight of them were operated on the day of the accident. The fractures of the other 16 patients were immediately reduced, the leg placed in traction and the operation carried out between 2 and 8 days later. Osteosynthesis was performed by means of cancellous bone screws without opening the joint. If this internal fixation was stable, the incidence of femoral head necrosis was reduced to one tenth. For fractures of the group Pauwels III this method was not suitable. Early reductions seems to be of more importance for the final results than immediate surgery.

[Material testing of modern plaster substitutes].

4 Plaster substitutes were tested in order to obtain some information about their reaction to stress. 2 materials are made of glass fibre with resin (Scotchcast and Lightcast), one from another artificial product (Baycast) and the fourth consists of a mesh of pure resin (Hexelite). Their resistance to bending forces was correlated to the weight of the samples used. The results indicated that a product containing glass fibre as carrier for the resin is the most robust.

[Indications for osteosynthesis in childhood hand injuries (author's transl)].

The general indication for osteosynthesis in childhood can also be applied to the bones of the hand. The preferred operative procedure is to reduce and stabilize the fracture with one or two percutaneous Krischner-wire. Where there are defects in the bones or soft tissues, the use of a mini external fixation has proved rewarding, even on growing bones.

[The creeping post-operative hip joint empyema (author's transl)].

The case histories of 10 patients with chronic empyema of the hip joint following nailing of the femoral neck, respectively following intertrochanteric osteotomy are being described. With only a few exceptions their post-operative course showed no signs of infection. After an interval of between 5 weeks and 2 years their condition deteriorated rapidly. By the time the patients returned to hospital, usually, the joint was already destroyed or septic luxation had occurred. Because of their poor general condition only resection of the femoral head was possible. As an alternative to continuous suction drainage, the implantation of PMMA chains seems to have advantages.
