25(OH)D levels in diabetic pregnancies relation with neonatal hypocalcemia.

The levels of 25(OH)D have been quantified in 42 insulin diabetic pregnancies (DP) through the three trimesters of pregnancy with a total of 177 determinations. In parallel we quantified this metabolite in 114 normal pregnant women (NP) and also in 116 normal controls (NC). In addition 25(OH)D was quantified in 18 (DP) and 19 (NP) at delivery in the 35-37th week of pregnancy, and ionic calcium was measured in their newborns at 24 h of life. Grouping by trimesters of gestation, the (NP) group had 25(OH)D levels similar to those of (NC) and none showed significant differences between trimesters of pregnancy. (DP) showed in all seasons lower (25(OH)D levels than (NC) but did not have differences in these levels between trimesters of pregnancy. The newborns of (DP) had lower ionic calcium levels than newborns of (NP). Eight newborns of (DP) had hypocalcemia and seven of their mothers showed 25(OH)D levels lower than 10 ng/ml. These findings suggest that lower 25(OH)D levels in (DP) can influence the neonatal hypocalcemia in their newborns.

[Parathyroid hormone: perinatal aspects].

Authors have measured parathormone levels in 28 mothers at delivery and their respective newborns at 40-50 hours after birth. They also quantified these levels in umbilical vessels. Control groups was formed by 48 healthy non pregnant women. PTH levels in pregnant women were similar to controls. In umbilical artery these values were higher than in umbilical vein and in both, lower than other groups. Newborns at 40-50 hours after birth showed higher PTH levels than controls and pregnant women at delivery. A positive correlation between the PTH levels of the mother and those of the umbilical vessels is found. PTH levels of the neonate at 40-50 hours after birth correlated with levels of umbilical artery and umbilical vein. They do not observe a hyperparathyroidism at the delivery, although there is a slight decrease in Ca levels. Newborns at 40-50 hours after birth did show an increase in their PTH levels in parallel with hypocalcemia correlation found between PTH levels of the mother and those of the umbilical vessels could suggest a placental transfer for this hormone.

Serum CPK-BB isoenzyme in the assessment of brain damage in asphyctic term infants.

Serum creatine phosphokinase-BB activities were measured at 4 and at 10 hours of life in 33 asphyctic full-term infants. The infants were followed for a mean period of 16 months. Enzyme activities at 4 hours of life were significantly higher in those neonates who died of severe hypoxic-ischemic encephalopathy or developed neurological sequelae than in those who did not present neurological abnormalities during the follow-up time. The specificity of the enzyme assay in order to predict the neurological outcome was inferior to that of cranial computed tomography or a combined clinical-electroencephalographic evaluation. It appears that the presence of elevated serum creatine phosphokinase-BB activity can be a sensitive indicator of conspicuous brain damage, but it is of limited value to predict the neurological outcome after neonatal asphyxia.

Caudal deficiency and asplenia anomalies in sibs.

The caudal deficiency and asplenia anomalies have been described separately in children. Two sibs with these two entities are described. Only another similar case, with caudal deficiency and polysplenia anomalies was found in the literature. It is possible that this association could be a new syndromic entity. An autosomal recessive mode of inheritance is suggested.

Hydranencephaly after maternal butane-gas intoxication during pregnancy.

A newborn infant who suffered intra-uterine anoxia is described, whose mother inhaled butane gas accidentally during the sixth month of pregnancy. The infant was born at 39 weeks. Ultrasonography and neuroradiological studies (CT scan and angiography) showed an almost complete absence of both cerebral hemispheres. The thalamus, brainstem and cerebellum were preserved. These findings were compatible with hydranencephaly. The authors believe that the malformation was due to intra-uterine anoxia occurring during fetal brain-development.

LDH isoenzymes in CSF in the diagnosis of neonatal brain damage.

Lactate dehydrogenase isoenzymes in cerebrospinal fluid were measured during the first 24 h of life in 25 asphyctic neonates. The infants were subjected to clinical and developmental examinations for a mean follow-up period of 15.2 months. The mean values for cerebrospinal LDH2(MH3) and LDH3(M2H2) isoenzymes were significantly higher in asphyctic infants who died from hypoxic-ischemic encephalopathy or who survived but suffered neurologic sequelae, than they were in those asphyctic infants who survived and were normal in the follow-up studies. Therefore, the authors conclude that the lactate dehydrogenase isoenzyme pattern in cerebrospinal fluid may be of value when assessing anoxic brain damage.

Hydrothorax, ascites, and right diaphragmatic hernia.

Hydrothorax and/or ascites may be the most striking finding in children with right diaphragmatic hernia. The clinical, radiographic, and pathologic findings of five children with right diaphragmatic defects through which the liver had herniated are described. Three presented with a right hydrothorax, one with a right hydrothorax and ascites, and another with ascites. All four children with large right hydrothoraxes were found to have an incarcerated peritoneal sac filled with fluid in the right side of the chest at surgery or autopsy. Lymphatic congestion and obstruction was the probable cause for the fluid collection, which tended to enlarge with time. This condition may be life threatening, and two of the four patients died soon after birth because of hypoplasia of the lungs. Fetal ultrasonography in both had disclosed right intrathoracic cystic masses, and in one, intrauterine aspiration to decompress the lungs had been attempted. The other two patients are alive and well following surgical repair at 1 week and 7 months of age. Ascites was present in two patients and was believed to be due to hepatic venous obstruction, a mechanism similar to that responsible for the Budd-Chiari syndrome.

Ionic calcium levels during pregnancy, at delivery and in the first hours of life.

Free calcium ion concentration (mmol/l) and pH were determined in whole blood using a semiautomatic electrode system (ICA-1 Radiometer, Copenhagen, Denmark) in 37 normal women, 90 pregnant women (30 from each trimester of gestation), 28 mothers at delivery and their respective newborns. The blood samples from normal controls, pregnant women, umbilical cord and 40-50-hour-old infants were collected anaerobically in vacuum tubes. Duplicate samples drawn from newborns shortly after birth by heel puncture were collected in special heparinized capillary tubes. We observed that Ca2+ concentrations in the second (1.20 +/- 0.04) and third (1.20 +/- 0.05) trimesters of pregnancy, and at delivery (1.18 +/- 0.05), were lower than in the control group (1.23 +/- 0.04). The [Ca2+] in samples from the umbilical vein (1.44 +/- 0.11) and artery (1.45 +/- 0.08) and from newborns 2-5 min after birth (1.34 +/- 0.12) was greater than in control samples. The [Ca2+] in newborns 40-50 hours after birth was lower (1.16 +/- 0.14) than in the control group.

[Triad syndrome in the neonatal period: an anatomoclinical study of 11 cases].

Eleven newborn children with clinical and anatomic characteristics of Prune-Belly syndrome were studied. This establish an approximate incidence of 1:28,000 newborns. All these children died in the first-days of life, nine of them as a result of pulmonary hypoplasia secondary olygohydiamnios. All of them except one had urinary tract dilatation and in four anatomic stenosis of urethra was found as the cause of obstruction. These findings support mechanical theory more than a primary mesodermic disorder as a pathogenic mechanism. All this, together with establishment of an early prenatal diagnosis, opens the possibility of an intrauterine treatment which could free the urinary tract and prevent oligohydramnios and its, sequelae.

[Early detection of congenital hypothyroidism. Study of 31,609 determinations].

As a result of congenital hypothyroidism (CH) early detection program carried out in our clinic during the period 1978-1981, 31.609 capillary blood samples (35,7% from newborn infants) were taken on filter paper cards. An evaluation of TSH following the specific radioimmunoanalysis technique was made in all the samples, and an evaluation of T4 whenever TSH values were higher than 25 microU/ml serum. Thirteen cases of CH were detected: nine with permanent H and four with transient H. Two cases of hyperthyrotropinemia were also detected. Frequency of permanent CH in our population is of 1/3.512. Corresponding biochemical data and detailed clinical characteristics detected, including the cost of the procedure are shown. Authors emphasize on possibility of preventing brain damage and subsequent subnormality through early detection and treatment of congenital hypothyroidism, together with a suitable cost-benefit ratio, makes necessary to carry out a nationwide campaign in accordance with those centers already established.

[Mucolipidosis II or I-cell disease in the neonatal period].

A newborn with the clinical, radiological and biochemical features of mucolipidosis II or "I cell disease" is presented. Some aspects of differential diagnosis in neonatal period and of the outcome are commented. The infant had also a single right kidney and agammaglobulinemia without further alteration of the humoral or cellular immunity at 10 months of age.

[Pathogenic factors and therapeutic criteria in 82 newborn infants with necrotizing enterocolitis].

Sixty newborn infants with necrotizing enterocolitis (NEC) were retrospectively studied in 1979. The incidence was 0,49/1.000 living newborns: 81% were under 2,500 g and 30% under 1.500 g. We found a very low incidence of the so-called pathogenic factors. Feeding started in this group on the 24 h of life, with adapted formulas; 28% underwent surgery with a postoperative mortality of 62% and an overall mortality of 28%. Ten out of the 17 deceased were under 1.500 g. From 1979 with a similar medical treatment we introduced a prospective study consisting of a protocol of 10 different parameters including clinical, radiological and analytical values, the aim being a more objective way of indicating when to operate the patient. Special meticulous care of the nutrition of the low-weight babies was taken; 22 patients were treated til 1982, only 4,5% were under 1.500 g. The initial feeding time in all high risk babies under 1.250 g being 7 days with gastric gavage and elementary diets. All the infants with more than 60% values in the protocol underwent surgery (38%). The postoperative mortality was 37% with an overall mortality of 19%. A look back to our series suggests a clear usefulness of the protocol and the suspicion that the delay on feeding with intragastric continuous perfusion in low weight babies reduces NEC.

Dietary treatment and biochemical studies on a neonatal case of propionyl-CoA carboxylase deficiency.

The case of a patient with neonatal propionic acidaemia is reported. Despite an initially favourable response to the administration of an artificial formula, the patient finally died when 9.5 months old. Protein tolerance never exceeded 1.3 g kg-1 day-1. During the remission periods, when ingesting the formula, 3-hydroxypropionic acid was excreted alone or together with tiglylglycine and/or methylcitrate. In a period of ketoacidosis, in addition to these three metabolites and those of ketoacidosis, elevations of 2-methyl-3-oxovaleric acid, propionylglycine and 2-methyl-3-hydroxybutyric acid were found. A severe deficiency of propionyl-CoA carboxylase in cultured fibroblasts was detected; biotin, when added to the fibroblasts culture media, did not stimulate this enzyme activity. The effectiveness of the administered formula is discussed.
