Twenty four neonates affected of osteoarticular infection, treated on the Neonatal Unit throughout a four and a half year period are analyzed. In the majority of instances they represent a complication in the course of neonatal sepsis. In more than half of them there was a previous umbilical vessel catheterization. "Staphilococcus aureus" was the most frequent etiologic agent. Involvement of the hip was the location more often seen and of worse prognosis. Treatment besides of supportative measures, and specific antibiotic therapy, included posterior arthrotomy in the 13 cases of hip arthritis, and diagnostic puncture and/or arthrotomy in the rest of locations. There was no mortality, and only 20% of the patients exhibits long-term residual defects of variable degree.
During the period January'69-June'81, 45 cases of listeriosis in neonatal period were observed. 37 of them had an early onset (82%) while other eight had late infection disease (18%). Incidence was 1/6,346 newborns with most frequent presentation in spring. No epidemic forms or asymptomatic carriers were discovered. Perinatal, clinical, analytical, radiological, bacteriologic and pathological data are shown, emphasizing differences between the two clinical forms. Overall mortality of 43%, although brought to zero in the last six years, together with the same percentage of neurological sequela in the late form, urges for better knowledge of epidemiology and preventive measures, as well as a closer obstetric-neonatal relationship.
A retrospective study is made on the results of the treatment of 38 preterm infants with symptomatic PDA; they represented an incidence of 4% of all the admissions to our Unit from June 1978 to March 1980. 30 of the 38 infants (79%) had PDA associated with RDS. Conservative medical treatment failed in 42% of the patients, requiring the administration of indomethacin for pharmacologic closure of their PDA. The different responses to the drug in each of the established groups are commented, being the group with a birth weight less than 1,500 g who presented the highest percentage of re-openings (62.5%) and of therapeutic failures (50%). An early closure of the PDA can contribute to decrease the morbidity and mortality of these infants, specially those with lower birth weight.
Authors communicate four cases of bloody diarrhea in neonates, with Campylobacter fetus jejuni in the stools. In one case, the same bacteria was isolated from the mother and, in another case, existed possibility of a cross-infection. The mildness of the evolution is marked, specially in those cases in which maternal feeding was continued. Some diagnostic and epidemiological aspects, concerning the neonatal period, are commented.
There is a higher incidence of delayed closure of the patent ductus arteriosus in premature babies with respiratory distress syndrome. From July, 1975, to December, 1977, 57 small, preterm infants with patent ductus arteriosus were diagnosed at our neonatal intensive care unit. From July, 1975, until September, 1976 (first period), 23 patients were diagnosed, and 11 underwent surgical ligation of a patent ductus arteriosus. There were 3 early deaths. From October, 1976, until December, 1977, out of a total of 34 patients with diagnosed patent ductus, 18 were treated with indomethacin, and only 3 required ligation. Our present policy for patent ductus arteriosus with respiratory distress syndrome in the premature baby is to initiate early treatment with indomethacin. If this treatment fails and the infant's status deteriorates, we perform early surgical ligation of the ductus in order to minimize the time on mechanical ventilation and lessen the chances of the development of bronchopulmonary dysplasia.
Three patients with Meconium Aspiration Syndrome (S.A.M.) and severe hypoxemia are reported. From the first hours of life they needed mechanical ventilation, showing no improvement in PaO2 in spite of usual respiratory support measures. In one case, pulmonary hypertension and ductal right to left shunt through ductus arteriosus and foramen ovale was objetivated by cardiac catheterization. Following intravenous prefusion of a pulmonary vasodilator (Tolazoline) patients showed a clinical amelioration and a definite increase in PaO2. Results and evolution with the use of this drug are commented.
Twenty seven newborn with serious meconial aspiration syndrome are studied. Two groups are stablish weather they require or not mechanical ventilation during evolution. When comparing various perinatal and clinical data, only the Silverman test showed statistically significant differences; pH and gases in blood during the first hours of life did not showed differences. Pneumothorax incidency in both groups was 33%. Twelve newborn required mechanical ventilation. The starting age was 24 hours of life range 3-54 hours. Most frequent indication was hipoxemia and apnea. Use of intermitent positive pressure improved PaCO2 with a light increase in PaO2 though it also increased alveolo-arterial oxygen gradient. Hipoxemia was the most relevant data in evolution of these patients. In seven cases continuous distending pressure was applied, with a light increase in PaO2 only in three patients. No patient suffered pneumothorax during mechanical ventilation. In patients with mechanical ventilation mortality was 25%. The possibility of an addition in certain cases of pulmonary hypertension with right-to-left shunting ductal and atrial is reported.

A study was made of the blood levels, half-life and elimination of fosfomycin in two groups of underweight infants during the 1st to 3rd days of life and after 3-4 weeks. The test dose was 50 mg/kg i.v., administered in a single dose. The times at which the blood was controlled were 5 min, 1, 3, 6, 9 and 24 h; and 0-24 h for the urine. All samples were evaluated by the microbiological method of diffusion in plate. The results obtained indicate that the elimination of fosfomycin after 24 h is slower and occurs in smaller quantities in the group of infants 1-3 days old than in the group of 3- to 4-week-old infants, the half-life of the antibiotic being 7 and 4.9 h, respectively.
A serie accounting for 32 newborn sick infants with a mean weight of 1,324 g. and gestational age of 30.6 weeks in whom parenteral nutrition was applied alone or as supplementation of oral alimentation is presented. The mean duration was of ten days, and mean weight increment of 14.6 g. Infectious complication were the most frequent (25%). Mortality was 46% due in part to the important associated pathology. The general lines for management of these infant are given. Finally it is pointed out that, intravenous nutrition is only justified by the failure to use oral feeding.

The prognostic value of cerebrospinal fluid (CSF) lactate concentration after an hypoxic-ischemic brain insult was evaluated. Three groups of neonates were studied and followed. Group 1A included 19 asphyxiated neonates who subsequently developed normally, Group 1B included 6 asphyxiated neonates who died or developed neurologic sequelae, and Group 2 consisted of 20 control (comparison) infants. A CSF sample was obtained within the first day of life for cytochemical analysis, culture, and lactate determination with simultaneous determination of serum lactate. The mean CSF lactate level in Group 1B (4.5 mmol/L) was significantly higher than in Group 1A (2.5 mmol/L) (p less than .001). In comparison to the highest CSF lactate concentrations in the normal range in Group 2 (2.7 mmol/L), all Group 1B neonates had elevated CSF lactate levels, and eight neonates (34%) of Group 1A had increased levels. A correlation was found between blood and CSF lactate levels in asphyxiated infants (r = .42, p less than .05), but its meaning is uncertain. We conclude that CSF lactate determinations may be helpful in the evaluation of perinatal hypoxic-ischemic encephalopathy.
